There's ghouls living in the Royal Waterways under the city I fucking guess
Humanoid species that resemble ants in their social structure and life cycle, starting off as small very much not humanoid larva, eventually pupating to one of the many adult forms. Some more humanoid than others.
Before the infection they weren't at all hostile and they consumed parasites and waste in the waterways and didn't venture outside it so the king opted to leave them the fuck alone and simply monitored the colony's growth
