EL PASO, Texas — Residents in the Texas border city of El Paso have been urged to stay home for two weeks as a spike in coronavirus cases overwhelms hospitals.

The uptick in virus cases has also prompted the state to dedicate part of the city’s civic center as a makeshift heath care center for the ill. On Sunday night, El Paso County’s top elected official issued a stay-at-home order that imposes a daily curfew from 10 p.m. to 5 a.m. Violators could be fined $500 under the order.

Earlier Sunday, Texas Gov. Greg Abbott said 50 hospital beds will be set up in the city’s convention center and another 50 beds could be added if needed. The state has already sent over 900 medical personnel to El Paso, some of whom will be staffing the convention center site.

El Paso County health officials reported 772 new coronavirus cases Sunday, a day after a record 1,216 new infections were reported.

MADRID — Authorities in Spain’s northeastern region of Catalonia are considering a mandatory stay-at-home order for weekends only, one of the strictest measures being imposed across the country to combat a sharp resurgence of the coronavirus.

Under a new state of emergency declared by the national government on Sunday, Spain’s 17 regions and two autonomous cities can tailor restrictions that include a nationwide overnight curfew, a cap of six people on social gatherings and possible regional travel bans.

Only Spain’s Canary Islands, where the curve of contagion has been kept at bay, will be exempted from the 11 p.m.-6 a.m. general curfew, which other regions’ officials can decide to push one hour earlier or later.

The aim is to rein in infections from gatherings of family and friends at home or in bars.

The Spanish government wants the state of emergency to be extended later this week until May with as many votes as possible in the country’s parliament.

Spain last week became the first European country to surpass 1 million officially recorded COVID-19 cases, although officials say that the real figure could be way above 3 million.

LONDON — British authorities are likely to tighten restrictions on more areas of the country this week, amid mixed signs about whether recent measures have stemmed a steep rise in coronavirus infections.

Government scientific advisers say there are some signs the increase has begun to level off since a three-tier virus risk system of restrictions came into effect, but that it’s too soon to be certain.

A large chunk of northern England, including the major cities of Liverpool, Manchester and Sheffield, has been placed in the top tier of “very high” risk, with pubs closed and people from different households barred from mixing.

The regional disparities are causing friction between local politicians in the north and Prime Minister Boris Johnson’s Conservative government, which has been accused of not doing enough to support people and businesses hit by the local lockdowns.

PARIS — Virus patients now occupy more than half of France’s intensive care units, and some doctors are urging tougher restrictions after another record jump in confirmed infections.

Dr. Jean-Francois Delfraissy, head of the government’s virus advisory body, expressed surprise Monday at the “brutality” of the rise, after more than 52,000 new cases were reported Sunday.

Speaking on RTL radio, he floated the idea of local lockdowns or extending France’s 9p.m. to 6 a.m. curfews, which currently cover about half of the country and more than two-thirds of its people.

The number of people hospitalized in France with the virus has climbed sharply in recent weeks, putting renewed pressure on ICUs. COVID patients now fill more than two-thirds of the ICUs in the Paris region.

Dr. Eric Caumes, head of the infectious and tropical diseases department at Paris’ Pitie-Salpetriere Hospital, told broadcaster Franceinfo on Monday that “we have lost control of the epidemic, though it doesn’t date from yesterday.”

France has been among countries hardest-hit by the pandemic, reporting 34,761 virus-related deaths. It is currently registering more than 340 positive cases per 100,000 people nationwide each week.

Delfraissy warns this latest wave of the virus could be “stronger than the first” and is spreading all around Europe.

ZAGREB, Croatia — Croatia’s minister of tourism has tested positive for coronavirus as the small European Union nation continued to report a surge in the new cases.

The state Hina news agency said Nikolina Brnjac tested positive after attending a government meeting Saturday. Croatian Prime Minister Andrej Plenkovic tested negative, the government said Monday, but two ministers who sat next to Brnjac have been told to self-isolate.

Experts in Croatia and some neighboring countries blame the country’s summer tourist season for the rise in new cases, saying there was little or no distancing on Adriatic beaches or in nightclubs.

On Saturday, Croatia’s daily number of new infections soared past 2,000 for the first time.

From 11:59 p.m. on Nov. 8, Melbourne residents will no longer be restricted to traveling within 25 kilometers (16 miles) of their homes.

Melbourne, the nation’s second-largest city, has been under strict lockdown measures since early July and the state government has been accused of inflicting unnecessary economic damage by not relaxing restrictions sooner.

The last time Victoria recorded a 24-hour period without a single case was June 9 before a second wave of infections began. A week has passed since Victoria lost a life to COVID-19. The death of a man aged in his 90s on Oct. 19 brought the state’s death toll to 817. Only 88 people have died with COVID-19 elsewhere in Australia.

Victoria’s daily infection tally peaked at 725 in early August.

COLOMBO, Sri Lanka — Sri Lanka’s Parliament has closed for the premises to be disinfected after COVID-19 was diagnosed in a police officer serving there.

A coronavirus cluster that began among garment factory workers earlier this month has grown to 4,400 cases, more than half the country’s total of 7,872. The death toll climbed to 16 on Sunday.

During the last 24 hours, 351 new patients have been detected and the majority are from the garment cluster.

To contain the spread, the government has closed schools and banned gatherings across Sri Lanka, and a curfew is in effect in many parts of Western province, where the infections have been concentrated.

Several thousand people have been asked to quarantine at home, while another 8,421 people are being quarantined at military-run centers.

NEW DELHI — India’s coronavirus cases are continuing to decline but rising air pollution and Hindu festivals are raising fears of new infections.

The Health Ministry said 45,148 new cases have taken India’s tally to 7.9 million on Monday. It also reported 480 deaths in the past 24 hours, raising total fatalities to 119,014.

The Indian capital is seeing an upsurge with nearly 4,000 new cases, the highest in the past five weeks. Experts expressed concern over severe air pollution caused by farm fires, exhaust from diesel generators, dust from construction sites and burning of garbage.

“When you have high levels of air pollution you will see an increase of severe COVID-19 infections,” said Dr. Randeep Guleria, a government health expert.

The southern coastal state of Kerala is the second-worst state for active cases in the country. India’s Health Minister Harsh Vardhan blamed “gross negligence” during the 10-day Onam festival in late August.

BERLIN — The head of the United Nations says that “the Covid-19 pandemic is the greatest crisis of our age.”

UN Secretary-General Antonio Guterres opened an online session Sunday of the World Health Summit with a call for worldwide solidarity in the global crisis and demanded that developed countries support health systems in countries that are short of resources.

The coronavirus pandemic is the overarching theme of the summit, which originally had been scheduled for Berlin. Several of the leaders and experts who spoke at the opening stressed the need to cooperate across borders.

“No one is safe from COVID-19. No one is safe until we are all safe from it,” said German President Frank-Walter Steinmeier. “Even those who conquer the virus within their own borders remain prisoners within these borders until it is conquered everywhere.”

More than 42 million have been infected with the virus and over 1 million people have died of Covid.

ROME — Italy’s one-day caseload of confirmed coronavirus infections jumped past 20,000 on Sunday, with more than a quarter of the new cases registered in Lombardy, the northern region which bore the brunt of the pandemic in the country earlier this year.

According to Health Ministry figures, there were 21,273 new cases since the previous day, raising Italy’s total of confirmed COVID-19 infections to 525,782.

Health Minister Roberto Speranza said the government’s latest crackdown on social freedoms, including closing restaurants in early evening and shuttering gyms, for the next 30 days, was warranted by the growth of the contagion curve worldwide, with a “very high wave” in all of Europe.

“Every choice brings sacrifices and renouncing” activities, Speranza said. “We must react immediately and with determination if we want to avoid unsustainable numbers.”

Italy’s confirmed death toll in the pandemic rose to 37,338, with 128 deaths since Saturday.

190 Indian Bank Locations Raided in Massive Fraud Crackdown

India’s Central Bureau of Investigation has reportedly raided over 190 bank locations nationwide in an effort to crack down on fraud involving at least 15 banks. As many as 1,000 officers were involved in what is now one of the largest coordinated searches in India this year.

Major Bank Fraud Crackdown

The Indian Central Bureau of Investigation (CBI) carried out searches at over 190 bank locations nationwide on Tuesday, local media reported. An officer of the bureau said that the CBI has registered 42 new scam cases involving Rs 7,200 crore (~$1 billion). PTI reported that CBI’s teams knocked on doors of the accused to collect evidence and question them, elaborating:

The operation was spread across 16 states and union territories with as many as 1,000 officers of the agency involved in it making it one of the largest coordinated searches this year.

“Out of these 42 cases, there are 4 cases of more than Rs 1,000 crore and 11 cases of fraud amount of Rs 100 crore to Rs 1000 crore,” a statement issued by the CBI reveals. “During searches, incriminating documents have been recovered so far.”

The statement further details that most of the loan fraud cases were detected at 15 banks, including the State Bank of India (SBI), the country’s largest government-owned lender. The other banks involved were Andhra Bank, Oriental Bank of Commerce, Indian Overseas Bank, Allahabad Bank, Canara Bank, Dena Bank, Punjab & Sind Bank, Punjab National Bank, Central Bank of India, Union Bank of India, IDBI Bank, Bank of Baroda, Bank of Maharashtra, and Bank of India, Business Today detailed.

The searches were conducted throughout Tuesday, most of which were in Maharashtra where 58 bank locations were raided, followed by Punjab with 32 branches searched. Other searches were carried out in New Delhi, Tamil Nadu and Madhya Pradesh, Uttar Pradesh, Andhra Pradesh, Chandigarh, Kerala, Telangana and Dadra and Nagar Haveli, Gujarat and Haryana, Karnataka and Uttarakhand, and West Bengal.

PMC Bank Fraud Case Continues

A recent, widely-publicized fraud case involving one of the largest cooperative banks in the country is still ongoing. Punjab and Maharashtra Cooperative (PMC) Bank, with 137 branches in multiple states, was put under regulatory restrictions by the Reserve Bank of India (RBI) in September, limiting customer withdrawals to 100 rupees for six months.

The withdrawal limit has been increased several times, following protests and court petitions. On Nov. 5, the central bank raised the limit to 50,000 rupees, but some depositors are still battling with the central bank to gain access to all of their money. In the PMC fraud case, bank officials allegedly created more than 12,000 fictitious accounts to cover up bad loans worth approximately Rs 4,300.

Last year, India’s second-biggest state-controlled lender, Punjab National Bank, was hit by a $2 billion fraud scandal. The RBI said in June that over 6,800 cases of bank fraud involving an unprecedented Rs 71,500 crore (~$10 billion) have been reported in 2018-19.

Doctors at a hospital here have used a new drug called RLF-100, also known as aviptadil, that has led to rapid recovery from respiratory failure in critically ill COVID-19 patients, reports PTI.

The drug has been approved by the FDA for emergency use at multiple clinical sites in patients who are too ill to enter the FDA's Phase 2/3 trials.

Eight patients in the ICU ward of a private COVID-19 designated hospital died after fire broke out there on Thursday, news agency PTI reported.

Fire broke out at the Shrey Hospital in Navrangpura area of Ahmedabad during the early hours of Thursday, an official said.

Around 40 other COVID-19 patients at the facility were rescued and shifted to a civic hospital in the city, he said. The cause of the fire is not known yet, he said.

Zydus Cadila announced Wednesday that its plasmid DNA vaccine to prevent COVID-19, going by the name ZyCoV-D, was found to be both safe and well-tolerated in healthy volunteers in Phase I human trials, which began on 15 July 2020. The vaccine candidate will begin Phase II trials on Thursday, 6 August, the company said.

Virus testing in the US is dropping, even as deaths mount

US testing for the coronavirus is dropping even as infections remain high and the death toll rises by more than 1,000 a day, a worrisome trend that officials attribute largely to Americans getting discouraged over having to wait hours to get a test and days or weeks to find out the results.

An Associated Press analysis found that the number of tests per day slid 3.6 percent over the past two weeks to 750,000, with the count falling in 22 states. That includes places like Alabama, Mississippi, Missouri and Iowa where the percentage of positive tests is high and continuing to climb, an indicator that the virus is still spreading uncontrolled.

Novavax signs Covid-19 vaccine supply deal with India's Serum Institute

Novavax Inc said it has entered a supply and license agreement with the Serum Institute of India for the development and commercialization of its COVID-19 vaccine candidate reports Reuters.

The Indian drugmaker will have exclusive rights for the vaccine in India during the term of the deal and non-exclusive rights during the "Pandemic Period" in all countries other than those designated by the World Bank as upper-middle or high-income countries.

Coronavirus LATEST Updates: Fire broke out at the Shrey Hospital in Navrangpura area of Ahmedabad during the early hours of Thursday. Around 40 other COVID-19 patients at the facility were rescued and shifted to a civic hospital in the city.

India's COVID-19 infection tally raced past 19 lakh on Wednesday, two days after it crossed the 18 lakh, with 52,509 people testing positive for the novel coronavirus in a day. However, the recoveries have also increased to 12,82,215, according to the Union Health Ministry data.

The recovery rate among COVID-19 patients has risen to 67.19 percent while the fatality rate has further dropped to 2.09 percent, the data stated.

The total coronavirus cases in the country surged to 19,08,254 while the death toll climbed to 39,795 with 857 people succumbing to the disease in a span of 24 hours, the data updated at 8 am showed.

There are 5,86,244 active cases of coronavirus infection in the country presently which contribute 30.72 percent of the total caseload.

This is the seventh consecutive day that COVID-19 cases have increased by more than 50,000.

According to the Indian Council of Medical Research (ICMR), a cumulative total of 2,14,84,402 samples have been tested up to 4 August with 6,19,652 samples being tested on Tuesday.

Meanwhile, three Indian drug manufacturers on Wedensday launched Favipiravir, an antivral which has been approved by the Drug Controller General of India (DCGI) for restricted emergency use in COVID-19 patients.

While Lupin's Covihalt costs Rs 49 per tablet in India, BDR Pharmaceuticals's BDFAVI will be available at a price of Rs 63 per tablet. Alkem Laboratories too launched ''Alfluenza'', but the cost per tablet is not available.

Case fatality rate drops to 2.09%

A total of 51,706 patients have recuperated from COVID-19 in India in a span of 24 hours, the highest in a day, while the case fatality rate has dropped to 2.09 percent, according to the health ministry.

"With an increasing number of COVID-19 patients recovering, there has been a 63.8 per cent increase in the recovered cases in the last 14 days," the ministry said in the statement.

With 6,19,652 tests conducted on Tuesday, the cumulative testing as on date has reached 2,14,84,402.

The tests per million have seen a sharp increase to 15,568.

SC suggests cheaper treatment in smaller towns

The Supreme Court has suggested that there should be cheaper treatment for COVID-19 infected patients in smaller towns and asked the Centre to consider the aspect of prompt release of claims by insurance companies which cannot keep their hands off at the time of pandemic.

The apex court was told by the Centre that health is a state subject and primary responsibility in terms of management of COVID-19 rests with the states.

In a report filed in the top court, the Centre proposed that states "may on a priority basis arrive at fair rates for private hospitals to ensure that there is no over-charging from COVID-19 patients or indiscriminate hikes for patients".

A bench headed by Chief Justice SA Bobde, which was hearing the pleas including the one filed by advocate Sachin Jain who has sought direction for regulating the cost of treatment of COVID-19 at private hospitals across the country, suggested that there should be cheaper treatment for coronavirus infected patients in smaller towns.

The top court made the suggestion after counsel appearing for private hospitals said that uniform price capping may not be feasible as it may vary in big and smaller cities.

State-wise figures

Of the 857 fresh deaths reported by the central health ministry on Wednesday, 300 are from Maharashtra, 110 from Karnataka, 108 from Tamil Nadu, 67 from Andhra Pradesh, 54 from West Bengal, 39 from Uttar Pradesh, 25 from Gujarat, 20 from Punjab, 17 each from Rajasthan and Bihar, 13 from Telangana, 12 each from Madhya Pradesh and Delhi and 10 from Jammu and Kashmir.

Nine fatalities have been reported from Odisha, eight each from Chhattisgarh and Haryana, six from Assam, five from Uttarakhand, four from Goa, three from Kerala and Jharkhand, two each from Andaman and Nicobar Islands, Puducherry and Tripura, while Chandigarh has recorded one fatality each.

Of the total 39,795 deaths, Maharashtra has reported the maximum at 16,142 followed by 4,349 in Tamil Nadu, 4,033 in Delhi, 2,704 in Karnataka, 2,533 in Gujarat, 1,817 in Uttar Pradesh, 1,785 in West Bengal, 1,604 in Andhra Pradesh and 962 in Madhya Pradesh.

So far, 732 people have died of COVID-19 in Rajasthan, 576 in Telangana, 462 in Punjab, 448 in Haryana, 417 in Jammu and Kashmir, 347 in Bihar, 216 in Odisha, 128 in Jharkhand, 115 in Assam, 95 in Uttarakhand, 87 in Kerala.

Chhattisgarh has registered 69 deaths, Goa 60, Puducherry 58, Tripura 30, Chandigarh 20, Himachal Pradesh 14, Andaman and Nicobar Islands 12, Ladakh and Manipur seven each, Meghalaya and Nagaland five each, Arunachal Pradesh three, Dadra and Nagar Haveli and Daman and Diu two and Sikkim one.

90% of recovered patients in Wuhan suffering from lung damage: Report

Ninety percent of a sample group of coronavirus-recovered patients from a prominent hospital in China's Wuhan city where the pandemic broke out have reported lung damage and five percent of them are again in quarantine after testing positive for the virus, according to a media report on Wednesday.

A team at the Zhongnan Hospital of Wuhan University led by Peng Zhiyong, director of the hospital's Intensive Care Unit, has been conducting follow-up visits with ''100 recovered patients'' since April.

The first phase of this one-year programme finished in July. The average age of the patients in the study is 59.

According to the first phase results, 90 percent of the patients'' lungs are still in a damaged state, which means their lungs ventilation and gas exchange functions have not recovered to the level of healthy people, state-run Global Times reported.

Peng's team conducted a six-minute walking test with the patients. They found that the recovered patients could only walk 400 metres in six minutes while their healthy peers could walk 500 metres in the same period.

Some recovered patients have to rely on oxygen machines even three months after being discharged from the hospital, Liang Tengxiao, a doctor from the Dongzhimen Hospital, Beijing University of Chinese Medicine, was quoted as saying by the report.

Ayushman Bharat - Pradhan Mantri Jan Arogya Yojana

Background

In the General Budget 2018-19, the Government announced a major initiative in health sector- Ayushman Bharat programme aimed at making path breaking interventions to address health holistically, in primary, secondary and tertiary care systems, covering both prevention and health promotion. Ayushman Bharat has 2 components-

Health and Wellness Centre: As envisioned in The National Health Policy, 2017, 1.5 lakh centres will provide comprehensive health care, including for non-communicable diseases and maternal and child health services. These centres will also provide free essential drugs and diagnostic services.

National Health Protection Scheme: cover over 10 crore poor and vulnerable families (approximately 50 crore beneficiaries) providing coverage upto 5 lakh rupees per family per year for secondary and tertiary care hospitalization. Pradhan Mantri Jan Arogya Yojana is launched as this component.

7.1. Features of the Scheme

Beneficiary Identification: PMJAY primarily targets the poor, deprived rural families and identified occupational category of urban workers’ families, 8.03 crore in rural and 2.33 crore in urban areas as per the latest Socio-Economic Caste Census (SECC) data for both rural and urban areas as well as the active families under the Rashtriya Swasthya Bima Yojana (RSBY). There is no cap on family size and age as well as restriction on pre-existing conditions.

71st Round of National Sample Survey Organization (NSSO) has found

85.9% of rural households and 82% of urban households have no access to healthcare insurance/assurance.

More than 17% of Indian population spend at least 10% of household budgets for health services.

Catastrophic healthcare related expenditure pushes families into debt, with more than 24% households in rural India and 18% population in urban area have met their healthcare expenses through some sort of borrowings.

Hospitalization cover from inpatient care to post hospitalization care:

The Yojana will provide a coverage up to Rs. 5,00,000 per family per year, for secondary and tertiary care hospitalization through a network of Empanelled Health Care Providers (EHCP). The services will include 1350 procedures covering pre and post hospitalization, diagnostics, medicines etc.

Universality: One unique feature of the PMJAY is its national portability once fully operational. The beneficiaries will be able to move across borders and access services across the country through the provider network seamlessly. The beneficiaries will not need a special card. Their Aadhaar numbers will suffice.

Implementation agency: The National Health Agency (NHA) will provide overall vision and stewardship for design, roll-out, implementation and management of Pradhan Mantri Jan Arogya Yojana (PM-JAY) in alliance with state governments.

Alliance with the States: The Scheme is principle based rather than rule based -

It allows enough flexibility to states in terms of packages, procedures, scheme design, entitlements as well as other guidelines while ensuring that key benefits of portability and fraud detection are ensured at a national level.

The States will have the option of implementing this scheme through a Trust model or Insurance Company based model, though the Trust model will be preferred.

States have the option to use an existing Trust/Society or set up a new Trust/Society to implement the Scheme as State Health Agency and will be free to choose the modalities for implementation.

Centre’s contribution being 60 per cent and the states’ 40 per cent.

For giving policy directions and fostering coordination between Centre and States, it is proposed to set up Ayushman Bharat National Health Protection Mission Council (AB-NHPMC) at apex level Chaired by Union Health and Family Welfare Minister.

IT based: In partnership with NITI Aayog, a robust, modular, scalable and interoperable IT platform will be made operational which will entail a paperless, cashless transaction.

Fraud detection and Data privacy: NHA Information Security Policy & Data Privacy Policy are being institutionalized to provide adequate guidance and set of controls on the secure handling of Beneficiaries Personal Data & Sensitive Personal Data in compliance with all laws and regulations applicable.

Pradhan Mantri Aarogya Mitra (PMAM): The scheme is creating a cadre of certified frontline health service professionals called Pradhan Mantri Aarogya Mitras (PMAMs) who will be primary point of facilitation for the beneficiaries to avail treatment at the hospital and thus, act as a support system to streamline health service delivery.

7.2. Significance of the PMJAY

Road to universal health coverage: According to NITI Aayog, the scheme would increase the public spending on health to around 4 per cent from approximately 1 per cent at present, thereby, dramatically improving provision of healthcare for the poor.

Catalyst for transformation: It will be an enabler of quality, affordability and accountability in the health system.

The empaneled hospitals have been tasked to follow the treatment guidelines. Patient outcomes will be monitored.

Another impact of the PMJAY will be rationalisation of the cost of care in the private sector. With an increase in demand created, it is expected that private sector will move from a low volume-high return paradigm to a high volume-fair return (and higher net profit) model.

The earnings of public hospitals under PMJAY will be available for their upgradation and also for incentivising the provider teams as these funds will be deposited with the Rogi Kalyan Samitis. Up to 30 per cent of the overall public spending on the scheme may return to public sector institutions.

Poverty-reducing measure: Each year, six to seven crore people, above the poverty line, fall below it because of health-related expenses. PMJAY would reduce this number significantly. More than a third of the out-of-pocket expenditure (around Rs 5,000 per household) is due to inpatient hospitalisations. One out of eight families have to incur health expenditure of more than 25 per cent of the usual household expenditure each year. PMJAY will ease this burden on the poor.

Employment Creation: The scheme will create lakhs of jobs for professionals and non-professionals — especially women. It will give a boost to the health technology industry.

NHPM will subsume the on-going centrally sponsored scheme “Rashtriya Swasthya Bima Yojana” (RSBY) and Senior Citizen Health Insurance Scheme (SCHIS).

Concerns ahead

NITI Ayog estimates Rs 12,000 crore will be required to run the scheme. However, allocation of just ₹2,050 crore during the current year to the PMJAY cannot provide the promised cover to the large population sought to be included. Not all States and Union Territories are in a position to raise their own share, and a few have not even joined the scheme. The challenge of funding, therefore, remains.

Health is a state subject under Indian constitution. State governments should regulate the hospital sector under the Clinical Establishments (Registration and Regulation) Act. The law broadly provides for standardisation of facilities and reasonable rates for procedures. Costs are a contested area between the care-providers and the Centre, and many for-profit hospitals see the government’s proposals as unviable.

Union government scheme covers only the deprived beneficiaries as per the caste census, thus bringing down the number of people to be covered. But many schemes implemented by states have a wider range of beneficiaries. For instance, Karnataka’s health insurance scheme covers all the residents of the state. This leads states hesitating adopting PMJAY.

Sustainability of insurance companies has to be ensured. According to the Insurance Regulatory and Development Authority of India’s data on incurred claims ratio (premium earned versus pay outs) for government-sponsored health schemes, it went up from 87 per cent in 2012-13 to 122 per cent in 2016-17. In case of PMJAY, the government has set a premium of Rs 1,050. Insurance companies find this amount very low to provide coverage. This would be more of an issue in states like Kerala where claims ratio is quite high.

Though hospital care is a big expenditure, it is still a very small percentage of people’s Out of Pocket (OoP) expenses on healthcare. People spend much more on illnesses that don’t need hospitalisation and are not covered under insurance. NSSO 2014 round shows there was no relief from increasing health expenditure w.r.t 2004.

Along with Insurance model focus must be on strengthening the country’s health infrastructure. It would have a long-term impact. Globally, countries with adequate public health facilities have been successful in implementing health insurance schemes. e.g. Thailand, extensively focused on strengthening the public health infrastructure before rolling out its Universal Coverage Scheme in 2001.

Conclusion:

The “best health care at the lowest possible cost” should be: inclusive; make health-care providers accountable for cost and quality; achieve a reduction in disease burden, and eliminate catastrophic health expenditures for the consumer. Ayushman Bharat – Pradhan Mantri Jan AarogyaYojana (AB-PMJAY) is a paradigm shift from sectorial, segmented and fragmented approach of service delivery through various national and State schemes to a bigger, more comprehensive and better converged and need based service delivery of secondary and tertiary care.

SC suggests cheaper treatment in smaller towns The Supreme Court has suggested that there should be cheaper treatment for COVID-19 infected patients in smaller towns and asked the Centre to consider the aspect of prompt release of claims by insurance companies which cannot keep their hands off at the time of pandemic. The apex court was told by the Centre that health is a state subject and primary responsibility in terms of management of COVID-19 rests with the states. In a report filed in the top court, the Centre proposed that states "may on a priority basis arrive at fair rates for private hospitals to ensure that there is no over-charging from COVID-19 patients or indiscriminate hikes for patients". A bench headed by Chief Justice SA Bobde, which was hearing the pleas including the one filed by advocate Sachin Jain who has sought direction for regulating the cost of treatment of COVID-19 at private hospitals across the country, suggested that there should be cheaper treatment for coronavirus infected patients in smaller towns. The top court made the suggestion after counsel appearing for private hospitals said that uniform price capping may not be feasible as it may vary in big and smaller cities. They found that the recovered patients could only walk 400 metres in six minutes while their healthy peers could walk 500 metres in the same period.
