Imma ramble about this cuz I like Tsuna (not based in anything especially concrete; I'm just running my mind)
Honestly with all the flaws (and great stuff) in KHR, Tsuna's a great protagonist for the sole reason that he is so stubborn, so willing to weave out of trouble... Just to stay in his lane 🤣
He's sensible in that way. It's always "I don't want to be a Mafia boss! That's dangerous!" Which is true. Like the boy was hardly a teen when the series starts. He's already bullied a lot and struggles in school. He's "Dame Tsuna," so just let him float on by and do his thing. Tsuna is always nervous of being the center of attention, even if it's his friends (Yamamoto's & Gokudera's popularity). He made his comfortable little goals and he's sticking by them.
On top of his self-esteem being low, throughout the whole series he comes to recognize himself as someone who can be strong, can protect his friends and family, and has grown. But he just wants to live his life as what he believes is normal. All the pazzazz and control that comes with being in the mafia is terrifying—as it should be. I'm sure even the "moderate" members couldn't have been sparkling great (Timoteo's sons were offed by Xanxus & Co. 😬 shu, the Varia as an assassin squad is a thing under Timoteo's rule, so). The potential risks would remain great.
Tsuna might understand he's a better leader then he thought he ever could be, but ultimately he doesn't want to "lead" anything. Knowingly putting himself and his loved ones in constant danger is terrifying to him. Of course, the Vongola goes beyond his own needs and wants, and he will have to ultimately give in (as he kinda does by the last arcs... Kinda). But even in the Future arc: all versions of future Tsuna—except one—we're dead. Within only ten years. That was a real threat that came true.
Not wanting to be in the mafia was one of his most mature decisions from the start. Tsuna has to realize that running from it won't actually help, but he isn't wrong for trying to do so. Kid just wants safety and security in his life for once.
