ER lobby is genuinely the most miserable place on planet earth I swear to fuckin god I'm losing it. Everyone's been asking me about the woman that's been screaming and loudly vomiting for the past two hours but honestly I'm more concerned about the guy with a note in the trackboard that says "NEEDS MAGNESIUM, LASIX, ROOM FOR HOSPITALIST TO EVAL" and it's been three hours and he doesn't even have an IV yet

m4a btches!!!! deprivatize healthcare and im NOT just saying that because im so fucking tired of trying to puzzle out how to code what when there are fifty bajillion companies and fifty bajillion plans! these companies are bloated monstrosities eating the money that would be better spent on your care, and the care of all around you, relics of times long past! fuck ‘em! even the ACA needs work, and not in the way people think! for most people who state medicaid won’t cover and who don’t get insurance through their job, exchange plans (ACA) arent an option due to how expensive they can be. essential plan 1+2′s, too.
do y’all know how much it took the wind out of me to start explaining the medicaid application process to a patient, just for her to immediately tell me that she knows she won’t qualify because of how much she makes? so many people have employers that either don’t offer coverage since they’re so small, or cheat them out of coverage entirely. still, i gave her the papers. i hope the financial aid office can knock her bill down.
y’know, in communities like the one my hospital’s located in, there can be days where a good fifth of the patients on our trackboard don’t have any insurance at all.
this doesn’t have to happen. this doesn’t have to keep happening.
we can do better.

Thoughts as I'm trying to fall asleep
I'm thinking back to experiences of interactions with police as a medical professional- when we called security/police for a patient threatening people, or when they bring a person in to the ED for medical clearance
And I distinctly remember one story from the ED trackboard- in one room, a toddler with a head injury- abuse suspected, in another, his father in police custody. I initially think - that is a case when we need police
And then I think. What if. (In the spirit of the other posts of scenarios that are currently handled by police and how they could be handled differently)
Because I know that most people guilty of harming a child didn't do it out of malice - they do it because they are overwhelmed. They shake a baby because they haven't slept in 20 hours straight and have been short on sleep for months and the baby is crying and they don't know why and they can't make it stop.
As a new puppy parent, I know I absolutely never would hit or harm my dog. But I have, in times when she is biting me, or going wild, or wanting to play at 3 am, grabbed her or pushed her away or held her down for a minute while I took some breaths to figure out how to handle the situation. And that's a puppy, who is perfectly happy to be gently or even firmly mandhandled, and is much more managable and more quickly maturing than a human
So. What if there was a number you called where you could say- my baby is driving me insane. I feel overwhelmed and i haven't slept and I need a moment to take a few breaths. My toddler is having trouble potty training and had his 5th accident and is being stubborn and I have a million other things to do. And someone (trained, licensed, trustworthy) could show up to your house and hold your baby while you shower, or start a load of laundry for you, or help you figure out how to pump, or let you sleep, or talk through better ways to discipline children. A person who would understand that parenting is overwhelming. A person who you aren't scared will remove your children from you. Just there to help
My initial thought was- what, you just want people on call to show up wherever they are needed at short notice? Because the time between feeling overwhelmed and things escalating to violence against a child could be too fast for this to help, you would need someone on call all the time and ready to say I'll be there in 15 minutes. And then I thought back at myself- well yeah. We have the police set up that they can show up in just a few minutes whenever they are called. If we can do that to react to harm, why couldn't we do it to prevent it? Wouldn't that be so much better?
I literally am about to go to bed so the wording of this is rough and it needs proofreading but I got very excited about the possibility and had to write it down. Because once you have seen a case of child abuse you want to do anything you can to prevent even one more. And there is so much possibility if we stop accepting that the way things are is the way things should continue to be
So yeah. Defund the police. Fund communities. Funs people. Fund prevention

Hospital Fun Facts (at least for the big hospital i work at):
they dont call code blues code blues anymore, too many people prolly cottoned onto what that meant and got scared when codes were called. now its Code CAC when its overhead paged, still commonly referred to as coding, which still means cardiac arrest, which still majorly sucks
we also dont tend to do that thing where we cover bodies with sheets so you can just pass by a body in RZ and be none the wiser until you check the trackboard or notice the crash cart parked in front of the room
also, it can take a good 3 hours or so to actually transfer them out, so they’ll just kinda be hanging out in the ED for a while.
