Kenzou Tenma
Morality
Tenma’s morality is something that is very stable, despite it being questioned multiple times throughout the series. I will get more into it and why it’s the way it is shortly, but, before so, I would like to place him on my scale. He would be in the paler region, somewhere between the grey area and the extremely pale area.
Although people debate otherwise, he’s an extremely moral person. With the reason why he is this way aside, it shows clearly in the anime and manga just how much he cares and is willing to give to others. There are myriads of examples for this, but the one I find to be the clearest on just how much he’s willing to do for someone is when he wanted to protect Eva from Roberto. At that point in time, she had ruined his life. She wasn’t going to help him in any way, and all that she had done for him, up to that point, was either horrible or slightly beneficial. She’s someone who didn’t care about him at all, who wanted to ruin his life to prove a rather petty point. In spite of all of that, he still cared about her deeply and wanted to save her. He knew why she was doing this, and he knew that she hated him, yet he wanted to help her because it would be “wrong” to not do so. Furthermore, helping people was all that he was used to. He knows how to help people and it’s the one thing that he could say that he’s most familiar with. It is completely outside of his nature to not help people. It’s something that’s embedded within him, that he cannot get rid of, and it could even be considered his fatal flaw.
His morality could be attributed to many things, but the most prominent would be his childhood. He knows pain. He knows what it’s like to feel lonely and isolated. He knows what harsh feelings are like, and, because of so, he is extraordinarily kind to everyone. His kindness was implanted within him due to the experiences that he had.  You never truly know what someone’s facing, and, because of that, he is exceedingly kind and always willing to help people. He is, simply, someone who genuinely cares about others.
Despite being deeply concerned about others’ feelings and wellbeing, I cannot fully call him altruistic. This is because it’s unclear why he helped Johan in the first place. Was it because he was the first patient, so, logically, he would be the first to be operated on? Was it to prove a point, that the hierarchy and system of the hospital are corrupt? Was it because he felt bad for Johan and genuinely wanted to help him? We shall never know.
Something that I would say just conveys the degree of rigidness and perfection that he requires from himself in terms of morality is how he wanted people to stop calling him a “doctor”. He didn’t think that he was worthy of this title, because he injured people and have wanted to kill them. He feels that it’s simply inacceptable for someone who’s supposed to help people to think like that, and, by that logic, he cannot be considered a doctor anymore. The reason why he thinks this way is almost definitely due to his upbringing and the way that he was taught about morality, which would make sense as this is a common way of teaching people about “right” and “wrong”. However, it is extremely rare when someone sticks to it so much. It’s incredibly easy to find a doctor who’s committed crimes and is a shady person with underground and illegal dealings to get more money or more fame. Furthermore, he knows that fixing his “mistake” by killing Johan is wrong, and, because of this acknowledgement of his, he feels that it’s wrong to be called a doctor. It’s wrong to be considered a “good” member of society, because he considers himself a horrible person for thinking the way he does.
Although this is the case, he is physically incapable of killing Johan. He cannot, at least not mentally. He’s physically prepared, and he knows how to use a weapon, but he cannot kill him. It’s something he’s incapable of and it’s almost definitely due to his morality
At the end of the day, Tenma is who would be considered a “good” person, regardless of his thoughts on the matter.
Personality and insecurities
If I were to describe Tenma with one adjective, it would almost definitely be kind. There are numerous examples of him being a generous and considerate person all throughout the series. Alongside that, I would say that he is extremely gentle. Because of him not knowing what type of situation people could be dealing with, he’s very careful with them, as to not hurt them accidentally. It’s a part, in my opinion, of the reason why people persist in calling him a doctor. It’s a behaviour mostly expressed by doctors and people who work in the medical field. They’re always extremely kind-hearted, and Tenma is no exception.
Although there are smarter characters in the series, Tenma’s still incredibly smart. It’s because he’s very knowledgeable about many things, as he’s a doctor, and because he’s emotionally intelligent. He uses this rather unique skill of his to understand others. He doesn’t use logic, even though he follows its path often, because he finds feelings and emotional matters to make more sense compared to mental and logical ones. It’s a bit bizarre for someone in his work field, but it’s also what makes him so unique and loveable. 
To be concise, Tenma is who you’d imagine when thinking of the word “doctor”. He stands for exactly what his career would expect of him.
I think that it’s rather evident that Tenma is a rather insecure person. The reason for this varies for the insecurity, but it’s something that would be considered rather logical about him.
He’s afraid of disappointing people. This is because he has always had people expecting things of him, his parents and Eva are examples of such, and, due to this, he expects that everyone would always be expecting things of him. However, with people he doesn’t know well or who aren’t involved with him in terms of career, he would never be able to please them because he has no clue what they expect of him. Due to this, he would think of himself as a failure, because he isn’t what people expected him to be, and because he relies, although this changes throughout the series, on people’s opinions of him to determine his own worth. Even though he’s used to feeling like he’s a disappointment, he doesn’t want to feel the familiar pain once more. Another fear of his is losing those he loves. This is because he doesn’t know how to not live without them now that he has people like them. He’s never had people who truly cared about him in the same way that he did for them, so he doesn’t know what losing them would feel like. He imagines it would hurt, but he doesn’t want to risk it. He’s been able to achieve something he couldn’t for most of his life; there’s no way he’s going to give it away for anything now. 
He’s also scared of getting close to people. Unlike most people, he doesn’t view vulnerability and emotions as weakness. He knows why people would consider them as such, but he, personally, doesn’t view them in that light. What truly frightens him is that he might be taken advantage of. He knows that, due to his caring nature, he will be put in a position where it’s extremely easy to use this in practically anything. He’s also very aware of the fact that Eva had done something similar, where she took advantage of him loving her, where she benefited from being someone of his position and salary’s partner.
Something that’s not quite an insecurity, but is something that Tenma is rather guilty of is hurting people, especially Roberto. He feels extreme remorse for it, even though he’s fully aware that he wasn’t a “good” person in the slightest, and it haunts him. The reason for this could be because he believed that he killed Roberto, but it could also very well be due to his rigid morality. Feeling guilt in general is expected after committing something similar, but it’s never really to the same degree of guilt, to the point that it haunts you, that he experienced.
Conclusion
I just hope I did Tenma justice in this analysis. Tenma’s a character I’m rather fond of. I love him beyond description. He’s just so sweet, kind, and I cannot help but admire him for his character writing, which is as incredible as ever; I’m always amazed by Naoki Urasawa-san’s character writing skills, because all of the Monster characters are so humane and realistic, which is such a difficult thing to write for characters most of the time. Tenma’s also a character whom I find comfort in, for reasons I think are quite obvious by now, and I just really hope that I was able to explain his personality, morality and mannerisms well within this analysis.
