About the Special Rapporteur on extreme poverty and human rights

I. Introduction

1. I have spent the past two weeks visiting the United States, at the invitation of the federal government, to look at whether the persistence of extreme poverty in America undermines the enjoyment of human rights by its citizens. In my travels through California, Alabama, Georgia, Puerto Rico, West Virginia, and Washington DC I have spoken with dozens of experts and civil society groups, met with senior state and federal government officials and talked with many people who are homeless or living in deep poverty. I am grateful to the Trump Administration for facilitating my visit and for its continuing cooperation with the UN Human Rights Council’s accountability mechanisms that apply to all states.

2. My visit coincides with a dramatic change of direction in US policies relating to inequality and extreme poverty. The proposed tax reform package stakes out America’s bid to become the most unequal society in the world, and will greatly increase the already high levels of wealth and income inequality between the richest 1% and the poorest 50% of Americans. The dramatic cuts in welfare, foreshadowed by the President and Speaker Ryan, and already beginning to be implemented by the administration, will essentially shred crucial dimensions of a safety net that is already full of holes. It is against this background that my report is presented.

3. The United States is one of the world’s richest, most powerful and technologically innovative countries; but neither its wealth nor its power nor its technology is being harnessed to address the situation in which 40 million people continue to live in poverty.

4. I have seen and heard a lot over the past two weeks. I met with many people barely surviving on Skid Row in Los Angeles, I witnessed a San Francisco police officer telling a group of homeless people to move on but having no answer when asked where they could move to, I heard how thousands of poor people get minor infraction notices which seem to be intentionally designed to quickly explode into unpayable debt, incarceration, and the replenishment of municipal coffers, I saw sewage filled yards in states where governments don’t consider sanitation facilities to be their responsibility, I saw people who had lost all of their teeth because adult dental care is not covered by the vast majority of programs available to the very poor, I heard about soaring death rates and family and community destruction wrought by prescription and other drug addiction, and I met with people in the South of Puerto Rico living next to a mountain of completely unprotected coal ash which rains down upon them bringing illness, disability and death.

5. Of course, that is not the whole story. I also saw much that is positive. I met with State and especially municipal officials who are determined to improve social protection for the poorest 20% of their communities, I saw an energized civil society in many places, I visited a Catholic Church in San Francisco (St Boniface – the Gubbio Project) that opens its pews to the homeless every day between services, I saw extraordinary resilience and community solidarity in Puerto Rico, I toured an amazing community health initiative in Charleston (West Virginia) that serves 21,000 patients with free medical, dental, pharmaceutical and other services, overseen by local volunteer physicians, dentists and others (WV Health Right), and indigenous communities presenting at a US-Human Rights Network conference in Atlanta lauded Alaska’s advanced health care system for indigenous peoples, designed with direct participation of the target group.

6. American exceptionalism was a constant theme in my conversations. But instead of realizing its founders’ admirable commitments, today’s United States has proved itself to be exceptional in far more problematic ways that are shockingly at odds with its immense wealth and its founding commitment to human rights. As a result, contrasts between private wealth and public squalor abound.

7. In talking with people in the different states and territories I was frequently asked how the US compares with other states. While such comparisons are not always perfect, a cross-section of statistical comparisons provides a relatively clear picture of the contrast between the wealth, innovative capacity, and work ethic of the US, and the social and other outcomes that have been attained.

By most indicators, the US is one of the world’s wealthiest countries. It spends more on national defense than China, Saudi Arabia, Russia, United Kingdom, India, France, and Japan combined.

US health care expenditures per capita are double the OECD average and much higher than in all other countries. But there are many fewer doctors and hospital beds per person than the OECD average.

US infant mortality rates in 2013 were the highest in the developed world.

Americans can expect to live shorter and sicker lives, compared to people living in any other rich democracy, and the “health gap” between the U.S. and its peer countries continues to grow.

U.S. inequality levels are far higher than those in most European countries

Neglected tropical diseases, including Zika, are increasingly common in the USA. It has been estimated that 12 million Americans live with a neglected parasitic infection. A 2017 report documents the prevalence of hookworm in Lowndes County, Alabama.

The US has the highest prevalence of obesity in the developed world.

In terms of access to water and sanitation the US ranks 36th in the world.

America has the highest incarceration rate in the world, ahead of Turkmenistan, El Salvador, Cuba, Thailand and the Russian Federation. Its rate is nearly 5 times the OECD average.

The youth poverty rate in the United States is the highest across the OECD with one quarter of youth living in poverty compared to less than 14% across the OECD.

The Stanford Center on Inequality and Poverty ranks the most well-off countries in terms of labor markets, poverty, safety net, wealth inequality, and economic mobility. The US comes in last of the top 10 most well-off countries, and 18th amongst the top 21.

In the OECD the US ranks 35th out of 37 in terms of poverty and inequality.

According to the World Income Inequality Database, the US has the highest Gini rate (measuring inequality) of all Western Countries

The Stanford Center on Poverty and Inequality characterizes the US as “a clear and constant outlier in the child poverty league.” US child poverty rates are the highest amongst the six richest countries – Canada, the United Kingdom, Ireland, Sweden and Norway.

About 55.7% of the U.S. voting-age population cast ballots in the 2016 presidential election. In the OECD, the U.S. placed 28th in voter turnout, compared with an OECD average of 75%. Registered voters represent a much smaller share of potential voters in the U.S. than just about any other OECD country. Only about 64% of the U.S. voting-age population (and 70% of voting-age citizens) was registered in 2016, compared with 91% in Canada (2015) and the UK (2016), 96% in Sweden (2014), and nearly 99% in Japan (2014).

About the Special Rapporteur on extreme poverty and human rights: Professor Philip Alston is the current Special Rapporteur on extreme poverty and human rights. The Special Rapporteur is an independent expert appointed by the Human Rights Council and undertakes the following main tasks: (1) conducting research and analysis to be presented in separate thematic reports to the Human Rights Council and the General Assembly; (2) undertaking country visits and reporting on the situation in those countries in relation to the concerns of the mandate; (3) sending letters to governments and other relevant entities in situations in which violations of human rights of people living in extreme poverty are alleged to have taken place.

The mandate on extreme poverty was first established in 1998 by the United Nations Commission on Human Rights, and was taken over by the Human Rights Council in June 2006. It is one of a number of mandates that together form what is known as the United Nations system of special procedures. Yoko Ono and John Lennon

Yoko Ono was born in Tokyo in 1933. Yoko's father, Eisuke came from a long line of aristocratic samurai warrior-scholars. Two weeks before Ono's birth, Eisuke was transferred to San Francisco by his employer, the Yokohama Specie Bank. The rest of the family followed two years after, with Ono meeting her father for her first time when she was two. The family moved to New York City in 1940. The next year, Eisuke was transferred from New York City to Hanoi, and the family returned to Japan. Starvation was rampant in the destruction that followed the Tokyo bombings; the Ono family was forced to beg for food while pulling their belongings with them in a wheelbarrow.

Yoko moved back again to US, this time to New York city in 1953 to pursue her career as painter, singer and songwriter. In 1956, Ono married the Japanese composer Toshi Ichiyanagi, but they filled for divorce in 1962. Ono returned home to live with her parents and was suffering from clinical depression when she was placed into a mental institution. Later that year, on November 28, 1962, Ono married Anthony Cox, an American jazz musician, film producer, and art promoter, who was instrumental in securing her release from the mental institution. She gave birth to their daughter Kyoko Chan on August 8, 1963.

Yoko met John Lennon in November 1966 at the Indica Art Gallery in London, where Ono was preparing for an exhibition of her work. By that time, Yoko was still married to Anthony Cox However, Yoko and John, instantaneously connected and began a whirlwind romance. Yoko divorced Anthony Cox and married John on March 20, 1969 in Gibraltar and they spent their honeymoon in Amsterdam. Lennon changed his name by deed poll on April 22, 1969, switching out Winston for Ono as a middle name. Although he used the name John Ono Lennon thereafter, official documents referred to him as John Winston Ono Lennon, since he was not permitted to revoke a name given at birth.

Soon after, the couple embarked on their “bagism” period which included creating a series of lithographs that depicted the pair on their honeymoon. This was one of many collaborations that Lennon and Ono would ultimately undertake. Famously, the duo staged a series of unforgettable “bed-ins,” which were akin to the sit-in form of protest popular at the time.

Ono influenced Lennon to produce more "autobiographical" output and, after "The Ballad of John and Yoko", they decided it would be better to form their own band rather than put the material out under the Beatles name. In 1969, the Plastic Ono Band's first album, Live Peace in Toronto 1969, was recorded during the Toronto Rock and Roll Revival festival. This first incarnation of the group also consisted of guitarist Eric Clapton, bass player Klaus Voormann, and drummer Alan White.

After the Beatles disbanded in 1970, Ono and Lennon lived together in London and then moved permanently to Manhattan to escape perceived tabloid racism towards Ono. Their relationship became strained because Lennon was facing the threat of deportation due to drug charges that had been filed against him in England, and because of Ono's separation from her daughter. In 2015 Yoko declared that she had a lesbian affair with Hillary Clinton when Yoko I living with John in Manhattan and Hillary was studying at Yale. Yoko and John separated in July 1973, with Ono pursuing her career and Lennon living between Los Angeles and New York with had personal assistant May Pang (Ono had given her blessing to Lennon and Pang's relationship).

By December 1974, Lennon and Pang considered buying a house together, and he refused to accept Ono's phone calls. The next month, Lennon agreed to meet with Ono, who claimed to have found a cure for smoking. After the meeting, Lennon failed to return home or call Pang. When she telephoned the next day, Ono told her Lennon was unavailable, because he was exhausted after a hypnotherapy session. Two days later, Lennon reappeared at a joint dental appointment with Pang; he was stupefied and confused to such an extent that Pang believed he had been brainwashed. He told her his separation from Ono was now over, though Ono would allow him to continue seeing her as his mistress.

On 1975, Yoko and John had a son, Sean, and John took a hiatus from the music industry and became a stay-at-home dad to care for his infant.

On the morning of of 8 December 1980 Annie Leibovitz took a portrait of Lennon and Ono. That evening Lennon was shot dead in New York City at close distance from Yoko. The perpetrator was a recently unemployed resident of Hawaii who stated that he was incensed by Lennon's lifestyle and public statements, especially his much-publicized remark about the Beatles being "more popular than Jesus".

After nearly 4Months I have now revived a letter stating that the claim has been closed on a Without Prejudice Basis. When I contacted Aviva to see what the 3rd party has not accepted full liability they said it was dealt with like this as the 3rd party who crashed into me was not responding to and calls or letters that have send him. I did see a bill for one night I spent in the emergency room when I had this awful infection and a terrible fever, and without insurance that one night would have been over $7000! With insurance I think it was like $2400. All of my treatments and doctor's offices are in posh medical facilities in affluent parts of Los Angeles like Beverly Hills and Westwood / Bel Air, so perhaps that's why my medical care was so much. I honestly don't know. I wish I knew how to support or advise this girl. She's too old to attend the support group I went to, and I don't really know that much about NH-L because it's quite different than HL. You can pick and choose your premiums/deductibles/co-pays, hunt around for what suits your needs best. The single plans especially, can be and most of the time are, affordable. I don't understand why people do not apply for basic health insurance that would cover them in an emergency, or for general routine health-care. Poverty Is Both a Political and a Moral Choice Made By the Powerful

The U.S. views itself as the exceptional nation, the beacon of liberty and justice for the world. In the popular imagination, it is the land of plenty where everyone can thrive, the land of opportunity where anybody who works hard enough can realize the dreams of their heart. But is this really so or just a comforting illusion?

For two weeks this past autumn, Professor Philip Alston, the UN Special Rapporteur on Extreme Poverty and Human Rights, sought to find out, traveling over the U.S. to assess the state of extreme poverty in this country and its impact on human rights. His travels brought him to California, Alabama, Georgia, West Virginia, and Washington, D.C., as well as Puerto Rico. His report, published in mid-December by the Office of the UN’s High Commissioner of Human Rights, pulls the curtains on the illusions this country cherishes about itself and reveals the startling truth about where we stand and where we are headed.

Wherever he went Alston met people living at the edge of survival: homeless people on Skid Row in Los Angeles; unemployed workers forced into unpayable debt; families and communities devastated by the scourge of drug addiction; people who lost all their teeth because they did not have access to dental care. In the American South he saw yards filled with sewage because state governments don’t consider it their obligation to provide sanitation. Native American tribes told him about their degrading poverty, cultural debasement, and shocking suicide rates. In Puerto Rico, decimated by Hurricane Maria, he met people living next to a mountain of coal ash, which brings them illness, disability, and death.

Numerous studies have shown that along with its wealth and technological achievements, the U.S. also stands out by its extreme levels of economic inequality. Some 40 million people—12.7% of the population—live in poverty; almost half of these, 18 million, in deep poverty, with family incomes below half the poverty line. Infant mortality rates in the U.S. have been among the highest in the developed world. Americans live shorter and sicker lives than people in other affluent democracies. On scales measuring poverty, social safety provisions, wealth inequality, and economic mobility, the U.S. comes last among the ten most well-off countries and eighteenth among the top 21. Yet lack of financial clout is not the problem, for the U.S. spends more on national defense than the next seven countries combined, has the highest expenditures on healthcare, and the highest rate of incarceration.

Alston points out that poverty has become so widespread in the U.S. because successive administrations have refused to recognize that economic and social rights are full-fledged human rights. International law affirms that everyone has the right to an adequate standard of living, including health care, food, work, and social protection in times of need. While these rights are often trampled upon by more repressive regimes, among the economically developed democracies the U.S. is virtually alone in callously disregarding them. It stands out in the extent to which it allows its citizens to suffer from food insecurity, homelessness, lack of access to affordable healthcare, and lack of suitable housing. Due to cutbacks in basic services, poverty has become a trap—a trap into which anyone can fall, and from which it is extremely hard to escape. Thus, according to Alston, “The American Dream is rapidly becoming the American Illusion as the U.S. now has the lowest rate of social mobility of any of the rich countries.”

What are the Most Common Symptoms and Signs?

The first sign of alveolar osteitis may be the partial or total loss of a blood clot in the tooth socket. The jawbone may sometimes be visible and the surrounding tissue may appear gray due to poor circulation and lack of healing.

The symptoms of alveolar osteitis may include a throbbing pain shortly after a tooth extraction. The pain may radiate to other parts of the head such as the ears and the eyes. This pain may be accompanied by bad breath or a bad taste due to an accumulation of food debris and bacteria. Sleep Apnea in Children Linked to Detrimental Brain Changes Dr. Mercola By Dr. Mercola Sleep apnea occurs when you have obstructions in your airway that interfere with your breathing during sleep. The flow of air can be partially blocked or even completely stopped altogether. Sleep-disordered breathing, a common problem, affects more than half of all men and over one-quarter of women.1 Disturbingly, it’s also becoming more prevalent among children, largely due to processed food diets and lack of breastfeeding. Sleep apnea in childhood has been linked to adverse brain changes that can affect behavior and cognition. Estimates suggest up to 5 percent of children are affected by sleep apnea.2 An even larger percentage of children may be susceptible to sleep-disordered breathing, which is characterized as snoring, noisy breathing or open-mouth breathing during sleep. Fortunately, there are ways to prevent sleep apnea from occurring in the first place, and ways to address it without having to resort to a continuous positive airway pressure (CPAP) machine. Sleep apnea and sleep-disordered breathing are related to suboptimal growth of the mouth, teeth and face. This may be prevented by exclusive breastfeeding for at least six months after birth and avoiding processed food diets. If you’re pregnant, please consider breastfeeding. Also, be sure to pay careful attention to your child’s diet during his or her early years. A few simple changes can have an incredible impact of their lifelong ability to breathe and sleep comfortably. If your child already struggles with impaired breathing, two treatment alternatives offer a great deal of hope: oral myofunctional therapy and learning how to breathe properly through the nose while awake. Breathing Problems Can Exacerbate and Promote Other Health Problems It’s important not to ignore any breathing problems your children may have, as it can have serious repercussions for their health and academic performance. According to recent research,3 children with sleep apnea display troubling brain changes in areas involved with thinking and problem solving. These areas were found to be smaller in those with sleep apnea compared to children without sleep apnea.4,5 Previous studies have found sleep apnea can produce changes in the brain by causing nerve cells to die. Studies6 have also shown that kids with sleep apnea tend to struggle more in school and perform worse in language arts, math and science compared to those who do not have sleep or breathing problems, scoring on average 11 percent lower on academic tests. In this latest study, they used magnetic resonance imaging (MRI) to analyze and compare the volume of gray matter in children with and without sleep apnea. Gray matter is the outer layer of the brain associated with high-level brain functions such as problem solving, language, memory, personality, planning and judgment. Indeed, children with sleep apnea were found to have substantially lower gray matter volume than those without sleep apnea. Paul Macey, director of technology and innovation at the Los Angeles School of Nursing and one of the authors of the study, told Reuters, "It really does seem that there is a change in the brain or that the brain is affected." Lead author Dr. Leila Kheirandish-Gozal, director of pediatric clinical sleep research at the University of Chicago, added:7 “The Images of gray matter changes are striking. We do not yet have a precise guide to correlate loss of gray matter with specific cognitive deficits, but there is clear evidence of widespread neuronal damage or loss compared to the general population.” Next, the researchers aim to find out whether treating the sleep apnea will help children to recover and regain full function. Considering what we now know about brain plasticity, chances are your child will be able to make a complete recovery once he or she starts breathing and sleeping normally. The key is treating it, ideally getting to the very root of the problem, which can often be traced back to the size and shape of the child’s oral cavity. How Size and Shape of Mouth May Contribute to Sleep Apnea While obesity is a common factor that increases your risk for sleep apnea, an increasingly common root cause is related to an improperly shaped mouth and incorrect positioning of the tongue. This is particularly true for younger generations raised on infant formula and processed foods. Dr. Weston Price's pioneering work showed how diet can affect your entire mouth, yet most people are still clueless about this effect, and how the size and shape of your oral cavity affect the placement of your tongue and your overall ability to breathe properly. According to Dr. Arthur Strauss, a dentist and diplomat of the American Board of Dental Sleep Medicine, our mouths have progressively gotten smaller through the generations due to lack of breastfeeding and not chewing enough, combined with poor childhood nutrition. Breastfeeding actually helps expand the size of your child's palate, shifting the jaw forward — two important factors that help prevent sleep apnea by creating ample room for unobstructed breathing. Tongue placement also plays an important role, as revealed in a 2015 study on pediatric patients.8 The newborn palate is as soft as a drum, and if we place into it a bottle, pacifier, a spouted cup or the child finds his or her thumb, the palate may deform, making the nasal airway smaller, as well as preventing the ability of the teeth to erupt normally. Having an abnormally short lingual frenulum9 can result in impaired orofacial growth in early childhood, reducing the width of the upper airway. The upper airway is very pliable, so this increases the risk of it collapsing during sleep. The study found that children with an untreated short frenulum developed abnormal tongue function early in life, which also impacted their orofacial growth and led to disordered breathing during sleep. The researchers suggest pediatricians and otolaryngologists should systematically examine the lingual frenulum in children exhibiting difficulties such as trouble sucking, speech impediments, snoring or other breathing problems. They also note that while removing the frenulum can be helpful, it typically will not resolve all abnormal breathing patterns, so oral myofunctional therapy, both pre- and post-surgery is recommended to restore normal breathing through the nose. Four years ago, I interviewed Joy Moeller, a leading expert in this form of therapy, about its many benefits. I’ve included that interview here for your convenience. In a meta-review10 of nine studies that included a total of 120 patients with obstructive sleep apnea, oral myofunctional therapy reduced the severity of sleep apnea by about 50 percent in adults and 62 percent in children.
