Pain Assessment
According to the dictionary pain is a physical feeling caused by disease, injury or something that hurts the body. It also caused by mental and emotional suffering.
Pain Assessment is needed to make the correct diagnosis and to determine the most effective treatment plan for clients experiencing pain.
OLDCART can be used for assessing the client’s pain easily. OLDCART stands for:
O - onset
L - location
D - duration
C - characteristic
A - aggravating factor
R- relieving factor
T - treatment
It is a must for us to know when did the pain start, where it came from and to where it is going, how long does it occur, what factors make it more worse, what factors that lessens the pain and what will be the treatment in order for the pain to go away.
We can also use the 0-10 pain scale for our clients to rate their pain.
0 - no pain
1
2 - mild pain; 1 2 and 3
3
4 - moderate
5 - moderate
6 - severe
7
8 - very severe; 7 8 and 9
9
10 - worst pain
For the deaf and mute the Wong-Baker’s face Pain rating Scale is used. It is a pain scale that was developed by Donna Wong and Connie Baker. The scale shows a series of faces ranging from a happy face at 0, "No hurt" to a crying face at 10 "Hurts worst". The patient must choose the face that best describes how they are feeling.
