Allmänna eller spinalanestesi utförs under operationen. Anestesiologen och patienten bestämmer vilken av dessa metoder som tillämpas. Om mängden ben som är tänkt att förlängas är mindre än 5 cm, görs en enda nivå och ett snitt av två nivåer. 2 eller 3 spikar placeras på båda sidor av snittet innan benet skärs. Sedan prepareras och appliceras Holyfix enheten. Dessa processer varar i 90 minuter i förlängningar upp till 5 cm och 2-2,5 timmar i förlängningar upp till 10 cm.

Die Holyfix-methode, die seit 1997 praktisch angewandt wird, ist die verbesserte version von Wagner und De Bastiani. Diese nennt sich „Monolaterale (Ein-Nadel-Stift)-Methodik“. Im Gegensatz zur „Ilizarov-Methode“ hat die Holyfix weniger komplikationen in der ausstattung und bessere patientenversorgung. Der nagel wird ins bein einseitig außerhalb des Holyfix-geräts eingefügt. Die nägel werden in einer blutgefäße- und nerven freien zone eingesetzt, die sich am nächsten an der haut befinden und an der sicheren zone befestigt sind, so dass der nagel im bein bleibt und nicht bis zur anderen seite hin ausstreckt. Diese methode verhindert nämlich das risiko einer infektion. Holyfix wurde aus speziellen und leichten aluminium- und titanlegierungen gemacht, die gleichzeitig MRT-kompatibel sind. Dieser vorgang ist ein biomechanisches system, dass äußerst stabil und sicher ist und den patienten behandeln lässt. Der erweiterungsprozess ist sehr bequem/komfortabel und kann leicht am patienten umgesetzt werden.
