BARIATRIC SURGERY GAVE ME NEW LIFE - Mr. BABU RAO, SPICE GARDEN RESTAURANT, ANANTAPUR - Dr. V. AMAR
Mr. Babu Rao, Owner of Spice Garden Restaurant, Anantapur underwent Sleeve with Loop Duodenal Switch (SLDS, Single Anastomosis Duodenoileal Bypass with Sleeve, SADI S) Bariatric Metabolic Surgery by Dr. V. Amar in November 2021. He lost 62 kg weight (from 130 kg to 68 kg) in 1 year.
Bariatric surgery gave him a new life. Bariatric & Metabolic surgeries are very safe and highly effective to induce long lasting significant weight loss and long term remission from Type 2 diabetes.
SLDS Surgery is the most powerful combination of Sleeve and Bypass surgeries. It is more powerful than Sleeve Gastrectomy (SG), Roux En Y Gastric Bypass (RYGB), Mini Gastric Bypass (MGB) - One Anastomosis Gastric Bypass (OAGB) or any other Sleeve and Bypass combination surgeries such as Sleeve with Proximal Jejunal Bypass (SG PJB), Sleeve with Duodenoileal Interposition (SG DII), Single Anastomosis Sleeve Ileal Bypass (SASI Bypass) and Single Anastomosis Sleeve Jejunal Bypass (SASJ Bypass).
99% people respond very well after SLDS surgery. Weight regain and diabetes recurrence chances are less than 5%. If anyone regains weight after any of the above mentioned surgeries, converting them to Loop Duodenal Switch re-induce weight loss and diabetes remission. It addresses several problems associated with RYGB and MGB - OAGB. Unlike in RYGB, MGB-OAGB, after SLDS Surgery, endoscopic access to the stomach is maintained, there is no at risk gastric remnant, dumping syndrome chances are less due to preservation of pylorus, calcium and iron deficiency risks are less due to preservation of first part of the duodenum. Unlike in RYGB, after SLDS surgery, there is no risk of marginal ulcers, no risk of internal hernias. SLDS surgery is technically more advanced surgery compared to other surgeries.

Bariatric surgery induces significant and long lasting weight loss by means of altering hormones that control total body fat. But weight loss is not straight in a smooth downward slope. Weight fluctuations are common during weight loss journey. - You lose weight. - You stop for a while. - You gain sometimes. - Weight loss can stall upto 4 to 8 weeks sometimes. - Then you lose weight again. Weight fluctuations after bariatric surgeries are generally because of - Change in diet. - Taking diet high in salt. - Weight training that leads to muscle tears (Body retains water to repair them). - High carbohydrate diet. - Eating outside foods high in sugar, fat and salt. - Lack of sleep. - Increased stress. - Irregular food timings. - Lack of physical activity. - Certain medications. - In females, before menstruation. After bariatric surgeries - Don't check your weight daily. - Check only once in a month on the day of your surgery and maintain a graph. - Measure your waist, hip, arm and thigh circumferences monthly. - Observe clothes becoming loose. When you lose weight naturally with diet and exercise, your Basal Metabolic Rate (BMR) reduces and body tries to conserve fat. In contrast, after bariatric surgery, because of the hormonal changes your basal metabolic rate and heat production increases, and fat melting increases. Still it is mandatory to follow healthy diet and do muscle strengthening exercises to increase weight loss and to prevent weight regain. ARE YOU SUFFERING FROM OBESITY AND SEVERE DIABETES? Low back pain is one of the common problems in individuals with severe and morbid obesity. It is not life threatening, but is a common cause of work related disability and absence.
Excess weight adds additional burden on the weight bearing joints such as lumbar spine and knees. This results in or worsens low back pain and knee joint pains. Fat accumulation in the tummy leads to stretching and weakening of abdominal muscles. To maintain balance individuals adapt to a backward curved posture (Exaggerated lumbar lordosis).
Bariatric surgery is the most effective intervention to induce long lasting weight loss. Studies have shown that weight loss after bariatric surgeries significantly decrease severity of low back pain. And in some patients there will be resolution of severity and disability associated with low back pain. There will be significant increase in work productivity after bariatric surgery because of improvement in low back pain.
Pain scores significantly improve, there will be a significant symptomatic relief of pain of the head, neck, shoulder, low back, and knee. There will also be a decrease in pain on physical movement during sports and work- related activities, reduced usage of pain killers, and an increase in quality of life.

Diabetes increases risk of black fungus infection after Covid-19:Individuals suffering from diabetes are at high risk of developing severe Covid-19 infection and it's related complications including mucormycosis (black fungus).
Mucormycosis is an opportunistic fungal infection. It affects individuals with decreased immunity. High blood sugar levels in diabetic patients cause immune dysfunction. Diabetes medications can suppress immune response. In addition medicines used in the treatment of severe Covid-19 such as steroids and IL-6 inhibitors (tocilizumab), suppress immunity. Presence of uncontrolled diabetes and indiscriminate use of steroids to treat Covid-19 lead to mucormycosis.
Mucormycosis affects eyes, nose, nasal cavities and brain. Symptoms include facial swelling, loose teeth, headache, stuffiness in the nose, black nasal discharge, blurring of vision. Severe infection can lead to blindness, seizures, blood vomitings and altered sensorium.Maintain hygiene, regularly use povidone-iodine mouth washes, use masks, maintain social distance, avoid crowded places, garbage dumping sites and construction sites.
Keep your blood sugars under control. Avoid indiscriminate use of steroids to treat mild to moderate Covid-19. Use sterile water for humidification while administering oxygen. Early diagnosis, early surgical debridement and liposomal amphotericin B usage are necessary to control black fungal infection.

Bariatric surgery results in long lasting weight loss in individuals suffering from severe obesity by altering the hormones controlling the body fat. It is generally indicates for individuals between 18 years to 65 years age.
When body mass index (BMI) is 30 and above, 96% of individuals fail to achieve long lasting weight loss by lifestyle modifications alone, because of the effect of hormones controlling the body fat. This is true even in case of teenagers less than 18 years age and children suffering from severe obesity.
Since these are in growing age, they need to try lifestyle modifications first. If these methods fail, then bariatric surgeries can be offered. Parents and pediatricians don't consider bariatric surgery as an option because of the fear of long-term consequences, psychological impact on teenagers and children.
But we need to weigh the risks of carrying excess weight vs. benefits and risks of undergoing bariatric surgery. Bariatric surgeries are very safe and they can be considered even in teenagers less than 18 years age and children suffering from severe obesity on individual basis, if they fail to achieve long lasting weight loss with lifestyle modifications alone. Benefits of bariatric surgery significantly outweigh the risks of not undergoing surgery.

Obesity and diabetes increase risk of Covid-19 related complications. Bariatric surgeries for obesity and metabolic surgeries for type 2 diabetes reduce those risks. After surgeries even if anyone gets Covid-19 infection, it is either asymptomatic or usually mild and recovery chances are significantly high. Several people are calling me and asking if they can undergo bariatric and metabolic surgeries if they got Covid-19 infection and recovered.
You can safely undergo bariatric and metabolic surgeries after recovering from Covid-19 infection but the waiting time depends on the severity of Corona infection, requirement for hospital or ICU admission.
Waiting time from the date of Covid-19 diagnosis to bariatric surgery - 4 weeks - For asymptomatic patients. 4 weeks - For mildly symptomatic patients with non respiratory symptoms.   6 weeks - For symptomatic patients who did not require hospitalization. 8 to 10 weeks - For symptomatic diabetic, immunocompromised, or hospitalized patients. 12 weeks - For patients admitted in ICU.

During the lockdown, kids are forced to spend more time at home away from school and friends. They tend to spend more time in watching television or playing with smart phones and laptops. Increased screen time tends to delay the development of speech and language in kids. However, video calling is not considered as a part of the stipulated screen time.
Childhood obesity is a serious consequence of increased screen time, as binge watching also leads to binge eating. And there is no exercise while watching television or playing with gadgets. Same lifestyle leads to development of diabetes early in the age. Increased screen time also leads to headache, eye irritation and dryness.
However, it is not possible to stop screen time altogether. So parents can restrict screen time by fixing timings for watching television and for playing with gadgets. Set alarms for screen time. Encourage kids to read books. Parents can engage kids with other activities like drawing and painting. They can make them do exercises for a minimum of 1/2 hour to 1 hour at home.
Kids tend to irritate parents to get what they want. Parents should not lose temper. If parents yield, then irritating parents becomes a habit for kids. Be firm on the set rules and make kids understand that you don't yield.

Severity of COVID-19 infection is significantly less in individuals doing exercises consistently. Those indulging in regular physical activity require significantly fewer hospital and ICU admissions if infected with COVID-19. Even mortality is significantly less in these individuals.
Exercise improves immunity. Immune response to infections such as common cold, influenza and corona is adequate if you do exercises regularly.
Even in those individuals suffering from obesity, but doing exercises regularly, disease severity is less. In these individuals regular exercise is important to lose weight and to maintain weight loss.
Even make children exercise daily. Involve them in any activity, games or sports involving physical activity.
In addition to routine COVID-19 precautions, start doing exercises daily if you don't have a habit of exercising. If already physically active, then continue doing them.
Involve in any physical activity such as muscle strengthening exercises using machines, dumb bells or bar bells with weights; walking; running; cycling or swimming and improve immunity against infections such as corona. Regular exercise reduces corona severity.

Second wave of Covid-19 turned out to be more infectious and deadlier. New cases keep on increasing exponentially everyday. Situation is going to worsen further. New cases are nearing 5,00,000/day and shooting towards touching 10,00,000/per day.
And there is no sight of curve flattening in near future. Approximately 10% of patients requiring treatment need ICU beds. Through out the country, doctors and hospitals are facing the brunt. Hospital and ICU beds are full every where. Every day all doctors are getting several enquiries about availability of hospital and ICU beds. Sudden spike of cases increased the burden on healthcare system. There is severe shortage of hospital and ICU beds, ventilators and oxygen.
So don't drag until you will be the one in need of an ICU bed with ventilator and oxygen requirement. Get vaccinated now. Don't neglect Covid precautions - wear a mask, follow social distancing, avoid social gatherings, postpone all non essential works, postpone travel and impose self lockdown whenever possible. Life is priority - All other things come next.

Bariatric Surgery reduces severity of corona virus in individuals suffering from obesity.
Previous history of bariatric surgery significantly reduces risk of hospital admission, intensive care treatment and ventilator requirement in covid positive patients suffering from obesity.
Obesity significantly increases covid related mortality and morbidity because of defective immune response to infections. Obesity with BMI > 30 kg/m2 is an independent risk factor for COVID-19. Additional presence of diabetes further aggravates the risk.
Bariatric surgery results in long term and significant weight loss and diabetes remission. It also normalises functions of severy body systems including immune system. Those who underwent bariatric surgery if affected with corona virus, will have asymptomatic or mild infection and the hospital admissions are low. So bariatric surgery offers protection against severe COVID - 19.

WHAT A PERSON 'SUFFERING FROM OBESITY OR DIABETES' SHOULD DO TO DEAL WITH COVID-19.
Obesity and associated co-morbid conditions such as type 2 diabetes enormously increase risk of Covid-19 complications and mortality. Situation outside is very scary. There is shortage of ICU beds, Oxygen, Ventilators, Remdesivir, Tocilizumab and Vaccines.
So TAKE PRECAUTIONS TO PREVENT CORONA - Impose self lockdown if possible - Wear mask in public places (To cover nose and mouth, Not to cover chin) - Don't touch nose, mouth, face without washing hands - Wash hands frequently - Do steam inhalation by adding karvol plus and turmeric powder three times daily - Do salt warm water gurglings or betadine mouth washes three times daily.
IN ADDITION TO THE ABOVE REVIEW YOUR DIET AND EXERCISE - Start doing muscle strengthening exercises. Exercise boosts your immunity - Stop junk foods, refined foods - Change to healthy diet rich in protein and fibre - The more weight you lose, the more protection you get - Get vaccinated - If you are in severe or morbid obesity category, consider bariatric and metabolic surgeries.
IF YOU GET INFECTED WITH CORONA - Don't panic - Don't neglect any symptoms - Fever, Cough, Breathlessness, Weakness, Cold, Stomach cramps, Loose motions - Consider any symptom as corona symptom until otherwise proven - Remember negative RTPCR test doesn't rule out Corona - If symptomatic, take medical help immediately.

Obesity leads to several medical co-morbid diseases. Hyperlipidemia is one of them. Elevated levels of blood fats such as Total Cholesterol, LDL Cholesterol, VLDL Cholesterol and/or Triglycerides is considered as Hyperlipidemia.
High blood fats can lead to heart attack, brain stroke, and pancreatitis. It is necessary to keep the blood fat levels under control to prevent complications. While healthy diet and muscle strengthening exercises are necessary to control weight and blood fat levels, some individuals need addition of medications.
Individuals with severe or morbid obesity with hyperlipidemia benefit from bariatric/metabolic surgeries. High end metabolic surgeries result in remission of hyperlipidemia in > 90% of the individuals in addition to achieving long lasting weight loss.
If you are having excess weight with high blood fat levels, take an expert opinion.

Can you take vaccine after Bariatric Surgery?
Obesity and diabetes significantly increase the risk of complications and death associated with Covid 19.
On the otherhand bariatric and metabolic surgeries significantly reduce that risk. Even if anyone gets infected with Covid 19 after bariatric surgery, recovery chances are significantly high and life risk is minimal.
Individuals who underwent bariatric and metabolic surgeries CAN SAFELY TAKE COVID VACCINE. These surgeries are not a contraindication for Covid 19 vaccination. However wait for 4 weeks to take Covid 19 vaccine after bariatric and metabolic surgeries.
