but now, he’s nearing fifty, retired, still “in shape,” but not anywhere near where he was when he was part of the task force. his breasts are a bit more noticeable now, enough where he’d do a double-take in the mirror if he wasn’t wearing a binder or tape. the familiar feeling of top dysphoria came creeping back, as sniveling and pathetic as it is. (not that he felt pathetic for feeling it – he just felt as if it was a benign tumor that he’d be much happier without.)
you and simon had tackled this as a team, as you always did. you both did research about types of top surgeries, doctors, hospitals, recovery, the like. you had reassured him when you noticed any amount of hesitancy, (mostly about him being “too old” or not qualifying for surgery), calling him your “big man” and holding him tenderly, soothing your hand over the softness of his belly.
you took care of him in soft ways like that. you made sure he was comfortable, laying him down as you wiped an alcohol swab over his belly. your hands were careful as you pinched the fat of his stomach and injected his testosterone shot, soothing the injection site with a bandage and a kiss. 
“i can do that myself, y’know,” simon grumbles, but he does nothing to stop you.
“i know,” you say softly. you rub a thumb over the bandage, pushing down on it gently to calm any lingering pain – though usually, there was none. “just practicing taking care of you for when you’re recovering.”
“you take care of me just fine, lovie.” simon takes the syringe from your hands and caps the needle before putting it on the bedside table. then, he pulls you down so that you’re laying on his bare chest. he was shirtless and binderless – a true man in his true form. 
you hum and move so that your cheek is pressing against his chest, your hand resting on his sternum. you breathe in deeply, taking in the scent of the musk between his breasts. it was a heady and intoxicating smell, just like simon.
you trace the scars on his sternum – acne scars from when he first started testosterone. “hm… i’m gonna miss this,” you mumble.
simon’s hand comes up and pets the back of your head, messing with your hair. “miss what?” 
“this,” you say. “resting on your chest. hearing your heartbeat.”
simon huffs out a laugh. “the recovery is only two months.”
“two months too long,” you whine, then press a kiss to his chest, right over the darkest, dipping acne scar. “how am i gonna kiss you like this when you have all those bandages on you?”
“you’re just gonna have to wait,” simon chides, but you can hear a smile in his voice.
you sigh dramatically and nod. “you’re right. and i know it’s for the best. i’m just gonna miss loving on you like this, okay? even if it’s just for two months.”
simon just hums softly in response. his hand continues to play with your hair, combing the strands with his fingers. 
“mh… you gonna miss my chest?” he asks.
“i don’t think so,” you say. “it’s your choice, really. i’ll support you through everything, you know that.” you glance up at him with a teasing smile. “and i’ll finally get you to rest while you’re recovering instead of you busting your ass every day.”
simon rolls his eyes, but there’s still a smile on his scarred lips. “yeah, yeah. you sound like you’re looking forward to it, lovie.”
“maybe i am.” you bury your face in his chest again, pressing another kiss to his scarred skin. 
“don’t expect me to stay down for long,” simon says.
“i won’t,” you mumble. 
simon presses a soft kiss to the top of your head, his hand drifting down to rub one of your shoulders. “i know.”
you sigh softly as you relax into his touch. you know it’ll be like this, now and forever. even after simon’s surgery and recovery, he’ll cradle you to his chest and exchange kiss for kiss like he is now, loving and soft and sweet. 
he’ll always be your boy. your lovely boy.

Just want to write a bit about my experience with the whole starting testosterone process in PA. I have had a really great experience and I really hope this can help other trans people to feel more confident going into this process.
I’ve been out since I was in 4th grade and this year I started the process to go on t. I made an appointment with my regular doctor, just a “well checkup” and spoke to her about my desire to start testosterone. She was incredible, no invasive questions, she was respectful of my desires, spoke to me about some options she was aware of and recognized that she didn’t known enough. I went home, she did her research, and she made me a referral to a gender clinic near me within the week.
My mom called the gender clinic for me, they put me into their system and said I’d get a call within 2 weeks to schedule a therapy appointment. The therapist would be the one to give me a referral to the medical doctor at their clinic. They called us back within 2 days NOT weeks. And much to my joy and disbelief there wasn’t the wait list I expected. My appointment with the therapist was going to be the very next week.
Today I went to the therapist. She was the most queer informed cis het individual I have ever met. She was understanding of and knowledgable about not just the medical and psychological side of being trans and queer, but also of the culture and media. She didn’t make judgements of my competency based off of my past mental health experiences or my autism. It was the first time I ever felt understood by or seen by a therapist or health professional. She was non-invasive, her questions were respectful, her understanding of gender and transition as existing outside of just a “hatred” for one’s body and as a range of experiences in and outside the binary was beautiful. At the end of the appointment after speaking to me and my mom she had decided to give me the go ahead for the medical doctor in order to receive testosterone. Once again I was surprised: I had faced no ignorance from the therapist, there weren’t a million more appointments for me to attend, or months of begging for something I have clearly and consistently wanted for years of my life.
When I went to the front desk to make the appointment I once again expected some disappointment or an obstacle. Perhaps a months or even a year long wait to see the medical doctor, after all everything had gone so well so far, surely there would be an obstacle here. But no, my appointment was scheduled for the 4th of this upcoming month, 12 days from today. Not even 2 weeks. On that day I will more likely than not receive my prescription for my testosterone.
I am so full of joy and hope with this experience. For the first time in my life I feel like my future is real, like my life will be fully mine. I feel confident and full of anticipation for the future. And I’m so touched by the understanding and support from the medical professionals I’ve worked with to get to this point, who have worked so hard to be understanding and inclusive and kind. I understand my privilege here, I’ve read the horror stories, I know how so many people struggle, and I feel deeply in my heart for everyone in our community who has been met with barriers and ignorance and fear in their journeys to medically transition. But I hope that my story can spread some joy and hope for others, and maybe alleviate some fear for some. Sometimes, as rare as it is, we do get breaks, things do get to be easy and kind for us, and with the way things have gone in this country the last several years I’ve needed proof of that so much more than ever.
I’m looking forward to sharing more of my journey with medically transitioning! I’m happy to answer any questions about the process to getting there, what my therapy session looked like, and going forward about the physical and mental changes testosterone causes!

A doctor said to me that he cannot give me testosterone, but I can buy it and inject it myself! They don’t sell medicines like that without a doctor’s permission.
I look like a woman, or a 12-year-old cis boy.
I am 19 years old
And when people meet me, a grown man, they see a child and act accordingly. They call me little and short, and I can’t tell people in public how old I am, but they always ask.
I hate myself because I don’t look like the grown man I am. I am 153 cm and 42 kg. I am short, skinny, and have a baby face.
I sometimes present as feminine, and when I do, people in public say unkind things to me. They even try to harm me.
I live in a place where the government executes gay men and I am afraid when they see me as a gay boy.
I live in a Muslim country, so they expect me to wear a hijab, even though I am not Muslim.
I can’t transition here, even if they allow it. The doctors don’t know what they’re doing. I don’t want to be a laboratory mouse. Once, the most famous doctor was accused of killing a person just from a mastectomy! I want a healthy, beautiful, normal body.
I can’t travel for transition because I am very poor, and in my country, even $10,000 is a lot. Even with 100 years of working, I couldn’t accumulate that much money.
But they won’t even let me work or study! Many LGBTQIA+ people here have been expelled from school.
In my country, a trans person is a psychopath. Many of us don’t have an ID ( of our true gender) , and we can’t live like this.
I can’t attend classes, such as an art class, or visit any doctor. They require an ID, and even when they don’t, I don’t want to out myself or have them touch and examine my body.
I experience all forms of dysphoria that exist. I am dealing with depression, childhood trauma, ADHD, social anxiety, among other issues.
I tried to kill myself twice. I have left school. I don’t want to leave the house, but I am trying to change these things, and I can’t seem to do so.
And you know what? Nobody cares!
Do you think all transgender individuals speak English and reside in countries that are friendly to the queer community?
I cannot create a GoFundMe here; there is no supportive organization or similar entity available. Everyone here hates me and can easily kill me.
I am gay, and my relationships have always been toxic.
Men do not perceive me as a man.
My father left me; my mother just doesn’t care about me, and my brother is my biggest enemy.

Actually, it’s the psychologists, but mostly the government. You need to prove yourself to them, and I tried hard, but I failed.
Even my family doesn’t see me as an adult—a man who is 19 years old.
Most of the day, I talk to AI because it is kind and knows what it is doing.
Here people think we are sex workers. That Trans people are always horny!
I have dysphoria, so I am not horny, even when I want to be. I can't even masturbate. I can't even look at it.

Queer people in my country are the most miserable people on the planet. And they are against each other more than anywhere else. Gay men don't want me around them here ,same as Trans men. And they all hate non-binary people, Polyamorus people and people like me who have more than 10 labels.
I want to grow one day and become an artist, a writer, and an LGBTQIA+ activist. But also I want to kill myself. I want to become manly, sexy, hairy, and big But on the other hand, I want to hurt myself. I want to study philosophy, literature, and languages, but I also hate them because they don’t include someone like me.
I want to write LGBTQIA+ stories in my native language to contribute to my community. But this is illegal here.
I want to do anything and everything, but I know all of this is a dream, and just a dream
All I can do is cry and wonder if I should kill myself

i just wanna talk about hyperhidrosis for a minute because it's something that has caused distress for me since i was a kid but i never see it talked about. for context, hyperhidrosis is when your body produces way more sweat than is needed. it can be a symptom of other things or can just be something that happens on its own and is pretty commonly hereditary.
both of my parents and i have hyperhidrosis. i'm an only child so this meant that my entire household experienced hyperhidrosis since i was young, which made me think that these symptoms were much more typical. i would be constantly sweating, i couldn't go outside in the summer without being supremely uncomfortable. i would often sweat through shorts and shirts. it was very easy for me to be dehydrated and on more than one occasion i would near passing out because of it. it made taking care of hygiene even more stressful, as being trans and autistic i already struggled with hygiene, and sweating all the time meant i had to be more rigorous and routine to make sure i smelled okay, but i often struggled, leading to more social anxiety and often bullying.
i'm on medication now for it, as when i started testosterone i started getting severe hot flashes often (which my mother did also when she went through menopause). i talked to my doctor about possible solutions and she recommended a hyperhidrosis medication for it (because going on estrogen isn't exactly ideal), which has decreased the hot flashes and my overall sweating. i don't think i would have started this medication if it weren't for my severe hot flashes and avoidance of estrogen medication due to being on testosterone. it's made my experience of testosterone a lot different because where most people start sweating more, i have been sweating less than i have ever been. my body temperature is much warmer but i'm not overheated and sweating all the time.
i don't hear about hyperhidrosis, like ever, and while i still hesitate to call it a disability, it does shape my disabled experience in the way it interacts with my ability to function, especially in conjunction with things like my chronic pain and autism. it shapes my trans experience too, and my trans experience has shaped my experience of this disorder. i wanted to share my experiences with it in case anyone else has experienced something similar because you don't hear about it all that often and i think it's important to know other people experience this too.

Hi! I just happened to see your post from 2023 about vaginal atrophy and it's such a eye-opener! I've been struggling with it for a while now (as an asexual afab with a male partner... Let's say that feeling uncomfortable and too tight during intercourse is my normal) and I suspect it must have to do something with my contraception ring, but all I got from my gyn was to use lube and that I'm only too tight (sometimes even putting ONE finger in to put the ring in irritates my entrance, but I need it to not have painful periods and diarrhea) because I only see my partner 1-2 monthly, so I'm not constantly used to stimulation, according to him. I've caught a candida infection recently (with no previous history of it) and I had had struggles with my vaginal ph nearing that of menopausal women before, but all I got from my doc were a cream (once! And he said that if it comes back I'll need extensive medical therapy) and every time I complain of any symptoms I get boric acidic vaginal insertions (idk the word, that cone thing which you have to insert then it melts and gets absorbed) which feel like inserting chili peppers and I'm struggling to keep taking that for even a week, so I never finish the whole pack. Your post made me realise that I might need to ditch my obgyn (and get a female one). I hope it's not my ring that is causing my athropy though since he never recommended oral contraception cause of my history of mental illness, which he says would be negatively effected by the greater hormonal swings of the pill. But at this point, I'm not sure if that's not him being misogynistic again lol. Anyways, your post kept me from gaslighting myself about my worries so thanks ❤️ I'm wishing all cis and trans vagina owners less struggling and better doctors! You deserve it.

Vaginal tightness can absolutely be impacted by how regularly you're using the muscles and by extension how often you're having sex, but to be so tight as to have difficulty inserting your finger as a constant is definitely a sign that something might be up!
Extreme tightness can be a symptom of vaginismus as well as vaginal atrophy - "vaginismus" like other vulvodynic conditions is kind of used as a catch-all term for tightness that doesn't have a specific diagnosable cause. Mine improved considerably when I started testosterone (which improved my arousal, my blood flow, and probably impacted my feelings of gender dysphoria) and then cleared up almost entirely when I started receiving counseling for my experiences of child sexual abuse.
The thing is though, while vaginismus is often assumed to be caused by psychological issues and concern, it is the basic responsibility of a medical care provider to eliminate potential physical causes before immediately sweeping to diagnoses of the psychological and psychosomatic.
This sort of involuntary tightening of the muscles can be something to look out for particularly when you're under stress or feeling anxious about penetration, sure, but what you're describing does sound like you're immediately getting irritation and discomfort rather than just physical muscle tightness, and even if it's not an ongoing atrophy, it certainly sounds potentially like a lubrication issue or an issue with the sensitive mucous membranes around your vagina.
Absolutely get another gynecological consult if you can, and yes, woman doctors are always a good shout over men, especially in these fields - they're not perfect, of course, but definitely bring up your concerns and ask them to have a look at your medical notes and see if anything specific rings a bell.
Remember when you do for a vaginal exam that if you're particularly anxious about penetrative exams themselves, you can often ask in advance for a paediatric speculum which is generally a lot smaller than the regular specula, and a lot of doctors are able to apply a topical anaesthetic to aid the internal exam. When I went for a pap smear when my vaginismus was quite bad I had the topical anaesthetic in combination with an oral muscle relaxant as well - your doc should be able to provide more info if this is a concern for you.
Good luck, Anon, and I'm so, so sorry your doctor has been so shitty, it's honestly so common for doctors to routinely dismiss vaginal pain and especially vaginal tightness and to immediately work on the "problem" of how open that vagina is to presumed men's penetration of it rather than the actual vagina owner's comfort, safety, health, and pleasure.

He does so for safety, because he sometimes goes to rural Putnam County: “very small, really Southern places,” doing HIV education. “If they knew I was queer, let alone trans, I would probably be killed, so I kind of have to keep all of that very much on the ‘down low’ when I’m doing work out in the community.” But to his co-workers he is “very, very out.” For Lucas, undergoing top surgery is an assertion of what some feminists call bodily autonomy. Pro-choice activists argue that the government has no right to tell women what to do with their body; transgender activists say that they have the right to change their body if they please.
Lucas is at the surgeon’s office with Oliver, a former boyfriend who is also a bearded trans man; and Rachel, a bisexual Latina, his “soul mate and sister.” Lucas says he has “always known” he wanted top surgery,” even before he began injecting testosterone.
