They Had Mild Covid. Then Their Serious Symptoms Kicked In.

In the beginning, her fatigue was so severe that walking two or three laps around her 600-square-foot apartment would exhaust her for the rest of the day. In addition, she said that she had “really intense mood fluctuations that don’t feel like they’re mine.”“Waking up every day in this body, sometimes hope feels a little dangerous,” said Ms. Khan, who will soon start the cognitive rehab program. “I have to wonder: Am I going to recover, or am I going to just figure out how to live with my new brain?”In his job, “my clients would tell me things like a passcode or an address and I couldn’t remember it,” he said.At Mr. Palacios’s first appointment with the Northwestern clinic, “I did the cognitive tests, and I failed them all,” he said. On a return visit, he did another battery of tests, he said, “and I didn’t do so hot on that, either.”Mr. Palacios was referred for cognitive rehab at a long-established program in Chicago that helps give patients strategies to manage and improve memory, organizational and cognitive difficulties. But he didn’t go, he said, because “I completely forgot.” He plans to go now.In the Northwestern study, 43 percent of the patients had depression before having Covid-19; 16 percent had previous autoimmune diseases, the same percentage of patients who had previous lung disease or had struggled with insomnia.Experts cautioned that because the study was relatively small, these pre-existing conditions might or might not be representative of all long-term patients. “We are all seeing very small pieces of the elephant in terms of the long Covid group,” Dr. Bell said. “Some of us are seeing tail; some of us are seeing trunk.”Along with neurological symptoms, 85 percent of the patients were experiencing fatigue, and nearly half had shortness of breath. Some also had chest pain, gastrointestinal symptoms, variable heart rate or blood pressure. Nearly half of the participants were experiencing depression or anxiety.“I was cleaning my gutters and I forgot where I was, I forgot what I was doing on the roof,” Mr. Palacios said. When he remembered, he added, the idea of doing “something as simple as climbing on a ladder all of a sudden became a mountain.”Dr. Allison P. Navis, a neuro-infectious disease specialist at Mount Sinai Health System in New York City who was not involved in the study, said that about 75 percent of her 200 post-Covid patients were experiencing issues like “depression, anxiety, irritability or some mood symptoms.”Participants in the study were overwhelmingly white, and 70 percent were women. Dr. Navis and others said that the lack of diversity quite likely reflected the demographics of people able to seek care relatively early in the pandemic rather than the full spectrum of people affected by post-Covid neurological symptoms.“Especially in New York City, the majority of patients who got sick with Covid are people of color and Medicaid patients, and that’s absolutely not the patients one sees at the post-Covid center,” Dr. Navis said. “The majority of patients are white, often they have private insurance, and I think we have to figure out a little bit more what’s going on there with those disparities — if it’s purely just a lack of access or are symptoms being dismissed in people of color or if it’s something else.”In the Northwestern study, Dr. Koralnik said that because coronavirus testing was difficult to obtain early in the pandemic, only half of the participants had tested positive for the coronavirus, but all had the initial physical symptoms of Covid-19. The study found very little difference between those who had tested positive and those who had not. Dr. Koralnik said that those who tested negative tended to contact the clinic about a month later in the course of the disease than those who tested positive, possibly because some had spent weeks being evaluated or trying to have their problems addressed by other doctors.Ms. Khan was among the participants who had a negative test for the virus, but she said she later tested positive for coronavirus antibodies, proof that she had been infected.Another study participant, Eddie Palacios, 50, a commercial real estate broker who lives in Naperville, a Chicago suburb, tested positive for the coronavirus in the fall, experiencing only a headache and loss of taste and smell. But “a month later, things changed,” he said.Across the country, doctors who are treating people with post-Covid neurological symptoms say the study’s findings echo what they have been seeing.“We need to take this seriously,” said Dr. Kathleen Bell, the chairwoman of the physical medicine and rehabilitation department at the University Texas Southwestern Medical Center, who was not involved in the new study. “We can either let people get worse and the situation gets more complicated, or we can really realize that we have a crisis.”Dr. Bell and Dr. Koralnik said many of the symptoms resembled those of people who had concussions or traumatic brain injuries or who had mental fogginess after chemotherapy.In the case of Covid, Dr. Bell said, experts believe that the symptoms are caused by “an inflammatory reaction to the virus” that can affect the brain as well as the rest of the body. And it makes sense that some people experience multiple neurological symptoms simultaneously or in clusters, Dr. Bell said, because “there’s only so much real estate in the brain, and there’s a lot of overlap” in regions responsible for different brain functions.“If you have inflammation disturbances,” she said, “you can very well have cognitive effects and things like emotional effects. It’s really hard to have one neurological problem without having multiple.”In the Northwestern study, many experienced symptoms that fluctuated or persisted for months. Most improved over time, but there was wide variation. “Some people after two months are 95 percent recovered, while some people after nine months are only 10 percent recovered,” said Dr. Koralnik. Five months after contracting the virus, patients estimated, they felt on average only 64 percent recovered.The study of 100 patients from 21 states, published on Tuesday in The Annals of Clinical and Translational Neurology, found that 85 percent of them experienced four or more neurological issues like brain fog, headaches, tingling, muscle pain and dizziness.“We are seeing people who are really highly, highly functional individuals, used to multitasking all the time and being on top of their game, but, all of a sudden, it’s really a struggle for them,” said Dr. Igor J. Koralnik, the chief of neuro-infectious diseases and global neurology at Northwestern Medicine, who oversees the clinic and is the senior author of the study.The report, in which the average patient age was 43, underscores the emerging understanding that for many people, long Covid can be worse than their initial bouts with the infection, with a stubborn and complex array of symptoms.This month, a study that analyzed electronic medical records in California found that nearly a third of the people struggling with long Covid symptoms — like shortness of breath, cough and abdominal pain — did not have any signs of illness in the first 10 days after they tested positive for the coronavirus. Surveys by patient-led groups have also found that many Covid survivors with long-term symptoms were never hospitalized for the disease.A new study illuminates the complex array of neurological issues experienced by people months after their coronavirus infections.
In the fall, after Samar Khan came down with a mild case of Covid-19, she expected to recover and return to her previous energetic life in Chicago. After all, she was just 25, and healthy.
But weeks later, she said, “this weird constellation of symptoms began to set in.”
She had blurred vision encircled with strange halos. She had ringing in her ears, and everything began to smell like cigarettes or Lysol. One leg started to tingle, and her hands would tremble while putting on eyeliner.
She also developed “really intense brain fog,” she said. Trying to concentrate on a call for her job in financial services, she felt as if she had just come out of anesthesia. And during a debate about politics with her husband, Zayd Hayani, “I didn’t remember what I was trying to say or what my stance was,” she said.
By the end of the year, Ms. Khan was referred to a special clinic for Covid-related neurological symptoms at Northwestern Memorial Hospital in Chicago, which has been evaluating and counseling hundreds of people from across the country who are experiencing similar problems.
Now, the clinic, which sees about 60 new patients a month, in-person and via telemedicine, has published the first study focused on long-term neurological symptoms in people who were never physically sick enough from Covid-19 to need hospitalization, including Ms. Khan.

Florian’s life has been an uphill battle from the start — the eldest of six children, he was stuck with the duty of a caretaker from the very beginning. Whenever his parents were too busy with tending to their business, he’d end up taking care of his younger brothers, even when half the chores really shouldn’t have been done by a kid. He was an obedient child, figuring he’s got no other choice, only putting his foot down when he turned eighteen. He told his parents that he wanted to go to college, instead of, as planned for years, work at his family’s tailor shop, which, later down the line, he was supposed to take over. There were many arguments which in the end he won. Florian wanting to go his own way was definitely the start of the relationship between him and his parents deteriorating.
The plan he’s set up for himself was simple, college, med school, residency and a start of a career that would let him live independently. He was all work and no play during college, focusing on doing his best and more. As hardworking as he was, he wasn’t going to settle for anything other than perfect, no matter what. Everything seemed to be going according to plan until he was half-way through med school where he met Mauro. The whole relationship happened quickly and before either of them knew what they actually wanted, there was a child on the way and pressure from both their families to get married. So they did, even though both Florian and Mauro were close to breaking everything off. Florian was forced to drop out of school and take care of their child. Three years later there was more family on the way, a set of twins this time, which pushed Florian’s dreams even further away from him. 
It took another four years for Florian to finally put his foot down again — he left his husband and then soon enough he was divorced and free to do whatever he wanted. He started with coming out and transitioning, something that’s always been on his mind but due to his circumstances, wasn’t possible before. Going back to school was the next thing he wanted to focus on and thanks to his two brothers (only two, because the rest of his family has cut all ties with Florian by then)  who offered to help with his children and money, Florian was able to finish his studies and complete his residency. He took up a position at the local hospital and when his skills improved, he moved to Chicago where another, better job was lined up for him.
For a couple years things were good and then all of a sudden they weren’t — one of his brothers was killed and because Florian and Cesare were really close, the loss hit him very hard. Florian started working too much and then drinking too much and sometimes even mixing the two. Everything caught up to him when one night he almost operated under the influence. He was let go from his job soon after and once again, one of his brothers, the only other one who was still speaking to him, stepped in to help. Alberto offered an unconventional solution — to work with him. Florian knew that his brothers were involved in what was pretty much a gang, for years, the money he got from them couldn’t come from anything legal, considering how quickly and how much of it was coming in. And since Florian couldn’t see himself going back to work anytime soon, he figured that stitching up criminals is the next best thing. 
When Alberto told him he’s moving south, Florian was desperate to follow, afraid to lose the last of his family he had left. His kids have long moved out and got on with their lives, he was alone, there was nothing left to lose. Florian followed Alberto to New Orleans but decided to stay away from his brother’s business this time, which put a strain on their relationship. Florian managed to land himself a job at the hospital to get himself back on track and he’s been working there ever since.

Women’s Shoes Are Painful by Design
It’s not that I think wearing Toms with socks to work is a good look, per se.
I admire your d’Orsay oxfords and fun mules. But unfortunately, when it comes to shoes, my only criteria is, “Will these cause my feet to dribble blood all over my open-plan office?”
I am simply not meant to wear professional office flats. I can’t wear Payless flats, but I also can’t wear the premium, handcrafted-in-Italy, join-the-waitlist flats. I don’t even attempt heels. No matter what brand they are, what width or size, any shoes that you can’t play basketball in will, inevitably, rub Medieval-looking holes into my foot skin.
Every month, I succumb to an Instagram advertisement or a Strategist post purporting to have found the holy grail of women’s footwear: comfortable and work-appropriate. Reluctantly, my shoe-price ceiling has risen with the number of subcutaneous infections brewing on my heels. I buy them with a renewed sense of hope every time. Every time, I eagerly unwrap the shoes, slide them on in my carpeted apartment, and note how comfortable they feel—you get what you pay for, I think, smugly.
Then I hit the streets, where it all goes wrong. The unyielding little back digs itself into the area right below my Achilles tendon, then shreds it with every step. And so every time, I send boxes of bloody shoes back in the mail like a serial killer. (I will say that you do get the return policies that you pay for.)
I always just thought this was part of being female. You wake up, you apply powders to your face to make parts of it different colors, you walk to work, your feet bleed. There is simply no way to look as professional in a shift dress and Allbirds as you can in a fleece vest and Allbirds. Beauty is pain, I figured, and so is professionalism.

More women than men suffer pain from their footwear, according to podiatric surveys, and similarly, more women than men say they’ll suffer for the sake of their shoes. A few years ago, there were stories of, ahem, well-heeled women getting “pinky tucks” and toe-shortening surgery so their feet could fit their shoes, rather than the other way around.
Studies reveal the excruciating outcomes of this disparity: “In women ... those who wore good shoes in the past were 67 percent less likely to report hind-foot pain, after adjusting for age and weight,” one big paper from 2009 notes. “In men,” the study authors added, “there was no association between foot pain, at any location, and shoewear” ... possibly due to the fact that fewer than two percent of men in the study wore “bad” shoes.
My own bad-shoe wearing peaked in college. My friends and I would walk home from bars so wracked by agony that we would take our shoes off and walk on the freezing, filthy cement. It was only recently, when I was in so much pain after work that I seriously considered reliving my college days—walking down the Metro escalator barefoot and carrying a pair of blood-splattered loafers—that I decided to take this problem to a couple of podiatrists.
The foot doctors, first, validated my feelings. “Women’s shoes are tighter, they’re meant to fit tighter around the toes and the heel,” said Cary Zinkin, spokesman for the American Podiatric Medical Association. “They’re made to make your foot look as slim as possible.”
But Marlene Reid, a podiatric physician in Naperville, Illinois, told me “it’s not normal” to be julienned by your own footwear like I am on a regular basis. I was surprised to learn you shouldn’t have to “break in” shoes. They should not be painful the first few days you wear them. “If you try on a shoe that hurts from the outset, don’t buy it,” Zinkin told me.
All feet are different, and so are all shoe sizes. Any shoe that’s too loose or too tight can cause a blister, Zinkin said, and a lot of friction can actually rub away the skin, which appears to be what’s happening to me.
My feet don’t look like any of the feet that show up in a Google image search for this, but Reid suspected that I might have what’s called a “pump bump,” a bony growth on the back of the heel that makes it hard to wear pumps—and other kinds of dressy shoes, apparently. She suggested I try “heel lifts,” little insertable wedges that might hoist my bump out of harm’s way.
Both Reid and Zinkin recommended that I use moleskin, gel pads, or even lubricants—like the body glide that athletes use to keep their nipples from chafing—to help me tolerate any shoes I just can’t bear to part with. Zinkin told me that your skin tends to be more blister-prone if it’s dry, so I guess I could also try to lotion up my heels, as though I’m preparing to make a coat of my own skin.
One reason why so many women are injured by their shoes these days might be the rise of online shopping, in which you don’t know how the shoe fits till you’ve already bought it. Instead, you're supposed to shop in the store, preferably in the afternoon, after your feet have swollen to their fullest. When you shop, you’re also supposed to bring along whatever tights you wear, plus any gel pads or heel lifts you use, and to stick the whole contraption inside the shoe when you try it on. Also, most shoe stores are carpeted, Reid said, so you want to walk to an area of it that has tile or wood.

If you get the shoes home and they’re still too tight, take them to a shoemaker to stretch. (Just keep in mind that natural fabrics like leather stretch more than synthetics.) Reid debunked the common notion that flats are always better for you than heels. “Some of the flats that have been out there are really hard and don’t stretch,” Reid said. “They’re rigid. I’d rather someone wear a heel that they can tolerate than wear a flat.” She recommended the Fly London brand as a good mix of comfortable and acceptable-for-work, and while they look a little too ’90s punk for me, YMMV.
But, ultimately, Reid told me something I didn’t want to hear: I’d have to spend even more money on something that has already become a very expensive problem. “If you have the same recurring problem with multiple pairs of shoes, see your podiatrist,” she said. “They will show you how to wear shoes that work with your feet.”
Something tells me their answer is gonna be Toms—preferably with socks.

But Marlene Reid, a podiatric physician in Naperville, Illinois, told me “it’s not normal” to be julienned by your own footwear like I am on a regular basis. I was surprised to learn you shouldn’t have to “break in” shoes. They should not be painful the first few days you wear them. “If you try on a shoe that hurts from the outset, don’t buy it,” Zinkin told me.
All feet are different, and so are all shoe sizes. Any shoe that’s too loose or too tight can cause a blister, Zinkin said, and a lot of friction can actually rub away the skin, which appears to be what’s happening to me.

Julie Valdez had a bad case of “mom guilt.”
“You know, the guilt moms feel that they aren’t doing enough,” she said. “I had all this guilt that I wasn’t doing enough to balance all my kids’ needs.”
Valdez, 32, is mother to two sets of twins: Aiden and Isaiah, 4, and Augustine and Isabella, 2.
“The biggest challenge is that they all have different needs, and one of my sons is autistic and has a sensory disorder as well, so it’s a balancing act to make sure they all get what they need from me,” she said.
“Mom guilt” kept Valdez from taking care of herself. A perceived lack of time and energy meant she didn’t prioritize her nutrition, and she let her fitness slide. As a result, by the time Augustine and Isabella were born in 2016, she had reached 210 lb. at 5 foot 4.
“I wasn’t organized to plan meals ahead of time, so when I was hungry I’d just grab whatever was there, like my kids’ fruit snacks, or I’d eat out,” she said. “Especially after my second set of twins were born. I went into survival mode, so eating out was my go-to.”
She felt unhealthy and lethargic and suffered from postpartum depression. In July 2016, when her youngest twins were 6 months old, Valdez hit rock bottom.
“I went on a family vacation to Florida. … I saw a picture of myself after the vacation and hardly recognized who I was,” she said.
She knew it was time to make a change—not just for her own sake, but also for her children. So she went to the doctor, who told Valdez she was obese.
After the appointment, Valdez sat in her car and cried.
“I realized I wasn’t taking care of myself, and I wasn’t happy with my own health and the way I looked. … I knew if I didn’t do anything about it, my health would continue to deteriorate and I would just get heavier,” she said.
Before CrossFit, Julie Valdez weighed 210 lb. at 5 foot 4 and suffered from back pain, fatigue and postpartum depression.

Valdez immediately turned to CrossFit 630 in Naperville, Illinois, for help. She began working out four days a week and eating whole, unprocessed foods.
Planning and preparing meals in advance was the key to staying on track, Valdez said.
“I figure out every Sunday what I’m going to make for dinner for the whole week,” she said. “As long as I have a plan and I don't have to think about what I’m going to make, it’s easy to stay on track. It makes it so much easier knowing I have everything I need for each meal.”
Because her husband works late hours and travels for work during the week, Valdez is the primary cook. To entice her picky children to eat healthy foods, she has discovered she needs to be creative and cook with variety. This means finding new ways to sneak vegetables into meals and using healthier ingredients such as coconut or almond flour when she wants to bake a treat for her children.
“I have learned throughout this journey that eating healthy doesn’t have to be eating a salad every day or having the same meal every day,” she said. “I have practically learned how to cook all over again.”
Though Valdez said she now enjoys cooking, like any mother, she also appreciates help—so she finds jobs her kids can do to help prepare meals.
“Not only are they learning a life skill, they are learning how to make those healthy choices to nurture their bodies,” she said.
Once Valdez embraced a diet of mostly meats and vegetables, her weight began to drop.
“After a pregnancy, it can be hard for any woman to lose that stubborn weight your body seems to want to hang on to, but once I cut out dairy—and then grains and sugar—I started leaning out a lot,” she said.
Today, Valdez weighs 130 lb., 80 lb. lighter than her heaviest weight. Her mental health has also improved, which she attributes to attending the 5:30-a.m. class at CrossFit 630 four to five days per week.

“I knew if I was going to stick to a gym routine, my only chance to get there would be 5:30 a.m.,” she said.
At that time, her husband is still home and her children are usually still asleep, so it’s a rare opportunity for Valdez to escape and do something for herself.
“After my second set of twins, I suffered from postpartum depression,” she said. “I was always tired, didn’t have energy, didn’t want to go out. In a way, I felt stuck doing the same daily tasks. Mentally that can be very hard, especially as a stay-at-home mother.”
Taking just one hour a day to work out with her friends helped pull her out of that mental rut, she said, while her improved fitness enhanced her quality of life.
“It’s crazy how just by holding on to extra weight, it really does affect doing the simplest things in your daily life,” said Valdez, whose back used to hurt when she walked upstairs.
“Now I can run up and down the stairs without feeling like I ran a marathon, or I can run around with my kids while still having the energy to go to CrossFit. It's pretty amazing what our bodies can do once we take care of ourselves,” she added.
Losing weight also gave her more confidence.
“I used to live in my little box, and I was always nervous all the time. CrossFit has definitely pushed me outside my comfort zone, and that has really built my confidence to do things with my kids,” she said. “I used to always stay covered up at the beach because I wasn’t happy with myself, for example. Now I’m more confident as a parent and just a person.”
Valdez’s coach, Nate Steele, says he uses her transformation story as an example to other clients, who give excuses for why they aren’t prioritizing their health and fitness.
“You can’t have a better real-life rebuttal to those people who tell you, ‘I just don’t have the time,’ than Julie. She’s the absolute example that recognizes the importance of doing whatever it takes to concentrate on health. … Two sets of twins all under the age of 4—one with special needs—won’t deter her from spending that hour at the gym,” he said.
Steele added: “We talk a lot about building healthy habits and that once those habits become part of the routine, they really are part of you. Julie has made improving herself a priority and (has) built the habits needed to see that her goals come true.”

In addition to better health, CrossFit has given Valdez a new outlook on parenthood.
“CrossFit has made raising four kids easier by giving me the extra push I needed, as well as giving me an outlet for those stressful days,” Valdez said.
She added: “People always make comments like, ‘Oh, good luck to you. You must have your hands full.’ … But you know what? It’s so much fun. I have silly kids who like to make me laugh, and I just think how lucky I am to have four little ones who like to give me kisses at the end of the day.”
Valdez no longer suffers from “mom guilt,” and she hopes other mothers will learn from her story and lose the guilt, too.
“It’s not selfish to go to the gym and work out,” she said. “A lot of moms think, ‘Oh, but I could be doing laundry at 5:30 a.m. or cleaning up the house. That’s more important than the gym.’ But it’s not. You have to take care of your own needs.”
She added: “Especially if you have a family to take care of. You have to take care of yourself first so you can take care of them.”
