Meet Johanna, ❤️🤍💙SSVF CASE MANAGER Veterans & Community Housing Coalition. She helps our veterans identify their goals 🏃‍♂️related to medical 🏥 , substance abuse, financial 💵, mental health, housing 🏡legal, vocational, transportation 🚗 , and family 👨‍👦‍👦👩‍👦‍👦👩‍👩‍👦👩‍👩‍👧‍👦👨‍👨‍👦needs while developing strategies to achieve their goals.🏆 Johanna assists veterans with self-sufficiency and financial independence through the following comprehensive services, individualized assessment, development plan and case management, supplemental life skills instruction and counseling, job training, employment, and transit, benefits eligibility determination and assistance, and affordable housing placement. She also makes sure our #veterans have food on their table. She loads her 🚗 with food 🍱 🍎 🍲 🍖 🐣 from our #veteranfoodbank and delivers to our veterans. That’s #dedication 😇 Johanna is our #BootsOnTheGround 😍 We have a 98% success rate with our veterans… This is one of the reasons why. #casemanager #ssvfprogram #veteranfamilies #rapidrehousing Through #donations… And Community’s support JOHANNA able to provide these services and more to our #veterans.
