Hi! I strive to be a veterinary technician but where I live there are no schools or programs at the moment and have no money to travel in regards to school. I am going to graduate soon and have already applied to a local college in hopes to pursue a bachelor's degree in bio. I am wondering if this seems like the right choice? Some of my friends have told me it is not required to go to college to become a vet tech but my mom is making me. Thank you in advance!

Phew, this is a loaded question.

Let’s start by saying: this varies A LOT by location. It is definitely not straightforward, and every veterinary assistant/technician you ask will have a wildly different answer.

Every US state (I’m sure it varies even more wildly by COUNTRY) has different laws in regards to what people with different levels of certification can do.

I currently work as a veterinary assistant who has been trained on the job. I don’t have any official certification - I was just taught by other assistants, technicians, and doctors over a few years.

In addition, I have my bachelor’s degree in Animal Science, and am planning to go on to veterinary school in the fall.

That being said, I don’t have a lot of experience with veterinary technician schooling, exams, or licensure. Any of my followers with more experience, please feel free to add on or correct me!

According to NAVTA (The National Association of Veterinary Technicians in America) and the AVMA (The American Veterinary Medical Association), the correct term is “Veterinary Technician” in the US (as opposed to terms like “Veterinary nurse” used in other countries) - another source of debate and varied opinions!

There are people with zero official certification, people who are LICENSED veterinary technicians (LVTs), people who are CERTIFIED veterinary technicians (CVTs), and people who are REGISTERED veterinary technicians (RVTs). Again, the difference varies by state.

After completing a Veterinary Technician program (~2 years) and receiving your degree, most jobs require you to take the VTNE (Veterinary Technician National Examination) - the nationwide exam. Once passing this, depending on the state, you may have to pay a fee (no exam required - just proof of your degree and your passing exam grade). Paying the fee will make you “licensed”. I believe this needs to be repaid every 2-3 years to maintain your licensure.

In addition, there may be a mandatory amount of C.E. (Continuing Education) credits you must obtained every year to maintain your licensure.

I’m not sure if this is still in effect - but in some states, you are able to take the VTNE exam WITHOUT attending veterinary technician school. (In other states, you need to show proof of your degree in order to even take the exam). Passing the VTNE will then enable you to pay for your license.

Now, once a veterinary technician or technologist, you can also move forward by becoming a veterinary technician SPECIALIST (they usually work under a veterinary doctor who chose to specialize in a certain field and become a board certified specialist).

Let’s do a brief overview of the medical staff in a veterinary hospital.

Many veterinary technicians and technologists are placed in a supervisory role in veterinary practices, research institutions and other employment options. Veterinary technicians can find employment in veterinary practices, biomedical research, zoo/wildlife medicine, industry, military, livestock health management, pharmaceutical sales, etc. Almost every state requires a veterinary technician/technologist to take and pass a credentialing exam. Passing this exam ensures the public that the veterinary technician has entry-level knowledge of the duties they are asked to perform in the veterinary clinic or hospital.”

That’s important to keep in mind. You don’t have to be working in a general practice small animal hospital giving vaccines to cats and dogs. You have the option to work with small animals (cats/dogs), large animals (horses/cows/sheep), exotics (birds/reptiles/rodents), zoo animals, wildlife animals. You can work in a hospital, or you can work in a research lab. You can work in industrial jobs or government/military jobs. There’s a huge variety of options.

VETERINARY TECHNICIAN: “A veterinary technician is a graduate from a two-year, American Veterinary Medical Association (AVMA) accredited program from a community college, college or university.”

VETERINARY TECHNOLOGIST: “A veterinary technologist has graduated from an AVMA accredited bachelor degree program.”

VTS: “A veterinary technician or technologist specialist has met the same requirements as above plus spends about 75% of their time doing a specific task and has passed a specialist certification exam administered by a Specialist Academy. Currently, there are eleven academies offering specialty certification.”

Keep in mind this is a huge amount of work. It requires a lot of experience, hard work, and studying to become a VTS.

CREDENTIALED: “The current terminology recognized by decree of both NAVTA and the AVMA is “Veterinary Technician”. Whether you are an LVT, RVT or CVT the term used is mandated by the technician’s state of residence. Here are some definitions to help understand why all three terms are in use.”

CERTIFIED: “Is the recognition by the private sector of voluntarily achieved standards. Certification is usually bestowed by a private sector, nonprofit, professional association or independent board upon those members who achieve specified standards. Certification is therefore distinguished from licensure because it is generally non-governmental and voluntary. Confusion can result when the title “certified” is used for a licensed profession, such as Certified Public Accountant. Many CVTs in the U.S. are recognized by government agencies, such as boards of veterinary medical examiners, which also adds to the confusion. States that currently certify veterinary technicians include Arkansas, Arizona, Colorado, Florida, Idaho, Illinois, Massachusetts, Minnesota, Montana, Mississippi, New Hampshire, New Jersey, Oregon, Rhode Island, Pennsylvania, South Carolina, Vermont, Wisconsin and Wyoming.”

REGISTERED: “Refers to the keeping of lists of practitioners by a governmental agency. It can be equivalent to licensure but may also be distinguished from licensure in that criteria for registration may not exist, and registration may not be required for practice. States that currently register veterinary technicians include California, Indiana, Iowa, Kansas, Kentucky, Louisiana, Maine, Maryland, Missouri, New Mexico, North Carolina, Ohio, Oklahoma, South Dakota, and West Virginia.

LICENSED: “Is understood as the permission to do something as given by an authority, with the implication that one would not be permitted to do this thing without permission. To be licensed is more than a statement of qualification, as certification is. It is a statement of qualification, and it is the right to do a thing otherwise not permitted by a given authority. Both certification and licensure, however, carry the connotation of trust, belief and confidence; for without these attributes, the certification or the license would have little worth. States that license veterinary technicians include Alaska, Alabama, Delaware, Georgia, Michigan, Nebraska, Nevada, New York, North Dakota, Tennessee, Texas, Virginia, and Washington.”

It’s actually very difficult to find Licensed Veterinary technicians, so a lot of practices use on-the-job training, and teach people (over the course of years) the different skills a veterinary technician needs. Again, the legality of this varies by state, but it is done VERY often.

Legally, if licensure is required in these states, these employees are “assistants” and have a limit to what they should be doing. Some LVTs are insulted by these employees being called “technicians”, as the LVTs worked hard through their schooling and exams to earn that title. On the other side of the spectrum, some un-credentialed employees working as technicians will tell you that licensing means nothing - and that some of the most skilled technicians never went to school.

I am not going to TOUCH this very controversial area - as I have seen both perspectives.

Personally - I don’t see the point in a veterinary assistant program since most are inexperienced people who learn everything on the job. The veterinary assistant programs are very new.

NAVTA on not going to school:“In most states you can’t become credentialed without graduating from an accredited veterinary technology program. Very few states currently have an “alternate route” that allows people to sit for the exam, however there are a number of prerequisites that must be met before taking the exam. In 2000 there was a ruling by the Association of American Veterinary State Boards that within ten years they will no longer allow the National Veterinary Technician Exam to be used under these circumstances. If you are in a position to attend a veterinary technology program in your state it is well worth the effort. The amount of knowledge behind the skills you already know will astound you. If you cannot physically get to a program, there are five Distance Learning Programs that are currently accredited by the AVMA that can be taken via the Internet.”

Again, I have to recommend going to an actual school if at all possible. In my experience, online classes don’t give you the hands-on experience you need in this particular field. Being in a classroom and having labs will help immensely. (In addition to getting LOTS of experience in a hospital!)

In regards to not having money for school - most people don’t! What kind of graduating high schooler has $50k/year for college?

Are you SURE there’s no programs around you? Check out NAVTA’s website and see if you’re missing any. If not, you unfortunately may have to travel.

You can save money by commuting and driving to class every day, but if that’s not an option, you may just have to use a loan for dorming/renting an apartment for the 2-4 years you’re away at school. You will NOT be alone in doing that.

If your mom wants you to get your bachelor’s so badly - is she willing to help you at all with college costs? If she can’t/won’t, look into (both private and government) student loans. Most people get through college on student loans, and once you have a steady salary, you can worry about paying them back.

If you’re lucky, maybe you’ll even get some financial aid that you don’t have to pay back! In addition, you can try applying for scholarships.

When you get a job, sometimes there is loan forgiveness. Not sure how this applies to veterinary technicians specifically, but if you work for a government job (public health, food animal production, military, etc.) or even a big corporation, sometimes they will offer to pay back your school loans if you commit to working for them for a certain amount of time.

You can definitely get your associates or bachelor’s degree while deciding what you want to do. If possible, I would aim for a program that has an Animal Science degree. You will have access to a lot more relevant classes and hands-on experience. For example, I was able to take classes like Comparative Mammalian Anatomy, Animal Nutrition, Animal Reproduction, Companion Animal Science, etc. in addition to working with laboratory animals and farm animals over 4 years. That will probably be harder to find with a program for a degree in biology. Biology will be focusing a lot on humans, in addition to plants and the environment.

However, if you definitely want to become a veterinary technician, I would just go straight to veterinary technician school.

If you (or your mom) want you to have your bachelor’s degree - it sounds like a 4-year veterinary technologist program (as opposed to a 2-year veterinary technician program) would work for both purposes. You would have your bachelor’s degree, and you would have finished a program allowing you to take the VTNE exam.

But now at home, I am trying a quote with Geico, and it seems that I could probably get a better price with this company (not completely sure, I need to call them). Back in October of 2010I was involved in a Hit and Run Accident. The Guy that hit us was found a few days later and we filed against his insurance. Me and my Fiance both went through Physical therapy for our backs. Medical bills totalled about 5K for me and about 8K for her. There were times when she was in tears because her back was hurting so bad. She Couldnt work some days either and she is self employed and works from home as a graphic Designer. I missed some days of work too but my pain was not near as much as hers. State Farm Called and offered a settlement with me to pay the 5k to the Doctor and then a $1000 check to me for my pain and suffering. They called my Fiance to settle but she did not answer. A few days ago I scraped a co-worker's vehicle in our work parking lot. She wasn't in a proper parking space, but whatever, it was clearly my fault. I am paying for repairs to her car. Overview

Juice Plus is owned by National Safety Associates (NSA) out of Collierville, Tennessee. The company previously produced unrelated products, such as water filters and fire protection equipment, but branched into nutritional supplements in 1993 with the creation of Juice Plus.

Within its first year, Juice Plus achieved $6 million in sales and become NSA’s best selling product. [1] By 2008, the product was nearing $300 million in annual sales, according to the company’s VP John Blair. [2]

Of course, this is MLM, so they haven’t been around for over 2 decades without their fair share of controversies.

Both consumer watchdogs and government agencies have criticized the company for making health claims with absolutely no scientific backing and employing deceptive marketing practices, but more on that later. [3]

Perhaps the most publicized of the company’s controversies happened after they selected O.J. Simpson to provide them with a celebrity endorsement. He signed a six-figure contract with the company in January 1994. Of course, Juice Plus never could have known what would happen 6 months later (O.J. Simpson’s wife was murdered in June 1994 and he was accused of the murder)…but it gets worse than unlucky PR.

While endorsing Juice Plus, O.J. Simpson claimed that its major benefit was helping with joint pain, inflammation, and arthritis. He’s videotaped at a big Juice Plus conference in March 1994 claiming that Juice Plus completely cured him of his arthritis, allowing him to stop taking harmful anti-arthritic drugs and to start golfing again. [4]

Well, during O.J. Simpson’s controversial murder trial, he defended himself by saying that he was physically incapable of murdering his wife due to his arthritis and showed proof that he’d been taking the anti-inflammatory drug sulfasalazine. [5]

So, not only does the company lack real scientific evidence to back its claims, but endorsements can’t even be trusted.

How much does Juice Plus cost?

The start-up fee to join Juice Plus is $50.

Products

Juice Plus sells fruit and vegetable-based nutritional supplements and drinks. The whole idea behind Juice Plus is based on the fact that most Americans don’t consume enough fruit and vegetables (true).

Juice Plus products were designed to solve this problem by providing consumers with convenient ways to get their daily fruit and vegetable needs in. They sell convenient products (capsules, juice, bars) that contain concentrated fruits and vegetables.

Produce is dried and reduced to powders (with unnecessary salts, sugars, and most of the calories removed). Additional fiber and enzymes are added, and then it’s all placed in capsules for quick and easy consumption.

Orchard Blend Capsules, for example, contain berries, apples, cranberries, oranges, papaya, peaches, and pineapple.

Garden Blend Capsules contain barley, beets, broccoli, cabbage, carrots, kale, oats, parsley, spinach, and tomato.

Better Bars contain real fruits, oats, bran, and “a host of other natural ingredients”.

Juice Plus+ Lite is a 110 calorie meal replacement shake containing 12 essential vitamins and minerals.

No where on these packages or on the Juice Plus website does the company explain the quantities of each ingredient. Presumably, a single capsule isn’t going to adequately cover your body’s fruit and vegetable needs. This is especially true because a big part of your body’s need for produce is due to the fiber it contains (which is removed during the reduction process) and the healthiness of having a balanced diet. Capsules are not meal replacements and don’t affect your daily dietary choices.

Furthermore, study after study has shown that supplements in general can never provide the full range of benefits that a balanced diet can.

Tests from ConsumerLab have proven that Juice Plus supplements don’t contain anywhere near enough of your daily vitamins and minerals to have a significant impact on your health. A test done on their Garden Blend supplement showed that it contained only 76.4% of the calcium it claimed to contain, which amounted to less than 4% of the recommended daily value per serving. [6]

Benefits:

Benefits are completely based on anecdotal claims, as there aren’t really any legitimate studies (i.e. studies that AREN’T funded by Juice Plus themselves) to back them up. Claims include that Juice Plus:

improves mood

improves overall health

increases energy

cures arthritis

clears skin

prevents illness

improves sleep quality

decreases inflammation

Side Effects:

Reported side effects associated with Juice Plus are pretty gnarly, and include:

diarrhea

All distributors make retail profit off their personal sales, which is the difference between the retail price and the wholesale price that they’re paying.

As you move up in rank, you can access additional commission bonuses. These include 6%-22% commission on your downline once you start hitting monthly sales requirements and recruiting consultants to your team.

Once you hit the 3rd rank and up, you access a 5% Performance Bonus. After that, a 4% “Promote Out Bonus”.

The highest ranks access a 20% Business Incentive Bonus of up to $500-$1,500 per month.

Consultants at the highest level, National Marketing Directors, also get to access a benefits package that includes health, vision, and dental insurance, disability, life insurance, prescription coverage, tuition reimbursement, and a holiday bonus, which is pretty awesome.

Of course, almost no one makes it that far.

They actually have a pretty generous comp plan, and it’s not overly complicated.

But the product has come up against a LOT of controversy, even for an MLM (see: DS Domination, Creative Memories, or Silpada). The benefits seem to pretty much be the placebo effect, and the potential side effects are pretty nasty. Who’s going to keep buying a pill that makes them cramp, vomit, and poop uncontrollably? Good luck keeping up your sales numbers.

The estate of Dr. Michael and Kim Perry finalized the sale of Perry’s practice, Mid-South Dental Implants and Periodontics, to his trusted business partner Dr. Brenda Fowler this week.

Robert Perry, brother of Michael Perry, said, “This is a great day for the Perry family. After conducting a full search and vetting potential suitors for the business, Brenda turned out to be the best person to whom to sell the practice all the way around. She knew the staff and the operation. The best scenario ended up happening.”

Perry started the practice in 2002 in Oxford, and he eventually opened an office in Southaven. Fowler and Perry joined forces six years ago after he reached out to her about a partnership that would ultimately bring the number of locations to five and be a solid working relationship in which all locations have grown. The airplane accident in which Dr. Perry and his wife Kim along with two other Oxford couples were killed happened two days it would have been exactly six years that Perry and Fowler began working together.

“Although it is bittersweet, I am thrilled to be able to be a part of this, long-term and to carry on his legacy. Michael had exceptional business sense and charisma,” Fowler said. “He was so loved by his patients, and it has been such a pleasure to get to know them and to get involved in the community the way he would have wanted us to. The staff and I have talked, and we are honored to be able to carry on his good work professionally and in the community.”

Right after the accident, the family was faced with the challenge of keeping the practice going until they could figure out what to do. The staff, which is known to many as being a great staff, had job offers from all over. But according to Perry and Fowler, the dental community continued to support them throughout.

“Periodontists depend on referrals from general dentists,” Fowler said. “And the referrals kept coming.”

“Brenda made a lot of sacrifices and started coming down (from Tennessee) to run these offices,” Robert Perry said. “But she could not do it by herself.”

According to Perry, that’s when several friends of Perry’s in the industry from all over the country got together, working with Fowler, to come up with a rotation of visiting periodontists for the Southaven and Oxford locations.”

“It was amazing,” said Robert Perry. “They all loved Michael and Kim, and they wanted to help the family through a really tough time.”

Perry and Fowler both told HottyToddy.com that the whole process took a lot longer than they would have imagined. But they believe it has all been worth it.

Taking Aim At The Opioid Industry

Late last year, the Associated Press combined reporting with the Center for Public Integrity to roll out a multi-part series on the $880 million spent on lobbying and political contributions by the opioid industry in a decade long effort to stymie efforts to control these powerful drugs. Reporters reviewed of hundreds of documents brought to light in a recent court case and interviewed over 200 officials, experts, and advocates to understand how the opioid epidemic unfolded.

The series had its genesis when the AP’s Matthew Perrone tripped across a group called the Pain Care Forum. Led by the top lobbyist for Purdue Pharma, the Pain Care forum held regular meetings with doctor and patient groups in Washington to discuss strategies for ensuring Americans had easy access to opioids.

Last year, over 50,000 Americans died from drug overdose, and six out of ten were caused by opioids. That’s 91 Americans every day.

From Lake Atitlan, Guatemala where he is on sabbatical after 10 years at the Associated Press, Perrone spoke with me about what this award-winning series taught him about the political power of the opioid lobby and whether Americans can retain trust in medicine and our federal science agencies. Our conversation follows, condensed and edited.

This series has many of the classic elements of corruption that we see in the pharmaceutical industry—front groups, paid off physicians, incredible amounts of lobbying. But what seemed unique?

The story of American’s opioids crisis has two chapters from what I’ve seen in my reporting. The first chapter closed back in 2007. It was a classic pharmaceutical corruption story. This was when Purdue Pharma settled—excuse me—they actually plead guilty—and paid $600 million in fines. Usually companies settle and neither admit nor deny the allegations in the lawsuit.

You would think that would be the end of the story. But opioids are a bigger crisis than ever—so bad that they are reducing the lifespan of people in middle America.

I believe it is the first time we have seen a reduction in the lifespan of Whites in a century.

Right. So what happens after justice is supposedly served in 2007? The companies and their allies begin chapter two and put lobbyists on the ground in all fifty states. We found 1,300 of them. They also placed doctors and patient advocates they had paid on panels at the National Institutes of Health, on panels at the National Academies—defending more treatment of pain using opioids.

The companies created a whole new apparatus for pushing their drugs. I should add that these are legal means. It’s not illegal to lobby; it’s not illegal to give money to members of congress; it’s not illegal to push your favorite doctors to serve on federal panels.

In a way, that’s the saddest thing. It was all pretty much legal.

One of the things interesting about this 2007 settlement—it was one of the few times pharmaceutical executives were charged with criminal conduct and plead guilty. But these three executives only did a few years of probation and community service.

Nobody went to jail, and the behavior just continued. That’s what I find fascinating.

I don’t think we appreciated how into the nitty gritty, these companies are willing to get. A group called the Pain Care Forum was so worried about Washington state implementing new opioid guidelines that they hired a public relations firm for that one state. The American Pain Foundation also helped fund lobbing efforts.

In Tennessee, they have a very bizarre law that if a doctor is not willing to prescribe opioids for pain, he is required to send the patient to another doctor who will. That’s a law that the drug makers helped to pass and then defended in one of our articles.

Oxycontin generates over $2 billion in US sales. It’s not that much to hire a couple lobbyists in a state to protect millions of dollars in prescription revenue.

There’s an interesting story about how you uncovered the Pain Care Forum. How did that unfold?

I was talking with a source who had come to Washington for a meeting with the Pain Care Forum, which I had never heard of. This group meets once a month in Washington at a law firm, and it is run by the main lobbyist for Purdue. This source sent me a list of the attendees and it was all the companies and patient groups . . . anyone who had a stake in the matter of pain and opioids.

How did you get your hands on these documents? I had a sense from reading the stories that you had piggy backed on a lawsuit from Cohen Milstein.

In 2015, I learned that the City of Chicago had filed a lawsuit alleging something along the lines of what Big Tobacco had done. They accused opioid makers of polluting the public understanding of the benefits and risks of opioids.

That lawsuit made a big splash but few reporters caught on that the City of Chicago is a government entity with public record laws. That was the beginning of a trial and error process, trying to figure out what we could get. Like any overly ambitious reporter, I wanted all the documents. Well that didn’t work.

Eventually, I realized that you could get access to trade groups and nonprofits, and I remembered about the Pain Care Forum, and asked for those documents. They sent over a couple hundred documents, and I spent months organizing everything.

When I was on the Hill, I spent a great deal of time investigating the drug companies by investigating the doctors and nonprofits that the companies funded. I referred to them as the proxy agents of the companies. As you dig into these nonprofits, how does it change your idea of a “nonprofit”?

Yes. [Laughs]. I feel like we should almost come up with a new name because “nonprofit” sounds so innocuous and has so many connotations that don’t serve this conversation. I’m sure you remember the American Pain Foundation. They got 90% of their money from drug makers and then used that money to testify against people who were suing drug makers and to testify on Capitol Hill.

[Note to readers: after this interview, STAT reported on a new nonprofit called Advocates for Opioid Recovery which is calling for wider use of medications to treat opioid addiction. The group is advised by Newt Gingrich and Patrick Kennedy and has been promoted by CNN commentator Van Jones. All three declined to disclose the organization’s funding.]

I recently wrote a piece noting how one group created studies in science journals to push opioids.

Right. The American Pain Foundation shut their doors the next day after the Senate sent them a letter. Apparently, all that money dried up, overnight. To your point, many of these groups are messaging arms of corporate America. In this case, it’s the pharmaceutical industry, but there are similar innocuous sounding groups for banking and other industries.

What is your opinion of the federal agencies? The companies are doing what they do: creating front groups, paying off doctors, lobbying, creating medical studies as part of their marketing. Can Americans feel secure that their federal science agencies are still looking out for them, nonetheless?

Right. [Laughs] After this sabbatical, then I have to go back and keep writing about these agencies…. As citizens, you and I are very optimistic and idealistic when we think about our government. We should expect them to be crusaders for public health, the way that David Kessler was when he ran the FDA under President George H. W. Bush and President Clinton.

Unfortunately, it’s very easy to just check the box and say, “This meets the definition of a safe and effective drug.” But if you just think that small, you’re never going to stop an epidemic like the one we’re in. The ramifications of these drugs go so much further than just that one patient. Maybe they take it for five days after dental surgery and put it in their cabinet. Then along comes a nephew who finds the bottle and he takes it with his friends and they get hooked on opioids. A year later, this kid can’t afford to buy opioids illegally, so he starts taking heroin.

There are these cascades of impacts that make the conversations the FDA has over whether a company submitted well-controlled studies look out of touch. Nobody in the federal government was looking at the broader picture until the Centers for Disease Control came out with prescribing guidelines.

But the CDC almost had to sneak that out of their headquarters in Atlanta, before the drug companies could realize what was happening and stop it.

Absolutely. That’s the sad conclusion to our story. It took an agency hundreds of miles from DC that previously did not get in the business of writing medical guidelines. That’s not what they do. They track small pox outbreaks and epidemics. But the FDA had been locked into this narrative that we needed more opioids to address chronic pain.

It’s almost miraculous that CDC published those guidelines. We showed in one story how industry, once they got wind of what CDC was going to do, they succeeded in forcing them to delay the guidelines by a month. But against all odds, the guidelines came out largely intact.

What happened at the National Institutes of Health?

We have these august science groups that do important work, like the National Academies, like the NIH, but you have to realize that so much of that comes back to legislation. In the case of the Pain Care Forum, they wanted a semi-permanent body at the NIH that would do nothing but look at pain and push more money to studying it. Well, they got that in the Affordable Care Act, which was passed in 2010.

What do you know, the patient representatives and the doctors that ended up serving on that NIH panel had long standing relationships with the Pain Care Forum.

One panelist was a patient advocate whose chair at the Center for Practical Bioethics was created with a $1.5 million gift from Purdue. She has a permanent job, courtesy of Oxycontin.

I’m cracking up. I hadn’t thought of that before, but you’re right. She has a job courtesy of the profits generated by Oxycontin.

You would think that this would be a red flag, and agencies would go, “No. We cannot have this on our panel!” It makes you question what comes out of these supposedly august federal science bodies that we put so much faith and trust in, when they choose someone like this as an acceptable expert.

Explain what happened at the National Academies, this purportedly important scientific organization that advises the federal government on policy.

The National Academies has put out some great reports, but it comes down to someone in Congress requesting them. The Pain Care Forum had been working for years to have the National Academies write a report on pain in America. And this got into the Affordable Care Act.

You know how this works—the National Academies creates a committee with researchers from universities all across the country. We looked back and half the experts who served on that committee had connections to industry and the Pain Care Forum. In short, they lobbied for the creation of the report and then had their friends serve on it.

Surprise, surprise, the report came out and the first sentence was that “100 million Americans experience chronic pain.” One hundred million Americans, to put in perspective, is about 40% of adults. [Laughs] That’s a damn lot of pain.

They didn’t say it all needs to be treated with opioids, but the report could then be reinterpreted and promoted by the same nonprofits and companies to say that there can’t be restrictions placed on opioids, because there are 100 million people with chronic pain that need treatment.

When these conflicts of interest were first broke by the Milwaukee Journal-Sentinel and MedPage Today, the head of the Institute of Medicine wrote to MedPage Today asking if the outlet would “bar your reporters from writing about alcoholism because you receive advertising revenue from beer companies?”

I actually emailed Ivan Oransky, the editor, to confirm that the letter was real. I didn’t believe that someone like the head of the Institute of Medicine couldn’t understand that stacking a report with experts tied to industry might be a problem.

Who do you think does their background checks? It must be the last thing they do.

I don’t think they do much at all. Okay, so we have this giant organism called the Pain Care Forum, whose entire reason for existence is to consume money from pharmaceutical companies and convert that cash into a medical culture that demands more opioids. What was the impact on Americans?

It takes my breath away to say this, but 180,000 Americans have died from using these legal medicines since 2000. More have died from heroin, after having first got hooked on prescription opioids.

One researcher explained to me that once you get hooked, it’s a lifelong trajectory, and you will have to deal with this addiction for the rest of your life. You either get treatment or one day you get a bad hit, you overdose, and you die.

When I was in the Senate and working on the diabetes drug Avandia, before we put out the report that ended up on the front page of the New York Times and on Good Morning America, there was a time when I was not sleeping at night.

It was early 2010, and I knew from internal documents that this drug was causing heart attacks and the company knew it. The FDA knew it. I was having problems sleeping at night, because every hour of every day that I wasn’t working to get this report out, more people were having heart attacks. Did you have some similar problems as you spent months working on this investigation?

Around 2010, the FDA brought in an expert panel to advise them on opioids. The panel said, “You need to mandate opioid training for doctors and do more about these drugs.” But the FDA went against the advice of its own experts and made the training for doctors voluntary.

I spoke to Dr. Nathaniel Katz, who was a former adviser to the FDA. He had traveled from Boston to Washington, as a private citizen, with his own money, to beg the FDA to do more. We quoted him saying, “The FDA failed to make a decision that could have averted many of the thousands of deaths we’re seeing per year,”

After having that conversation, I hung up the phone and it hit me that the FDA had a chance to take the advice of their own experts and save thousands of lives, and they just wouldn’t do it. I had been working on the story around six months at this point. I didn’t break down and cry, but I almost wanted to, because it all felt so futile.

These people with so much power and expertise just couldn’t make the tough decisions.

What do you think causes that? When I was on the Hill, my job wasn’t just to expose but to try and fix problems. I remember one time when the NIH finally got the message that they were funding doctors who were likely corrupt. They admitted this to us in meetings. They knew they had a problem, but they didn’t do anything about it.

I had a conversation with my boss afterwards, who was the Committee’s Chief of Investigations, and she was really mad, almost coming out of her chair, asking me, “What do you when you tell someone they have a problem. They admit, ‘Yes, we have a problem.’ And then they don’t do anything to fix it?! What is your next move? We all admit there’s a problem, but nothing happens to fix it?!”

Yes.

To this day. I don’t have a response to that. What do we do as a society?

You probably care more about the opinion of your neighbor, your friends, and your co-workers than people living on the other side of Earth. In the same way, the life of a senior official at the FDA involves a lot of talking to drug companies. Unfortunately, that’s kind of what the job is. It is a public health agency, but it’s also a gatekeeper for businesses. So all day, your job is interacting with drug company executives or doctors and researchers who are likely paid by drug companies.

In one story, we quote Janet Woodcock, a senior official in charge of drug approvals about why the FDA wouldn’t require doctors to take a class on opioids and safe prescribing. She said, “You can’t imagine the bitter screeds we hear from the prescribing community about the paperwork involved.”

Basically, she was more concerned about doctors complaining about the paperwork involved in some extra training instead of the advice of her agency’s own expert panel.

How has this series altered your understanding of the drug industry and medicine in general?

We only know what is happening to our country and our government, in many cases, after it’s already happened to us. It’s almost a secret history that we unearthed—how the federal government was spun.

It makes me appreciate the importance of investigative journalism. But it’s also sobering. You only know as much as the information you can get your hands on. We only find the tip of the iceberg, in many cases, I’m afraid. In so many cases, you never get to know who was in the room when decisions were being made.

Thanks to you, we can now see what the money doctors are taking. But still, somebody has to come along and read the Open Payments website to put together those relationships.

It’s funny you bring up the Physician Payments Sunshine Act and Open Payments. That’s why I worked so many hours to get that passed, so that we could get stories out like this. How has your opinion changed about physicians? That most august of august professions.

It’s like any profession. You can use that degree to do incredibly helpful things for people and society, or you can use it for pure monetary enrichment. In medicine, if you choose the monetary path, there are so many pharmaceutical companies waiting to help you cash that check.

With Open Payments, I can log on and see this. I think it’s thrown a wrench into the game played by the drug makers. I guess it comes down to shame. If you don’t mind a federal website that lists you receiving thousands of dollars for giving speeches for pharmaceutical companies, then you can do that.

Was there blowback for this series?

I was certain the day this story went out, we were going to get calls from lawyers representing companies or doctors. The first story went out on a Sunday, and I came into the office and sat by the phone the entire day, waiting.

They never challenged a single thing. Every single point in the stories—every medical study cited—was footnoted and fact checked. It pays to do your homework.
