What is Hiatal Hernia ?
Hiatus hernia or Hiatal hernia is a common condition affecting upto 10 to 15 percent adult population. It occurs due to enlargement or widening of the hole which lets the food pipe cross the diaphragm from the chest into the abdomen. There is weakening of the muscles around the normal hiatus(hole) which makes it enlarged.
As a resultant effect, a part of the stomach protrudes into the chest repeatedly and leads onto symptoms of reflux esophagitis or GERD. The larger the hiatal hernia, the severe is the reflux.
95 percent of hiatal hernias are sliding hiatal hernias. 5 percent are called para esophageal hernias.
Diagnosis of hiatus hernia is achieved by an endoscopy and barium meal examination. The length of the hiatus hernia is best diagnosed by esophageal Manometry.
In most cases of sliding Hiatal Hernia, patients don’t require any surgical treatment. Simple antacids serve as the treatment for the symptoms of reflux esophagitis. Surgical correction is required in extreme cases of reflux not controlled by antacids or if the patient is not tolerating antacids due to side effects. Fundoplication is the surgical treatment of hiatus hernia in which part of the stomach is wrapped around the food pipe strengthening the lower esophageal sphincter.
Paraesophgaeal hernias detected on tests require to be corrected by surgery even if the patients don’t have any complaints especially under 65 years of age. If the patient is older, it can be simply left alone untouched.

Following foods should be avoided if you have fatty liver-

Sugar: Reduce your intake of sweets. Even naturally occurring sugars from things like citrus juice and honey should be cut down.

Fried foods: Fried foods are rich in fat and sugar, including French fries, chips, chicken wings, doughnuts, etc. Thus, eating these meals causes your blood sugar to increase, which is one of the causes of NAFLD.
Saturated fat: Any form of saturated fat should be avoided, including that found in butter and red flesh. Instead, swap it out for healthy fats like cold-pressed nuts, avocado, and olive oils.
Red meat: Saturated fats are prevalent in red meats like beef and pig. If you have fatty liver, it is not advised to eat red meat as it will add to the excess fat content in the liver. If these foods are not processed properly they can cause inflammation which will worsen the condition of the liver.

What is Crohn`s Disease ?
Crohn’s disease is a chronic inflammatory condition of the gastrointestinal tract. It can affect any portion of the intestine starting from mouth till anus. It falls under the category of disorders called Inflammatory bowel disease. Ulcerative colitis is also a chronic inflammatory bowel disorder just like Crohn’s disease. Ulcerative colitis affects only colon or large intestine but Crohns can affect any part of intestine
The most often location Crohns affects is the terminal portion of small intestine (Ileum) and beginning of the colon. It affects the entire thickness of the wall of intestine. It can lead to ulcers, strictures in the intestine and can also result in intestinal obstruction rarely. Typically patients suffer from abdominal pain, bleeding, loose motions and weight loss over months to years. It can also cause anal fissures, Perianal fistulas and anal abscesses. Rarely it can also present as acute intestinal obstruction. Patients of Crohn’s disease can also suffer from extra intestinal symptoms such as joint problems, eye problems, mouth ulcers, skin manifestations, kidney stones, Osteoporosis due to bone loss.
Testing often includes a Colonoscopy and biopsy, an endoscopy and a biopsy, CT enterography, Stool calprotectin, capsule endoscopy along with basic blood work up. No single test confirms the diagnosis and the tests are repeated from time to time to assess the progress and severity of disease.
Treatment involves combination therapy of corticosteroids, Immunosupressants/immune Modulator, Biological therapy. Roughly, 20% of the patients may require surgery in their life time to manage complications of Crohn’s disease. Surgery becomes necessary if the medical therapy can no longer control the symptoms for conditions like anal fissure, fistula, abscess, intestinal obstruction. What is Fibroscan?
