Oh this is a whole can of worms!
There's the literal interpretation that Gortwog is descended from Tiber Septim (which is in teresting, considering Tiber Septim used anti-orc racism to unify man and mer -- yes, orcs are mer, but that wasn't really what most thought at the time). Of course, orcs proved themselves in the legions, but didn't really gain full equality until... after Gortwog, actually. Still, one of Tiber's relatives could have done it.
Yet, there's another interpretation. Martin Septim is not, in fact, the descendant of Tiber Septim (he is descended from Tiber's brother or uncle, one of the two); the direct line of Tiber died quickly. Yet he could use the amulet of kings.
Which brings us to the concept of Royalty in Elder Scrolls. In daedric it is called CHIM, but I don't mean the meme-y, transcend reality CHIM (though that is a thing too). Rather, the blood of Tiber Septim can be translated as the blood of Royalty or the blood of Man, indicating a sort of divine right as one of power; not divine right in the christian sense, but more the paganistic sense; the greek conception of the hero. One who is powerful, and thus ordained.
When Tiber Septim turned the jungles of Cyrodiil to farmland, he "Breathed royalty" -- uttered the syllable CHIM. The gemstone embedded in the Amulet is Chim-el Adabal which I think itself translates to something to the effect of Blood of Royalty as well. It (and the totem/mantella) can only be wielded by those with the Blood of Royalty -- originally, Alessia's lineage.
But Reman was not descended from Alessia (unless you interpret the Remanada literally, but I'm not sure you should, and even then it's more Alessia as a mountain), and Tiber Septim (unless you believe some extremely biased propaganda) is likely unrelated to Reman. Yet they can all wear the amulet.
All of this to say that, "blood of Tiber Septim" COULD be read as a bloodline descendant, but it could also be read as simply being Annointed with royalty; having a pre-ordained fate, doom-driven towards greatness.
Plus, you know. All of the candidates in Daggerfall are the rulers of the kingdoms in Iliac Bay -- Wayrest, Daggerfall, Sentinel, etc. Mannimarco can ALSO use the Mantella, and he predates Tiber Septim, yet is of the Blood of Tiber Septim. The Underking is either Zurin Arctus, Wulfharth or both, and both of them can't be descended from him either.
In short, I believe the blood of Tiber Septim refers more to the abstract concept of Royalty (CHIM, but the more grounded symbolic kind) rather than literal blood relation!

to any tes lore experts, what are mara's thoughts on lycanthropy? I've been looking around and the closest to anything I can get is that the ancient nords called her "she-wolf" bcs of the wolf symbol. ik lycanthropy was a daedric blessing/curse (I can be yuor angle or yuor devil type shit, insert other memes here) from hircine and therefore it might be frowned upon by the gods but I'm not sure

When your parent asks you one little innocuous question about something that they know a little about and it sends you into a 3 hour rant about the complex philosophy of the fallout games and the history of their development vs the elder scrolls games and how Bethesda is gradually devaluing all of its intellectual properties and how fallout fits into a very niche category of video games and why good writing and atmospheric storytelling and ambiguity are so important to it
