If you're new to me yelling about this: my neighbors feed a feral cat colony but provide no other care to the colony like neuter/spay or vaccination. They free feed the cats, which attracts skunks, and they have (at the moment) about 20 cats in the colony, which attracts coyotes.
My dog has been skunked twice, she can't be outside in our yard unsupervised for even a minute because otherwise she'll try to eat cat shit because there's cat shit everywhere in my yard because the neighbors also haven't provided litterboxes or a sandbox for their ferals (their entire yard is paved) and if she has to go outside at night I need to clear the yard for coyotes before I let her out even in the small fenced area by our bedroom.
Also there are fleas fucking *everywhere* and even though my dog is on medication to prevent fleas I need to de-flea her twice a week in the summer; this is the case for every dog owner on the street.
The street I live on has a speed limit of 25mph but I hear cats getting hit frequently, so frequently that we have a resident group of crows who hang out and wait for them to become roadkill.
I have a collection of photos on my phone that show kittens with broken legs, kittens with missing eyes, kittens with horrible ear mite infections, and I have a dedicated shovel that I use for moving cat corpses that show up on my property. The cats that die on my property die from abscessed wounds from fights with other cats, respiratory infections, renal failure, and injuries from being hit by cars.
My spouse is immune compromised, and while toxoplasmosis is not a serious risk for most people, it IS a serious risk for people who are immune compromised (as are all the other infections that cats can potentially carry), which means that it's unsafe for me to grow vegetables in my yard for us to eat and it's unsafe for him to work in the yard.
I'd love to maybe open my windows at night and keep down the electricity costs of using the AC, but I can't because the entire side of my house that faces my neighbor's yard reeks of cat shit and piss year round.
Those are my next door neighbors.
Last week I was walking my dog as a neighbor around the corner was pulling out of her garage; she paused and rolled down her window and pointed at the cats on her lawn and said "Head's up, my cats are weirdly aggressive about small dogs and they just got let out so they're full of energy right now" and I nodded and crossed the street and didn't yell at my neighbor but *the temptation was there* because A) why are you letting your aggressive animals roam and B) Why are you letting your cats shit all over the neighborhood and C) Why are you exposing your owned cats to the risks of the large and territorial feral colony that is a literal stone's throw away from your house?
Anyway, and as always, Keep Your Fucking Cats Indoors.

Vampire world biology rambling ft. Lorelai
Vampires "live" through the interaction of necrotic energy allowing them to ambulate. For this reason, despite the common misconception, vampires do have a heartbeat, it is however extremely slow, most accurately being compared to an Agonal rhythm (around 10 BPM), purely for the sake of circulating this energy through the vehicle of blood. Since most of the vampires organs have atrophied (their digestive, reproductive and renal systems), blood circulation is purely to facilitate the energy reaching all extremities and to discourage blood pooling, and causing swelling.
Since their digestive tracts are atrophied, vampires cannot digest food, and have to get their "nutrition" through liquid means. In order to replenish their unique homeostasis, they must consume the living force of other creatures, the more intelligent creatures leading to a much better ratio of blood to potential energy. Sickness, age, lifestyle and nutrition affect the potential energy that can be found within an organism, thus vampires typically seek out younger, healthier individuals, but rarely children due to their small size. Because of this, vampires do not need to feed as regularly if they consume blood from sapient, and go several days up to weeks without feeding (Akin to reptiles).
As such, since the vampire cannot eat meat to consume the energy, they must stick to bodily fluids, the easiest and easiest to replenish being blood, but things like CFS would also work. Occasionally starving vampires eat meat in desperation, but that is a lost cause as, they are forced to regurgitate it as they cannot digest it (Like Owl pellets). This usually only happens to 'feral vampires' - so lost in their hunger they step over their inhibitions and cave into pure instinct to get *something* to fill their belly.
Another system, while not fully atrophied, but heavily impacted is the nervous system causing vampires to experience Analgesia (Lack of Pain). They can feel sensations, however they are extremely muted. To feel cold a vampire must be in extremely freezing conditions, to feel warm they must be quite close to a flame, to feel pain they must sustain heavy damage etc.etc. This has an adverse effect on the psyche, and it is quite common for vampires to experience depression upon becoming undead.
But just because their body is dead does not mean it is impervious to damage. Their regenerative properties are part to none - Vampires still have to care for their bodies just as much as mortals, as due to their lack of an immune system, while they cannot fall ill, their body can still house fungi or bacteria that promote decomposition which while not a death sentence, is a promise of a painful and gruesome future.
In the sun, vampires burn, but not akin to a thermal burn, but rather chemical burn. There is no flame, but the sizzling of flesh, due to a unique interaction of necrotic forces and sunlight. As long as there is no direct sunlight on exposed flesh, the vampire can walk in the sun, however such a feat is quite difficult as it would involve covering ever single inch of your skin, most likely outing yourself as undead -hence why most prefer to simply rest during the day. Sunlight and holy forces are the only things that can scar a vampire.
How does Lorelai deal with the above?
Lorelai is still very young as far as Vampires go, only being one for several years, so many of the psychological long term effects remain a mystery to her.
She does require regular consumption of blood but she is acutely aware that by biting her companions, she would be putting them at a disadvantage for her own...he avoids it as much as possible (Only on extremely rare occasions will she feed from the wrist - never the neck because she finds that too intimate but that's for another post). Whenever the party buys ingredients for food, they also buy Pig's blood for her - a fairly innocuous and easy to acquire ingredient. When hunting, she is given time before the animal is butchered to drink. Since she mostly drinks of animals, she has to feed much more frequently, so it's common for her to be having her 'food' along with the party for dinner nightly.
She always had a soft spot for various fruits, with mangos being her favourite, so she indulges in tiny quantities of mango juice on special occasions.
Lorelai does feel the temperature difference very acutely, as I mention before she enjoys staying in her bat form to warm up by the fire or in various people's beds due to the much smaller size of body. She is fine without the cold.
She does have a lot of issues as I went over here about Analgesia, and how it impacts how she feels pleasure as well - so she is working through that, but she misses the positive sensations of when she was alive, feeling something soft or fluffy. To hold a warm cup. She however is not one of those people that wax poetically about how even pain she misses.
In general Lorelai isn't...someone that hates being a Vampire. Ultimately she wouldn't go back. She doesn't mind being tied to the night than the day, after all, most people rest during the night and miss the things that happen there (and with Raha only needing 4 hours of sleep and Blythe not needing sleep at all, she doesn't feel lonely). She enjoys that she feels full of energy at all times, that she can't get hurt as easily, that she is free to do what she wants. Vampirism to her was...the open door to freedom, to acceptance of what she wanted to do. She'd never want to go back (even if Raha continues to search for a cure for her. Even if she doesn't take it...at least this time she can make the choice to stay a Vampire)

Due to the overwhelming popularity of the Nuka-Berry mixture available in Nuka World, an exclusive run of Nuka-Berry was distributed in select markets and advertised in in magazines directed at upper-class women, such as SteelBarn and Publishing Clearance Home. Implied to be a cool, refreshing drink with sophisticated taste, the popularity of the drinks created a gap in the market which lead to resellers making excess profit, much to the displeasure of the greater public and the Nuka-Cola Company and its affiliated properties. Official Nuka-Berry would never be distributed again beyond its initial release, due to claims that the intense, glowing pink drink had caused the death of a woman in renal recovery who was reported only to have tried the drink. Many reports of renal damage in healthy individuals and rumors that the product turned a person's urine a concerning red, lead to the shut down of the project. A hand painted commission for the wonderful @juneiper-art of her Lone Wanderer Quinn. I wanted to do a Nuka-Cola ad with those pink curls and I'm delighted with how it came out. I hope to do more stuff like this!
edit!!: commissions still open bby! 🏄🏼‍♀️

Do Weed Edibles Make You Dehydrated? (What Experts Say)
Do you ever experience dry skin, mouth, or other dehydration symptoms while consuming Cannabis? Does that ever concern you? If so, you are not alone. Many people have certain concerns while trying new stuff, especially when it is drug-related, whether it is for pleasure or pain management. But is it really the truth or a myth? Do weed edibles dehydrate you? We will explore it in this article.
What is Dehydration?
It is the absence of enough fluids or water in the body. Dehydration occurs when your body loses more fluids than it takes in. Your body won’t work properly if the lost fluids aren’t replaced as soon as possible.
Usually, thirst is our body’s way of telling us we need to drink water. It has been shown, nevertheless, that thirst alone isn’t necessarily a good sign of hydration. Many people don’t feel thirsty until they’ve already lost a lot of fluid, which is especially typical in older people. In general, it’s a good idea to increase water consumption whether you’re sick, it’s hot outside, or you’re exercising.
Dehydration signs are obvious, such as darker urine and a considerable reduction in urination. Dehydration can also be indicated by less obvious symptoms like fatigue or lightheadedness, which can occasionally resemble the consequences of using Cannabis. It can also appear as a dry scalp or skin.
Dehydration results from regular biological functions that cause us to lose water, such as breathing, sweating, peeing, and producing saliva. Dehydration results from not drinking enough water to replenish these losses.
Dehydration can occasionally happen accidentally. Lack of thirst, being preoccupied with other things, not feeling thirsty, or even being under the influence of Cannabis can all cause dehydration.

Why Does Cannabis Dry Mouth?
Cannabis users frequently suffer from “cotton mouth,” a drying sensation in the mouth that is also associated with dehydration. The dryness that follows smoking, meanwhile, doesn’t always indicate dehydration.
Cannabis use does not automatically dehydrate you, but dehydration symptoms like dry mouth, lightheadedness, and dizziness, which are typical of a “weed hangover,” might be mistaken for these symptoms.
Contrary to claims of dehydration, Cannabis itself doesn’t significantly disturb the natural balance of water. However, actions like consuming alcohol and Cannabis together might cause dehydration and other problems.
Although Cannabis does not normally result in dehydration, a small percentage of long-term users may have a disease known as cannabinoid hyperemesis syndrome (CHS). CHS develops in phases, resulting in vomiting, dehydration, weight loss, and nausea. The only CHS therapy that works and results in complete recovery is ceasing to consume Cannabis.
But Is Cannabis Dehydrating Your Body?
Does Cannabis dehydrate? Yes, it does. Cannabis has dehydrating properties because of its diuretic properties, which encourage urine production and, therefore, increase water excretion.
Fundamentally, increased renal function can cause the body to feel dry when using Cannabis without maintaining enough water consumption. This effect is especially prominent in the case of edibles since the prolonged period of increased urine lasts for several hours due to their slow digestion.

What Can Be Done to Avoid Dehydration?
You can use the below-mentioned tips in order to avoid dehydration after consuming the weed edibles.
1. Maintain Hydration
Staying hydrated is essential, especially while using Cannabis. This means drinking enough water before, during, and after using Cannabis. You may prevent the possible dehydration effects of Cannabis by intentionally ingesting enough water. The body’s natural systems are supported by enough hydration, which also helps counteract water loss from cannabis’ diuretic effects.
2. Choose a Balance Diet
Diuretic intake must be balanced; therefore, paying attention to what you’re eating is important. The drying effects of Cannabis can be exacerbated by meals high in salt and beverages like coffee and alcohol. These drugs promote increased urination, which amplifies water loss. Maintaining optimum water levels can be greatly helped by monitoring and controlling your consumption of certain diuretic substances.
3. Exploring Cannabis-Infused Drinks
If you like edibles, choosing cannabis-infused drinks might be a smart move. These drinks give a hydration benefit in addition to the desirable cannabis benefits. Contrary to certain conventional ways of ingestion, cannabis-infused beverages increase your fluid intake and help you stay hydrated as you experience the effects of the drug.
4. Quickly Responding to Dehydration Symptoms
It’s crucial to spot early indications of dehydration. A few examples are a dry mouth, increased thirst, darker urine, or a feeling of exhaustion. The dehydration effects of Cannabis can be fought off by increasing water intake as soon as these symptoms appear. You may avoid the pain and potential health problems related to dehydration by practicing proactive hydration.
5. Factors Affecting Dehydration Caused by Cannabis
The degree of dehydration brought on by Cannabis varies depending on a number of variables. These variables include the amount and kind of Cannabis used, as well as one’s metabolism, body composition, and general health. An overall feeling of dehydration can also result from other cannabis side effects, such as an elevated heart rate or dry lips.
Should You Get Advice from a Medical Professional?
It’s important to recognize that the relationship between Cannabis and a person’s physiology can be complicated. It is advisable to see a medical expert before making any big adjustments to your cannabis use, especially if you are worried about how THC may affect your body’s processes. Based on your health history, medical specialists can offer individualized guidance to make sure your cannabis use is in line with your general well-being.
Maintaining proper hydration while using Cannabis entails a proactive approach to water intake, mindful use of diuretics, consideration of alternative consumption methods like beverages infused with Cannabis, prompt response to dehydration signals, and consulting medical professionals to make educated decisions about your cannabis use.

Conclusion
Drinking a lot of water is essential while using Cannabis; however, there should be a healthy balance to everything. When the body stores an abnormally large amount of water, water intoxication can arise from excessive ingestion. Drinking a substantial volume of water in a short period of time can give birth to this issue. For instance, consuming 100 ounces of water in only a few hours might result in a number of risks, even lethal ones. Make careful to spread out your regular water consumption equally throughout the day to protect your health.
There are a number of sensible methods for keeping hydrated when using Cannabis. The cornerstone of preserving good health is moderation, along with a balanced lifestyle and nutrition. Since thirst may not always correspond to our real hydration needs, including hydrating drinks and plain water in your diet can help. In addition, maintaining sufficient hydration is a tried-and-true way to reduce the effects of a “weed hangover” the next day.

These past few years, I have become a huge advocate for feeding fresh food to dogs to enhance their lives. However, you don't exactly have to transition your dog to a 100% fresh food diet in order to gain the benefits of fresh food.
Simply adding fresh food to your dog's kibble can go a long way in improving their nutrition and overall health. For instance, one study done a group of dogs in 2005 revealed that feeding vegetables just three times a week had significantly lowered their risk of developing cancer, compared to dogs who were only fed kibble.
Asian markets, in particular, have some very beneficial fresh food items that aren't typically found at your local grocery store. These food items, however, are not intended to fully replace your dog's food. Remember - too much of something good can be bad for you.
Instead, they should be offered in moderation as treats, or just small amounts topped over an already complete and balance meal. As a general rule of thumb, treats/toppers should never make more than 10% of your dog's overall diet.
Tumblr media
Bok choy (Chinese cabbage) is full of essential vitamins and minerals needed to support a dog’s eyesight, cardiac function, digestion, bone strength, and immunity. Sulfurophane - a compound found in bok choy and other cruciferous vegetables - has been linked to inhibiting cancer cell growth, lowering blood pressure, and activating the nuclear factor Nrf2 which prevents the development of diabetes and its complications. Bok choy’s low calorie, high fiber content also make it a good option for a snack/food topper for dogs who need to control their weight.
Preparation: Lightly steam or boil for better nutrient absorption. Cooking will also help neutralize the enzyme (myrosinase) that can decrease thyroid function. You should also chop/trim their long leaves to avoid choking hazards.
Tumblr media
Goji berries (wolfberries) are rich in amino acids, carotenoids which support healthy vision, and polyphenols that have anti-inflammatory, anti-tumor, and cardio-protective properties. Studies on goji berry supplementation in animals have shown that it has the potential to help lower cholesterol, protect the liver by increasing hepatic antioxidant activity, as well as enhance metabolic homeostasis and prevent diabetes-induced renal inflammation.
Preparation: Remove stems if still attached and cut/mash berries. If dried, soak in water until softened to avoid passing straight through the GI tract and losing the benefits of its nutrients.
Tumblr media
Offal refers to the internal organs and variety meats of animals that can be consumed as food (i.e. lung, heart, kidney, liver, gizzard, head, feet, etc.) The word 'offal' literally translates to "fall off", and thus, whatever falls off the skeleton during the butchering process. Offal is densely loaded with essential vitamins, minerals, proteins, and fats - so small amounts go a long way. Pet owners who use these as ingredients as part of a complete and balanced recipe should keep in mind that each organ is unique in its nutrient profile - dependent on the animal it is sourced from and how it is raised. For example, grass-fed beef may contain less fat yet more vitamin A and E than grain-fed beef, and beef liver has significantly more vitamin A and copper than chicken liver.
Preparation: Boil in water or low-sodium broth. May also be baked or dehydrated into jerky treats.
Tumblr media
Oyster mushrooms are a good source of protein and fiber, and vitamins that support healthy digestion. They can help to increase satiety and maintain healthy body weight. Oyster mushrooms are also full of pantothenic acid which help to maintain cognitive function and healthy skin/coat. Beta-glucans derived from oyster mushrooms have also been shown to reduce inflammation and strengthen immunity.
Preparation: Lightly sauté or boil in water or low-sodium broth. Raw or dried mushrooms can be difficult for dogs to digest.
Tumblr media
Purple sweet potato (Ube or Okinawan sweet potato) are typically found in grocery stores more commonly during the fall/winter seasons. They contain more antioxidants than regular sweet potatoes due to the anthocyanins that cause their violet hue. While all potato varieties may impact blood sugar levels because of their high carbohydrate content, purple potatoes may exert less of an effect because of their high polyphenol content that decreases the absorption of starches in the intestines. In addition, they have been linked to improving blood pressure, likely due to their high potassium content.
Preparation: Peel skin and bake until soft for better digestibility and to prevent intestinal blockages. May be boiled and drained to reduce oxalate intake for dogs with a history of bladder stones, or prone to urinary issues.
Tumblr media
Quail eggs are small, yet packed with protein and essential fatty acids, and have a higher ratio of iron, riboflavin, and vitamin b12 than chicken eggs. It is even safe and can be beneficial to feed them with the shells included for added calcium. Quail eggs have been linked to improving bone growth/healing, reducing liver damage, and increasing energy levels. While some studies have shown evidence of quail eggs treating allergies in humans and mice, they may be less likely to trigger reactions than chicken eggs in dogs who have poultry sensitivities.
Preparation: Wash thoroughly, and handle gently as they are more fragile due to their tiny size. Cooking methods are similar to that of chicken eggs, yet require less time.
Tumblr media
Sardines contain all the amino acids your dog needs for optimal health, making it a "complete protein". They are also notorious for being rich in omegas and coenzyme q-10, which help support heart and brain health. What sets sardines apart from other fish, too, is that they are smaller and more short-lived species than larger predator fish and may have less of a risk for mercury poisoning. Because they are so small and have soft bones, you can feed them whole to your pets.
Preparation: Bake or dehydrate. If already purchased as dried, be sure to make sure there is no added salt or seasonings. If canned, preferably those stored in water only.
Tumblr media
Sushi rice (Japanese sticky rice) has a high proportion of starch and moisture, giving it a stickier texture when cooked. Sushi rice has virtually zero fat, yet still provides a desirable balance of nutrients while remaining gentle on a dog’s digestive tract. Its higher iron content than regular rice supports healthy blood circulation and can provide more energy to dogs who are recovering from illness. Cooling/refrigerating the rice before serving also enables more retrogradation of prebiotic fiber, helping to lower glycemic responses and maximize intestinal function.
Preparation: Rinse thoroughly and soak in water (with 1tbsp apple cider vinegar - optional) before cooking. If you don’t have a rice cooker or instant pot, boil on the stove until the water is completely absorbed. May be cooked with bone broth for added flavor. You can also use small amounts of beet root powder, turmeric, or blue spirulina to add color and nutrients.

On The Effects of Prayerblossom on Shadow Afflictions
A note; local sources use the names Prayerblossom and Prayerbloom interchangeably. Both seem to be correct in the Common vernacular of the area.
Other medics have speculated on the effects of Prayerblossom and its interaction with fel, shadow, and even death magic afflicted individuals, often relating the plant to the Holy Light.
While other plants I've seen have consumed arcane from their surrounding areas, I fail to see why Prayerblossom would do the same. There is no natural font or beacon of Light magic noted that I can find in texts surrounding the Swamp of Sorrows, however the area in which Prayerblossom is prevalent is absolutely laden with natural energies.
Sources state nature spirits and pure elementals are very common, and the Green Dragonflight had activity in the area, even if they later fell to corruption. If anything, the plant may simply have incredible curative properties.
Since the supply of plant material I have is limited and my purpose of research narrow, the experiments conducted will be solely focusing on the idea Prayerblossom can aid in the treatment of shadow corruption in some form.
First, I broke down the different parts of the plant into different forms to see if there was a difference in how they could be used or applied. Of the blooms, the stems, the leaves and the roots, I made a tincture and a paste of each of them.
Secondly, I prepared the samples I would be testing on. I had small pieces of meat I had steeped in Shadow, hares I afflicted with Shadow, and soil tainted with Prideful Sha.
Testing each upon each, I recorded the results as follows;
Of the shadow-steeped pieces of meat, the paste of the blooms seemed to have the most potent effect. Applying a little didn't do much, but applying a generous portion, enough to cover the affliction, slowly pushed the corruption to the edges of where I applied the paste. The corruption, when increased by my hand, did not go past where the paste was applied, down to several inches past the surface.
The hares I tested on seemed to mostly want to chew on the stringy paste I had made from the stems, even though they'd been fed plenty prior to testing. Though I did not see a reduction in corruption levels, chewing and even eating the stem paste seemed to make them less nervous and more docile. They did not flee as before when a bright light was shone on them.
Tinctures made from the roots did seem to reduce the hares' corruption levels, though they did not like the medicine, likely due to its dryness and bitterness. The reductions were rather quick, not coming out in the renal process as many other Shadow Protection Potions work, but by shedding it out in the few sweat glands that exist around the mouth. Through careful care, I was able to aid the hares in removing their affliction.
The soil, when mixed with the tincture and even paste from the roots, seemed to slowly, over the course of hours and days, release the sha's taint upon it. Such as is the case of sha, the soil mound smouldered and smoked, though there was no indication of true 'burning'. Adding more root tincture or mixing in more root paste increased the speed of the reaction.
In conclusion; through these few studies, I believe I've worked out a few methods of further treating shadow afflictions.
Paste of the flowers of Prayerblossom seem able of 'chasing out' Shadow corruption. One might be able to use it to line a limb or an area to contain its spread in the body, where extraction can happen in a particular spot. It also may be able to be used to reduce the concentration of shadow corruption if placed on a shadow-infected wound.
While perhaps better as whole stems vs. stem paste, it does seem that the stems have some sort of calming agent in them. While it doesn't affect shadow corruption directly, it may prove useful to those suffering from shadow's mental effects and make patients easier to treat. The tincture did not have the same effect, so it stands to reason there's something in the fibres themselves or in the chewing and mechanical activation that matters.
Prayerblossom root tincture did wonders in general for shadow and void corruption. It not only seems to revitalise, but force the body to excrete the shadow through any means necessary. Applied to non-living material, it severed the bonds between shadow magic and material object given enough contact and time. It may be a useful alternative for those who cannot take SPPs, or a good addition for those already taking an increased dosage of SPP.

A Comprehensive Guide to Natural Kidney Health and Herbal Remedies
Kidneys are essential organs that filter waste and excess fluids from the blood, regulate blood pressure, balance minerals, and produce hormones necessary for red blood cell production and bone health. As vital as they are, kidneys can face challenges due to stress, disease, or age-related decline. Fortunately, natural herbs and supplements can help maintain kidney health and support its functions. This article delves into the best natural kidney health herbs, their benefits, and how they contribute to overall wellness.
Tumblr media
Understanding Kidney Function
Healthy kidneys process about 50 quarts of blood every day, removing waste and excess water to create urine. This urine travels through two ureters to the bladder for elimination. Besides filtering blood, kidneys also:
Regulate the balance of minerals like potassium, calcium, and sodium.
Manage blood pressure through hormone release.
Produce erythropoietin, which aids in red blood cell production.
Convert Vitamin D into its active form for bone health.
Given these vital roles, maintaining kidney health is crucial. Proper hydration, a balanced diet, and a healthy lifestyle form the foundation of kidney care. However, herbal remedies can offer additional support, especially as we age or face chronic conditions.

Redefining Post-Hemodialysis Bleeding Control with SURGISPON® DIAL
Tumblr media
Chronic kidney disease (CKD) is a significant global health issue, affecting over 750 million people worldwide. For more than two million patients with end-stage renal disease (ESRD), hemodialysis serves as a vital lifeline, enabling survival despite the loss of kidney function [1, 2]. As the need for hemodialysis in patients with end-stage renal disease (ESRD) continues to rise, the creation of hemodialysis access points—such as arteriovenous fistulas (AVFs) and arteriovenous grafts (AVGs)—has become a routine yet essential vascular surgery procedure, crucial for ensuring long-term dialysis success [3, 4].
Maintaining the patency of these dialysis access points is a significant challenge, as less than half of all AVFs and AVGs remain functional beyond three years due to frequent complications such as narrowing or thrombosis [4]. To address these issues, catheter-based interventions like percutaneous transluminal angioplasty are commonly used to restore blood flow. However, achieving hemostasis after these procedures remains a time-consuming challenge, underscoring the need for better bleeding control methods.
The presence of uremic toxins in ESRD patients complicates bleeding control, underscoring the need for improved hemostatic solutions. This is where haemostats for dialysis like SURGISPON® DIAL can revolutionize patient care.
Understanding the Need for Dialysis
Dialysis becomes necessary when kidney function declines to a point where 85-90% of it is lost, marked by a glomerular filtration rate (GFR) of less than 15. In this stage, without dialysis, toxins accumulate in the body, leading to serious health risks. Despite its life-saving properties, dialysis presents challenges, among the most pressing being the management of post-procedure bleeding.
The Challenge of Post-Hemodialysis Bleeding
After each hemodialysis session, controlling bleeding at the puncture site is a critical concern. Although this may seem straightforward, several factors complicate the process—most notably the use of anticoagulants, such as heparin, which are essential for preventing blood clotting during dialysis. However, these same medications significantly increase the risk of post-dialysis bleeding [5]. This complication, known as Post-Hemodialysis Site Hemorrhage (PHSH), becomes even more complex in patients with other conditions like hypertension, diabetes, or low platelet counts. These factors prolong the bleeding and make the hemostasis process time-consuming and difficult.
Methods for Controlling Post-Dialysis Bleeding
Today, healthcare professionals use two primary techniques to control post-hemodialysis bleeding:
Direct Pressure: This is the most commonly used method. It involves applying firm pressure to the puncture site with gauze or a cotton roll until the bleeding stops. The time required can vary greatly, from 20-30 minutes to over an hour, depending on the patient’s health status, comorbidities, and the extent of the bleeding.
Tourniquet Application: a sterile elastic tourniquet is applied along with direct pressure to control bleeding. While effective, applying excessive pressure can reduce the lifespan of the fistula, the vascular access point used for dialysis. This renders it a suboptimal solution for long-term use, particularly in patients requiring frequent dialysis.
The Need for Advanced Haemostats for dialysis
The current methods, while standard, have limitations. They take time, are labour-intensive, and may damage the vascular access, potentially compromising future dialysis sessions [6]. This highlights the need for advanced hemostats for dialysis, like SURGISPON® DIAL, that can offer rapid, reliable bleeding control without compromising the integrity of the fistula or graft. These advanced solutions would not only reduce the time required for post-dialysis care but also decrease the burden on healthcare staff, allowing them to focus on other critical aspects of patient care. Discover more about our hemostatic solutions here.
We at Aegis Lifesciences introduce “SURGISPON®DIAL”, an optimal haemostatic dialysis bandage to control bleeding post-renal dialysis.
The Role of SURGISPON® DIAL in Post-Hemodialysis Bleeding Control
As the need for hemodialysis continues to grow, the challenge of maintaining the functionality of dialysis access points persists, with frequent complications such as narrowing and thrombosis[3]. Despite advancements like catheter-based interventions, post-procedure bleeding remains a significant and unresolved issue, highlighting the need for improved hemostatic solutions like SURGISPON® DIAL.
SURGISPON® DIAL offers a revolutionary approach to managing post-dialysis bleeding, addressing the unique challenges posed by anticoagulants and other complicating factors, thus redefining post-hemodialysis bleeding control and enhancing patient care.
SURGISPON® DIAL
SURGISPON® DIAL is an absorbable hemostatic gelatin sponge specifically designed to meet the unique challenges of post-renal dialysis patients. By addressing the limitations of traditional hemostatic methods, SURGISPON® DIAL offers a faster, more efficient solution.
a) Key benefits
Highly Porous Structure: SURGISPON® DIAL is capable of absorbing fluids 40–50 times its own weight, ensuring rapid and effective bleeding control.
Function: SURGISPON® DIAL provides a matrix that promotes platelet aggregation and coagulation and prevents the loss of natural clotting factors.
Indication: It is specifically designed for use in post-hemodialysis patients to stop bleeding at the puncture site.
Mode of Action: SURGISPON® DIAL’s porous structure activates the thrombocytes at the moment blood comes in contact with the matrix of the sponge. This initiates the release of substances that promote thrombocyte aggregation while their surfaces undergo change, facilitating fibrin formation and arresting the bleeding.
Size available: offered in a convenient 30cm x 30cm x 10cm size, it is specifically designed to cover dialysis puncture sites.
Reduced Recovery Time: SURGISPON® DIAL is designed to minimize post-operative time and promote a faster return to normal activities, enhancing recovery outcomes for patients.
b) Clinical highlight
Cristóbal et al. (2021) aimed to evaluate the pressure generated by an adjustable hemostasis clamp on arteriovenous fistulas (AVFs) during the hemostasis process and compare it with the direct two-finger pressure applied by the patient. The study analyzed data from 51 hemodialysis procedures involving 15 patients. SURGISPON® DIAL, an absorbable gelatin dressing, was utilized as an adjunct hemostatic agent during the study to apply pressure to the puncture site, enhancing the effectiveness of both the adjustable clamp and manual compression techniques. The combination of SURGISPON® DIAL with these pressure methods contributed to improved hemostasis, ensuring adequate pressure while minimizing the risk of complications, such as external bleeding or hematomas [7]. Read the full case study here.
Conclusion
Managing post-dialysis bleeding is a vital but often underemphasized part of patient care. With the increasing number of patients requiring dialysis, the need for more effective and efficient hemostatic solutions is clear. SURGISPON® DIAL addresses this challenge by providing a fast, reliable method to control bleeding while preserving vascular access for future treatments. Its innovative design reduces recovery time, enhances patient outcomes, and lightens the workload for healthcare professionals. By integrating SURGISPON® DIAL into their practices, healthcare providers can significantly improve the quality of post-dialysis care. Discover our full range of hemostasis solutions here.
