Antiserum Arsenal: Strengthening the Fight Against Rabies.
Introduction Rabies remains a serious zoonotic disease caused by Lyssaviruses. Though vaccines are available for prevention of rabies in humans as well as animals, rabies antiserum plays an important supportive role in the management of rabies especially in developing nations. This article aims to provide an overview of rabies antiserum - its various applications, production process, mechanism of action and effectiveness.
Post-exposure Prophylaxis Rabies antiserum is an integral part of post-exposure prophylaxis (PEP) along with vaccination for category III exposures as recommended by the World Health Organization. It is administered through infiltration and intraneural routes at and around the wound along with vaccines. This helps in neutralizing any virus present at the site of exposure and prevents its entry into the central nervous system, buying time for the active immunization to work. Its role is crucial in immunocompromised individuals and children for whom cellular immune response may be delayed.
Pre-exposure Prophylaxis Some occupational groups such as veterinarians, animal handlers etc. who are at continuous risk of unapparent exposure can take antiserum alone as pre-exposure prophylaxis. The antibody levels achieved provide immediate protection until an active immune response develops following natural exposure or booster doses. However, vaccination should still be taken at the earliest opportunity to ensure sustained protection.
Production and Mechanism of Action Rabies antiserum is a pathogen-specific immunoglobulin preparation containing high titres of rabies virus-neutralizing antibodies. It is manufactured by hyperimmunizing horses/sheep with inactivated rabies virus. The neutralizing antibody content is standardized and purified from the antiserum. Upon infiltration around wounds, the antibodies bind to free virus particles, aggregates or soluble antigens thereby preventing viral attachment and entry into host cells. This passive transfer of immunity helps clear or control initial infection bought about by cell-associated virus.
Effectiveness and Limitations Several clinical studies and reports of PEP failures have demonstrated the effectiveness of antiserum in preventing rabies when used in combination with adequate wound treatment and full vaccination regimen. However, there are certain practical limitations to its usage. Production relies on large animals and is less sustainable. The short half-life requires repeat dosing and careful scheduling with vaccinations. Storage and maintenance of cold chain during transport adds to Logistic challenges especially in remote regions.
Recent Advances Newer purification techniques enable higher yields and more standardized product quality. Cell culture technologies can eliminate animal usage altogether. A human monoclonal antibody cocktail is being evaluated as alternative to equine antiserum post-exposure. Research also aims at improving stability and extending half-life of antibodies through molecular engineering and nanoparticle conjugation. Such advances hold promise to enhance availability, effectiveness and compliance to rabies biologics especially in resource-limited settings.
Conclusion In summary, rabies antiserum remains a valuable adjunct to rabies vaccination in post and pre-exposure rabies prophylaxis despite facing some practical constraints. Continuous efforts to improve production technologies, antibody formulations and develop substitutes are expected to help bolster rabies control programs worldwide in future.

Albay Rabies Deaths: A Preventable Tragedy
A Grave Public Health Concern
The Albay Provincial Health Office (PHO) is raising an urgent alarm, discouraging victims of animal bites from resorting to traditional or "quack" treatments for rabies. This appeal comes after the grim toll of four rabies-related deaths reported from January to September this year, highlighting the dire consequences of this deadly viral infection. Unconventional Methods Claim Lives In a candid interview on Thursday, Jan Edmund Dominic Bailon, the chief of the Provincial Epidemiology and Surveillance Unit (PESU), revealed a troubling pattern. All four victims tragically avoided seeking proper medical treatment at a hospital or animal bite center, with two of them opting for the unorthodox "tambal" method.
The Disturbing "Tambal" Approach
The questionable "tambal" cure involves the unsettling practice of making incisions and inserting small pieces of carabao horn into the wound, purportedly to "extract" rabies.
Fatal Neglect
Bailon disclosed, "The four death cases were from the municipalities of Guinobatan, Bacacay, Tabaco City, and Daraga. Lahat po hindi nagpa-vaccine (All of them were not vaccinated), both human and animal." He further clarified that these four cases were clinically diagnosed based on the observed signs and symptoms of rabies infection.
The Power of Vaccination
In response to this alarming trend, Bailon emphatically reinforced the importance of immediate vaccination as the best course of action for individuals bitten by animals. Lifesaving Measures "Still, vaccination remains the most effective safeguard. We have two dedicated animal bite clinics in the province offering free vaccination services. Additionally, the Municipal Agriculture Office extends free vaccination for dogs and cats," he emphasized.
A Call for Public Awareness
Last month, the PHO launched a province-wide information campaign through barangay assemblies to raise public awareness about the critical importance of rabies vaccination in the event of an animal bite. A Harrowing Past It is crucial to note that in 2022, the Albay PHO documented two tragic deaths resulting from rabies in the municipalities of Tiwi and Manito. These painful incidents serve as poignant reminders of the life-threatening consequences of neglecting timely vaccination against this lethal scourge. Sources: THX News & Philippine News Agency. What happens if you take rabies vaccines late?
The Rabies Vaccine needs to be given on a suitable timetable. Nonetheless, if a patient becomes off timetable for the vaccine the arrangement isn't restarted. Each exertion should be made to stick to the suggested PEP routine timetable, particularly the initial two days of treatment, days 0 and 3. After day 3 of the routine, deviations of a couple of days are adequate. For most minor deferrals or interferences, the immunization timetable can be moved and continued like the patient was on time. For instance, if a patient misses the portion booked for day 7 and presents for immunization on day 10, the day 7 portion should be managed that day, and the last portion was given several weeks later on day 17.
Rabies infection can persevere in tissue for quite a while prior to moving into a fringe nerve. Rabies immunoglobulin can be utilized something like and including 7 days after the presentation. Rabies resistant globulins (RIG) are not generally accessible. Rabies-uncovered patients regularly present to clinical focuses, especially in canine rabies invaded areas after an antibody arrangement has been begun without insusceptible globulin organization. It is realized that rabies insusceptible globulin can bring about concealment of the killing counteracting agent reaction which typically yields perceivable antibodies by day 7. We have indicated that it tends to be regulated with a postponement of as long as 5 days after the beginning of immunization treatment without critical neutralizer concealment inside the primary month. This investigation used the WHO endorsed multisite Thai Red Cross intradermal post exposure routine. Viable utilization of rabies insusceptible globulin in extreme and numerous injuries, especially in little youngsters, may require a weakening of the RIG in ordinary saline to give a volume sufficient to penetration, everything being equal.

60% Rabies Vaccinations Yields Healing for Sarangani
Sarangani has reached over 60% rabies vaccination coverage on dogs for two straight years. Federico Yadao, Medical Technologist II and Provincial Epidemiology and Surveillance Unit Disease Surveillance Officer, cited this as a good indication towards the province’s rabies elimination plan consistent with the Department of Health’s target to free Philippines from rabies by 2030 and the 80% vaccination coverage indicator that the World Health Organization had set to successfully achieve rabies global control. From the 46,138 dog population in Sarangani in 2020, the Provincial Veterinary Office (PVET) vaccinated 28,421 or 61.60% in 2021. While, 62.18% or 27,917 were vaccinated in 2022 out of the 44,895 reported population in 2021. As of March, 1,950 dogs were already vaccinated based on the 32,256 dog population in 2022. There were also 1,249 cats vaccinated in 2022 which composed 6.25% of the 19,976 population recorded in 2021. As of March, 55 cats were administered with anti-rabies vaccines based on the 3,856 population in 2022. In the Philippines, Dr. Bernard Cababat, Sarangani’s officer-in-charge Provincial Veterinarian, said dogs are responsible for 98% of cases of rabies virus transmitted to humans, while the remaining two percent were caused by bites of cats and other animals.
Rabies rally. Photo by Jake T. Narte of Sarangani Provincial Information Office. Dr. Cababat said they are stepping up efforts to eliminate the disease by urging Sarangans to vaccinate their dogs and cats especially that “rabies is 100% fatal but 100% preventable.” Rabies is a vaccine-preventable, zoonotic, viral disease affecting the central nervous system. Federico Yadao, Medical Technologist II and Provincial Epidemiology and Surveillance Unit Disease Surveillance Officer said: “We really do not want to have fatality dahil doon as preventable disease” and cited their collaboration with the Department of Education Sarangani Division." “With DepEd, mas more tayo sa control. Mas more tayo sa preventive na mga activities, rather than curative.” From 2019 to 2021, the Provincial Health Office reported a total of 4,583 animal bite patients administered with post exposure prophylaxis and 17 deaths distributed as eight in 2019, one in 2020, and eight in 2021. Although Yadao said this is not “alarming” since post exposure prophylaxis are available, but he encouraged everyone to: “limit exposure by being responsible pet owners. Kung ano lang ang kayang alagaan, ‘yun lang ang alagaan.” The massive vaccination drive is in line with the celebration of the Rabies Awareness Month, but Dr. Cababat pointed out they are doing vaccination all year round in partnership with the offices of the Municipal Agriculturist (OMAG), where the program is lodged. Rabies Awareness Month, this year, is anchored on the theme "Rabies-free na pusa't aso, kaligtasan ng pamilyang Pilipino." As part of the campaign, the Provincial Veterinary Office and offices of the Municipal Agriculturist in Malungon held an information drive dubbed as Pet Talks at Malalag Cogon National High School on March 28. Dr. Cababat emphasized the importance of making students agents of the right information about rabies. He said they have considerable leverage to spread the information “being active on social media.” If anyone is bitten by suspected rabid animals, Dr. Cababat advises to directly go to the nearest Municipal Health Office or hospital for assessment and proper medical care.
