Raccontino.
A seguito di tosse insistente ad origine faringolaringea, ovvero " di gola", durata alcune settimane giunge in radiologia un giovane cardiologo che lavora nel mio ospedale per eseguire RX torace che risulta negativa per infiltrati polmonari.
Gli dico che potrebbe trattarsi di pertosse da confermare, se vuole, con titolo anticorpale.
Lui sbianca in viso.
E mi racconta che prima della insorgenza della tosse aveva fatto proprio vaccino antipertosse raccomandato a lui e alla moglie incinta di 7 mesi.
Pare ovvio che questa possa essere intesa come reazione avversa al vaccino sotto forma di malattia indotta dal vaccino acellulare, che, come sappiamo, ha indotto resistenza del ceppo originario ormai sostituito dai ceppi pertactina negativi.
Il cardiologo risbianca in viso e mi chiede come faccio a sapere queste cose.
Gli rispondo che leggo articoli del professore Paolo Bellavite e del professore Davide Suraci, ex docente presso nostro ateneo nonché coautore del libro " Operazione Corona, colpo di stato globale".
E trascuro di perdere tempo con le comunicazioni del Quotidiano Sanità.

Il cardiologo mi informa inoltre di un altro fatto importante che mi riguarda.
Vi ricordate la ragazza che io segnalai sul mio referto ecografico come possibile reazione avversa a 4 dosi di vaccinazione anticovid e ad una dose di vaccinazione antipertosse che lei aveva fatto in gravidanza?
Ebbene Lei, con sospetta sindrome proliferativa post vaccinale sviluppò anche una pericardite per cui fu ricoverata proprio nel reparto di cardiologia del medico in questione.
Pare che il suo caso sia stato oggetto di segnalazione all' AIFA come sospetta reazione avversa dal primario della cardiologia in persona.
Bene.
Ma io non sono mai contenta, e la conversazione continua.
In particolare emergono fatti interessanti che riguardano anche i farmaci di pertinenza prettamente cardiologica: sapevate che i farmaci usati per contrastare la pressione alta chimato beta bloccanti di prima generazione, passando la barriera ematoencefalica causavano turbe del comportamento, alterazioni psichiatriche, incubi notturni, insonnia prolungata e incremento dei tentativi di suicidio?
Ora lo sapete.
Per questo sono stati rimossi dal mercato.
E sostituiti con altri che non causano il suicidio.
Per esempio gli antipertensivi chiamati Ace inibitori.
Come andiamo con gli Ace inibitori?
Con gli Ace inibitori non si è riscontrato un incremento dei tentativi di suicidio tuttavia è stato notato un effetto cancerogeno che si manifesta con netto incremento dei casi di tumore polmonare soprattutto se usati continuativamente per più di 4 anni.
Bene.
Come il Ramipril, rimosso dal mercato perché dichiarato cancerogeno dopo anni di uso, per essere attualmente riimesso sul mercato sotto altro nome ovvero Triatec.
Ripeto: uso prolungato degli Ace inibitori è associato alla insorgenza di tumore polmonare.
E allora come possiamo curare la pressione alta?
Non c' è problema, le linee guida parlano chiaro:
Evitare quando possibile la prescrizione degli Ace inibitori ai pazienti giovani, in quanto, in questi soggetti, avendo una aspettativa di vita più lunga, potrebbe essere incresciosa la insorgenza di una neoplasia polmonare.

As I've been getting used to being in a pharmacy tech job again, I'm re-familiarizing myself with the wacky linguistic world of medication names.
"Ramipril" is a pleasing one to me because it seems like almost a mashup of "Ram" and "April" (and April contains part of the date-range of Aries, the Ram sign).
All in all, besides having the name of Ram (my favorite TRON character), it also appeals to my Pattern-Recognizer and my sense of all things in the universe being connected through puns.
(However, it is a type of med called an "ACE inhibitor." And if you've read my fanfic and my headcanons, you know I... do not exactly view Ram as either being ace, or having any inhibitions.)

Work is long and you have become tired. You go through the motions, you smile when you’re supposed to. It never stays for long. Where did your joy go? You’re still so young. Why are you always so tired? What happened to you?
Nobody can predict the downspiral. It creeps into the cracks and squeezes your lungs. You just need to rest, and try it again tomorrow.

Unfortunately, "Holistic" is one of those words in medicine that has been so misused it's functionally meaningless.
Holistic medicine SHOULD mean "medicine that considers all aspects of a patient, including how their diseases interact and what matters to them in life." The idea is a counter to a lot of how medicine was practiced in the 70s-90s, when Tablets Solved Everything and the only thing that mattered was "curing" your disease.
This is actually a really important concept. Classic example: As a doctor, it's really easy to get into the pattern of "fixing" diabetes with Metformin, "fixing" blood pressure with Ramipril, and "fixing" period pain with HRT. Unfortunately what I've also done is made a 30-year-old woman functionally infertile, given them a permanent cough and lightheadedness, and meant the next time they get a UTI it lasts a lot longer. I've also failed to diagnose their PCOS, because I've managed each problem on its own as it's come up. And maybe the tour guide I've given this medical combo to could do without coughing, weeing and feeling lightheaded all the time?
Holistic medicine SHOULD be about taking all of that into consideration and saying "ok, maybe your BP isn't your biggest problem. Maybe if we get on top of your stress with therapy, and get your diabetes under control, it will be ok, and maybe it doesn't matter compared to everything else anyway?"
It should also be about advising Camomile, Mint and Ginger tea for period pain, and getting a strap on heat pad which lets them do their walking tours as comfortably as possible, and here are some exercises to do at the gym to increase core strength. That way we avoid HRT, so that they can try for a kid in a few years without having to change all their meds and get bounce back pain.
Importantly, it ALSO means saying in no uncertain terms that untreated diabetes will ruin your life and you MUST take Metformin, be aware UTIs might be worse and get antibiotics earlier but do NOT STOP TREATING YOUR DIABETES YOU WILL SUFFER IF YOU DO.
That is what holistic medicine is SUPPOSED to be. Unfortunately, a lot of this has been caught up in what I will politely call "fluffy thinking" - people who are unwilling, or unable, to engage in the complex process of good holistic medicine (the above scenario might not work for an office worker who doesn't walk very much! Ramipril might be a good idea and she might not be allowed a hot pad under her uniform! Holistic medicine is HARD), but who want to appear like they are doing it.
That leads to two big polluters. The first is Quakery. A chiropractor is often cheaper than a physio, sounds impressive, and can make people "feel" like they're doing something. And they are! It's a massage! But if it's dressed in some pseudoscience language, people buy into it more. And then the well-meaning doctor is stuck, because SURELY if it's helping someone feel better and meaning they need fewer opioids it should be encouraged? So maybe they should recommend it to people? You know, to be "holistic?" And they're easier to get to than the physio, and chiropractors don't set much homework, and and and ....
Acupuncture, chiroprathy, and herbal medicine all work on this basis. There's some evidence that some of what they do is beneficial, the science behind it is either misapplied or straight up bullshit, but it's hard to explain exactly how if you're a busy doctor with a suffering patient in front of you who can't afford the 6-week intensive physio programme.
That brings me to the second polluter: money. More specifically, who decides where it's spent. In the UK, and other countries where the government pays for most healthcare, it's normally nonmedical people deciding what gets covered. They've read that "holistic" means offering non-conventional treatment, and in front of them are two options: pay for Loractformin,* an expensive new version of Metformin that doesn't prolong infections, OR offer on-line weight loss courses which cost almost nothing to run. A holistic practitioner would understand that the BEST option is "both, for specific groups that would benefit." But the purchaser doesn't practice medicine and doesn't get that. Or maybe they do, but their brief says they can't choose both. Or the budget means choosing both will result in poor delivery of both (this is a legit limitation). So they pick the weight loss course, and tell their clinicians this is about a "holistic" approach because that sounds better.
In fee-paying countries, some of it is about what patients can afford. A doctor wants to offer SOMETHING to the patient who can't by Ramipril, Metformin and HRT; an online diet course convinces both the doctor and the patient that Ramipril can be skipped at least, maybe Metformin if times get tight, and hey why not try some herbal nonsense pills for pain? Placebo is powerful right?
On the other end of the scale, insurance companies and hospital executives can make good money out of insisting people go on courses, or see a chiropractor, BEFORE they authorise Metformin. Hey, it might help some people right? The fact you can tell beforehand who will and won't be helped isn't the point, EVERYONE deserves access to ALL the treatment options? That's "holistic" right? Sorry, can't hear you Mr Dr sir, the radio in my new BMW (courtesy of Totally Not A Scam Weightloss LTD) is a bit loud!
And THAT is how Holistic medicine gets fucked. It's part of why I don't do general practice, because the external influences are unbearable (also I find a lot of it very tedious; I'm an anaesthetic/intensivist trainee, I like my patients sick and unconscious).
TL;DR: Holistic Medicine is a very difficult thing to do well, and almost anyone telling you that's what they're doing is probably bullshitting for Buzzword Bragging points.
*(I made Loractformin up, but drugs like this crop up from time to time. Anyone on Omeprazole, please ask your doctor why you're not on Lansoprazole instead).
