So, from my understanding, Caterpillar is a hiluchurl that got his consciousness back by whatever the fuck Renee did to him before. He doesn't remember anything from before this ceremony. We also don't know if he's a Khaenri'ahn-turned-hilichurl or some other random hilichurl that Renee just picked up.

Interesting that whatever ceremony Renee did actually granted Caterpillar his consciousness back when both Clothar and the Abyss sibling failed to do so (or at least, failed to stick in Clothar's case). Like, Clothar had the blessing of the Sinner (whoever he is) and the Abyss sibling has what we assume to be the Primordial Sea Water and yet those two failed while this random ass kid managed to do it.

And of course there's Caterpillar's hands/feet and it's similarity to Arlecchino and whatever that implies.
