2.3k | angst, drinking irresponsibly If Simon could do it all again (part 1) (part 8/end)
“You don’t look good, sir.” The sergeant stood at attention, looking straight into his lieutenant’s eyes.
Simon had to commend the balls of Kevlar required to walk right up to him to point the fact out unprompted, but that was why he liked Sgt. Eric Jefferies the most. You had no time to waste when you raced with death on the regular - he would tell anyone they didn’t look good.
He knew he didn’t - it was the same bland face he had the pleasure to look at in the mirror each day. Annoyed, but not surprised by the darkening circles under his eyes, stark against his pale complexion. It didn’t help that he nicked himself in the jaw shaving that morning.
“Dining hall, sergeant,” he grunted.
“You’re barely eating, Riley,” Lt. Ramsay said, the same bloke who’d catch him sneaking back to his room. “You know you’re contributing to the food waste when you don’t ask for seconds, yeah?”
It was true, and the table chuckled, but Simon continued to shove whatever was on his plate into his mouth. It was enough to not starve.
“He never leaves his room anymore, not even on the weekends,” another lieutenant quipped, but was promptly elbowed by the officer next to him.
That, too, was true.
Simon had nowhere else to be, like how it always was before his mum came to Hereford. These days his flat was too empty and cold with the hole in his chest. He never came back after that night.
It wasn’t like he was thriving in his quarters either, but it was still a little better – at least it was untouched by you. Though his nights were dreamless at first, he kept waking, and waking until the dreams started.
It was a glitch in the universe, wasn’t it? That the memory that played in his mind to insanity was the last time he saw you, about crawling back to your door with limbs that didn’t feel like his, vision swaying with the lights, coming on and off, his heartbeat ringing in his head.
It’s not supposed to end this way… I want to try…
He sighed at another disturbed night. Tea would slow his mind. Instead, he found the box of Darjeeling you gifted him to take back to base. ‘So we can have the same tea over the phone,’ you’d said.
Was there a way to escape you, make you stop haunting? He needed an exorcism.
He put it back in his drawer. One day, it wouldn’t have to hurt anymore.
And the nightmares came back. It was once, then twice, and thrice a week of waking up in cold sweat in the dark.
Simon’s performance slipped. There was a reason sleep deprivation was a popular torture method. He requested sleeping medications - his career was the last thing he had and he wasn’t about to let it go. Any unrestful sleep interrupted by the vivid images his sickly mind conjured up was still better than no sleep at all.
Quitting you was impossible when the thoughts still followed. If pushing you away didn’t work, maybe basking in the memories would, even if it hurt more. Aching for your warmth, the scraps of it, he’d go anywhere you’d been to see your ghost. The pain was better than the void.
“You lads are volunteering at the soup kitchen this Saturday,” he announced to Sgt. Jefferies after hours.
“Saturday, sir?”
“It’s good for you. Reminds you why you’re doing all this.”
“Can’t tell me what to do,” he teased. “You’re not my L.T. on the weekends.”
Simon’s stare didn’t waver and the other bloke’s smile dropped.
“Copy, sir. I’ll tell the others.”
When the four burly SAS soldiers entered the kitchen, chatter and clanks stalled as all eyes turned to them.
“May… May I help you young lads?” one of the middle-aged ladies said.
Simon recognised her from his last visit, but he quickly realised this was a silly idea. He was out of place, knowing no one there.
He flashed half a smile. “Just wanted to give a hand. Got any lifting to do?”
The lieutenant and his sergeants hauled the food items to the kitchen, including the bread which he taught his sergeants to half and butter. They were offered to peel potatoes, but Simon decided it was wise to leave it to the pros instead.
People still avoided his gaze while his boys exchanged pleasantries with the other volunteers; Eric even got called handsome by the group of older ladies he impressed with his strength as he hefted the sack of potatoes. While the night was as pleasant, it wasn’t the same if you weren’t there to hold his hand and laugh at his jokes.
When the boys invited Simon to the pub at the end of the night, he said no. He thought he was ready, but even after weeks, coming back to his flat was just as sickening.
The silence pierced. Despite all the lights flicked on, the place made his skin crawl, the space too vast and empty. But he didn’t become a lieutenant from succumbing to his emotions.
As he lay in bed, he recalled that you too slept there once. That the mattress once dipped with the gentle weight of you, but unlike the bed that bounced back, you’d left a lasting imprint that disfigured his soul.
Simon wondered what you were up to, if you knew he was there drowning, miserable in his cold room. He couldn’t decide if he preferred your door to be closer or further: closer so he could catch a glimpse of you without meaning to, or further so he wouldn’t be so tempted to go over and get on his knees.
You said begging only reduced you to nothing, but for you, he’d beg and beg. There wasn’t much to lose when he wasn’t much to begin with. He was a stray for a reason.
He tossed and turned, and was granted a wink of sleep before the same bloody dream flashed in his mind.
I don’t care how hard it gets…
He sat up, feet thudding on the floor as he rubbed his face with a heavy sigh. It was always that one moment, like a broken record. Why couldn’t it be you on a night out, or kissing you on the kitchen counter, or simply, you smiling? It was a curse. If only the heart could follow where one’s feet went.
With no plans on coming here, his sleeping pills lay on his desk at base. He looked through the cabinets to distract himself, finding various bottles of dusty, unopened spirits he was gifted. They weren’t his cup of tea.
So he packed, to get his mind off you, from spiralling and digging a deeper grave for itself.
It was time for a change. With the accommodation he was provided, he never needed to rent, but he did anyway in case his mum ever needed the place. It was a good call he did, but with the divorce on the way, keeping it was pointless. He’d rather spend the extra money on his mum and nephew.
Yes, he came to remember- not to forget, but you wouldn’t leave, would you? In the dead of night, when he pulled the hoodie he’d forgotten about out of his wardrobe, he decided he’d had enough of his bloody flat and drove back to base.
He still had another weekend to before his next deployment, a two-month mission. He’d finish packing then.
“You’re right, sir, it feels good volunteering.” Eric grinned at his lieutenant. “We’re going again tomorrow. Also one of the ladies is introducing her daughter to Sam. See you there then?”
Never again. “Dining hall, sergeant.”
Simon was a fool for not finishing his lunch sooner and bolting, instead lingering for the announcement. With how atrocious he did on his tests, he must have been beyond high to still hope for a miracle, that despite everything, he still had a chance at a promotion.
He didn’t make to the top 3.
Amidst the wishes from the table, Lt. Ramsay’s turned to him. His grateful smile faltered.
Simon’s fists clenched. It was supposed to be him, his. But who was he to be mad. It was the fruit of his incompetence. He knew this was coming. Things were going to shit. The unforgiving truth was staring right at him mercilessly: he had nothing else.
He left for his office.
“Sir, sir!” Sgt. Jefferies called. “We’re heading to the pub tonight. Come with us.”
He gritted his teeth. Word travelled too fast.
“Let’s get out of the base for a bit,” he continued when he caught up to his long strides. “It’s the last weekend before we ship out.”
Simon eyed the display of vibrant bottles behind the bar as he listened to his sergeants’ orders, the names foreign to him. Above, the telly showed a rugby match rerun no one paid attention to.
“Jefferies, how much you reckon it takes me to get pissed?”
He chuckled. “You, sir? At least 10,” he said before taking a swig of his beer.
“Nah, 15 sounds more like it.” Richie, the designated driver for the evening piped up.
Sam downed his first two shots, hissing as he slammed the glasses on the bar. “Agreed. Do you know how much he lifts?” He nodded at Simon’s biceps, bulging under his loose black shirt.
It was a genuine question. Simon didn’t want to get pissed, he only wanted to forget. He didn’t mean to go over his limit he had no idea was at seven.
Drunk Simon was a weeping, blabbering mess. It didn’t help that he was massive, because his sergeants had trouble getting him to the car before Richie drove him to the address of his flat he barely managed to gurgle out before passing out.
“Sir, you’re paying for the bloody cleaning if you get sick in my car!”
Why did he think this was a good idea? He was never a drinker, barely even touched alcohol socially. It was the poison that turned his dad into a demon, and it too became his downfall. The only thing he thought he would always have – his resolve, let him down too. He’d lost you, his mum whom he was supposed to protect, his future, and now his dignity.
Desperation was a lethal sentiment.
And that dream came again, that he stumbled to your door. Legs wobbly, his vision in and out as the world spun in slow motion.
“Luv… Luv, it’s not supposed to end like this,” he slurred, the same line he always opened with.
A marionette, a prisoner in his own head, it was a loop he couldn’t escape. The awful show had to commence to end the same way each time.
“I’m sick of losing and I wouldn’t know what to do when you leave, after how much you’ve given. Instead, I left when you needed me. I should have been there for you, gone through all this with you, no matter how hard it got.
“If you would give me a chance, I’ll quit the SAS. I’d start all over again. I’ll butcher the carrots and apples with the bloody peeler, I’ll let the steakhouse mess up our reservation and eat a dozen soapy tacos… If you ever show up at my door with your pie again, I swear I’d kiss you, not scare you. And I’ll never let go. If it has to hurt, I want it to be you.”
The door clicked open, and like how it always went, it meant the dream was coming to an end.
“You make it worth it,” he muttered as his vision faded.
…
Simon gasped for air, this time staring up at blinding lights. He shielded his wet eyes, chuckling to himself.
“Bloody hell, I think I’m sick on the inside.”
“Only your past, but you are not your past.” Your voice echoed in the distance.
His body was too heavy to move. “Could you forgive me, for all of this?”
“Could you? You need to forgive more than you need to be forgiven.”
He laughed as another tear slipped.
Simon woke on his couch, still in his clothes from the night before. Dreaming of you always drained him, leaving him hollow and out of touch with his body.
He sat up with a groan, rubbing his face as the dizziness settled. He didn’t remember much after getting dragged to Richie’s car. Judging by the gnarly bruise on his arm, he probably fell last night, but he was glad he found his way back to his flat in one piece.
Stumbling to the shower, he hissed when his toe stubbed one of the boxes on the floor. It was a horrendous decision to drink so much, still having to pack the rest of his stuff. He leaned over the sink, staring at his bloodshot eyes.
His sergeant was right. He didn’t look good. He never did. What the fuck are you doing to yourself, Riley?
With his hair damp, he made his way to the kitchen. As he realised he’d packed all his tea stash in one of the bloody boxes, a series of knocks echoed in his flat.
He grumbled. It better be important for someone to disturb his peace, especially with the pounding of his head. He couldn’t be bothered putting a shirt on before he swung the door open.
It was you, a pie in hand like the first time he met you all those months ago.
“Hi, is Simon in?”
His heart lurched as he crushed you in a hug.
“Thought you said you were going to kiss me.”

okay fine i'm falling back into the hole i'll summarise all my doctor whomst aus again
clara - canon. victorian clara au where she travels with the doctor after surviving the snowmen, otherwise following the events of canon (with eps set in her home life translated to the victorian era).
molly - bf canon. eight's companion, an irish maid-turned-wwi VAD. mostly set either during her travels or back in her own time once she stops travelling with him, when she's living in the doctor's house in london, a full nurse, and helping people out there. always alien things in london, even in 1918.
georgie - seconded to UNIT from the regular army after her courageous actions in horrifically traumatic situations, sgt lane is a combat medic, and on the front line of every awful alien event that seems to happen in the uk, because it's constant and it's all terrible. wants to leave the army, UNIT is declining, she's heavily traumatised, and she knows any time the doctor appears, something awful is about to happen. also has a similar au for third doc's time stuck on earth with UNIT in the '70s.
sidney - not technically an au; she's set in the future of our universe (which i can give more details about if necessary). an android with a gun for an arm, roller skates for feet, a cybernetic eye, and a love of grenades, dressed like a 1950s diner waitress, sid was originally designed to be an any-purpose droid. first and only of her line, has sentience and free will that she's still trying to figure out what it means. her canon is chaos and would cross over with dw very well (let her have some fun tardis adventures i Beg).
pippa - she had a marvellous time ruining everything. a regency era Eligible Young Lady (nonbinary if they had the word for it ty) who stole a time machine to escape a suffocating, miserable life, with absolutely no idea how any of it works. they love chaos, disrupting order and the way society is "meant" to be. keeps going back in time to change what's meant to be a fixed point so their best friend doesn't die, keeps destroying the fabric of the universe in the process and the universe keeps killing him so they do it again, and knows it bc they are time sensitive. they're just both selfish and terrified of being alone.
bea - from a future human post-apocalyptic colony, she has psychic abilities she is afraid of and doesn't know how to use. loves drawing, the stars, herbalism, and taking things apart to see how they work. travelling on her own when she likely stumbles across the doc + companion.
nate - UNIT captain with terrible commanders who seem to actively want the aliens to kill his platoon, fully begins operating by his own (lawful/neutral good) morals and not by what he's told to protect people, including refusing to fire. definitely leaves UNIT before it's disbanded but still actively ends up at scenes of Shit Happening to help people. might end up working with people who've had their lives displaced by Alien Shit? or even helping non-violent aliens stuck on earth in a very non-torchwood way. he'd hate torchwood.
#the first two obv aren't aus but my clara's a lil au and a lot of people haven't listened to big finish (which is valid i've only#listened to a few alksjdhf molly just wormed her way into my heart)#also any historical/modern muses can just be Found in their own settings#i wanna come up w smth for my wot gals but til i get specifics like. magic is just power and powers exist in dw

Police in El Paso, Texas, say a second person has been taken into custody following a shooting at a shopping mall Wednesday that left one person dead and three other people wounded.
Interim police chief Peter Pacillas said that Cielo Vista Mall was still considered a crime scene, and that it would remain locked down until authorities had completed their investigation.
Pacillas stressed that the danger had passed.
“There is no more danger. I want to repeat that: There is no more danger to the public,” Pacillas said.
The two people in custody and as well as the victims are males, Pacillas said. THIS IS A BREAKING NEWS UPDATE.
AP’s earlier story follows below. EL PASO, Texas (AP) — Police in El Paso, Texas, say one person was killed and three other people were wounded in a shooting Wednesday in a shopping mall.
One person has been taken into custody, El Paso police spokesperson Sgt. Robert Gomez said. No immediate information was given about that person.
Police were looking for another person who could have been involved, Gomez said. No description was given.
A weapon was recovered at the scene, Gomez said, but he could not provide more information about that.
“It’s too early to speculate on motive,” he said.
The three who were wounded were taken to local hospitals, Gomez said.
University Medical Center in El Paso said in a statement that two male gunshot victims were being treated there. They were in critical condition. The condition of the third victim was not immediately known.
Gomez said police believe the scene is secure and that officers are sweeping through the whole mall to verify that.
“This is a large scene,” Gomez said. “It’s going to take some time to clear the mall.”
Authorities have set up a reunification center at a nearby high school.
Police earlier said the shooting was reported at the shopping mall’s food court.
Wednesday’s shooting at the Cielo Vista Mall happened in a busy shopping area and across a large parking lot from a Walmart where 23 people were killed in a racist attack in 2019.
The United States has seen dozens of people killed in mass shootings so far in 2023, most recently Monday at Michigan State University, where three students were killed and five more were wounded. In January, 11 people were killed in the Los Angeles-area city of Monterey Park as they welcomed the Lunar New Year at a dance hall popular with older Asian Americans.
In 2022, more than 600 mass shootings occurred in the U.S. in which at least four people were killed or wounded, according to the Gun Violence Archive.

Canadian police arrested eight teenage girls who know each other through social media for allegedly "swarming" a homeless man on the streets of Toronto and brutally stabbing him to death.
Three 13-year-old girls, three 14-year-old girls, and two 16-year-old girls have each been charged with second-degree murder in the deadly stabbing of a 59-year-old man living in the homeless shelter system shortly after midnight Sunday, according to an announcement by the Toronto Police Service on Tuesday.
Police withheld the names of the eight minor suspects under Canada’s Youth Criminal Justice Act, which prevents their publication. The name of the victim was initially held pending next of kin notification.
At a press conference Tuesday, Det. Sgt. Terry Browne of the Homicide and Missing Persons Unit declined to describe the group of girls as a gang, saying they carried out a "swarming-type behavior."
TWO ARRESTED AT NYC COUNCILMAN'S APARTMENT IN ‘GAYS AGAINST GROOMERS’ PROTEST OVER DRAG QUEEN STORY HOUR
"From what we gathered so far, is that they met each other through social media. They come from varying parts of the city. That’s to say they’re not from one specific geographic location. We don’t know how or why they met on that evening or why the destination was downtown Toronto," Browne said. "We don’t know how long they’ve been acquainted together with each other. But I wouldn’t describe them as a gang at this point."
"What they are alleged to have occurred that evening would be consistent with what we would traditionally call a swarming or a swarming-type behavior," he added.
A Toronto resident, the victim had only been living in the shelter system since late fall of this year.
"He does have a very supportive family in the area, so I wouldn't necessarily call him homeless, maybe just recently on some hard luck," Browne said.
Three of the suspects have had prior contact with Toronto Police Services, but the other five had no prior contact with police whatsoever, Browne said. "A number of weapons" were secured from the girls at the time of arrest, Browne added, declining to go into more details at that time.
At approximately 12:17 a.m. Sunday, police responded to a call for an assault in the York Street and University Avenue area. Emergency medical personnel were initially flagged down by a group of individuals reporting the stabbing. The man was transported to the hospital by medics with life-threatening injuries. He was pronounced dead a short time later at the hospital. The eight teen girls were subsequently arrested nearby, police said.
They appeared at Old City Hall for a 10 a.m. court hearing later Sunday and remanded into custody. Their next court appearance is scheduled for Dec. 29. Police are seeking assistance from anyone who had contact with the suspects that night and are trying to obtain for any video surveillance footage that might have captured the brutal attack.
The suspects are believed to have first arrived in the area around 10:30 p.m. Saturday and are believed to have been involved in an earlier altercation in the time leading up to the fatal stabbing.

PLEASE tell me about the epic of rexamesh this sounds so fascinating
Oh goodness! My fan fic is more like an epic saga, or a book series. Every chapter is like 10-15k words. It covers the adventures of my OC General Lara Lin and her relationship to the clones. Hope this helps!!
General Lara Lin - a character made of a fusion of LOTR and GOT, the source of the living and cosmic Force, immortal, a warrior goddess created to maintain the Balance in the universe as a whole, not just one galaxy. She primarily rides a dragon into battle, though she also has an old warhorse. She counts her home world as Earth because she was first sent to Middle Earth at the time of her creation and has served ever since. Married Captain Rex, dated Echo, had a small tryst with Fives due to exigent circumstances (and secretly might even love him more than Rex which is like a photo finish type race). She is the leader of Dragon Company, well respected and well believed by the Jedi Council to be the most powerful entity in the universe.
Personality: laid back
Jaded
Dry, witty humor
Enjoys sending prank supply orders to General Windu because the two of them can’t stand each other
Leader, completely loyal to her troopers and the clones as a whole
Beautiful, tall as the clones, long brown curly hair with blonde/silver highlights
Luminous blue eyes that reflect the power that resides within
Excellent fighter with two swords - short and long, quarter staff and dagger, guns and hand to hand
Fiercely independent, intelligent
Deeply loving, possibly to her detriment
OC LT Primer (promoted to Captain following the end of Order 66) - completely loyal first in command to General Lin, over the Dragon Company.
Personality: careful
Discerning
In love with Lara
Fiercely loyal to both his General and his men
Smart, savvy, quick thinking
Steadfast, serves as a pillar to Lara’s life
OC Sgt Boost - 1st SGT of Dragon Company
Personality: a typical Sgt, great with herding people
Rule follower but able to think outside the box
Dry humor
OC Chief Medical Officer Volte - first medical officer of Dragon Company, trained both by the GAR and personally by General Lin to treat just about any type of medical emergency. Learned in the first response of many galaxies including Earth. Becomes a community doctor following the events of Order 66, caring for the men and families of Dragon Company on their clone colony.
Personality: kind, caring
Highly emotionally intelligent
Loyal to his General, becomes one of her best friends.
Savvy medical professional, steadfast
Smart but not arrogant.
Somewhat haunted by the people he couldn’t save, the choices he could have made, the skills he could have used but didn’t.

November 6, 2024: Upright Nine of Swords
How Can I help myself and my loved ones cope with the feelings of grief we are having today, after feeing a loss due to the election?
Wow.. my deck thinks she’s cute.
The Upright Nine of Swords is a card that represents PTSD and nightmares associated with it. There are multiple examples of people who have suffered from this sort of anguish and trauma (myself included), but to keep this pop-culture and fandom focused, I chose Bucky Barnes, the Winter Soldier from the Marvel Cinematic Universe.
Note: I have not read many comics with him, don’t sue me. That doesn’t make me admire the stories any less.
If you don’t know SGT Barnes, he is the best friend of Captain America, a soldier, and a WWII Prisoner of war who is captured and brainewashed into a ultimate, super-soldier killing machine known as “the Winter Soldier,” only brought out of cryofreeze to commit murders upon murders (including some of his friends).
There was a whole “Communist Vibe” with his story, and it took 573 films and a Wakanda vacation to cure him, but once he was deprogrammed, Bucky had lingering memories of his assassinations. The two images are of him as the assassin and of him post-deprogramming, waking from a nightmare.
Now, I drew Bucky Upright.
In an upright Nine of Swords situation, the trauma may be something that is not often talked about, but is a daily recurring nightmare. Even if it has been years, or decades, we cannot get the situation out of our head. If this is what the card is saying, then you need to talk about it… preferrably with a licensed medical professional to talk out any PTSD issues, as neither my deck nor I are licensed counselors. In absence of a medical professional, reach out to friends and loved ones. They are invaluable Resources.
If there is no trauma, the Nine of Swords is most likely talking about general anxiety, and advising that you concentrate on the things you can control.
As always, seek Help.
Since I asked about helping my friends and loved ones, I need to be aware of any of my friends having anxiety during the next several weeks/etc about this election outcome, and be an ear to listen.
Reversed, the nine of swords still indicates the trauma, anxiety, or nightmares of his upright counterpart, even the idea of reliving the experiences. However, as he shows up upside down in that situation, we see someone trying to get past the problem and move on.
There is much work to be done.

Genetic Therapies: A New Horizon
One of the most promising areas in DMD treatment is the development of genetic therapies. These therapies aim to address the root cause of the disease by targeting the faulty gene responsible for producing dystrophin, a protein crucial for muscle health.
Exon-Skipping Therapies
Exon-skipping is a technique that uses synthetic compounds to skip over faulty parts of the dystrophin gene, allowing for the production of a functional, albeit shorter, version of the protein. Sarepta Therapeutics has been at the forefront of this innovation with its drug, eteplirsen (Exondys 51). Approved by the FDA in 2016, eteplirsen targets exon 51 and has shown promising results in stabilizing the disease's progression. Following eteplirsen, Sarepta has developed additional exon-skipping therapies targeting different exons, broadening the treatment scope for more patients.
Gene Editing and Therapy
Gene editing technologies, such as CRISPR-Cas9, have opened new avenues for potentially curing genetic disorders like DMD. In a landmark development, researchers at the University of Texas Southwestern Medical Center successfully used CRISPR to correct the dystrophin gene mutation in a mouse model of DMD. This breakthrough paves the way for human clinical trials, potentially providing a permanent solution to the disease.
Additionally, companies like Solid Biosciences and Pfizer are making strides in gene therapy, which involves delivering a functional copy of the dystrophin gene to muscle cells. Solid Biosciences' SGT-001 and Pfizer's PF-06939926 are among the leading candidates in this space, with both therapies showing encouraging early clinical trial results.
Regulatory Milestones and Support
The U.S. Food and Drug Administration (FDA) has played a crucial role in accelerating the development and approval of DMD therapies. The agency's commitment to supporting innovative treatments is evident through various regulatory designations and expedited approval pathways.
Orphan Drug Designation
Several DMD therapies have received orphan drug designation from the FDA, providing incentives such as tax credits for clinical testing, fee waivers, and market exclusivity upon approval. This designation is vital for encouraging pharmaceutical companies to invest in developing treatments for rare diseases like DMD.
Accelerated Approval Pathway
The FDA's accelerated approval pathway allows for the early approval of drugs based on surrogate endpoints that are reasonably likely to predict clinical benefit. This pathway has been instrumental in bringing DMD therapies to market more quickly, offering patients earlier access to potentially life-changing treatments. Eteplirsen, for example, was approved under this pathway based on its ability to increase dystrophin production.
Patient Advocacy: A Driving Force
Patient advocacy groups have been indispensable in driving DMD research and treatment progress. Organizations like Parent Project Muscular Dystrophy (PPMD) and the Muscular Dystrophy Association (MDA) have tirelessly worked to fund research, support clinical trials, and advocate for policies that benefit the DMD community.
Funding Research and Clinical Trials
PPMD and MDA have collectively raised millions to support research initiatives and clinical trials. Their efforts have helped bridge the funding gap, enabling researchers to explore innovative treatment approaches and bring them closer to clinical application.
Advocacy and Policy Change
Advocacy groups have successfully influenced policy changes that facilitate better access to treatments and care for DMD patients. For instance, PPMD's advocacy efforts were instrumental in securing the passage of the MD-CARE Act, which has significantly increased federal funding for muscular dystrophy research.
Real-World Impact and Future Directions
The advancements in genetic therapies, coupled with regulatory support and advocacy efforts, are transforming the DMD treatment landscape. However, the journey is far from over, and several challenges remain.
Access and Affordability
One of the significant challenges is ensuring that these advanced therapies are accessible and affordable for all patients. The high cost of genetic treatments poses a barrier, and there is a need for strategies to address pricing and reimbursement issues.
Long-Term Efficacy and Safety
While early results from clinical trials are promising, the long-term efficacy and safety of these treatments remain to be fully understood. Ongoing and future studies are crucial to monitoring patients over extended periods and gathering comprehensive data on the outcomes.

Comprehensive Care
Beyond genetic therapies, comprehensive care involving multidisciplinary teams of healthcare providers is essential for managing DMD. This approach includes physical therapy, cardiac care, respiratory support, and nutritional guidance to improve quality of life and extend longevity.
Conclusion
The Duchenne Muscular Dystrophy landscape in the United States is witnessing a period of unprecedented innovation and hope. Advances in genetic therapies, robust regulatory support, and the unwavering dedication of patient advocacy groups are driving significant progress. While challenges persist, the collective efforts of researchers, clinicians, regulators, and advocates pave the way for a brighter future for those affected by DMD. As the field continues to evolve, the goal remains clear: to find a cure and improve the lives of all individuals living with Duchenne Muscular Dystrophy.

What Do I Owe the Dead of My Generation’s Mismanaged Wars?
The New York Times Opinion Guest Essay, published May 26, 2024
By Phil Klay
Fairfield University MFA Professor
Mr. Klay is a novelist and a Marine Corps veteran of the Iraq War. His most recent book is the essay collection “Uncertain Ground: Citizenship in an Age of Endless, Invisible War.”
About 10 years ago, as the war in Afghanistan was slowly, painfully winding down, I walked through Arlington National Cemetery with a fellow Marine veteran and a relative of mine visiting from Ireland. We passed row after row of pristine white tombs, the dead of all the just wars and unjust wars that made and remade this country, and my relative told us he found it quite moving; he hadn’t been expecting that. Perhaps he thought it’d be more bombastic, or obviously militaristic, and he was taken by the beauty and serenity and quiet dignity of the place.
So we brought him to Section 60 to see some of the newest graves, of kids born in the ’90s, and I told him the sight filled me with rage, these young lives thrown into a mismanaged war, where even their deaths, at that late stage, were mostly ignored. Just the background hum of a global superpower.
A couple of years later, in 2021, the Afghan war finally ended, taking with it a few American children of the 2000s, and, in a moral failure laid on top of the military failure, leaving tens of thousands of Afghans who worked with us at risk in the now completely Taliban-controlled country. The last Marines to fall died in a suicide bombing at a gate to Kabul’s airport, a blast that killed 11 Marines, one Navy medic, one soldier and about 170 Afghan civilians. The Marines were trying to manage the chaos of the poorly planned evacuation of Afghans from Kabul — a humanitarian mission at heart, trying to help those we were abandoning. A week before she died, one of the Marines, Sgt. Nicole Gee, posted a photo of her cradling a baby in Kabul and captioned it, “I love my job.”
America responded to those deaths with a drone strike against a Kabul vehicle the military claimed was transporting ISIS members who were about to carry out another attack, but that, in a twist that felt grotesquely emblematic of so many of our failures, turned out to carry an Afghan aid worker. The blast killed the aid worker and his relatives, seven of whom were children. The sort of people those Marines died trying to help.
How do you memorialize the dead of a failed war? At Arlington, it’s easy to let your heart swell with pride as you pass certain graves. Here are the heroes that ended slavery. Here are the patriots who defeated fascism. We think of them as inextricably bound up with the cause they gave their life to. The same can’t be said for more morally troubling wars, from the Philippines to Vietnam. And for the dead of my generation’s wars, for the dead I knew, the reasons they died sit awkwardly alongside the honor I owe them.
I watched a lot of Marines go off to Afghanistan, a war that I could have gone to but that I chose to avoid. Mostly, they were young. That’s the thing Hollywood most often gets wrong about war when they cast grown men to portray America’s finest killers. Look at a Marine infantry platoon, so many of whose members joined at 17 or 18, and you see boys. Boys who haven’t grown into cynicism yet. Some find it in the middle of their tours. Some keep that idealistic flame burning through multiple deployments. And some die before it can be extinguished.
For so many of the kids I saw, their mission mattered to them, and so their mission should matter to all of us when we remember their deaths. And the mission was a catastrophe. Memorial Day should come with sorrow and patriotic pride, yes, but also with a sense of shame. And, though it has faded for me over the years, with anger.
A few months after Kabul fell I went to the Bronx to see a war photographer I admire, Peter van Agtmael, taking a group of adult learners through a display of his photography from 9/11 to the present at the Bronx Documentary Center, photographs now collected in the book “Look at the U.S.A.”
“I just got back from Afghanistan, and it’s controversial to say, but it’s beautiful,” he told the group. “It’s beautiful to see Afghanistan at peace.”
Beautiful. I thought of a Marine in 2009, just back from Afghanistan, hollow-eyed, telling us in a monotone about his best friend taking a bullet to the head in these beautiful regions of the country, now at peace. What would he make of such a claim? Around me on the walls I saw a burned soldier in a combat hospital, the arm of a Trump supporter climbing over a wall by the Capitol on Jan. 6, the dust cloud of an improvised bomb detonation in Iraq.
Toward the end of the gallery, there was a huge print hung high up. You craned your neck and saw a homeless encampment in Las Vegas, and then, craning further, you saw an F-16 fighter jet, an aircraft that costs tens of millions of dollars, flying above. Amid our national forgetting of the wars, there was something powerful about seeing this accounting of America in the South Bronx, in a community whose struggles have so often been subject to forgetting, effacing, indifference. And, God, it was painful.
In the past when I’ve thought about the recent dead, I’ve told myself that service to country, service unto the point of death, is a momentous enough sacrifice to overshadow all other questions. The cause doesn’t matter so much if the fallen I knew served courageously, looked after their fellow Marines and kept their honor clean. But I’ve come to feel that airbrushing out the complexities of their wars is, ultimately, disrespectful to the dead. We owe it to the dead to remember what mattered to them, the ideals they held, as well as how those ideals were betrayed or failed to match reality.
This Memorial Day, as I get ready to take my sons to march in our local Memorial Day parade, our country is in the midst of the most divisive antiwar protests since the early days of the Iraq war, protests my friends characterize as either “objectively pro-Hamas” or as “opposing undeniable genocide.” Questions long dormant, about how we use our might and whom we help kill, feel like live political questions once again (even if we’re not talking much about actual American military deployments, or the troops who have most recently died at the hands of Iranian proxies). The debate is raw and angry.
Good. What a good, uncomfortable, painful national mood for remembering the dead. This year, when I remember them, I will not just remember who they were, the shreds of memory dredged up from past decades. I will remember why they died. All the reasons they died. Because they believed in America. Because America forgot about them. Because they were trying to force-feed a different way of life to people from a different country and culture. Because they wanted to look after their Marines. Because the mission was always hopeless. Because America could be a force for good in the world. Because Presidents Bush, Obama, Trump and Biden didn’t have much of a plan. Because it’s a dangerous world, and somebody’s got to do the killing. Because of college money. Because the Marine Corps is cool as hell. Because they saw “Full Metal Jacket” and wanted to be Joker. Or Animal Mother. Because the war might offer a new hope for Iraq, for Afghanistan. Because we earned others’ hatred, with our cruelty and indifference and carelessness and hubris. Because America was still worth dying for.
