What Do Vaccine Efficacy Numbers Actually Mean?

During the first week of March 2021, Johnson & Johnson began delivering millions of doses of its coronavirus vaccine across the United States after receiving an emergency use authorization from the Food and Drug Administration. Central to getting the green light was a trial that Johnson & Johnson ran to measure the vaccine’s efficacy.

Efficacy is a crucial concept in vaccine trials, but it’s also a tricky one. If a vaccine has an efficacy of, say, 95 percent, that doesn’t mean that 5 percent of people who receive that vaccine will get Covid-19. And just because one vaccine ends up with a higher efficacy estimate than another in trials doesn’t necessarily mean it’s superior. Here’s why.

For statisticians, efficacy is a measurement of how much a vaccine lowers the risk of an outcome. For example, Johnson & Johnson observed how many people who received a vaccine nevertheless got Covid-19. Then they compared that to how many people contracted Covid-19 after receiving a placebo.

The difference in risk can be calculated as a percentage. Zero percent means that vaccinated people are at as much risk as people who got the placebo. A hundred percent means that the risk was entirely eliminated by the vaccine. In the United States trial site, Johnson & Johnson determined that the efficacy is 72 percent.

Efficacy depends on the details of a trial, such as where it took place. Johnson & Johnson ran trials at three sites: in the United States, Latin America and South Africa. The overall efficacy was lower than that in the United States alone. One reason for that appears to be that the South Africa trial took place after a new variant had swept across that country. Called B.1.351, the variant has mutations that enable it to evade some of the antibodies produced by vaccination. The variant didn’t make the vaccine useless, however. Far from it: In South Africa, Johnson & Johnson’s efficacy was 64 percent.

Efficacy can also change when scientists look at different outcomes. Johnson & Johnson’s vaccine had an 85 percent efficacy rate against severe cases of Covid-19, for example. That’s important to know, because it means that the vaccine will prevent a lot of hospitalizations and deaths.

When scientists say that a vaccine has an efficacy of, say, 72 percent, that’s what’s known as a point estimate. It’s not a precise prediction for the general public, because trials can only look at a limited number of people — in the case of Johnson & Johnson’s trial, about 45,000 volunteers.

The uncertainty around a point estimate can be small or large. Scientists represent this uncertainty by calculating a range of possibilities, which they call a confidence interval. One way of thinking of a confidence interval is that we can be 95 percent confident that the efficacy falls somewhere inside it. If scientists came up with confidence intervals for 100 different samples using this method, the efficacy would fall inside the confidence intervals in 95 of them.

Confidence intervals are tight for trials in which a lot of people get sick and there’s a sharp difference between the outcomes in the vaccinated and placebo groups. If few people get sick and the differences are minor, then the confidence intervals can explode.

Last year, the F.D.A. set a goal for coronavirus vaccine trials. Each manufacturer would need to demonstrate that a vaccine had an efficacy of at least 50 percent. The confidence interval would have to reach down no lower than 30 percent. A vaccine that met that standard would offer the kind of protection found in flu vaccines — and would therefore save many lives.

So far, three vaccines — made by Pfizer and BioNTech, Moderna and Johnson & Johnson — have all been authorized in the United States after their trials demonstrated they surpassed the F.D.A.’s threshold. AstraZeneca and Novavax, which have ongoing U.S. trials, have published efficacy results from studies in other countries. Meanwhile, the makers of the Sputnik V vaccine have published results based on their trial in Russia.

For a number of reasons, it’s not possible to make a precise comparison between these vaccines. One vaccine may have a higher point estimate than another, but their confidence intervals may overlap. That effectively makes their results indistinguishable.

Making matters more complicated, the vaccines were tested on different groups of people at different stages in the pandemic. In addition, their efficacy was measured in different ways. Johnson & Johnson’s efficacy was measured 28 days after a single dose, for example, while Moderna’s was measured 14 days after a second dose.

What’s clear is that all three vaccines authorized in the United States — made by Johnson & Johnson, Moderna, and Pfizer and BioNTech — greatly reduce the risk of getting Covid-19.

What’s more, all the vaccines look as if they have a high efficacy against more serious outcomes like hospitalization and death. For example, no one who got Johnson & Johnson’s vaccine had to go to the hospital for a Covid-19 infection 28 days or more after getting an injection. Sixteen people who got the placebo did. That translates to 100 percent efficacy, with a confidence interval of 74.3 percent to 100 percent.

A clinical trial is just the start of the research on any vaccine. Once it goes into widespread use, researchers follow its performance. Instead of efficacy, these scientists now measure effectiveness: how much the vaccine reduces the risk of a disease out in the real world, in millions of people rather than thousands. Early studies on the effectiveness of coronavirus vaccines are confirming that they provide strong protection.

In the months to come, researchers will keep an eye on this data to see if they become less effective — either because the immunity from the vaccine wanes or because a new variant arises. In either case, new vaccines will be created, and manufacturers will provide new measures of their efficacy.

Lines engraved between 125,000 and 105,000 years ago on two animal bones found in northern China held some sort of meaning for their makers, researchers say.

These ancient markings provide the oldest evidence of symbolic activity by humans or our close evolutionary relatives in East Asia, says a team led by archaeologists Zhanyang Li and Luc Doyon, both of Shandong University in Jinan, China. A mysterious Stone Age population called Denisovans, which had close genetic ties to Neandertals, may have carved sets of parallel lines into the pair of bone fragments, the scientists suggest in the August Antiquity.

Denisovans inhabited East Asia at the same time that someone carved lines into bones at northern China’s Lingjing site (SN: 3/2/19, p. 11). But either Homo sapiens or Neandertals, who also left behind Stone Age creations with apparent symbolic meanings (SN: 3/17/18, p. 6), might instead have modified the Lingjing bones.

“Nonetheless, the two objects from Lingjing suggest that symbolic capacities were within the realm of cognitive abilities of [Homo] species that lived before and during the evolution of Homo sapiens in Africa,” Doyon says.

Abstract markings on the Lingjing bones resemble engraved lines on roughly 100,000-year-old pigment chunks from South Africa (SN Online: 6/12/09), says archaeologist Paul Pettitt of Durham University in England. “As Homo sapiens was responsible for that early symbolism in Africa, and Neandertals were responsible for such in Europe, it is a fascinating possibility that these [Chinese] examples were created by another Homo species,” Pettitt says.

Until now, a roughly 40,000-year-old stone containing a set of parallel and intersecting engraved lines, previously found at another northern Chinese site, represented the oldest evidence of symbolic behavior in East Asia. An engraved geometric design on a roughly half-million-year-old seashell found on an Indonesian island stands as the oldest example of symbolic behavior anywhere in the world (SN: 12/27/14, p. 6). Researchers suspect the now-extinct hominid species Homo erectus carved that pattern. The oldest known drawing, another type of symbolic behavior, dates to around 73,000 years ago in South Africa (SN: 10/13/18, p. 6).

Almost one-quarter of 227 animal bone fragments excavated at Lingjing between 2005 and 2015 display stone tool incisions typical of butchery, the researchers say. But that sample included two exceptions. Seven nearly parallel lines had been cut into a partial rib from an unidentified large, adult mammal. Microscopic analysis indicated that the lines were made with a sharp point that was run across the bone’s surface after it had suffered some damage from weathering. Special care was taken to create each of the first five lines with a single pass of the engraving tool. Red residue in four engraved lines indicated that a pigment had been smeared on the pattern, possibly to increase its visibility.

A second rib fragment from a large mammal contained 10 roughly parallel lines that had been sliced with a sharp stone point, probably in a single session, the researchers say. Engraving of these lines also occurred after the bone had been damaged by long exposure to the air. No pigment residue appeared on this specimen.

Estimated ages of the engraved bones relied on calculations of the time since sediment in which they were found was last exposed to sunlight.

Whoever cut lines into the Lingjing finds also fashioned animal bones into tools. Bone and antler artifacts found in the same sediment as the engraved ribs were likely used to retouch and sharpen used stone tools, Doyon and his colleagues reported in 2018.

I thought I would try create a different prototype today, but I struggled to think of ideas. So, a few of us took a trip to Look Sharp. I ended up buying five glass jars and some string with the intention of wrapping each jar in string depending on which drink driving death toll rate it represents. This time I focused on the top five countries. These were:

· South Africa – 58%

· Canada – 34%

· United States of America – 31%

· Australia – 30%

· France – 29%

I went about wrapping the bottles in string by measuring the height of them first (9.5cm) then calculating the specific percentage of that measurement (58% of 9.5cm = 5.51cm). I then wrapped the bottle with string up to the specified height.

I liked this idea but I preferred my initial one over it because I feel like this was quite plain and simple and after talking to Charles, I realised that these jars resembled a bar graph, just a 3D version of one. So instead, for a final product, I thought I would go with my initial idea of filling each bottle with a certain amount of water (depending on the rate) then wrapping the bottles fully in string so the water is no longer visible. This way, the water itself won’t represent the data, but the sound it makes when you blow into it will (higher pitch = higher rate).

Although I didn’t really like this idea, I think it was good to experiment because it helped me to expand my initial idea further and develop it in a way that involves using your senses rather than just looking at it and being able to read it as if it’s just a bar graph.

We have observed the antecedence of Peter Obi, right from the period he was Governor of Anambra State, as far back as 2013. He is an exemplary leader, who can be compared with Nelson Mandela of South Africa and Julius Nyerere of Tanzania, in simplicity, honesty and integrity.

He chose to live simple lifestyle, devoid of long convoy of vehicles; excessive travelling with multiple allowances claims, inflated contracts, etc. There was a time he rejected free lands allocated to him. As well, he turned down inflated budgetary allocation to Government House. He was impeached by State Assembly for his integrity; as he refused to mismanage or share state fund. He was later reinstated by law Court.

He lectured political office holders on prudent spending and accountability. He took the decision to leave People’s Democratic Party (PDP), as he refused to join Ghana-Must-Go money sharing bazaar, which overwhelmed delegates at the party’s Presidential Primary Election.

It appears he is the only Governor, from Southern Nigeria, who didn’t have issues with EFCC, or any other Anti-Corruption Agencies. They have not found any sharp corrupt practices against him, to warrant his arrest and prosecution. The fact remains he is easily the best, among all those contesting to be President of Nigeria come 2023. But, will he be allowed to occupy the seat, even if he eventually wins the election, being an Igbo man? There is a long standing conspiracy in the country, against Ibos to rule Nigeria. It started with the brutal assassination of Aguiyi Ironsi in 1966, after being barely 6 months in power. At the end of Nigerian civil war in 1970, Northern leaders in collaboration with Yorubas, held meeting and resolved that nobody from Ibo tribe will ever rule Nigeria. That was about fifty-two years ago. And it has remained so till date.

Our people have been excluded from the scheme of things, to the extend we are merely part of Nigeria by territory. There has been systematic repression and killings of Igbos, even in this current government. The few Igbos who made spirited efforts to contest and clinch power at the centre, have been blackmailed, accused of one embezzlement or the other, arrested, tried, humiliated and frustrated. They have remained at the corridor of power for more than half a century; picking crumbs which fall from their slave masters’ tables.

From the look of things, it appears Obi may break through the barrage of obstacles, and become the next President. He and his Labour Party, are having overwhelming support, across the length and breathe of the country. Virtually, people of all tribes are fed up with this tribal and retrogressive government. Killings everywhere! Terrorists and Bandits have even penetrated Abuja, the Federal Capital Territory. Just recently, they broke into Kuje Prisons in Abuja, to release their colleagues held there. Nobody is safe anywhere in the country.

Now, enemies of progress are laying all forms of stumbling blocks against Peter Obi. His magnificent superstore at Abuja was burnt down by strange fire outbreak. His goods were looted in the inferno. He is being warned by sponsored armed gangs, not to campaign anywhere in the North. These are calculated attempts, to weaken and intimate him to quit the presidential race. Voter registrations are going on in Northern neighbouring countries like Niger Republic and Chad. They are predominantly Muslims. It could be recalled during the last Presidential elections, those nationals flooded the country to vote. Large number of them still remain in Nigeria, engaging in banditry and other criminal activities. Can they quench this lone voice from the East? Time shall tell.

Igbo man being President of Nigeria, will go a long way in addressing some of the injustice meted out to us. However, it can never be a lasting solution, to what we are passing through in the hands of our taskmasters; who will eventually come back to take power in near future. The way out of our problems with Nigeria, is to eventually leave the country. If not, we will still be back to square one.

Take a cue from the Yorubas; and there present situation, after their sons have ruled Nigeria for long. Obasanjo governed the country for a total of almost 12 years: both as Military Head of State, and as Civilian President. Apart from him, Ernest Shonekun was Interim President for three months, before he was kicked out of power. Osinbajo will soon complete his tenure of eight years as Vice President. He was Acting President, most of time the incumbent President was abroad for medical treatment.

When Obasanjo was in power, Yoruba land was peaceful: devoid of terrorist activities. Things were moving smoothly for them that period. He has since left the stage; and the Northerners have resumed the stranglehold of the country. Are the Yorubas smiling now, despite the fact that their son Obasanjo, presided over Nigeria longtime ago? The answer is no. They are at the mercy of Jihadist terrorists who have taken over their forests. The so-called bandits from the North, are killing people in every part of the country including South West. Those at the helm affairs pretend to be ignorant of what is happening. There’s a grand plan to allow those cattle rearrers living in the forests (disguised Jihadists), to drive away the inhabitants from their ancestral homes and take possession.

Biafra project is unstoppable. We are like Israelites who were living under bondage in Egypt. Later, God rescued them out of the land to a place He prepared for them. We need a country to call our own, to enable us attain our full potentials, unhindered. Biafra is the future of our unborn generation. The earlier we leave the better for us. Had it been Israel remained in Egypt till today, they wouldn’t have been able to reach such enviable heights in development, freedom of worship and expression.

If somebody from South East becomes President of Nigeria, it is good. We all pray for that. It will be a temporary relief, from the terrible condition we have found ourselves in. It can never be a permanent solution. We wish Peter Obi success in his endeavours. And let the will of God be done in his life. However, Nnamdi Kanu has an edge as Biafra leader. He and his colleagues are highly committed, to the emancipation of our people from that forced marriage called Nigeria. Biodiversity in Africa and Latin America at risk from oil palm expansion, new report warns

Zero deforestation commitments may inadvertently leave vital habitats in Latin America and Africa vulnerable to agricultural expansion, a new study has found.

The study highlights how sustainability commitments, which play an important role in preventing the destruction of tropical rainforest, fail to protect nature in tropical grassy and dry forest habitats such as the Llanos in Colombia, Beni savanna in northern Bolivia, and Guinean and Congolian savannas in West and Central Africa.

The research team, led by the University of York, calculated that if oil palm producers cleared these habitats to make way for new plantations, a third of vertebrates on the International Union for Conservation of Nature’s red list of threatened species could be affected, including the blue-throated macaw in Bolivia, the giant pangolin in Congo, and the Hellmich’s Rocket Frog in Colombia.

For the study, researchers mapped the areas around the globe that are at risk from new oil palm plantations. They identified 167 million hectares that are potentially suitable for the crop while still meeting the Roundtable on Sustainable Palm Oil’s (RSPO) definition of ‘zero deforestation’. Of those 167 million hectares, 95 million are in grasslands and dry forests, mostly in South America and Africa.

As global demand for agricultural land increases researchers are calling for urgent protections for these habitats, which support a rich array of species and act as an important carbon store.

Co-author of the study, Professor Jane Hill from the Department of Biology and the Leverhulme Centre for Anthropocene Biodiversity at the University of York, said: “Palm oil is at the sharp edge of debate on how we can balance the need to feed the world and sustain livelihoods, while protecting nature.

“With a yield estimated to be six times higher than many other vegetable oils such as oil seed rape, palm oil is regarded as a miracle crop and it supports the livelihoods of millions of people in tropical countries around the world. So rather than avoiding or banning palm oil, we need to ensure effective international policies and governance to protect, not just tropical rainforest, but tropical grasslands and dry forests too.

“Our study highlights how current sustainability commitments could have the unintended consequence of putting areas of remarkable biodiversity at risk from the expansion of oil palm agriculture.”

Since 2018, many oil palm companies have signed up to the RSPO’s zero deforestation commitments, which means they cannot expand plantations into tropical rainforest or peatlands.

While concern from buyers and consumers about the environmental impact of palm oil has helped to drive membership of the scheme, many oil palm producers are yet to sign up to these commitments.

First author, Dr Susannah Fleiss, who carried out the study while researching her PhD at the University of York, said: “Although we found that oil palm yield in areas currently covered by grassland and dry forest would be lower than in tropical rainforest, these sites would still be attractive for the expansion of oil palm agriculture. We also found that irrigation would improve yield in many of these locations, potentially making them more attractive for expansion.

“Clearing these areas for plantations would have a serious impact on biodiversity, potentially reducing the ranges of one quarter of vertebrate species that are currently threatened with extinction. Plantation development would replace the existing habitat in these areas, disrupting the ability of the species present to find food and water, and affecting their migration routes.

“Large numbers of people live in tropical grassy and dry forest regions, where they often play a critical role in ecological processes such as burning and grazing. The expansion of oil palm agriculture in these areas could lead to a number of interlinked issues for local people and biodiversity.

“Our study highlights the strong need for internationally-coordinated governance to protect these habitats, in addition to the existing global efforts to protect tropical rainforest.”

Co-author Dr Phil Platts, Honorary Fellow at the University of York and Director of Earth Observation at BeZero Carbon, said: “Sustainability guidelines for palm oil were developed in the context of Southeast Asia’s rainforests, and so reflect the structure and function of those habitats. Now expansion is shifting to different ecological contexts, the scope of sustainability commitments must similarly expand, in line with the distinct biodiversity and carbon stocks now under threat.”

The research, published in the journal Nature Ecology and Evolution, is funded by Unilever, in collaboration with the University of Liverpool, Oxford, the Potsdam Institute for Climate Impact Research, Unilever and BeZero Carbon.
