There are two types of teeth whitening products that can help you get rid of your discolored teeth. One is the tray system and the other is the bleaching pen. However, both have their own advantages and disadvantages. Toner tray system involves applying the bleaching gel directly onto the teeth. However, this method is not very effective as it only brightens the shade of teeth that are discolored and not eliminate the discoloration completely. On the other hand, using the bleaching pen is less effective as compared to the trays as the gel gets accumulated in between the teeth unlike the trays, which remain on the teeth for an extended period of time.

Americans support Black Lives Matter but resist shifts of police funds or removal of statues (Washington Post) A majority of Americans support the Black Lives Matter movement and a record 69 percent say black people and other minorities are not treated as equal to white people in the criminal justice system. But the public generally opposes calls to shift some police funding to social services or remove statues of Confederate generals or presidents who enslaved people, a Washington Post-ABC News poll finds. These findings underscore the mixed fallout after the brutal killing of George Floyd while in the custody of Minneapolis police in May. There is increased public scrutiny of police treatment of black Americans, but less unity on broader questions about how to address the country’s treatment of black Americans since its founding. Americans’ confidence in police appears shaken after a wave of national protests following Floyd’s killing. Compared with 2014, fewer Americans say they are confident that police are adequately trained to avoid using excessive force. Meanwhile, more people say recent police killings of black people are “a sign of broader problems” in police conduct.
Opposition grows to US agents in cities (AP) The Trump administration is facing growing pushback—in the courts and on the streets—to sending federal agents to Portland, Oregon, where protests have spiraled into violence, and vowing to do the same in other Democratic-led cities. Far from tamping down the unrest that followed George Floyd’s death at the hands of Minneapolis police, the presence of federal agents on the streets of progressive Portland—and particularly allegations they have whisked people away in unmarked cars without probable cause—has energized two months of nightly protests that had begun to devolve into smaller, chaotic crowds. President Donald Trump’s administration also faces multiple lawsuits questioning its authority to use broad policing powers in cities. One filed Tuesday says federal agents are violating protesters’ 10th Amendment rights by engaging in police activities designated to local and state governments. Oregon’s attorney general sued last week, asking a judge to block federal agents’ actions. The state argued that masked agents have arrested people on the street, far from the U.S. courthouse that’s become a target of vandalism, with no probable cause. The use of federal agents against the will of local officials also has set up the potential for a constitutional crisis, legal experts say. It could escalate if Trump sends federal agents elsewhere, as he says he plans to do, including to Democratic-led Chicago.
New Yorkers ditching city for elbow room fuel housing boom (AP) If the outbreak roars back in New York City, Anil and Joyce Lilly will not be sheltering again in their Bronx apartment. They just bought a house an hour north in the Hudson Valley. “We need more elbow room,” said Joyce Lilly, explaining their move to Washingtonville, New York. “Because we were locked into the apartment for three months, a solid three months, I feel like I’m getting out of prison and I want to run as far away as possible.” New Yorkers anxious after weathering the worst of the coronavirus pandemic are fueling a boom in home sales and rentals around the picturesque towns and wooded hills to the north. Real estate brokers and agents describe a red-hot market recently, with many house hunters able to work from home. “There has been a big uptick from Manhattan people, no doubt about that,” said Steven Domber, president of Berkshire Hathaway HomeServices Hudson Valley Properties. “Number one, it’s cabin fever, which is wanting to get out of an apartment and having some land if, God forbid, there’s a lockdown again.”
Powerful 7.8 quake hits Alaska isles (AP) A powerful 7.8 earthquake struck the Alaska Peninsula late Tuesday, triggering a tsunami warning that sent residents fleeing to higher ground before it was called off without any damaging waves. According to the U.S. Geological Survey, the 7.8 magnitude quake struck Tuesday at 10:12 p.m. local time. The quake was centered in waters 65 miles (105 kilometers) south-southeast of Perryville, Alaska at a depth of 17 miles (28 km), deeper than an earlier estimate. According to the USGS, since 1900 there have been six other earthquakes of magnitude 7.0 and higher within 155 miles (250 km) of Tuesday’s quake. The largest of those was a 8.2 quake in 1938.
US Navy problems (Bloomberg) The inferno on the $4 billion USS Bonhomme Richard caps a dismal few years for the U.S. Navy, James Stavridis writes. The retired admiral and former NATO supreme allied commander points to a litany of black eyes, including a series of corruption scandals; two deadly collisions involving $3 billion Arleigh Burke-class destroyers; the mishandled prosecution of an accused war criminal; and the coronavirus outbreak on a nuclear aircraft carrier in which one crew member died, the commanding officer was relieved of duty, and the acting secretary of the Navy quit under pressure.
U.S. seeking arrest of Venezuela chief justice, offers reward for info (Reuters) The United States on Tuesday imposed sanctions on Venezuelan Chief Justice Maikel Moreno and announced a $5 million reward for information leading to his arrest or conviction for allegedly participating in transnational organized crime. In a statement, Moreno said Venezuela’s judiciary would not accept “tutelage” from a foreign government. The United States charged Moreno in March with money laundering under a round of indictments against Maduro and more than a dozen other top Venezuelan officials on charges of “narco-terrorism” in an escalation of the Trump administration’s campaign aimed at ousting the socialist leader.
Nearly 40% of Europeans in their late 20s still live at home (Quartz) Do you have fond memories of living with your parents? An attachment to the neighborhood where you grew up? For two in five Europeans in their late 20s, the four walls of their parents’ home is their daily reality. In 2017, 39.3% of young adults in the 28 countries of the European Union still lived with at least one parent, according to data from the European Union Statistics on Income and Living Conditions survey. The proportions varied wildly between countries: While only 4% of Danes between 25 and 29 still lived at home in 2017, 75.4% of their Croatian peers did. Economists blame the phenomenon on the 2008 recession and the following European debt crisis, which created a “boomerang generation” that moved back home because they either couldn’t find jobs or were the first to lose them.
Fearing new coronavirus wave, Barcelona limits number of beach-goers (Reuters) Barcelona authorities on Tuesday cut the number of people allowed on to the city’s beaches to 32,000 from 38,000, after crowds flocked to the sea at the weekend despite advice to stay home to curb a local rise in coronavirus infections.
Ukrainian president ends hostage crisis (Foreign Policy) Ukrainian President Volodymyr Zelensky was forced to record a video recommending a 2005 Joaquin Phoenix-narrated documentary on animal cruelty in order to help free 13 citizens being held hostage in the town of Lutsk. The demand was made by Maksym Kryvosh, an animal rights activist who had seized a bus and began an armed standoff with police with the 13 hostages on board. Shortly after the hostages were all freed, Zelensky deleted the video.
U.S. tells China to shut Houston consulate in move China calls ‘unprecedented escalation’ (Reuters) The United States has told the Chinese consulate in Houston to shut down in three days, citing a need to protect American intellectual property and information, amid a sharp deterioration in relations between the two countries. Beijing condemned the order and threatened retaliation. A source said China was considering closing the U.S. consulate in the city of Wuhan. Ties between the United States and China have become increasingly tense since the outbreak of the novel coronavirus in Wuhan at the beginning of the year.
Hong Kong orders masks indoors in public as daily cases hit record (Reuters) Hong Kong will extend strict social distancing measures from midnight on Wednesday, ordering masks in all indoor public areas including malls and markets, as the global financial hub reported a daily record increase of novel coronavirus cases. Hong Kong tightened social distancing measures in July after a spike of locally transmitted cases and as authorities warned about a third wave of infections. The new order for masks in indoor public places would be for two weeks.
Taiwanese rebranding (Foreign Policy) Taiwan’s legislature has voted to rename the country’s main flight carrier, currently known as China Airlines, and to remove the phrase Republic of China from Taiwanese passports. The increasingly anti-China mood in Taiwan is likely to produce more such rebrandings: The country’s official name, the Republic of China, is now seen as a holdover from a different era. Such moves are likely to produce hysterical reactions from Beijing, reemphasizing the need for Taiwan’s long-term national strategy of decoupling from its largest trade partner.
Crisis hits Lebanon’s hospitals, among the best in Mideast (AP) Lebanon’s hospitals, long considered among the best in the Middle East, are cracking under the country’s financial crisis, struggling to pay staff, keep equipment running or even stay open amid a surge in coronavirus cases. Private hospitals, the engine of the health system, warn they may have to shut down. Chronically underfunded public hospitals, which have led the fight against the virus, fear they will be overrun. Across the country, hospitals and doctors are reporting shortages in vital medical supplies such as anesthesia drugs and sutures. With power cuts that run through most of the day, they pour money into fuel for generators, and many are turning away non-critical cases to conserve resources. “The situation is really catastrophic, and we expect a total collapse if the government doesn’t come up with a rescue plan,” said Selim Abi Saleh, the head of the Physicians Union in northern Lebanon, one of the country’s poorest and most populated regions.
Aid from top donors drops even as need soars (AP) A new snapshot of the frantic global response to the coronavirus pandemic shows some of the world’s largest government donors of humanitarian aid are buckling under the strain: Funding commitments, for the virus and otherwise, have dropped by a third from the same period last year. At a time when billions of people are struggling with the pandemic and the ensuing economic collapse—on top of long-running disasters like famine, drought or unrest—more, not less, money is urgently needed. New virus protection equipment must be bought for almost everything, from maternity wards in African villages to women’s shelters in Syrian refugee camps. During the first five months of this year, overall aid commitments from the largest government donors were $16.9 billion, down from $23.9 billion in the same period last year, according to the new analysis, which drew on data from the United States, the United Kingdom, European Union institutions, Germany, France, Canada and others. Many of these donors—notably the U.K., whose aid commitments have dropped by nearly 50% from last year, according to the analysis—are struggling as their economies contract. The sheer magnitude of the crisis is another challenge as every part of the world needs help—and now.
Power cuts return, adding to frustrations in COVID-weary South Africa (Reuters) Keitumetse Modise was already struggling to juggle her appointments as a freelance beautician while home-schooling her daughter during South Africa’s nearly four-month-old COVID-19 lockdown. The last thing she needed was the lights going out. But after being forced to turn kitchen tables into classrooms and bedrooms into home offices, pandemic-weary South Africans now face an added challenge in the form of a familiar headache: rolling blackouts. Months of pandemic-induced shutdown and reduced demand had actually resulted in uncharacteristically reliable electricity for those stuck at home, as South Africa’s creaking grid was given some respite. Since last week however, with restrictions easing and businesses cautiously reopening, planned blackouts—known locally as loadshedding—are back and with them the frustration of unstable internet, lost revenue and disrupted schedules.

Curb Your Enthusiasm
While we have made tremendous strides fighting the coronavirus, we want to temper some of that enthusiasm as it relates to the markets as we have a long way to go until things are settled. Clearly it is good news that growth in the number of new cases of the coronavirus is cresting which will permit some states and countries to open. We believe that the pace of the recovery will be slow until we have a therapeutic, antivirals, contact tracing, more testing including antibody tests and eventually a vaccine. While we finally can see light at the end of the tunnel, we will not see a semblance of how things were until 2022.
The financial markets are fighting a tug of war between global economic weakness and tremendous liquidity to the tune of over $7 trillion having been created so far by all the monetary authorities and governments around the world. We find it amusing to listen to the pundits who constantly remain one step behind getting bearish as markets decline and bullish when the tape is rising.
We are in a unique economic environment where we see only a handful of fundamental winners over the next few years.  These companies are benefitting directly and indirectly from the new normal where we spend more time in our homes utilizing smart devices to access services heretofore done in person. We would continue to avoid all companies needing government assistant who are in financial trouble due to the drop off in demand as a result of the coronavirus. We do not see demand for most of these companies returning to levels achieved just a few months ago until 2022 therefore profitability and balance sheets will remain challenged and at risk. Many may not even make it. We see the market tiering further over time giving higher than historic valuations to the winners in the new normal while avoiding the losers.
More than ever this is a market of stocks rather than a stock market. Active management is the only way to invest in the new normal as passive money managers will hold many of the losers that need to be avoided at all costs. It will be interesting to hear what Warren Buffett says tomorrow at Berkshire Hathaway’s online Annual Meeting. While he is the ultimate active manager, he has sung the virtues of passive investing for years.
The global number of coronavirus cases as of Friday hit 3.28 million with 234,000 deaths while the number of cases in the U.S now exceeds 1.1 million with over 63,800 deaths.  While the rate of gain has crested, we remain concerned that countries and states are opening too soon as we do not have enough testing nor is there a therapeutic available so we can feel safe. We get how hard it is for politicians to risk of opening too soon. They have to weigh economic, social and emotional impact of not opening vs. the risk of the virus continuing to flourish and possibly resulting in death. Personally, my biggest concern is an acceleration of cases as we open, which, of course, would cause another lockdown. As we open slowly, we need to maintain strict social distancing and making the wearing masks mandatory for several more weeks.
We heard last week that Gilead’s Covid-19 treatment Remdesivir showed encouraging results from a key U.S trial. Patients who received the drug recovered in 11 days, on average, while those taking a placebo took 15 days. Both Drs. Fauci and Hahn, after reviewing the data, said that this could be a game changer and are supporting emergency authorization by the FDA to permit its use. And the FDA did grant permission for emergency usage yesterday.  There was also some positive news on a vaccine from Moderna and Oxford University/Astra Zeneca. Bill Gates, who is investing heavily in this area, went out on a limb predicting a vaccine possibly as soon as in 9 months but certainly within 18 months. There are over 100 drug companies working on therapeutics, testing and vaccines. All good stuff!
It appears that monetary authorities have unlimited capacity to provide liquidity to the financial markets. We heard last week from the Fed, BOJ, Bank of England, and the ECB that each will buy unlimited amounts of bonds in response to the coronavirus. Again, all this liquidity will not create demand which really is the only way out of the global recession/depression. We will need as Fed Chairman Powell and ECB President Lagarde both said major fiscal spending programs to boost demand while also providing money to individuals/companies to make it to the other side. We expect our government to implement Phase 4 and 5 programs as needed to supplement individual incomes and provide money to small/medium size businesses to stay afloat rather than filing bankruptcy.
Bonds spreads continue to narrow which shows that monetary policy has provided enough liquidity to assuage market concerns. The bond market is operating very efficiently permitting companies to raise money up to 100 basis points better than expected. Boeing’s 25 billion offerings were oversubscribed by 3-4 to 1. U.S high-grade bond issuance continues to set new records each week.  All good news!
We focus on long term profitable investing with a 12- to 24-month time frame. The market environment remains favorable as the Fed is providing so much excess liquidity not needed by the real economy such that it is pushing investors further out on the risk curve. We always maintain cash reserves knowing that the market could have a correction at any time providing opportunities to add to our portfolios at lower prices. We believe that the U.S economy will be bottoming out shortly which is good news even though we think the recovery will be an elongated U.
Not all stocks are equal. Corporations have never faced an environment like this so it is important to listen to earnings calls so you can differentiate the winners from the losers. Especially in today’s environment, you need to focus on management, strategies, and financial strength. Avoid at all cost companies with demand and financial problems as it will last well into 2021 at a minimum.
We continue to focus on the winners in the new normal. Our list includes companies whose business models are mostly tied to the web/internet/data centers where usage has gone through the roof. Our positive view on these companies was supported by their first quarter earnings reports and conference calls which demonstrated all their favorable short- and long-term strengths. The best is yet to come for each of them. The Democrats are proposing an $80 billion-dollar bill to promote broadband expansion and buying smart devices for all that cannot afford them. 5 G will play a major role as the next generation for cellular technology. If you can, listen to the Qualcomm conference call. We also added a financial and an industrial. Both are best in class and will gain market share/profitability as we move through the other side.
We do not own bonds, the dollar nor private equity funds.
Finally, we would hope that all countries work together investigating the origin of the coronavirus rather than doing it individually.
Our weekly webinar will be held on Monday May 4th at 8:30 am EST. You can join the webinar by typing https://zoom.us/j/9179217852 into your browser or dialing +14086380968 or +16465588656.
Remember to review all the facts; pause, reflect and consider mindset shifts; turn off your cable news; do independent research listening to the earnings calls and…
Invest Accordingly!

There is a thin line between sacrificing a lamb and striking a deal with the Devil.
We give up whole parts of ourselves to belong in our families. In turn, for those of us who dare to come home to ourselves, we risk losing our family and severing the ties that bind us.
When I was twenty-one, I became the first member of my family to earn a college degree. In hindsight, this seemingly positive milestone, or the culmination thereof, both gave and spared me a lifetime of heartache. By achieving an advanced education and moving just an hour from home, I unknowingly left my family, and in doing so, embarked on the long, arduous task of breaking through the invisible (but formidable) barriers of class and intergenerational trauma.
Pittsfield is a city people never leave or never return to; I only knew I had to go —that hanging out with girls who were “dating” their father’s friends and losing five of my cohort in just ten months to alcohol, suicide, and drugs filled me with foreboding. My peers and I shared a unique darkness. One that went beyond the cynical, independent, and pragmatic nature that hallmarks Generation X. We shared history rooted in trauma bonds. Collective memories steeped in Black Sabbath and Pink Floyd, psychedelics and Jack Daniels, sex hallmarked by confusion versus consent, a blur between victim and perpetrator —think Lord of the Flies meets Heavy Metal.
Despite having just over forty-one thousand residents, my hometown lays claim to one of America’s highest crime rates (from the smallest towns to the very largest of cities). If you visit, you have a 1 in 27 chance of being a victim of a violent crime. Put differently; you’re more likely to be mugged or collide with a drunk driver than to get COVID19 while not wearing a mask. The irony is the city lies nestled in the center of the sleepy Berkshire hills. The surrounding landscape, a living Norman Rockwell painting, populated by wealthy New Yorkers and nineteenth-century “cottages.” Home to the Boston Symphony Orchestra and Tanglewood, where tourists eat bacon-wrapped figs and sip Sauvignon Blanc on the lawn. The Berkshires —where you can visit Herman Melville’s house in Lenox and score crack in Pittsfield, all in the space of an hour.
My twenty-one-year-old self-fled to the Pioneer Valley, and misfit though I was, I claimed it as my home. Just fifty-one miles as the crow flies, it kept me within driving distance of my closely knit (but) turbulent clan while affording me the possibility of a new life. Northampton was both academic and bohemian, brimming with universities, bookstores, cafes, and the arts. It was an altogether different planet, and it terrified me.
I had no idea of the implications of this move —of what it meant to transition from a working-class family in a post-industrial ghost town ravaged by racial and class warfare to a white-collar world steeped in privilege and academia. I could not foresee the coils that spun out from my childhood to my future. How they’d wrap around my life like the tentacles of a giant squid, choking me, pulling at my dreams, dragging me under —how I’d thrash, how it would take decades before my lungs acclimated to the water that would birth me, and the casualties of connection to be incurred along the way.
*****
When we were teens, we traversed Pittsfield via an underground network of train tracks. We believed that if we put an ear to the railway metal, we would hear the train coming long before seeing it. That as long as we maintained a vigilance by pressing an occasional cheek against the hot-rolled steel, we’d anticipate the train’s arrival —hear the hissing of the rails, feel the engine’s vibration in our skull. In hindsight, this is how we lived our days. A trick we played to maintain the illusion of immortality –we believed that a car full of balloons would cushion a crash, that powder and smoke were less lethal than needles.
The reality was, we were often too stoned or just plain afraid, so we never actually listened for the train. Never anticipated the deaths of our friends.
Tumblr media
We lost the first one to suicide. Pinned between two car bumpers on a Friday night bender, Paul never acclimated to his right legs’ amputation. Several months following the accident, he shot himself in the face in front of his fiancé. Then there was the motorcycle crash. Timmy was a bad boy from the town’s outskirts; he had warm cocoa curls and a smile sweeter than John Travolta. He flew his Harley around a corner, jacked on cocaine, and never landed. That same Autumn, up Barker Road, Ryan and Ellen wrapped their green Chevy Nova around a maple tree — he lived, she did not, their newborn baby home sleeping in her grandma’s arms.
Dearest to me was Bill, driven mad by an excess of Gooney Birds —that particularly potent blotter he partook of as a daily sacrament, so much so that the blur between his tripping and psychosis became indistinguishable. I can personally attest to the magic in those dime-size tabs, how it tingled your tongue and altered reality for days. Under its influence, I saw a bag of marshmallows breathe, watched my cousin’s hand melt into the ochre shag of a van rug. That November, Bill’s delusions drove him wild and deep into the woods of Hatfield; his body found unmarred amongst the ashen brush. The authorities said it was a lacerated liver, that he bled to death internally —that it was like going to sleep.
*****
At what moment do we begin the slow and steady handing over of our hearts? I remember being six and staring at dirty linoleum, my mother sobbing on the kitchen floor by the dishwasher. There were shards of glass underfoot; to walk toward her would require cutting myself. I believed that I had broken her —that my sister and I spawned a storm so vast that our home would not see sunlight for months. Our Italian grandmother and father concurred. So, I clapped my hand over my mouth each time my voice yearned to escape and swallowed it whole. Again, and again, I walked barefoot on glass to reach her. A little blood seemed a small price to pay. Slowly, I learned about relational transactions, equating love with pain, and silence with safety.
There is a thin line between sacrificing a lamb and striking a deal with the Devil. The first (we hope) affords us blessings and wishes. The latter steals our soul and damns us. When we offer up our voice in exchange for belonging, we silence our longing. It is a curious thing to consider; that to no longer Be our Longing, we must sever something, and it leaves me wondering what becomes of our hunger?
For me, my father’s blows and punches — an act of desperation intended (literally) to knock some sense into my inebriated fifteen-year-old head, no longer registered pain. My mother’s second wave of melancholy did not inspire compassion. The afternoon five girls ambushed me in a ballfield, and I felt the bubble gum on my tongue crumble like chalk when mixed with blood (a chemical reaction few have experienced) —I floated above the grass. Any part of me that longed for tenderness, validation, reassurance, and kindness burned down
—this is what trauma does; it begets and destroys, permeates, and empties.
*****
Fortunately, memory is malleable. To evoke a memory is to flick a switch —light up a constellation of neural pathways that are as intricate and ever-changing as the night skies. Our recollections are not so much facts as they are stories, and like all works in progress, they are subject to edits and revisions. Memory is as affected by our perceptions of the present as our perceptions of the past. This concept offers immense hope for those of us who have had bad things happen, which is to say —Everyone.
Implicit in this idea is that our perceptions can radically shift our stories —that when we mine our past for meaning, we will arrive at new understandings concerning our misfortunes, sorrows, and pain. Our divorce will no longer be a disaster, but rather a turning point that catalyzed a life otherwise not possible. A malignant tumor might serve as a wake-up call to a life otherwise spent underwater and holding our breath. I’m not implying we should wish adversity on ourselves but rather acknowledging that ultimately, we will all belong to some club. The “I lost my spouse to suicide” club. The “I had seven miscarriages and ten years of fertility treatment” club.” The “My mother was an alcoholic and my father left when I was two” club. To be alive is to be in a club.
Tumblr media
I believe the road to wholeness begins with the slow and steady patching of our hearts’ fractured pieces. That by stitching together tiny moments of connection, risk, and vulnerability, we find our way Home. That it’s not a straight line, but a somewhat never-ending journey where hopelessness, fatigue, and lapsing into old habits is standard. As we age, there lies the potential to write our story versus having our story write us. And if we stay the course and remain open, we will slowly assemble a network culled through friendship, psychotherapy, surrogates, and self-made kin. We will come to a deeper understanding of the hows and the whys of our life and we will find our people.
It took me thirty-one years of individual therapy, earning my master’s degree in Psychology, becoming licensed as a psychotherapist, moving one hour and a lifetime away from home, one marriage, a divorce, and a child to find my way. The cost —immeasurable. To paraphrase Maya Angelou, I belong nowhere because I belong everywhere. I belong to myself. I belong to a tribe of tattooed scavengers who have mastered the art of melding dung to feathers —a band of gypsies, ravens, and heretics who hover between scrappy and soulful —who happily fly alongside Icarus, broken wings and all.
What we share beyond our common humanity is a visceral knowing that suffering is here to stay. That trauma is inseparable from life. That loss is both holy and abysmal, and that grief is, in turn, the most sacred and proper response to joy. We are all wretched and omnipotent, sitting in the sun and soaked to the bone.
This is what trauma does.

Health @ Retail – Prelude to GMDC SelfCare Summit with Updates from Hims & Hers, GoodRx, Sam’s Club and Amazon Care
“We knew millions of people weren’t getting the care they needed — they were either too embarrassed to seek help or felt stuck in a system that was confusing and intimidating. Digital health has the potential to radically change the way people approach their wellness and, since launching in 2017, we’ve outpaced even our own expectations, delivering more than 1 million Hims & Hers products to our customers. In collaboration with highly-qualified doctors and healthcare providers, we’ve built a digital health platform that is changing the way people talk about and receive the care they need.”
That’s  a verbatim paragraph from a blog on the Hims & Hers’ Medium channel. published on 30 September 2019.
As we approach this coming weekend’s SelfCare Summit sponsored by GMDC, I continue to update my retail health ecosystem graphics and landscape. This post will be my latest read on the issue which I’ll discuss on Friday afternoon 5th October in a session titled, “The New Health/Care: Patient-Led, Retail-Enabled.”
Over the past sixty days, the business rationale for “getting people the care they need” could have been written into press releases from Sam’s Club and Walmart, Amazon, Best Buy and GoodRx, all of which announced plans in the past few weeks to extend health/care services in the U.S. beyond hospitals and bricks-and-mortar doctors’ offices.
Start with Hims and Hers, moving in the opposite direction from telehealth care focused on men’s and women’s health, morphing into a physical pharmacy distribution model. Hims and Hers will invest in a Columbus, Ohio-based mail order pharmacy to be operational in 2020, featuring customer support and fulfillment centers. Up to now, the companies have been working with third-party pharmacies to pick-and-pack medicines for consumers looking to support their gender-specific wellness needs.
Now consider GoodRx, linking up with HeyDoctor to help the prescription drug discounting platform expand telehealth services. The organization will be known as GoodRx Care.
The press release announcing the acquisition of HeyDoctor on 26th September noted that, “This new service comes at a time when Americans find that getting access to primary care physicians to be increasingly difficult….As healthcare costs continue to rise and many insurance plans cover fewer services than they did a year ago, consumers need affordable options for their routine health needs. With the introduction of GoodRx Care, Americans now have easy access to a network of qualified medical professionals so they can get the treatment they need, when they need it….GoodRx Care aims to help fill in the gaps in care to improve access, adherence, and affordability of medical care for all Americans.”
HeyDoctor branded itself as an online urgent care provider with an app that’s generated good word-of-mouth among users. Among its most-used services are help for urinary tract infections, birth control, acne and cold sores, giving you an idea of the organization’s key (young) demographic.
Addressing the access and affordability pillars, Sam’s Club launched a collaboration with Humana called Care Accelerator. As the third graphic illustrates, consumers can opt into one of four health care “bundles,” including components such as generic medications, primary care, vision and dental care, hearing and alternative medicine services. This isn’t meant to be a fully-loaded insurance plan, but a product that “curates” selected medical services consumers want to access at relatively low-cost to care for basic health care needs.
As I often say, “collaboration is the new black” in health care, and the Care Accelerator program brings together different partners to deliver components of care to the packages: in addition to Humana, Sam’s Club is working with Quest Diagnosis for lab testing and 98point6, an on-demand text-based app for primary care consultations. The 98pointsix primary care virtual visits cost $1 per visit and are included in the lowest-priced program at $50 per year, which also includes free select generic drugs sourced at Sam’s Club pharmacies, and discounted eye exams and eyewear.
The program is being piloted in Michigan, Pennsylvania and North Carolina and if it’s found viable and consumer-loved, Sam’s Club will expand it to club members across the U.S.
Programs range from $50 to $240 a year for the most complete plan. That $240 plan covers up to six family members and includes a preventive health screening looking at diabetes and heart disease.
Note that Sam’s Club is part of the Walmart business family, which has been fast-expanding its footprint in health care and wellness in the past several months. I’ve addressed the company’s growing health/care ethos here in Health Populi in these posts written this year —
Walmart’s Growing Footprint in Health/Care, from Guns to Mental Health and Gardens
FANGs & MAGA – Meet WaWa for Health, Walgreens and Walmart
How Walmart Could Bolster Healthcare in the Community
And for historical perspective, this post published over a decade ago in 2008!
Drugstore Dominance, and the Walmart Wild Card.
Finally, Amazon announced Amazon Care, its first clinic for employees in the headquarters Seattle area. This is not so much Amazon hanging up a brick-and-mortal medical center-shingle (like Walmart’s recent launch of a freestanding clinic model)  but a hybrid of virtual care and in-person services.
The Los Angeles Times analogized Amazon Care‘s disruptive to health care providers to Amazon Prime up-ending the retail shopping landscape.
It’s important to call out the technology media’s interest in this development: here’s Computerworld‘s take on the project, titled Why Amazon Care May be the New Model for Corporate Healthcare. Beyond the obvious link between Amazon as a tech company and the Haven alliance between AMZ, Berkshire-Hathaway and JPMorganChase, Computerworld mentions Apple’s AC Wellness concept for employee health clinics and smart businesses’ tighter focus on healthcare spending as, “a wake-up call for providers, payers and employers…(ushering in) a new operating system for health, and big technology companies are not going to wait for everyone else to figure it out.”
Health Populi’s Hot Points:  Stay tuned to my tweets @HealthyThinker and posts here on Health Populi between Thursday 3rd through Saturday 5th October as I listen to and learn from fellow speakers at the GMDC SelfCare Summit. We’ll cover a wide range of topics, from the future of retail health channels and trends, health plans’ approaches to inspiring members in self-care, new pharmacy formats, food-as-medicine, and consumer self-care forecasts.
I’ll be focusing on the patient-as-payor with high expectations for service and support for self-care health care for ourselves, our families, and in our communities.
Immediately following the SelfCare Summit, I’ll fly from Indianapolis to Kansas City where I’m delighted to meet with the Cerner user conference to brainstorm health consumers and healthcare costs in the context of health IT, revenue cycle management, and the patient-as-payor. Stay tuned to my tweets and posts there on Monday and Tuesday, 7th and 8th October.

Central Florida 100: Housing, UCF and mourning New Zealand
Thad Seymour should be the interim president of UCF for a year or more, trustees decided Thursday. (Courtesy of UCF)
Chris Carmody, shareholder, GrayRobinson
Last week: On Thursday, for the second straight year, the Florida Senate passed legislation aimed at protecting vegetable gardens on private property. Wheat a second, that’s illegal? Well, not here in central Florida, but some south Florida cities got all artichoked up about front lawn gardens and outlawed them. Senator Rob Bradley thought it was about thyme to change that. His bill would prevent local govern-mint from restricting gardens, and exempts water control, fertilizer and invasive species ordinances. The bill still has a long road to hoe in the House, but perhaps this year the seeds will sprout. After all, it ain’t over lentil it’s over.
Last week: Recently, I had the opportunity to speak to two different organizations, a local LGBT group and the Trump Club of Seminole County. Clearly, these organizations have little in common. However, my presentation at both focused on recent successes and challenges regarding infrastructure, environment and public safety. Ironically, both had similar questions and concerns. It reminded me that perception is not always reality. People are people and at their core, everyone has the same desire for a quality life, safety in their homes and freedom to pursue happiness. If we concentrated on our shared goals, instead of our differences, achieving them would be infinitely easier.
Last week: So here is a great question…why would you leave your pet tied up or abandon them during a natural or manmade disaster? You don’t! If this is the kind of person you are, then you don’t deserve the privilege of having a fur baby as a friend. A bill, which was filed on March 1, would make it a criminal act to leave pets chained up during such events, specifically by making it a first-degree misdemeanor. Under the bill, those who leave their pets unattended or restrained during a disaster would face up to a year in jail or a fine up to $5,000. Perhaps this light sentence will make a person think next time they abandon man’s/woman’s best friend. Probably not.
Last week: In the aftermath of the horrific terrorist attack in New Zealand, it was heartwarming to see so many Orlandoans rallying together to support our brothers and sisters on the other side of the world. For those who lived here at the time of the terrorist attack at the Pulse nightclub, we know first-hand how important these rallies are in the aftermath to a community simultaneously grieving and healing. While we’re thousands of miles removed from each other, every modicum of support that we demonstrate towards the Muslim community of Christchurch is an affront to the terrorists and serves as a small step forward in the healing process. As those in Christchurch stood with Orlando in 2016, we stand with you now.
Francisco Gonzalez, philanthropy director, National Review Institute
Last week: Congratulations to Andrew Gillum for finding a job! Florida voters rejected him in November, but he still had $3.9 million cash on hand from that campaign and he’s found a way to use it: on a voter registration drive. Andrew seems to be really good at getting out the vote — and finding a way to pay himself for this new job.
Looking ahead: It’s amazing how far to the Left – and how far outside the Constitution – many in the Democratic Party have gone recently. There was a day when the biggest fight between Left and Right was about the size and role of government. Now, it’s about the foundation of the Republic itself. Top candidates for the Democratic ticket want to get rid of the Electoral College and have suggested reinventing the Supreme Court, while also consistently attacking the Bill of Rights. This is scary stuff. For the future of our country and the world, let’s work to keep it fantasy.
Eric Jackson, president/CEO, Total Roof Services Corp.; board member, Children’s Home Society of Florida
Looking ahead: The recent passage of the federal Family First Prevention Services Act sends a clear message: Children belong with their families. I’m encouraged to see the federal government’s commitment to invest in front-end services like parenting education, substance abuse treatment and counseling so more children can remain safely at home. This is a direction Children’s Home Society of Florida has been moving toward for years as it strives to end the need for foster care as we know it by keeping more families safe, strong and together. As Florida prepares to implement this legislation, I look forward to seeing the collaboration among the state and community-based care lead agencies.
Last week: In November, Floridians adopted Amendment 4 by a nearly two-thirds majority, restoring voting rights to convicted felons who had served their time and completed all other requirements. Now the Legislature is considering a bill that can potentially delay or take away those newly-reinstated rights — especially for lower-income people. Voter enfranchisement is supposed to level the playing field.
Looking ahead: Warren Buffett is offering a million dollars a year for life to anyone (or, at least, any of his employees) who picks a perfect Sweet 16 bracket for the NCAA basketball tournament. I took a look at the schedule, and I’m confident I’ve got a winner. I’m picking "TBA" for all sixteen. Now all I need is a job at Berkshire Hathaway.
Ken LaRoe, founder and CEO of First GREEN Bancorp
Last week: Thanks to Gov. DeSantis and Agriculture Commissioner Fried, the will of the people was finally met with the smokable marijuana bill. What this really means is that the whole flower — with its unique set of medicinal properties — can now be purchased and used in its many different methods of administration. What we need next is recreational. I’ve heard repeatedly the horrors of recreational. That is absolutely untrue as cannabis is not a gateway drug, is not addictive and cannot cause overdose. Let’s move the discussion out of the dark ages.
Looking ahead: I live in rural Lake County where I grew up. It used to be peaceful and quiet. Since all gun laws were made illegal by our NRA-pandering Republican legislature, every gun nut has set up a DIY gun range in their backyard. It’s basically like Aleppo all day long with semi-automatic and fully automatic gun fire going off. That is bad enough but now a number of the inconsiderates have starting shooting at night! Our sleep was rudely interrupted the other night at 11 p.m. by gunfire that lasted 15 minutes. When is the gun insanity going to stop?
Last week: The Altamonte Springs autonomous shuttle project has been in the works for over 20 years. Officials could not foresee autonomous vehicles back then, but they certainly could foresee a need for an S.R. 436 parallel corridor for transit. Businesses along the corridor have been giving the City easement and right-of-way access ever since, at no cost to taxpayers. What could cost Florida taxpayers is House Bill 4043 – a $2 million gift to the city to kick start a project between an unnamed private venture and the city. Privately run public shuttles could succeed. However, taxpayers subsidizing private entities are often on the hook for more.
Looking ahead: Police lurking in the shadows to catch speeders is neither safe, nor is it effective in reducing speeding. It’s also a sleazy way to enforce the speed limit. We need our police to be out in the open and visible to drivers. Assuming the police are there in the interest of public safety, what’s better, a cop behind a bush or a police vehicle in the open to remind all drivers to drive safely? Seminole County Sheriff’s vehicles are a good example of where you will almost always see the patrol car out in the open for everyone to see.
Last week: As Orlandoans, we mourn for the victims of the white nationalist inspired anti-Muslim terrorist massacres at the Al Noor Mosque and the Linwood Islamic Center in Christchurch, New Zealand, which killed 50 people. The cowardly terrorist mass shootings, which were live-streamed, horridly remind us of the deadly Pulse nightclub attack on June 12, 2016. The victims included New Zealanders of all racial backgrounds as well as nationals of Afghanistan, Bangladesh, India, Jordan, Pakistan and Saudi Arabia. And, while I can’t blame New Zealand for seeking to ban assault weapons after the massacre, I wonder what it will do to combat the root cause of the tragedy, violent white extremist ideology.
Looking ahead: The Mueller Report will soon be complete and will undoubtedly spawn a cannibalistic frenzy among politicians and pundits. Will it be released to the public? Let’s hope so, otherwise media outlets will be consumed in a bottomless sinkhole of Trumpian conspiracy theory and sensationalism for the next two years. Personally, I’d like to watch real news. Is there a smoking gun that will overwhelmingly demonstrate that President Donald Trump is a Manchurian Candidate for the Russians? Probably not. Did Trump fail to disclose business dealings with Russia opening the door for compromise? Probably so. So where does that leave us?
Last week: As our country becomes more divided, we must not forget one of the most critical lessons we teach our young children – the lesson of compassion and kindness regardless of differences. We are now being reminded of this valuable message by a local kindergarten class in Merritt Island in Brevard County. The class designed a symbol for kindness and hope for it to gain worldwide recognition. If enough people sign their online petition, it will be sent to the leaders of Congress, Mitch McConnell and Nancy Pelosi. Lately, it seems we could all benefit from this little reminder, so let’s hope the class achieves its goal!
Looking ahead: We are overlooking a group that was strongly affected by our country’s college-admissions scandal. The disastrous impact this systemwide abuse has had on students with learning disabilities is severe. They are already stigmatized and many students with learning differences report experiencing various forms of discriminationby peers and even teachers and staff. Many falsely believe that students with learning challenges get an unfair advantage. Sadly, the only unfair advantage here is the ridiculous sums of money parents are willing to pay college preparatory businesses so their children — whom they apparently believe can only be successful in college through dishonest and unethical scams — can attend an Ivy League school.
Last week: There was a modern-day David vs. Goliath showdown that took place in the Fifth District Court of Appeal. Rural East Orange County residents squared off again against an incestuous giant of developers, lobbyists, lawyers and even their own county government paid for by their own tax dollars. Broken promises, conflicted representation and the powerful trying to crush the little guy describes the fight of these residents to prevent incompatible development east of the urban service boundary in a part of Orange County already neglected for far too long. May the giant fall again this time because the cause is right.
Last week: The opening of EarthFare at the corner of Gore and Orange Avenue on Wednesday morning was a sight to behold. Hundreds of people lined up to enter the long-anticipated urban market with a chance to win big money gift cards. The crowd included enthusiastic health food shoppers, hopeful social media “influencers” live streaming their experience as well as people who just love the freebies from a grand opening. Patrons of EarthFare can count on an easy-to-access parking garage from Gore or Orange Avenue and plenty of seating to enjoy made-fresh breakfast, lunch or dinner options.
Last week: A program exists that provides tax incentives, including temporary deferrals on capital gains taxes when those gains are reinvest into qualified opportunity funds, called Opportunity Zones. The objective is to invest those funds into low-income communities. Often these programs end up benefiting the investor and work against low-income families. These programs should be carefully monitored to make sure they’re benefiting the community and not only the investor. This could easily spin into a situation where government revenue is lost and expenditure goes up for section 8 housing subsidies to accommodate the newly created group of homeless. It takes concerned citizens to hold government accountable and hopefully these communities can invest the time to do so.
Last week: It took the New Zealand government less than a week to pass a ban on semiautomatic assault rifles after last week’s horrific terrorist attack on two mosques in Christchurch, New Zealand. In contrast, the Florida Legislature has refused to act on demands from a majority of Floridians to pass similar legislation nearly 3 years after the mass shooting involving AR-15s at the Pulse nightclub in Orlando, and over a year after the mass shooting at the high school in Parkland. A state ban on all semiautomatic assault weapons and large-capacity magazines is not a 2nd Amendment issue; it’s a public safety issue.
Last week: Last week’s news about an innovative economic project in Lake County is exciting! In what looks to be the start of bigger things to come, U.S. and British grocers are partnering to bring tech and jobs to a new warehouse in Groveland by 2021. Described as an “advanced robotics customer fulfillment center,” the tech comes from England, and allows for the creation of about 400 “higher wage” jobs that blend robotic efficiencies with human intuition and interaction. The best economic development initiatives create or compliment jobs, not replace them, so this project looks to be a win/win for Central Florida.
Looking ahead: Junior Achievement of Central Florida and Chepenik Financial are partnering for the fifth year to hold the original 4.01K Race for Financial Fitness in Orlando Cultural Park on Saturday, March 30 at 8 a.m. The annual walk/run event brings people and families together for healthy fun as well as to showcase the importance of financial planning. Proceeds benefit Junior Achievement, which supports 50,000 students across Orange, Osceola, Seminole, and Volusia counties each year through programs like work-readiness, entrepreneurship and financial literacy skills, in addition to experiential learning. Come on out to run, learn and/or volunteer!
Last week: Orlando has a strong jazz community. Much of that strength is thanks to Jeff Rupert. Rupert runs the jazz department at UCF and works tirelessly to cultivate a strong jazz community throughout Central Florida. He hosts a monthly show at the Bamboo Center for the Arts, which is always a strong musical experience. Last week, he facilitated the UCF Orlando Jazz Festival. Each year, the festival hosts some of the best jazz musicians from around the world. The festival has passed, but it’s not too late to experience Orlando’s jazz scene. Check out the Timucua House Blue Bamboo for regular jazz programming.
Looking ahead: Mark Baratelli is known for his innovative and interesting cultural events. For years, he has curated and run the Daily City Food Truck Bazaar. Now he has devoted time and energy to building “Orlando Flea.” This market features the best makers and craftspeople from around Central Florida. It’s well worth attending. And the next Bazaar is Sunday, March 31 at Celine Orlando. The multi-level venue is in Downtown Orlando and easily accessible from the nearby parking at The Plaza at Pine and Orange. It runs from noon to 4 p.m. Don’t miss it!
Brendan O’Connor, editor in chief, Bungalower.com
Last week: Bikes are taking over Orlando and I’m not mad about it. But I am a little anxious. Bike share systems are here to stay, scooters are on the horizon too. The newly opened Colonial Pedestrian Bridge just opened up over the weekend along with a new segment of Gertrude’s Walk and it’s tough not to be swept up in some sort of pedal-powered ecstasy but I’ve managed to maintain a portion of cynicism and worry if Orlando drivers are ready for more cyclists on the road and if those cyclists in turn are aware that while fun, they should still pay attention.
Looking ahead: A number of lawsuits that preceded an internal "civil rights audit" by Facebook has resulted in a $5 million settlement to groups that include ACLU, CWA, and NFHA. The lawsuits claimed that Facebook was enabling discrimination in housing, employment, and credit advertising by allowing users to hyper-target their advertisements based on characteristics like race, gender, and age. As a result over 5,000 categories that include targeting ethnicity and religion will be removed from the platform.
Beverly Paulk, founding member, Central Florida Foundation and The Orlando Philharmonic
Last week: Thad Seymour is well qualified to lead the University of Central Florida as interim president for as long as needed. His leadership style is based on a calm, thoughtful, collaborative approach. He has long demonstrated his commitment to service in our community. Thad has led the team for UCF’s new downtown campus, reported so far to be on time and on budget. Thad and Katie, his wife, have provided key leadership to dozens of nonprofits and important community projects as individuals and as a couple. Central Florida is fortunate to include Thad and Katie as they continue to serve.
Last week: GOAA’s legal assault on Orlando Melbourne International Airport and Orlando Sanford International Airport for trademark infringement is rather perplexing since trademark infringement requires “likelihood of confusion to the public." Both cities conspicuously identify their names and any traveler can clearly locate Melbourne or Sanford on Google Maps. Nothing confusing about that! A traveler flying to Midway in Chicago or Love Field in Dallas knows they are not the primary airports for those particular cities, but selects them based on travel needs anyway. Not only is GOAA’s action legally misguided, but it flies in the face of the regionalism Central Florida has fostered for over 20 years.
Looking ahead: The acquisition of Fox’s entertainment assets by Disney adds super fuel to a jet powerhouse capable of explosively metabolizing and leveraging content simultaneously through movies, television, theme parks, merchandise, and so much more. Disney’s prior acquisitions of Marvel, Lucasfilm and Pixar paid off handsomely for investors over the past five years. Its current acquisition of Fox, albeit on the high side for $71 billion, is likely to do the same. While Wall Street continues to undervalue Disney with a forward P/E Ratio of 14.96, investors with a long-term view would be best served to consider Disney’s $163.2 billion market cap a bargain.
Last week: Florida is in an affordable housing crisis of its own making. For years lawmakers balanced budgets at the expense of low income people by stealing nearly $2 billion from the Sadowski Affordable Housing Trust Fund. They can do the right thing now by passing a renter’s bill of rights and paying back every dime they stole. A report from the National Low-Income Housing Coalition exposes the consequences of Florida’s misdeeds. The state, and especially Central Florida, are the least affordable places in the country for cost-burdened renters. A bill of rights would protect renters from predatory landlords and unlawful evictions. More should be done. But this is a first step to right a terrible wrong.
Ed Schons, president, Florida High Tech Corridor Council
Last week: For those who worry about technology eliminating jobs, news that supermarket giant Kroger will partner with British online grocer Ocado to build a 350,000-square-foot warehouse north of Groveland suggests otherwise. Kroger and Ocado said the “advanced robotics” customer fulfillment center will employ about 400 people in “high-paying” jobs when it opens in 2021 to ship Kroger grocery items in Central Florida. Twenty such facilities are planned nationwide.
Rick Singh, property appraiser, Orange County
Last week: The new bridge across Colonial Drive for bikers and pedestrians opened this week just east of I-4. As downtown Orlando continues to morph into an urban center, the focus on safe transportation without the introduction of more cars will be critical to attract residents and workers. The diagonally-placed bridge also crosses train tracks and will allow bikers and pedestrians safe access to the Lynx and SunRail stations, while avoiding about 40,000 cars per day. In addition, the central location facilitates bike travel to local parks and neighborhoods through the use of existing bike lanes. This is a welcome amenity to downtown living.
Looking ahead: Dead last. That’s where the Orlando Metro area ranks for housing affordability in the entire country. According to the newest Affordable Housing Gap Analysis by the National Low Income Housing Coalition, the area offers only 13 affordable and available rentals for every 100 families who need them. Our region will lose its attraction for business and industry if the labor market cannot afford to house itself. Affordable housing is Central Florida’s highest priority, and should be at the center of every conversation about growth and development.
Michael Slaymaker, professional fundraising executive
Last week: As I scroll through photos of the mass shooting in Christchurch, New Zealand, I can’t help but see the eerie comparison to the Pulse shooting in Orlando. The candles, flowers, the number of lives taken. People from all walks of life crying, praying and hugging. I so respect Prime Minister Jacinda Ardern and the way she is handling this atrocity. She reminds me how proud I am of Mayor Jacobs, Mayor Dyer and Commissioner Sheehan. In New Zealand, semi-automatic, "assault-style" weapons are due to be banned. Gun reforms will be announced within 10 days. Wish our local politicians had that power.
Anthony Suarez, president, Puerto Rican Bar Association of Florida
Last week: THousands of Venezuelan immigrants are in Florida and Central Florida currently. The largest growth group of students in our schools is Venezuelans. it is obvious that the nation is in crisis, but will our nation and Central Florida be accommodating to this humanitarian crisis, or will our law and order neighbors let these legal immigrants who overstay their visas, be treated like criminals? I have been attending meetings, church gatherings and listening to so many horror stories of violence, starvation from this community and I wonder, will the Legislature continue its quest to shut down our communities?
Looking ahead: UCF has made great strides nationally with its football program. Now the basketball program is looking to make its stamp on the nation. Its first appearance in the NCAA men’s tournament in 14 years, followed by the performance in football. will ensure UCF is known around the nation. With all the controversy surrounding the school, this will be welcomed news.
Last week: On the heels of being named the worst region in America for affordable housing, the Florida House thought it would be a good idea to once again take all the money out of the affordable housing fund and spend those dollars elsewhere. Estimated at $352 million, the Sadowski Fund was stripped to $123.6 million for affordable housing. That money will then go to panhandle hurricane relief, meaning the state will spend zero dollars in addressing the most pressing need we have. It makes no sense to continue complaining about the lack of housing and then take the money that was intended to solve that problem.
Last week: The playing field for students attempting to qualify for top-rated colleges and universities just became more uneven with the recent college admissions scandal. And with the continuing erosion of our public school system, the education opportunities for the general student population remains increasingly challenging and often unattainable. More unfortunate perhaps is the lesson we are teaching our students that it is okay to bribe, cheat and lie to gain access to their preferred college or to request false accommodations when taking the SAT and ACT tests in order to gain an advantage over less affluent students. Time to turn those negative lessons into a positive for ALL students.
Looking ahead: Tragically, rates of death by suicide in the U.S. have increased by more than 25 percent in the past 20 years, according to the CDC. AdventHealth is collaborating with the University of Central Florida to address this alarming epidemic. Our goal is to track patients who come into our ER with suicidal thoughts or behaviors, and provide intensive support services after they’re discharged. I’d like to thank UCF’s team for working with us to assess the effectiveness of this approach, as we work to learn how best to support distressed patients and bring them back to health — and most importantly, apply our learnings far and wide.
Jen Vargas, Jen Vargas, producer/host, FilmSlam at Enzian Theater
Last week: The 180+ films of this year’s Florida Film Festival were announced this week. Great news! Todd Thompson, a Florida filmmaker, will debut his feature-length documentary, "Woman in Motion," and on opening night no less! This is big news because, historically, Florida made films are rarely seen during this 10-day event. Being an Oscar qualifying festival in three film categories (Documentary, Animated and Live Action shorts), Florida Film Fest is a very popular Oscar stop among filmmakers. Thompson is a longtime alum and past winner of FilmSlam and no stranger to Florida Film Fest, which begins April 12.
Looking ahead: Studio Movie Grill at Sunset Walk opens to the public on March 28! Located at Margaritaville Resort Orlando, this 12-screen, luxury dine-in theater has over 1,000 leather recliners, a full bar, chef inspired menu, and hand-crafted cocktails, all of which may be delivered right to your seat. Residents of Tampa and Seminole are already familiar with this brand which will be a first for us here in Central Florida and a slick addition to the unique dining, shopping, and resort options at Margaritaville’s outdoor Sunset Walk complex. Welcome to Orlando, Studio Movie Grill!
Carol Wick, principal, Convergent Nonprofit Solutions
Last week: The Kesse family won the right to have a private investigator examine all the files from the Orlando Police department regarding the disappearance of their daughter Jennifer. She has been missing since 2006 but her family has not given up hope that she may one day be found or at least, that they would have closure. While this is controversial on many sides, this long-standing mystery haunts us all here in Central Florida and we hope the Kesse’s can at least find some answers to what happened to their daughter all those years ago.

Pittsfield man gets a year in jail for drug, gun charges
News on drug arrest is very common. Whenever we watch TV or read news online, there is always a case about a person who got arrested with drugs in his possession. However, even with all these, still, there are so many who still keep doing the same thing. Here is another man from Pittsfield who gets a year in jail for drugs. It is so unfortunate and saddening at the same time that you have to spend a year living a difficult life inside the jail.
A city man who was found with a gun, cocaine and other contraband in his home last January was sentenced to a year in jail Wednesday after pleading out in Berkshire Superior Court. The plea hearing for Austyn M. Lanphear, 22, had to be rescheduled from its original Dec. 31 date after Lanphear failed to appear for the 9 a.m. event, prompting the issuance of an arrest warrant.
Click here to read the article.
On Wednesday, Lanphear, who had been taken into custody in the meantime, told Judge Daniel Ford he had transportation issues that prevented him from getting to court that day.
On Jan. 10, 2018, members of the Berkshire County Drug Task Force were staking out Lanphear’s home after obtaining search warrants for his him, his home and car, on suspicion he was using his home to distribute drugs.
Police waited until Lanphear left his home to take him into custody, out of concern he may have weapons inside. He left about 9:30 p.m. and was soon pulled over by police.
Inside the home, police found about 12.7 grams of cocaine, a loaded .45 caliber handgun, some other rounds of ammunition, three 30-milligram oxycodone pills and other evidence including a digital scale and other dealer paraphernalia.

5 fears physicians face today
Like prep sports or prime-time television, medical meetings have seasons.
In the spring and fall, my calendar fills with invitations to speak. I try to get to the venue a few hours before I’m scheduled to speak, so I can “take the pulse” of fellow doctors, asking them about their practices, patients and the future of medicine.
Figuratively speaking, the industry’s pulse is racing with fear. I’ve observed that in just the past 12 months, the once-mild anxieties of American doctors have transformed into clear and present phobias.
These are extraordinary times in medicine, giving way to five intense fears:
1. Peniaphobia: the fear of being poor
Ever since the Affordable Care Act (ACA) became law in 2010, policy experts have been wooed by the notion of a “pay-for-value” reimbursement model – a potential replacement for the traditional pay-for-volume approach (aka “fee-for-service”).
The swap makes sense on paper: Rather paying health care providers based on how many tests, procedures or office visits they complete, insurers who opt into a pay-for-value system pay doctors based on their outcomes and the excellence of their care.
Practically every physician supports pay-for-value, in theory. And yet almost all of them worry about the real-life implications.
One primary care physician at the National MACRA MIPS/APM Summit in Washington (a conference on Medicare financing) told me, “It feels like a bait-and-switch.”
By this, he meant that when the Centers for Medicare & Medicaid Services (CMS) first introduced its new “pay for performance” reimbursement criteria, it sounded like a good deal. The program was meant to emphasize “quality, value, and accountability.” But instead of rewarding all physicians who improve, the dollars are proving finite and program is becoming cost-neutral.
“No matter how well we practice, 50% of us will be in the lower half,” said another doctor at the same conference. “I figured that if we all improved, we’d all be rewarded. But by raising the bar every year, CMS guarantees there will always be losers. It’s just another way to pay doctors less.”
To be clear, physicians know they earn good salaries. And it’s not that they aren’t grateful. At the same time, most doctors feel as though they’ve earned a decent income after 10 years of higher education, three to eight years of residency training and $190,000 in average student-loan debt.
2. Metathesiophobia: the fear of change
For all the talk about cost containment, the health care industry is not – and has never been – known for optimal efficiency.
For instance, it’s not unusual for patients who call the doctor’s office at night to get an automated response, telling them to dial 9-1-1 or go to the local ER. And if they do, things don’t get much more efficient. Most patients admitted to the hospital on Friday night with a non-emergency will wait until Monday to receive a complex procedure or test. These delays aren’t just costly, they’ve been proven to increase death rates.
A more consumer-friendly form of change seems imminent. But doctors fear some of the new operating models being featured at national conferences will force them to work even longer days, putting further strain on their family and personal lives.
Physicians work hard, often having 15 minutes or less to spend with each patient. And during those precious few minutes, doctors now find themselves looking more at the computer screen than at the patient, needing to document every symptom and treatment in real-time just so insurers will pay them.
“The electronic health record was supposed to make my life easier,” said one of the doctors I met at the National Association of ACOs (NAACOs) Fall Conference, “but all the computer does is slow me down.”
3. Politicophobia: the fear of politics
Health care is the No. 1 issue for voters heading into the 2018 midterms. Now less than a month away, the elections are striking fear in physicians who worry that both sides of the aisle are advancing dangerous policies.
Doctors say Republicans want to lower health care costs by stripping away insurance coverage. They know this will impede access to care for many of their patients. At the same time, doctors also fear the “socialization” of medicine as the chorus of “single-payer health care” advocates grows louder on the left.
The most vocal proponents of so-called “Medicare for all” are planning to reduce physician payments by at least 10%. And since doctors spend about half of their reimbursement on office overhead, this reduction would effectively decrease their take-home pay by 20%.
Speaking on the condition of anonymity, a physician at the 2018 American College of Physicians (ACP) meeting explained, in shockingly candid detail, how Medicare-for-all would affect his practice: “Right now, I try to take care of everything my patients need in a single visit. But if all of my patients were suddenly covered by Medicare, I’d have no choice but split visits,” meaning he’d compensate for lost revenue by bringing the patient back a second time, thus being able to bill the government twice for treating the same problem.
4. Hypengyophobia: the fear of being blamed
Sometimes referred to as hypegiaphobia, this phrase combines the Greek words for “responsibility” and “fear.” It applies here to doctors who worry about being inappropriately scapegoated for health care’s high costs and rampant failures.
This is a common fear because (a) there’s plenty of blame to go around and (b) so many health care failures can be traced back to the doctor’s office. However, just because doctors are on “the front lines” doesn’t mean they deserve the blame.
For example, it’s true that doctors write prescriptions for pricey meds and order expensive radiological scans, but they have no influence over pharmaceutical-industry pricing or hospital billing procedures. They could practice more efficiently, but costs would still rise, even if somewhat slower.
One frustrated surgeon at the America’s Health Insurance Plans (AHIP) annual meeting in San Diego explained: “Pharma companies raise their prices 1,000% for brand-name drugs and Congress does nothing. Hospitals can bill patients $1,000 or more for a CT scan that takes two minutes. If I say no to either, my patient will find another doctor. I’m caught in the middle. I’m not the reason health care costs are out of control, but I get all the blame.”
5. Atychiphobia: the fear of failure
This is the No. 1 fear of most physicians and, for most physicians, it started well before they began practicing medicine.
To get into a good medical school or residency program, you need good grades. Anything less than straight A’s is considered a failure by most students who will go on to be doctors. As a result, physicians spend their lives trying to fulfill difficult (if not impossible) expectations.
As the pace and pressures of being a doctor increase each year, so do the fears of making a mistake. Doctors fret over errors all the time and worry their patients will be hurt by something they failed to do.
The result: growing reports of depression, burnout and suicide among physicians. These now-common side effects have had an unexpected consequence. Half of all doctors wouldn’t encourage their children to follow in their footsteps. Most physicians I speak with feel as though they’re swimming upstream, in rapids, paddling as fast as they can just to stay in place.
Doctors know they could’ve made more money in venture capital or investment banking. Instead, they dedicated their lives to helping those in need. That fulfillment is now eroding under the weight of intense financial demands and growing time constraints.
Backstage, at the annual conference of America’s Physician Groups (formerly CAPG) in San Diego, one prominent physician put it this way: “My classmates and I became doctors because we love helping patients. Too often these days, we feel like we’re letting them down. There’s so much administrative time-suck, and never enough time for our patients.”
Uncertainty: The root of all fears?
Almost every doctor I’ve spoken to recently has brought up the newly formed Amazon, Berkshire Hathaway and JPMorgan Chase health care venture. It’s on the mouths and minds of practically everyone in the industry.
Although many are unsure what it will mean for themselves, their practices or the future of health care, the majority of doctors trust that the venture’s new CEO, Dr. Atul Gawande, will seek to create a fairer, safer and more affordable approach to health care.
This particular brand of change (industry disruption) seems preferable to “death by a thousand cuts” that many doctors are experiencing today.
Should Dr. Gawande and this new health care enterprise succeed in doing what many in the industry have cautiously dreamed about, we may someday witness a health care system that: (1) integrates physicians into highly efficient medical groups, (2) ensures doctors are paid a capitated fee that rewards the quality (not quantity) of care, (3) implements advanced information technology and (4) develops physician leaders.
Three years ago, I doubted that doctors would be willing to take the risks required to achieve these so-called “4 pillars of health care transformation.” But, oddly enough, as their fears have grown, so too has their receptivity to change.
