Title TBA - Jily Fanfic
( Hey guys, I’m super new to Jily fanfic, and I’d never imagined I’d ever write any of it, but I just had this nagging idea in my head after reading @petals-to-fish‘s “On A Scale of One to Ten” and @elanev91‘s “The Yeast I Could Do,” I knew I had to just get it out, so I wrote a chapter. I honestly don’t quite know where it’s going, and I can’t even guarantee that I’ll finish it (my track record is......not so good), but hopefully I can find some direction and some semblance of a plot. Seems like it’ll be a Lily-centric Muggle!AU Jily story. Here’s a snippet below! )

Lily walked out of the staff room, slid her badge through the card reader, and clocked in. With slow steps, sleepy eyes, and red hair loosely thrown over one shoulder, she walked the quiet hallway to pass through the large double doors onto the unit. The bustle of activity and various alarms hit her senses immediately, and she deeply wished she were back home, snug and warm in bed.
Entering the nurses’ station, she saw that there were a few day shift nurses and nursing assistants already waiting to give report, and some of her fellow night shift nurses looking up their patients.
“Hey, Lily! Ready for your shift?” a day shift nurse asked cheerily. “I’ve got one patient for you!”
Lily grunted in response. “No way. And don’t you dare give me report until 7,” she replied, half-jokingly. Some of the day shift staff laughed in response, but they had reason to feel relieved. After all, with the arrival of the night shift, they got to go home.
She quickly jotted down her assigned room numbers and sat down at an unoccupied computer. It appeared that she would have five patients tonight. Scrolling through the unit list, Lily found three of her patients, but she could not find the fifth. She would probably be getting an admit or a transfer from another floor at some point then.
Lily swept her hair up in a high bun, and quickly perused the four patients’ charts, writing down the important details on her organizational brain sheet. There were the scheduled and as-needed medications, admission diagnoses, hospital events, past medical history of note, active orders written by healthcare providers, protocols in place; as well as the details that the average person might not realize—like if a patient needed assistive devices to get up and around. She even scrolled through the EMR, the electronic medical record, to find out when her patients’ bowel movements were last. “In the hospital, we celebrate bowel movements—unless you’re having too many,” she and her fellow nurses often told patients.
Minutes later, one of the day shifters, Kingsley, approached her. “I’ve got three for you when you’re ready,” he said.
“I’ll never be ready,” she joked, “so just tell me about these patients anyway so you can go home.”
Kingsley laughed and said, “I’m so ready to be out of here; this is my third day in a row. Only one more for me and then I don’t have to be at work for four days!”
“Shacklebolt, you poor, unfortunate soul,” Lily said with an eye roll, but laughed all the same. “I’m on day one of three, so maybe I’m the poor, unfortunate soul. I’ve only been off for two days and already I feel so off-kilter!”
The next half-hour was a blur of obtaining new information. Her patients seemed straightforward enough for a medical-surgical unit; one was an admit from earlier in the day for a blocked artery somewhere in his left leg, probably going for a bypass at some point; the second was a small bowel obstruction who seemed to be doing better with a nasogastric tube placement and bowel rest; the third had come in for cellulitis, a serious bacterial skin infection, of the right arm; and the fourth was here for a tibial-fibular fracture of the right leg and would be discharging soon.
“Hey, Lily,” a passing nursing assistant called out to her twenty seconds after she’d gotten report on her last patient. “The guy in room 15 is asking for pain meds. He says his IV site might be leaking, too.”
And so began her shift.

So fragging annoyed and frustrated with myself this evening/morning. Textbook executive dysfunction. Ask me why it took basically 7 hours of faffing about, watching youtube videos, reading Guardian articles about Brexit, reading TV tropes, etc -- for my to finish a clinic note on a patient I saw yesterday. 
I definitely have tons of room for improvement in clinic. I’ve heard so many times from upper levels and other interns who are better at clinic that you need to force patients to pick only two or three things to focus on during that clinic visit but it’s hard when the patient isn’t telling you a coherent story and we’ll be talking about a recent hospitalization one minute and then ripple into “yeah, I still get dizzy and lightheaded when I look up” and before I can get out “wait, what, how long has this been going on” they’re galloping off with new urinary incontinence and worsening vision. Wait, what was that about feeling tired and not wanting to go out of the house? Like fatigue fatigue or depression fatigue? Doesn’t know any of their meds, is on 16 of them, of course. Lives alone. Diabetes uncontrolled, Hypertension uncontrolled. Sees specialists but OF COURSE they don’t use our EMR so I can’t ever see anyone’s notes or co-ordinate care (which, yes, I know that’s the norm in the real private practice world and I’m super spoiled in residency by being able to see all of this with 99% of my patients).  
OF COURSE, once I got going the note wasn’t **that hard** to pump out but I hate that I let anxiety over what’s going on coupled with guilt and shame about how I should have finished the note yesterday, when I saw her that I had my worst procrastination/anxiety bout in a good long while.

Acer ConceptD 7 Ezel Review
Acer's ConceptD 7 Ezel (starts at $2,499; $3,999 as tested) might be the most radical convertible laptop on the market.  Aimed at pen-savvy creators, designers, and engineers, this 15.6-inch hybrid's gorgeous UHD/4K touch screen flips into many different positions thanks to a unique hinge design, making it more versatile than traditional convertibles with 360-degree hinges. It also offers a Wacom EMR pen for a natural inking experience. Like the smaller ConceptD 3 Ezel, the ConceptD 7 Ezel is a slam dunk for those able to leverage its uniqueness, as there's nothing else quite like it. Just be prepared to get equally creative with your finances to afford it.
A Unique Concept(D)
The ConceptD 7 Ezel's uniqueness starts with its marriage of desktop-class power to a large 15.6-inch pen-enabled screen. My review unit, model CC715-71-7163, has the same grade of components found in a top-shelf gaming notebook, such as the Razer Blade 15 Advanced (2020). Inside is an eight-core Intel Core i7-10875H processor (2.3GHz base, up to 5.1GHz turbo), an 8GB Nvidia GeForce RTX 2080 Super Max-Q graphics card, 32GB of memory, and a 2TB solid-state drive. The Windows 10 Pro system carries a one-year warranty, which strikes me as skimpy for $3,999.
The only convertible that offers remotely similar performance is the HP Spectre x360 15 (2020), priced at $1,649 at this writing with a six-core Core i7-10750H, a 4GB GeForce GTX 1650 Ti Max-Q, 16GB of RAM, and a 1TB SSD. That's a lot less than the base $2,499 ConceptD 7 Ezel, which matches the HP's specs except for a more powerful 6GB GeForce RTX 2060. That said, the comparison is purely for penny-pinching purposes; much of the ConceptD 7 Ezel's value comes from its design.
Versatile Usage Modes
The ConceptD 7 Ezel has a traditional display hinge that opens and closes clamshell style, but there's a second one halfway up the lid that allows the screen to rotate independently.
The display can be positioned at almost any angle and elevation by moving both hinges. It becomes a tablet with the screen rotated 180 degrees while the main hinge is folded flat, and by raising the main hinge, the screen can float as shown here 

The second hinge is stiff enough to keep the screen stationary for drawing or using the touch screen in this position. It also makes it possible to stand over the ConceptD 7 Ezel while doing so, an act that would require reaching several more inches with a traditional convertible that would be lying flat in tablet mode. A stand mode is also possible, which is like the photo below, but with the screen pressed down further so that it rests in front of the keyboard. The ConceptD lives up to its "easel" name in that position.
Last, it can operate in a display mode with its screen flipped over 180 degrees. It's a useful position if you're seated across the table from someone and wish to present something.
Creative Design Inspirations
The ConceptD 7 Ezel's white exterior may look like plastic, but it's metal; running a finger across it rewards with a high-pitched scratchy sound that plastic can't produce. The white comes from a ceramic coating that Acer claims is stain-resistant. I tested it by splashing dark roast coffee on the lid and letting it dry for a few hours. Sure enough, it wiped off without a trace.
At 1.13 by 14.1 by 10.2 inches (HWD), the ConceptD 7 Ezel is one chunky laptop considering today's relentless drive toward thin-and-light electronics, though it's not terribly overweight at 5.5 pounds. The display contributes most of the extra thickness; its base is of normal height for a notebook this powerful. All surfaces are satisfyingly flex-free.
The keyboard adds visual warmth with its atypical amber backlighting. Prettiness aside, the island-style keys unfortunately lack for tactile feedback, a casualty of their short vertical press distance.
The productive layout includes dedicated Home, Page Up, Page Down, and End keys. Only the arrow cluster is nonstandard, combining half-size up and down and full-size left and right keys. Below, the buttonless trackpad is just big enough. Its clicking action is slightly stiff but offers communicative, noiseless feedback. Meanwhile, the holes forward of the keyboard are for cooling, not for the speakers; the latter deliver unremarkable sound from under the palm rest.
Plenty of Input and Output
The ConceptD 7 Ezel offers Wi-Fi 6 and Bluetooth 5 wireless support, as you'd expect from a high-end laptop made in 2020. Its physical connectivity starts on the front edge with a full-size SD card reader, a nice break from the usual microSD slot.
The left edge holds a Kensington lock notch, a USB 3.2 Gen 1 Type-A port, two Thunderbolt 3 (USB Type-C) ports, and a headphone/microphone jack. Full-size HDMI and DisplayPort video outputs, another USB 3.2 Gen 1 Type-A port, and an Ethernet jack dot the right edge.
It's potentially inconvenient that most of the ports are located along the front half of each edge; connected devices can intrude on external mouse or elbow-resting space. The power jack's location at center right is also irksome—it should be further back to keep the cord out of the way—but at least the included adapter has a right-angle plug that doesn't stick out too far.
The power button, located on the left edge, doubles as a fingerprint reader for Windows Hello biometric logins. However, the ConceptD 7 Ezel has no IR webcam for facial recognition. Its average-quality 720p webcam also lacks a physical privacy shutter.
Legendary Inking: Wacom EMR Returns
The ConceptD 7 Ezel's best asset is its screen, a 15.6-inch IPS touch panel with detailed UHD/4K (3,840-by-2,160-pixel) resolution. Acer rates it for 350 nits of brightness and 100% coverage of the Adobe RGB gamut for eye-popping color. It looks great.
The ConceptD app provides controls for switching color spaces, including a native mode for those who plan to calibrate it themselves.
All I can discredit about the picture is its slight sparkle. It could be from the anti-glare surface treatment, a necessity given that the surface is glass (precisely, Corning Gorilla Glass 6) and would otherwise be reflection-prone.
The hard glass and the slight resistance from the anti-glare treatment provide an excellent pen-on-paper-like feel for inking with the ConceptD 7 Ezel's pen. Stashed in the right side of the display, it's slightly narrower than a normal ink pen and has two buttons. It never needs to be charged thanks to its Wacom EMR technology.
Wacom EMR, the oldest of the active pen technologies, is uncommon on convertible notebooks. It adds thickness since it requires an active digitizer to be built into the screen. By contrast, Wacom AES and N-Trig combine an active (powered) pen with a passive digitizer.
One of EMR's advantages is a long and responsive hover distance. The cursor appears when the pen is about three-quarters of an inch above the surface and moves perfectly in sync with the tip. With AES, the cursor lags since the tip's position isn't precisely known to the digitizer unless it's in contact with the display. EMR's tilt support, natural responsiveness to pen strokes, and up to 4,096 levels of pressure sensitivity are other key reasons to applaud the ConceptD's inking.
Testing the ConceptD 7 Ezel: Studio Power
The ConceptD 7 Ezel is a GeForce RTX Studio product, which means it packs enough performance for advanced creative tasks such as video editing. My unit's GeForce RTX 2080 Super Max-Q GPU is also capable of gaming, though its 4K screen resolution is too high to drive without lowering the detail settings in many newer titles. Its screen refresh rate is also just 60Hz, whereas even mid-grade gaming notebooks have 144Hz or higher.
Acer offers the ConceptD 7 Ezel Pro for those in search of professional Nvidia Quadro graphics and independent software vendor (ISV) certifications for popular apps. It's expensive; the top-end CC715-91P-X1XB model goes for $4,999 with a quad-core Xeon processor and a 16GB Quadro RTX 5000. But that sounds about right for a mobile workstation with those components.
For our benchmarks, I compared the ConceptD 7 Ezel to the following convertibles and high-performance notebooks...
The non-convertible HP ZBook Create G7 and MSI Creator 15 are thin-and-light creative laptops with GeForce-class graphics. Though they're not direct ConceptD 7 Ezel competitors, their overall performance should be similar.
Storage, Media, and CPU Tests
The ConceptD 7 Ezel started with an excellent 5,753-point showing in UL's PCMark 10, our general system performance assessment that simulates different real-world productivity and content-creation workflows. We informally look for high-performance PCs to post at least 4,000 points in that test. The Acer also did well in PCMark 8's storage subtest, though it didn't stand out from the others despite having two 1TB solid-state drives striped together in RAID 0 (they appear as a single drive in Windows).
Next up is a pair of CPU-crunching tests: Cinebench R15 stresses all available processor cores and threads while rendering a complex image, while in our Handbrake test, we transcode a 12-minute 4K video down to 1080p.
The higher clocks of the ZBook Create's Core i9 processor gave it a slight advantage over the ConceptD 7 Ezel in these tests. Predictably, those two and the eight-core MSI had no trouble leaving the six-core ConceptD 3 Ezel and Spectre x360 15 in the dust.
The final test in this section is photo editing. We use an early 2018 release of Adobe Photoshop Creative Cloud to apply 10 complex filters and effects to a standard JPEG image, timing each operation and adding up the totals. This test is not as CPU-focused as Cinebench or Handbrake, bringing the performance of the storage subsystem, memory, and GPU into play.
The ConceptD 7 Ezel finished with an impressively low time for a convertible laptop. The high 5.1GHz boost clock of its Core i7 chip and 32GB of memory are ideal for advanced Photoshop tasks.
Graphics Tests
Our first two benchmarks in this section measure the gaming performance potential of a PC. In UL's 3DMark, we run the Sky Diver (lightweight, capable of running on integrated graphics) and Fire Strike (more demanding, for high-end gaming PCs) subtests, both DirectX 11-based. Unigine Corp.'s Superposition is another gaming simulation that uses a different rendering engine to produce a complex 3D scene.
The ConceptD 7 Ezel's GeForce RTX 2080 Super Max-Q GPU gave it top scores in these tests, as it should have, though the ZBook Create G7 wasn't far behind with its RTX 2070 Max-Q.
For cooling, the ConceptD 7 Ezel relies on cooling fans that jet air out the sides and back of the chassis. They ran frequently and noticeably, especially during our benchmark runs. They did, however, keep the chassis cool top and bottom.
Battery Rundown Test
For our last benchmark, we measure a laptop's unplugged runtime while playing a locally stored video with screen brightness at 50 percent and audio volume at 100 percent. We use the notebook's energy-saving rather than balanced or other power profile where available, turn off Wi-Fi, and even disable keyboard backlighting to squeeze as much life as possible out of the system.
Eight hours of unplugged life from a convertible this powerful is impressive, especially considering its active digitizer; it's longer than we expect from gaming notebooks with similar components. It even outdid the much less powerful Spectre x360 15.
One of the Biggest, Baddest Convertibles Around
Acer's ConceptD 7 Ezel defies convention by combining the power of a top-shelf gaming laptop with a Wacom EMR-enabled screen. It's a blissful combination for designers and creators, though not one without compromise. This is one bulky device, and one that will set you back financially even in its base configuration. But for those willing to ante up, this one-of-a-kind convertible delivers an unparalleled creative experience.

Weekend Getaway (Emre Can OS)

Aurora was never a risk taker. But a trip to the mountains with no other than Emre Can may make her rethink her tactics of playing everything safe.                           
Aurora fixed her glasses and tried to focus on the paper that she was trying to write. As if this wasn't hard enough her phone screen kept lighting up with every new text message that arrived. Reading just a preview, she rolled her eyes and let out a sigh of irritation. Sometimes she really couldn't believe how irresponsible her friends were!
They have all agreed to go to the mountains near the city tomorrow, to catch up and take advantage of what was probably the last sunny weekends this year. However, one by one, all of them seemed to come up with an excuse not to go, in the last minute. Furious, she turned her phone face down and did her best to focus on her work.
She only allowed herself to look at her phone about half an hour later when she took a short break to fix herself a cup of coffee. Leaning on the kitchen counter as she waited for the water to boil, she skipped through the new messages. It seemed that the only one who actually still wanted to go, was the one who suggested this trip in the first place- Emre Can.
Aurora didn’t know him all that well. To be fair, this was a group of about twelve people  some of which she knew better than others. All of them have met at different points over the years and have somehow bonded together. For instance, there was her absolute best friend, Jenny, whom she’d been friends with over 14 years, and then there was Emre, a friend of Jenny’s boyfriend, who went hanging out with them once or twice and sort of just stuck around. She didn’t have a single clue why he would hang out them when he was a pretty famous footballer around here, but she was the last person to judge someone. To be fair, just because she wasn’t close to him didn’t mean that he hadn’t gotten along quite well with some of the people in their group.
Her phone buzzed signalizing another text  but the name on the screen surprised her. “Emre Can” it read. Hesitating for a bit, she opened the message and read it carefully. Apparently he was as annoyed as her and was asking if she still wanted to go to the mountains even though everybody else has canceled.  
“You mean just the two of us?” Aurora was quick to respond.
His answer came a moment later, “If you don’t mind.”
Ignoring her trembling fingers, she typed her response, “If you’re okay with that”.
“Is 9 a.m. okay for you?” read the text message that followed.
“Can we go at 11? Saturday’s my day for sleeping in.” She even added a smiling emoji.
“See you at 11,” he responded a few moments later.
Then came the second guessing. Aurora started wondering if this was really a smart decision. Admittedly, she was furious at all her friends for canceling and wanted them to regret not coming with Emre and her, but on the other hand, she barely knew Emre and was not even completely sure she liked him. He was polite and nice indeed, and never during their short interactions did he do or say anything to offend her, but they were just so different. He was a jock, a popular and outgoing person who seemed to have the ability to charm everyone he wanted to. She, on the other hand was an introvert. Not so much shy, but she liked spending time on her own, never too interested in trying new things or meeting new people. She was the most relaxed person around the people she knew and loved, but she just wasn’t the type to socialize very easily. And especially, not with someone like Emre.
Closing her laptop and turning the lights off, Aurora headed to bed before her thoughts made her cancel on him.

Their ride to the mountains only convinced Aurora more in what she was already thinking- she should’ve canceled too. It was the most awkward hour that she has had in a long while, and she is sure Emre felt the same. They’ve managed to exchange a few words when they were both complaining about their friends being complete jerks or when they used common friends as a possible topic of their conversation. However, Aurora lacked the ability to small talk for a long time and looks like Emre was no good at that either because they were only able to chit chat for a very brief time.
At one point, Aurora asked to take a look at the playlist in his car, and almost squealed in happiness when she saw that, among the songs that she has never even heard of, he had For the First Time by The Script.
“You listen to The Script?” she asked almost too eagerly. Finally a common ground she squealed in her mind.
“What?” Emre was confused, and when she showed him which song she was refereeing to, he waived his hand dismissively. “Oh it’s not mine; a friend added it to the list.”
A small “oh” escaped Aurora’s lips, and she leaned against the window trying to hide her disappointment. This was going to be a long day she thought.
The awkwardness between them continued and seemed to grow more by the second, even as they went to have breakfast. They exchanged a couple of words as they waited for their food, and Emre chuckled as she took pictures of the restaurant and the view to send to their friends to, as she said, “rub it in their faces”. But she secretly envied them because the awkward silence between her and Emre was becoming unbearable. She was actually hoping that something would come up so that they would have to leave early, for she had no idea how they were going to spend the whole day like this.
It was only after they’ve finished their breakfast that things began to ease up a little bit. They were both sipping their coffee and aimlessly scrolling through instagram, when Aurora stumbled upon a post about the movie The Intern.
“Awww this movie is sooo good!” she exclaimed excitedly, turning the phone to Emre to see.
“Oh yes, it is absolutely amazing,” Emre agreed with a grin.
Aurora’s mouth fell open. “You’ve watched it?”
“Yeah, I’ve actually seen it a couple of times,” he admitted sheepishly, scratching the back of his head.
“Wow, to be completely honest with you, I’d never think that you’d like these types of movies” she said, truly amazed.
“Why is that?” he asked with a smile. Aurora opened her mouth to answer but she realized she didn’t know what to tell him. She just didn’t think that he was that type of person. But which type was that exactly? The one that could love the same things as she did?
They started talking more and more about the movies they liked, both thrilled when the other one mentioned the movie that they found fascinating. Then one topic lead to the other one and soon they were both rambling about things they liked and disliked, laughing and teasing each other good-naturedly about some bad choices.
The atmosphere has slowly transformed from awkward to a quite pleasant one. Aurora thought to herself that maybe meeting new people and getting out of her comfort zone wasn’t as bad as she’d always portrayed it. In her mind, she made a self reminder to step out of her routine more often.
“Would you like to go for a walk?” Emre asked after a while, and Aurora gladly accepted, packing her belongings into her backpack.
The day was unusually beautiful for this time of the year. Had it not been for the snow melting on the ground, one could easily forget that it’s winter. They walked down the path into the woods and up the mountain, enjoying the beautiful view and soaking up the sun. Emre was retelling some anegdote from training and as he laughed, Aurora caught herself thinking how his was the prettiest smile she has seen in a long time. And really, on his slightly darker skin tone, his teeth seemed so dazzling white, but what was even more beautiful were his deep brown eyes that seemed so warm and soft as he laughed.
Once they have returned to the restaurant Aurora suggested that they play some board games. She asked the waiter if they had any, but he informed her that they only had UNO cards and chess, and chess was already occupied by the senior couple sitting by the window. Not really being an expert in chess and believing her friend wasn’t either, Aurora took the UNO cards.
Emre admitted that he hadn’t played since he was a kid, so she re-explained the rules to him. She won first two or three games, but Emre was a bad loser. He demanded they play more and more until he finally won and then he tried beating her score, but Aurora wouldn’t give up on it so easily.
“Ha! I win!” Emre exclaimed, throwing his fist in the air, after he threw his last card on the pile leaving Aurora with a couple of cards left in her hand. “10-7!” he said, indicating that he was now leading by 3 points.
Aurora chuckled rolling her eyes good-naturedly. Maybe she should’ve stopped the game while she was still winning.
She glanced out the window only to see that the sun has begun to set. The sky was painted in beautiful colors, and the city which was slowly being swallowed by the dark looked magnificent from up here.
“What a beautiful view,” she gasped, grabbing her phone to snap a few shots.
“It is indeed,” she heard Emre agree. But when she turned towards him he wasn’t looking through the window, but at her.
He quickly averted his eyes and Aurora tried to hide her blushing cheeks. Even though she convinced herself that she’d misunderstood him, suddenly all small signs that he had shown throughout the day started playing in her head. How she’d often catch him staring at her direction. How he helped her fix her scarf, his eyes lingering on her lips just a bit too long. How his fingers brushed against hers as he took the tea cup from her hands. How amazed he was by how soft her hear was after he’d asked to touch it. How softly he gazed at her when she was rambling.
Thinking about it now, Aurora started questioning herself if maybe there was a slight chance that he liked her. He was a very handsome man judging by his physique. His smile was definitely the prettiest one she has seen in a long time, and there was just something so calming about his eyes. But he got more attractive by the minute, with every conversation they shared. Aurora had no idea that one’s opinion about another person could be completely changed after just one day. 
“So, do you like me any more now?” Emre asked all of a sudden.
Aurora almost chocked up on the tea she was sipping. Did he have the secret ability to read her thoughts?
She cleared her throat, recomposing herself. “What do you mean?”
“Well I had a feeling that you didn’t really like me all that much before, but I guess that’s fair because you didn’t really know me. How about now?”
“Um, I- I’m sorry you got that impression.” Aurora stuttered, evidently surprised. “It’s not that I didn’t like you it’s just
you were kind of like an unexplored territory for me. At first I thought you were cocky and overly confident and full of yourself-”
“Wow,” Emre raised his eyebrows, amused.
“But now- now that I’ve had a chance to get to know you better I- I realized there’s much more to you than what I’ve thought. And I’m glad I got the chance to find that out.” She said honestly.
He smiled softly at her, “I am too.”

she liked him. But that was very unusual, for he was not at all the type of guy she would fall for. Making new friends is quite nice, but he just wasn’t for her. And she knew it. Yet, there was something so calming about talking to him, spending time with him. There was just something about the way he looked at her, smiled at her
or maybe she just wasn’t the object of male attention in a long long time.
They had spent the rest of the evening simply talking since Aurora refused to play any more games with him. Turns out she was even worse loser than he. Emre tried to teach her a few things about football and how the game functions but gave up on the task soon enough since everything he told her was met with her blank expression signalizing that she didn’t understand a single thing.
They were planning on heading back to the city soon, when a group of tourists occupied the table next to theirs. There were five or six of them and they seemed to have come from Spain. The group was talking and laughing very loudly and it seemed like they were having a great time.
Soon enough one of them pulled out a guitar from its suitcase and began fidgeting with the wires. Aurora had learned Spanish in high school, so she was able to understand a little bit of what they were saying. Apparently the guitar was new and he was explaining his friends that he thought it didn’t work properly. The redhead sitting opposite of him said that it was probably him- not knowing how to play it right, and everyone laughed. To prove them wrong, he started playing the notes of some Spanish song, the one that Aurora has heard somewhere and vaguely remembered. She started humming to the song while Emre looked at her amused, smiling softly. One of the guys from the group noticed her humming along and invited them both to join in. Emre and Aurora pulled their chairs closer to the group’s table, and soon more people followed, clearly intrigued by this cheerful young group. Even the senior couple that Aurora saw playing chess earlier today came by. Soon they had to move their little group to the sofas by the window because there just wasn’t enough room for everyone to sit anymore.
The atmosphere was so cheerful and everyone was just so happy and relaxed, and Aurora, who was just this morning wishing for this trip to end as soon as possible, was now mentally high-fiving herself for saying yes to Emre’s invitation.
The two of them were sitting on one of the sofas together. Emre’s body was so close to her that their arms were touching. Sitting so close, the scent of his perfume filled her lungs and as she hummed along to the song that they were playing, she fought the urge to lean on Emre’s shoulder. It was a bit chilly so she wrapped herself in her thick scarf and sipped her wine, and maybe she would’ve dared to make a move had this not been only her first glass.
As she closed her eyes and sang along, she leaned back against the sofa. But as she did, she felt Emre’s arm behind her back, resting on the sofa as well. She thought it was an accident so she quickly wanted to move away, but Emre wrapped his arm around her shoulder. She turned her head to look at him and their eyes locked for a moment before Emre leaned in and pressed his lips softly against hers. Moving on their own, her lips kissed him back, and she expected the kiss to last much longer but it didn’t.
Emre pulled back and gazed tenderly into her eyes, before he leaned against the sofa again, his arm still wrapped around her shoulder. He seemed so relaxed and peaceful that Aurora didn’t know what to think. He didn’t even say anything about the kiss. He didn’t even apologize. Not that he needed to- she didn’t have anything against it, but did he think he could just kiss her like that and don’t say anything?! She forced her inner self to stop freaking out and took a few breaths to cool down.
All of a sudden, the place became too crowded, too small and way too hot. She could feel her cheeks burning and her heart pounding against her chest. Afraid that Emre could hear it too, Aurora stood up and left the room under the excuse that she had to use the restroom.

Looking at herself in the mirror, she cursed at the fact that she had the tendency to blush even at the smallest things. Not that kissing a guy she barely knew was a small thing but still. She splashed some water over her heated cheeks and washed her hands, hoping to cool down a little bit. She tried telling herself that it was just a kiss, that he probably got caught up in a moment and nothing more.
But the thing is
she wanted it to be more than a moment. She hoped that he kissed her because he wanted to taste her lips on his. Because he liked her. And then her mind reminded her of all the small signs from and her heart jumped with hope that maybe he did like her after all. But she had awful experience with getting her hopes up so she shook her head and forced herself to not think about it. Aurora braced herself and stepped out of the bathroom promising that she’ll behave as if this wasn’t a big deal.
But as she did so, a pair of strong hands grabbed her, and soon she was pressed against Emre’s masculine figure, while his lips found their way to hers. He kissed her more passionately now, his tounge exploring her mouth as soon as it opened. Her hands creeped up his chest and wrapped around his neck and he wrapped his arms tighter around her pressing her more closely against his body if that was even possible. Once they finally stopped kissing it took them a couple of moments to catch their breaths and recompose themselves.
Aurora immediately opened her mouth to ask what was all of this supposed to mean, but Emre shut her up with a peck on her lips.
“Don’t ask,” he said looking into her eyes, “I have no idea either.” He chuckled a bit shyly as if he read her mind once again.
“Do you want to stay here or go home?” he asked. “Because all I know is that I want to keep kissing you for as long as you allow me.” He said sincerely, his fingers caressing her face.
Aurora was completely utterly speechless. She didn’t know what to think. She was a type of person who always needed to know all the facts before making a decision. Her mind demanded answers.
Taking Emre’s outstretched arm, she followed him back in the room, deciding that maybe it’s not the worst thing in the world to take a leap in the dark.

How to Succeed in Family Medicine (without really trying)
I wanted to do one last post on FM because I felt like I really got a good daily routine down and wanted to share. Some caveats:
- I do not know what my final rotation grade is yet
- I did not do very well on the shelf (I mean I passed, but it wasn’t super stellar), but I also recognized pretty quickly into the rotation that I was not studying effectively during the time and I will make note of what I should have probably done below (though it wasn’t what I actually did)
- I do have a letter of recommendation from my preceptor that (imho) is pretty positive and indicates that something I was doing worked. So here we go:
6 am: Wake up, go on a run (yay exercise!)
7 am: Shower, change, pack lunch and breakfast and snack, walk to work (ok also I got lucky because I lived across the street from work)
7:30 am: Get to work, eat breakfast while listening to OME videos/reading uptodate (no internet at home for me SO if you have internet obviously this could be done at home...)
8 am: Open up EMR, load up patients for the day, go through their charts and read pasts notes. Take note of recent changes in health (ED visits, specialists they saw, new diagnosis or meds) and what was discussed in past primary care visits (especially important when it was a general physical and they had a ton of things on their problem list). 
8:40 am: First patient of the day! My preceptor and I got in a good routine of me seeing patients, doing a full H&P, developing a plan and then presenting to him. He then would go back into the first patient’s room while I moved on to the next one. Some small variations were taken depending on schedule, for OB visits, and well-childs. 
12 noon: usually done with patients by now, eat lunch and finish up notes from the morning. 
1 pm: Afternoon patient time! 
4/5 pm: Done with patients for the day! Finish up notes. 
This is around the time that I should have spent at least an hour or two each day really studying. Instead I usually wasted time on the interwebs until I got hungry and went home. Closer to my shelf I started getting better about going through OME videos. 
7 pm: Go home and eat dinner.
7:30-9:30 pm: Practice questions. This was the one thing I was very good about doing every day. I was given PreTest by my school and I did all 500 questions. I then went over the ones I got wrong and made flashcards with those facts. Closer to shelf I went through those flashcards basically any free time I had. 
9:30 pm: Make breakfast for the next day (I eat overnight oats for breakfast), get in bed, read for pleasure. 
10 pm: Asleep!
Some general key things:
- I wish I had done more in the Case Studies book. I tried, but it was too dense at the beginning. Looking back I think I could have benefited from just going through it even if it wasn’t super detailed. 
- I wish I started studying my flashcards day two of the rotation 
- I focused a lot on coming up with plans and telling the patient what my plan would be while I was in the room with them. This was good practice for my future, but I also think my preceptor appreciated that he literally just would walk in the room, put orders into a computer and then leave because the patient knew exactly what to expect. 
- If you want to practice point of care stuff (blood draws, injections, vaccines) make sure you tell every single MA/NP/Nurse/Physician in the office because they will come get you if they know that’s what you want to do! I got to do a lot of cool stuff just because the MAs were constantly looking for me when they had to do something. 
- Going over the schedule in the morning was so key for me. It helped me understand how the flow of the day was going to go, what kind of patients to expect, and then when a patient came in I was already versed on what I wanted to ask them about/what their reason for coming was. 
- Basically the key to having fun and being successful in family medicine (and third year in general) is becoming self sufficient. Try as hard as possible to make your attending’s life easier and they will love you and reward your for it!
Those are my tips and tricks for family medicine! I promise this is the last post about FM/in general for a while....

Best Keto Instant Pot Beef Stew

Deliciously tender beef cooked in a pressure cooker with low-carb vegetables, in super creamy grain-free tomato sauce. This is comfort cooking at its best.
What I love most about it is that you can make the whole meal in the Instant Pot! In just 90 minutes you've got a tasty keto dish with fall-apart meat and juicy vegetables cooked to perfection.
The only extra work is thickening the sauce but it's totally worth it. Since there is no flour allowed on a keto diet, most low-carb stews are thin and runny. That's not the case with this recipe.
Egg yolks are a fantastic alternative to flour resulting in super creamy sauce without the extra carbs. I've been using egg yolks to thicken sauces, gravies and even soups, including those in my Keto Slow Cooker & One-Pot Meals Book - always with a success!
If you you don't have an Instant Pot, use one of the options listed at the end of this recipe.

Ingredients (makes 8 servings)
1 medium yellow onion, chopped (110 g/ 3.9 oz)
2 cloves garlic, minced
1.2 kg boneless beef such as brisket or stewing steak (2.65 lb)
1 rutabaga (swede), peeled and diced (400 g/ 14.1 oz)
3 kohlrabi or turnips, peeled and diced (600 g/ 1.3 lb)
4 celery stalks, chopped (160 g/ 5.6 oz)
2 tbsp ghee or duck fat (30 g/ 1.1 oz)
1 tsp sea salt, or to taste
1/2 tsp ground black pepper
1 tbsp paprika
1/4 tsp ground allspice
1 can unsweetened tomatoes or chopped fresh tomatoes (400 g/ 14.1 oz)
1/2 cup water (120 ml/ 4 fl oz)
8 large egg yolks
1/4 cup chopped parsley (15 g/ 0.5 oz)
Note: Not sure what to do with the leftover egg whites? Try one of the options listed in my Keto Lemon Curd recipe!
Instructions
Peel and dice the onion and mince the garlic. Cut the beef into large 2-inch (5 cm) pieces. Set aside.

Peel the rutabaga and kohlrabi (or use turnips instead of kohlrabi). Cut them into about 1-inch (2.5 cm) pieces. Chop the celery stalks. Set all the vegetables aside.
Set the Instant Pot to Sauté and add the onion (use 6 or 8-quart/L Instant Pot). Cook for just 3 minutes, until fragrant and then add the minced garlic. Cook for another 30 seconds.

Add the beef chunks and season with salt and pepper. Cook for 2-3 minutes, or until browned from all sides while stirring to prevent burning.

Add the paprika, allspice, chopped rutabaga, kohlrabi and chopped celery stalks.

Mix until combined and add the canned tomatoes. Turn the Sauté off.

Cover with a lid and set to Manual. Cook on high pressure for 35 minutes. When the program has finished, let the steam release naturally for 15-20 minutes, and then turn the valve to venting to release the remaining steam.

To make the creamy sauce, whisk the egg yolks with 1/2 cup of water. Keep the egg whites for another recipe (see suggestions listed in my Keto Lemon Curd).

Strain the juices into a small saucepan and bring to a simmer. Temper the red sauce into the egg yolk mixture while whisking.

When you add about half of the red sauce, return everything into the saucepan with the remaining red sauce. Cook while stirring for 4-6 minutes, until thick and creamy.
If the sauce is not smooth or has curdled, use an immersion blender and process until smooth and creamy (be careful not to get burnt!).
Pour the creamy sauce back into the pot with the cooked meat & vegetables. Stir to combine.

Serve immediately (about 2 cups per serving) with freshly chopped parsley. To store, let it cool down and refrigerate for up to 5 days. I haven't tried freezing it and I'm not sure whether the sauce would separate if you do. You could divide the mixture into single-serving containers or freezer bags and freeze for up to 3 months.

If you don't have an Instant Pot, try some of the following alternatives:
Oven Baking
Preheat the oven to 150 °C/ 300 °F. Place all ingredients in a large baking dish (optionally you can brown the onion, garlic and meat). Cover with foil or a lid and bake for 4 hours.
Add some water if needed (check half way through to ensure the haven't dried out). After 4 hours, remove the foil and increase the temperature to 200 °C/ 400 °F. Cook for another 15-20 minutes. Then, follow the instructions from step 8.
Dutch Oven
Grease a large Dutch oven (6 -quart/L or more) with ghee. Add chopped onion and cook over a medium heat for 4-5 minutes. Then add garlic and cook for a minute. Add the chopped rutabaga, kohlrabi, celery, salt, pepper, paprika, allspice and tomatoes. Add 1-2 cups of water. Bring to a boil over a high heat and then turn down to low.
Cook for 4 hours, checking every hour to ensure there is enough juices. If needed, add a cup or two more water. When done, take off the heat. Optionally, crisp up in the oven preheated to 200 °C/ 400 °F for 15-20 minutes. Then, follow the instructions from step 8.
Slow Cooker
Optionally, you can brown the onion, garlic and meat before adding into the slow cooker. Place all ingredients in the slow cooker (6 -quart/L or more).
Cover with a lid and cook for 3-4 hours on high or 6-8 hours on low. Optionally, crisp up in the oven preheated to 200 °C/ 400 °F for 15-20 minutes. Then, follow the instructions from step 8.

Meetings, training, and travel 05/02/17-05/28/17
The bus arrived at Bangkok’s main bus terminal at 7 PM. I looked up where my first meeting was being held and decided to walk to the area instead of getting a motorbike taxi and make sure I had my bearings before the meeting in the morning.
I was 6 kilometers away from my destination, so first I stopped at a 7/11 which is Thailand’s one stop shop for just about anything delicious besides the markets and roadside stands and picked up some supper.
After a couple hours of walking I arrived in the area and found a hotel for the night since all the businesses were closed and shuttered.
In the morning I discovered that my gps hit was a little off but after asking directions I took a motorbike taxi and made it to my meeting with just a few minutes to spare!
I, along with two other team members were meeting with both the managing director and medical director of a Burmese logistics company who specializes in helping sick or injured patients find quality emergency medical treatment, escort, and evacuation in SE Asian countries.
One of my old friends from the US has done business with their medical director and suggested that we meet and discuss ways to collaborate, and since our ambulance project is bogged down we were looking for another project to work on that will give us hands on patient care in addition to all the training we are involved in.
After the meeting- which was a success- we drove several blocks to find pizza for lunch to celebrate and then we headed off to our second meeting.
This meeting was with the managing director of the AREMT, an international counterpart to the NREMT. Because of all the training we are doing in Myanmar we wanted to partner with an organization that will allow us to offer an internationally recognized certification upon successfully completing our courses.
After the meeting we decided to pursue accreditation and currently both our side and the AREMT are working on finalizing the details.
That evening after my teammates went back to Kanchanaburi I spent one more night in Bangkok before taking a motorbike taxi back to the bus station the next morning.
Because there was still a week and a half before our next training I decided to take the bus up to Chiang Mai and visit some friends for a few days!
After 11 hours on the bus I was at my destination and was picked up by my friends in one of our original Thai ambulances which we had to sell!
Over the weekend I was able to hang out and relax with my friends and their cute children. One day we all piled into the ambulance and drove a couple hours even farther north to the small town of Pai. Pai is well known for the amazingly scenic drive up into the mountains before dropping down into a small valley where the town is. It it also known for its amazing street market that we were hoping to get to in time for lunch!
When we were about 10 km from the town we drove into a wall of torrential monsoon rain and were worried that the market would be completely shut down but when we arrived we were happy to see that we had beat the rain!
After stopping at a giant Buddha that stands guard over the valley and racing up its 250+ stairs with the kids to burn some energy, we drove down to the market.
Barely had we started to look around though when the tropical rainstorm caught up to us and it started pouring. We quickly grabbed some food and retreated back to the safety of the “ambulance”.
On the drive back up out of the valley we were amazed to find that the entire road had flash flooded after we passed and now there were large basketball sized rocks strewn all over the road as well as fallen trees, power lines, and a thick layer of sand and mud in places. We were glad for the knobby tires and high ground clearance as we negotiated all the obstacles!
On Sunday we went rock climbing on some volcanic cliffs outside of Chiang Mai. We climbed three routes including a crevice that was really challenging and a lot of fun!
Early Tuesday morning I had to say goodbye and headed to the bus stop to go visit my friends at Sunshine Orchard on my way back to Myanmar.
Once I arrived at the bus station, I discovered that my options weren’t so fantastic. I could either wait for 1 hour to get a bus that would only take me within 60 miles of the closest songtau circuit or I could wait for 5 hours to get a bus that would go all the way to the songtau route.
So since I still had 5 days until I had to be back I decided to go exploring!
I bought a seat on a small van that was driving to Mae Sariang Thailand which is southwest of Chiang Mai close to the border and several hours north of Sunshine Orchard.
The views along the way were phenomenal. Beautiful jungle mountains with little towns and villages dotted along the way. All the farmers rice paddies are green and lush and there were little well-tended garden patches behind all the huts.
Some of the coolest rice paddies I’ve seen in Thailand were along the way. Because the mountains are so steep the jungle farmers don’t have hardly any arable land so in between each hill they cut into the bases of the hills and widen the area between them to plant in. The cultivated areas are sometimes only 30-50 feet wide and are steeply terraced. They plant the rice in the terraces and it gets watered from rain runoff.
When we were maybe two hours from our destination we drove around a sharp corner and found that a car had failed to make the corner and flown off the very steep edge of the road and become trapped in the trees and vines about 20 feet below the road and 15 or 20 feet above the ground!
Fortunately everybody was out and the police were there and a tow truck was trying to attach his winch to the car so he could pull it down!
After a while we arrived in Mae Sariang. I found it to be a nice town, much smaller than I was anticipating, and not geared towards tourists at all like Pai is.
Striking off in a random direction I found a guesthouse to sleep in and hung out all my wet clothes to hopefully dry over night.
In the morning I went to the market and bought some dragonfruit, mango, and apples and started walking down the road towards Sunshine Orchard eating my breakfast.
I could have just gone to the station and waited for a songtau but I felt like walking and letting one catch up to me.
After 45 minutes a man on a scooter passed me and then turned around and came up beside me. Surprisingly he spoke excellent English and wanted to meet me and find out where I was going. After introducing ourselves and visiting for a few minutes, Mr. Nong San invited me to visit his house and wait for the songtau there.
After agreeing I jumped on the back of the scooter and off we went. On the way we stopped at a little roadside shop where he bought us both something to drink......It was the first locally made energy drink I’d had since I started working in SE Asia- the kind that looks and tastes like it’s made out of bulls blood, monkey testosterone, and caffeine with a shot of fresh coca tea- 😖.
After that experience we drove about 10 minutes further down the road to a very nice house set well off the road and surrounded by potted tropical plants.
After inviting me inside Mr. Nong San made coffee and we sat and talked at the kitchen table. I found out that Mr. Nong San legally immigrated to Thailand from Burma with his family 70 years ago. In 1966 he immigrated again to San Francisco and lived in the USA for 50 years teaching Thai in New York City and other places before retiring and moving back home.
One of his favorite hobbies is coin collecting and he showed me some of his amazing rare coins from the US and around the world. He also is a rare coin seller and urged me to let everyone I know know where to buy the best coins!
After an hour he said that the songtau would come soon and even though he sells coins he gave me a beautiful and rare Thai baht as thanks for visiting him! I gave him a dragonfruit as a token of thanks and got my backpack on.
I thought I would just walk out to the road and wait but Mr. Nong San instead drove me all the way back into town and got me all set with the songtau before going!
The songtau eventually got me to my destination and I spent that afternoon and the next morning with my friends at Sunshine Orchard before continuing on via songtau to MaeSot. I spent one night in MaeSot and crossed the border into Myanmar on the 12th to help finish preparations for the upcoming EMR course.
Monday, May 15 the EMR class went live with 40 students! First there was an opening ceremony with local religious and government leaders giving short speeches, next a emissary group from Taw Win foundation in Yangon that we helped Z-Rescue train gave a speech and donated a projector as a thank you gift. Finally classes could get underway!
Or so we thought
 Just after the first lecture had started one of the main sponsors for Z-Rescue who was also helping support this training asked if he could make an announcement. He announced that he would donate the uniform he was wearing to the student with the highest grade, and then he proceeded to strip right up on stage, using a longyi as a changing room!
Just in case that wasn’t a big enough incentive he pulled out a stack of 100,000 kyat (about $75.00) and threw that into the pot also! Of course, this performance got the whole class riled up and laughing and we had to take another break before we could really start.
This training is unique because it’s the first time we’ve had any other instructors besides the ones on our own team, and this time we have 4! So we have high hopes that their training will be dynamic and informative.
The afternoon of the first day of training we were honored to have two of our Swiss and German sponsors visit us. They have assisted us with sponsoring classes before and are finally able to come check us out in person!
By the afternoon of the second day everything was going according to plan when I received a call from the logistics company I had met with in Bangkok. They said they had a critical patient and asked if we could help them. We were still working on the necessary paperwork to allow M-EMS to contract for them, and the proposed start date was still a couple months away because we were still missing some key supplies like a cardiac monitor to be truly ALS.
I reminded them of this and they said that was ok because we were the only in-country team available so I hurriedly packed my backpack and double checked the aid bag and rushed to make it to the bus just before it left (there’s no airport in Myawaddy).
My translator and I took the night bus to Yangon and then took a taxi to the airport where we met the rest of the rescue team, a flight doctor from Bangkok and a communications specialist from the logistics company.
We chartered a Cessna Caravan and took off for towards our patient who was in northwest Rakhine state near the Bangladesh border.
Our flight plan had us at only 12,000 feet, so for most of the way there we were constantly zigzagging around trying to avoid the thunderheads of the monsoon.
The town of Sittwe is situated on a low peninsula in the Bay of Bengal, and as we descended to land I could see dozens of tiny fishing boats and canoes below us on the water.
As our plane touched down there was a loud noise and we suddenly tilted sideways and went shooting off towards the edge of the runway. After vibrating and wallowing to a stop, we climbed out and saw that the left side tire had blown!
The airport sent out a little truck to collect us, and after we got to the terminal we went outside and found the ambulance that was waiting to take us to the patient.
Our patient was a 32 year old French tourist who had tried diving into one of the nearby rivers and found out it wasn’t as deep as it looked. He hit a rock and scalped himself from the top of his head all the way down to his eyebrows.
First he was taken to a local clinic where they controlled the bleeding and then he was transferred to the hospital in Sittwe.
After we got to the hospital we went to the patient and found him laying in an open ward right next to another patient who looked and sounded like he had a terminal case of pneumonia or TB and had a giant unsecured oxygen tank standing upright between their beds.
The patient was extremely relieved to see us, and we were relieved to see that he was as in good shape as he was.
We completed our assessment and cut off the filthy dressing that he had on and discovered that the hospital had done an incredibly hacked up suture job without cleaning underneath first and the wound was starting to become infected. Amazingly he had no cranial or cervical vertebrae fractures per X-ray and no neck pain or neurological deficits. (He did have a headache for some reason though!)
After cleaning as best we could and re-dressing his injury and administering antibiotics we settled down to wait because the flight company had to send a new plane for us!
After we received word that the plane had arrived we loaded up our patient and rode in the ambulance again back to the airport. The airport had towed our old plane off the runway to over in front of the waiting area and the replacement was taxied up right beside it, not a confidence booster for our patient!
The flight back was uneventful and after landing in Yangon my translator and I transferred the patient to the hospital, and after waiting to make sure he was admitted and would be taken care of we took a taxi back to our apartment, exhausted.
We spent a couple days in Yangon waiting to find out if the patient would need to be evacuated out of the country or not and then took the bus back to Myawaddy on Sunday the 21st just in time for the second week of training.
I can’t say about the rest of week one but week two was fun with lots of scenarios and hands on skills practice interspersed in the lectures!
One night we were requested to go to the local hospital to see a patient. One of the government officials who was at the opening ceremony was sick and wanted us to look at him. He was also unhappy with the doctor who was taking care of him and wanted a second opinion.
After arriving at the hospital we were taken to a large open ward with 15 or 30 other patients and after visiting with and assessing our patient concluded that he had probably had a new onset seizure.
We asked the doctor if we could look at any labs or paperwork to confirm or find a differential but he was extremely unhelpful and wouldn’t share any information with us. I went back to the patient and asked if he could remember any medications that he had been given. The patient had a package he showed me that had Valproate which is for seizures!
Since there was nothing we could do for him we wished the patient good luck and a safe transfer to the Thai hospital in the morning and went back to our guesthouse.
After the classroom portion of class was over the final written and practical exams were taken, and overall the students did very well. We only had 3 students fail out of the 30 that stuck it out for the whole two weeks! Our highest scoring student was with a rescue group who came all the way from Sagaing region and he made a 96% on his written final! He was presented with the prize money and clothes from the sponsor in addition to his certificate and was so pleased he couldn’t stop shaking my hand and even smelled the clothes in front of the class 😆.
We had a graduation ceremony and then we packed all our manikins and teaching supplies up and loaded everything into the van In between downpours.
After spending one more day in Myawaddy some of us took the van and some of us took the bus back to our apartment in Yangon arriving on May 28th.

Low-Carb Instant Pot Osso Buco

Osso Buco is a traditional Italian dish made with veal shanks that are browned and cooked with onion, garlic, carrots and tomatoes. Veal shanks contain marrow bones that add fantastic flavour (and healthy fats!) to this low-carb dinner dish. Don't forget to eat the bone marrow — it can be hard to extract from the bones but you can use the thin end of a teaspoon or a meat skewer to do that.
Traditionally, veal shanks are served with a simple gremolata, risotto or polenta. To keep this meal keto-friendly, I like to serve mine with Gremolata Cauli-Rice or plain cauli-rice.
This meal makes four generous servings or up to six smaller meals. I used three large veal shanks but you may have to use up to six pieces depending on the size (make sure to use the correct weight). If you are short on time you can skip the browning although I would advice against it because browning adds a lot of flavour and takes this popular Italian-inspired dish to a whole new level. I also like to finish mine in the oven just to crisp it up a little.

Ingredients (makes 4 servings)
1 small yellow onion, diced (70 g/ 2.5 oz)
2 cloves garlic, minced
1 medium carrot, peeled and diced (65 g/ 2.3 oz)
2 large celery stalks (80 g/ 2.8 oz)
1.4 kg veal shanks, bone in (about 3 lbs) - will yield about 50% meat, 700 g/ 1.5 lbs raw meat)
3/4 tsp sea salt, or to taste
1/2 tsp coarse black pepper, or to taste
3 tbsp ghee, duck fat or tallow (45 ml)
1 small can chopped tomatoes (200 g/ 7 oz)
1-2 bay leaves
1 tsp fresh thyme or 1/2 tsp dried thyme
2 tbsp fresh lemon juice (30 ml)
2 tbsp extra virgin olive oil (30 ml)
Optional: best served with Gremolata Cauli-Rice Quick & Easy Gremolata
Instructions
Peel and chop the onion and garlic. Dice the carrot and slice the celery. Set aside.

Using a paper towel, pat dry the meat. Season with salt and pepper from both sides.

Set your Instant Pot to Sauté and grease with 2 tablespoons of ghee (use 6 or 8-quart/L Instant Pot). Add the beef chunks and season with salt and pepper. Cook for 2-3 minutes per side, or until browned and caramelised. This step is important as it will add flavour!

Remove from the Instant Pot and set aside on a plate.

Grease the Instant Pot with the remaining 1 tablespoon of ghee. Add the onion and cook for just 3 minutes, until fragrant and then add the minced garlic. Cook for another 30 seconds.
Add the carrot and celery and cook for another 1-2 minutes.

Add the tomatoes, bay leaves, thyme, lemon juice and water.
Add back the browned meat and cover with the sauce.

Cover with a lid and set to Manual. Cook on high pressure for 35 minutes. When the program has finished, let the steam release naturally for 40 minutes, and then turn the valve to venting to release the remaining steam (or let the pressure drop naturally in the sealing position).

Remove the lid once the pressure has dropped and the lid released. Season to taste and drizzle with olive oil and serve. Optionally, you can transfer the content of the Instant pot into a large shallow casserole dish and brown under a broiler preheated to high for about 10 minutes.

Don't Have the Instant Pot?
If you don't have an Instant Pot, you can use a large casserole dish or Dutch oven and prepare your Osso Buco in the oven. To do that, preheat the oven to 160 °C/ 325 °F. Brown the meat and cook the aromatics in the casserole dish — just like you would using the SautĂ© program on the Instant Pot. Add the browned veal shanks and the remaining ingredients. Cover with a lid and bake in the oven for 75 to 90 minutes.
