Abhinaya Yeddala's plan to become a family doctor and build a life in Nova Scotia is coming to fruition — but she says she nearly gave up and left the province because of a requirement that added more than two years to the process.
Yeddala, originally from India, is now a family medicine resident at a clinic in New Glasgow. She completed her undergraduate degree in the United Kingdom before moving to Malaysia to begin medical school.
But she had to wait almost two and a half years before taking the next step in her career. This is because there is an obligation in most Canadian provinces that she obtain permanent resident status before applying for medical residency.
She said that the tedious and lengthy process almost drove her away. It was very stressful as well as frustrating," Yeddala said. There was this fear of what if I got what I wanted in Canada. What do I do then?
According to the Canadian Resident Matching Service, every province is responsible for its additional eligibility criteria. Currently, every region except Quebec accepts only Canadian citizens or permanent residents.

"Everybody who graduated with me except [one student] left," she explained. "That's 18 doctors we lost in Nova Scotia, so that's pretty unfortunate." Though she is already through the most involved process, Yeddala said she wants to speak up for others. She said the current healthcare crisis in the province and the family doctor shortage made it challenging to seek change.
There's a crisis, and when everybody's talking about it, it's the right time to talk. This is because everybody is listening. Yeddala said it's easier for international students to begin their residency in the U.K. or the U.S. In the U.S., once a student is matched with a residency position, they get help working on their immigration status.
It's very different from here because they want to know their immigration status first. This is a catch-22 since if you do not have a job, your immigration status will not be approved.

Perrault said staff from the Office of Healthcare Professionals Recruitment are happy to speak to medical school graduates interested in working in the province's healthcare system.

'I was working nights in a call centre': Third Dalhousie-trained doctor says residency rules have to change
Another foreign-born Nova Scotia doctor has come forward to flag the strict residency rules he says are driving some willing young physicians away.
"They're just not doing enough," said Dr. Matthew Kumar on a short lunch break as he completes his medical residency at the hospital in Inverness, N.S.
Originally from Malaysia, he began his studies at the International Medical University before moving to Nova Scotia to complete them at Dalhousie University in Halifax.
Like others, the roadblock came when it was time to find a residency in the province he would come to love: out of the question unless he was a citizen or a permanent resident.
"And I took up a job as a research aid with the surgical department at the QE II, and I was working nights at a call centre, just up until I could get my permanent residency status," said Kumar, saying it took about two years to complete the process.
This is the third physician CTV News has spoken with, all having taken the same career path and offering the same complaint.
"As far as I know, a lot of them are either turning sights to the United States, the U.K. or going back to their own home countries. So, you have Canadian medical graduates who would prefer to stay there, but are unable to," said Dr. Taha Khan, a resident physician in Boston, Mass., who spoke to CTV News last Tuesday.
Dalhousie is the only current Canadian partner for International Medical University in Malaysia, but the school told us last week the long-running agreement is now winding-down with the final three students expected in the class of 2025.
"Rather than making meaningful policy changes and increasing the number of residency spots for graduates, they decided to can the program, which is like chopping off your arm because your thumb hurts," said Kumar.
Residency spots are sorted out online through a platform called "The Canadian Resident Matching Service" -- CARMS, for short.
The website notes applicants "must also be a Canadian citizen or hold a valid Canadian permanent resident card" but the CEO told CTV News it has nothing to do with setting the criteria, which is left to the provinces and their individual medical schools.
"They collectively determine the policies for that particular province and that medical school," said John Gallinger from his office in Ottawa.
While the health minister declined an interview, Khalehla Perrault, a spokesperson for Nova Scotia's Department of Health and Wellness, responded in a late-day email to CTV News.
"Residency seats and criteria for CARMS are established by each of the provinces. In Nova Scotia, eligible students who graduate from medical seats that are funded by the province have a pathway to residency training. Some international students are eligible for these seats. Nova Scotia also recently expanded a designated pathway to residency for 10 international medical graduates studying outside of Canada, with priority given to those with a connection to Nova Scotia," said Perrault.
"While we can’t speak to individual cases, we do know circumstances differ for each student. Staff from the Office of Healthcare Professionals Recruitment is happy to speak to medical school graduates interested in working in our province’s health-care system.
"Nova Scotia does have a high demand for our residency program and has been very successful in filling our residency spots. However, we are still very committed to attracting and retaining more health-care professionals to our province and will continue to explore how we can maximize opportunities," said Perrault.
A senior official in Dalhousie’s Faculty of Medicine was also unavailable for interviews Monday.
The experience has been disappointing for Kumar, who admits he considered going home, where his skills would be appreciated -- and put to work right away.
"Ultimately, it's the ineffective bureaucracy of the province and the university. Both of them share that burden," he said.

The one who didn't get away: Dal-trained med grad says residency rules have to change
A day after hearing from a Dalhousie-trained physician who says she was forced to look to Boston for her residency, a classmate who was successful in staying says the rules have to change.
Dr. Abhinaya Yeddala, or Abhi to her friends, is originally from India, and started her med school training in the International Medical University (IMU) in Malaysia.
She graduated with classmate Dr. Taha Khan in 2019.
While Khan wound-up heading south, Yeddala stuck it out in Nova Scotia - an option that only opened up to her after she obtained permanent residency status.
Without a job, getting it was a struggle - even if you're a trained doctor in a province that's desperate for them.
"I was ready to go anywhere at that point. I really wanted to be a doctor," said Yeddala Wednesday.
"I wanted to be a doctor in Nova Scotia and wanted to stay back here, so my first choice was always Nova Scotia and then New Brunswick."
In the end, Yeddala took on some research work for a couple of years before getting her permanent residency status and her first medical residency in Inverness, N.S.
"I should say that I was very close to leaving because I was applying for the States and I was applying to the U.K., if this didn't work out this year for me," she said.
Now, she's in New Glasgow, N.S., but she and her husband consider Halifax home - especially with a baby on the way.
If all of this seems complicated, it is.
After CTV News flagged the issue Tuesday, Nova Scotia's Office of Healthcare Recruitment responded with an emailed statement, saying Ottawa "currently requires international students training to become a doctor to have their permanent resident status to be matched with a residency opportunity in Nova Scotia. We know there is high demand for residency opportunities in this province, including Nova Scotian students studying at Dalhousie University. But the province is reaching out to find out more about this situation."
"We should do more to make it easier for people like her to stay," said Sean Fraser, Canada's minister of Immigration, Refugees & Citizenship, on Tuesday.
"This particular rule isn't the result of a particular federal piece of legislation but a practice baked into the national matching residency program, I understand."
"If they really wanted to, they could drop the rule wanting the PR [permanent residency]," said Yeddala. "So, CaRMS [the Canadian Residency Matching Service] has the authority to say, 'It's OK, as long as you have a work permit and a valid immigration status, you can work here.'"
Whether there's enough residency seats is another matter entirely, but Yeddala says the current rules are shutting too many out.
"I know multiple international graduates who left Nova Scotia after trying for multiple years because they don't have their entire life to dedicate just to get into residency," she said.
"There's so many international medical graduates who are not in the same boat as me. They go through extensive training, they're very well trained doctors in other countries, but when they come here, they have to go through a whole new process, and there's no guarantee they'll actually get matched. It's not an easy process in Canada. We make it so hard for international doctors who actually want to work here."

Why study computer science?

Typical International Baccalaureate requirements: 34 points. Typical A-levels requirements: ABB, with good grades in maths and computing. Typical IELTS requirements: 6.5 overall in IELTS Academic, with no lower than 6.0 in any of the 4 sections. If you haven’t studied in an IB or A levels school, you have to convert your scores to those scales in order to prove that you match the level.
