I'm actually going to bet money that most people who are -known- for looking really young for their age or "not aging" have gilbert's and just aren't diagnosed, or don't realize the impact, or some combo if it and a connective tissue condition or some autoimmune fuckery.
Bilirubin is an anti-oxidant. When your liver has a mutation and doesn't process it out very well, so long as the levels don't get high enough to cause visible jaundice and nerve damage, basically you just have high levels of antioxidants running around in your blood all the time for free without having to do anything to actively support/cause that.
If you add something to that like an autoimmune condition constantly causing enough damage do keep those levels hovering at a high but safe range, and then something like a connective tissue condition that makes your little skin attachment fibres stretchy and less brittle and causes you to develop -or not develop- features that are smaller or "cuter"... Or maybe both... Maybe even add a sun allergy that means you never go in the fucking sun... Maybe add some skin or hair growth rate mild abnormalities that means you have 'baby bangs' into your 40's
You suddenly get someone who looks like they haven't aged since they were a teen and gets carded regularly at 37 with much suspicion.
The secret isn't "not eating meat" or "thinking pure thoughts"... It's fucking:
Don't go in the fucking sun
Have a genetic mutation
Don't go in the fukcing sun
Have eds and multiple other conditions and try not to die of stress
The sun is a fucking deadly laser don't let it touch you
Don't tan either
Maybe drink a lot of tea or water or something idk I can't say I am not doing that...
Wear your fucking sunscreen

Let's talk about risk factors for neonatal jaundice, which is found in >80% of newborn babies and usually resolves in 1-2 weeks as the infant feeds and poops. However, a small percentage of babies develop hazardous hyperbilirubinemia (TSB≥30) --> brain damage from bilirubin crossing the blood-brain barrier (aka kernicterus). Visual assessment of significant jaundice does not reliably correlate with high total serum bilirubin levels! Therefore all newborns should be screened 24-48 hours after birth or before discharge, whichever is sooner. Both transcutaneous and serum bilirubin levels are reliable screening tools, although if a TcB is close to the treatment threshold, it should be checked with a TsB.

OH ok so the ghastly teenagers are 2x (GtN p. 170, p. 269) described as having a pituitary problem, likely due to the pituitary gland being the "grandmaster" of the endocrine (hormone) system and, being teenagers, they are in the THICK of it. and I love that actually like in tandem with Gideon having an acne problem, love to see that specific physical part of teenagerhood presented here.
It is actually Ortus who is described as hyperthyroid (GtN p. 295) (my guess is TM meant hypothyroidism and nobody caught the error -- Ortus is frequently described as variations of "large" and one of the classic symptoms of hypothyroidism is weight gain + fatigue, wheras hyperthyroidism is more frequently associated with uncontrolled weight loss + higher energy levels.)
Colum is described as jaundice (GtN p. 189, 297), which is due to excess bilirubin (related to liver malfunction) and can result in yellow skin and sclera (whites of the eyes turn yellow). (this also tracks with Colum elsehwere (p. 314) being described as looking as if he had a liver inflamation))

Approach to the evaluation of an adult with isolated unconjugated hyperbilirubinemia
Classification of hyperbilirubinemia – For clinical purposes, serum bilirubin is fractionated to classify hyperbilirubinemia into one of two major categories: unconjugated (indirect) hyperbilirubinemia and conjugated (direct) hyperbilirubinemia. Unconjugated hyperbilirubinemia is characterized by plasma elevation of predominantly unconjugated (indirect) bilirubin. This may be due to the overproduction of bilirubin, impaired bilirubin uptake by the liver, or abnormalities of bilirubin conjugation. In patients with conjugated hyperbilirubinemia, both unconjugated and conjugated (direct) bilirubin are elevated. This may be due to hepatocellular disease, impaired canalicular excretion of bilirubin, or biliary obstruction.
●Historical clues – Multiple clues to the etiology of hyperbilirubinemia can be obtained from the history:
•Use of medications, herbal medications, dietary supplements, and recreational drugs
•Significant alcohol consumption (>210 grams of alcohol (15 drinks) per week in males, >140 grams of alcohol (10 drinks) per week in females
•Hepatitis risk factors (eg, travel to endemic areas, blood transfusions prior to 1992, intravenous drug use)
•History of abdominal operations, including gallbladder surgery
•History of inherited disorders, including liver diseases and hemolytic disorders
•HIV status
•Occupational or recreational exposure to toxic substances
•Associated symptoms (eg, fever, right upper quadrant pain, myalgias)
●Laboratory evaluation – Initial laboratory tests include measurements of serum total and unconjugated bilirubin, alkaline phosphatase, aminotransferases (aspartate aminotransferase and alanine aminotransferase), prothrombin time/international normalized ratio, and albumin. Subsequent testing is guided by the predominant pattern of liver injury.
●Additional testing to determine etiology of hyperbilirubinemia based on pattern of liver tests
•Conjugated hyperbilirubinemia and elevated alkaline phosphatase – Elevation of the serum alkaline phosphatase out of proportion to the serum aminotransferases suggests biliary obstruction or intrahepatic cholestasis. Testing in patients with elevated bilirubin and alkaline phosphatase of hepatic origin typically starts with right upper quadrant ultrasonography to assess the hepatic parenchyma and bile ducts (algorithm 1). The absence of biliary ductal dilatation suggests intrahepatic cholestasis.
•Conjugated hyperbilirubinemia and hepatocellular injury – A predominant elevation of serum aminotransferase activity suggests that jaundice is caused by intrinsic hepatocellular disease. The evaluation is based on the degree and pattern of aminotransferase elevation and is discussed in detail separately.
•Isolated unconjugated hyperbilirubinemia – The evaluation of unconjugated hyperbilirubinemia typically involves evaluation for hemolytic anemia, drugs that impair hepatic uptake of bilirubin, and Gilbert syndrome (algorithm 2). However, patients with unconjugated bilirubin levels ≥4 mg/dL or abnormal aminotransferases should be referred to a hepatologist for further evaluation and consideration of a liver biopsy.
•Isolated conjugated hyperbilirubinemia – Conjugated hyperbilirubinemia without other routine liver test abnormalities is found in two rare inherited conditions: Dubin-Johnson syndrome and Rotor syndrome. Normal levels of serum alkaline phosphatase and gamma-glutamyl transpeptidase help distinguish these conditions from disorders associated with biliary obstruction.

I can't even check my blood pressure at home right now because cheap batteries from rexall ate through my blood pressure monitor that I have had for over 10 years...
So I have to estimate how fast my kidney's are actually processing things by how much I am peeing VS how high my blood pressure -feels-...
But actually the decrease in POTS symptoms over the past couple years, the getting gout every time I try taking a vitamin and even all the bone pain could all be from some level of kidney failure.
Which would make sense considering at some point during early post covid my complain to my doctor was that I hadn't peed in a week, hadn't pooped in two, and it felt like pressure was just building up in my abdomen.
My doctor and the hospital ignored that complaint until I slowly got better on my own... And didn't bother doing tests until the symptoms had already mostly lifted. Again.
But it may very well be my kidneys, and all the things I have been doing to help my liver and energy levels like having extra phosphorus, animals fats and proteins... Might all be very very bad for your kidneys.
So I am putting myself on a kidney failure diet, at least until the gout and bone pain go away, and monitoring for other symptoms [I mean I may still have occasional steak I kind of need the iron], but like... Without someone running tests I can't actually know how bad the problem is, but going to a walk in clinic about it and trying to convince them to run the tests I -actually- need run sounds a lot like adding errands to run when I don't even have the energy to go finish my groceries, for zero actual pay-off.
Regardless, know that I am on top of it and paying close attention anyway, since I am sure I will cause someone great concern by coming on here and being like "my bones won't stop hurting now"... Because my bones won't stop hurting now. I think high serum phosphorus levels might be fucking with the calcium in my bones again. I think everything is feeling like it's trying to break my forearms because the bones are actually fucked up right now.
Now I just have to balance doing what I need to do to support my liver with what I need to do not to tax my kidneys and honestly the answer might be to let myself get a bit jaundiced and prioritize my kidneys for a bit, the added anti-oxidant effect of bilirubin won't hurt my kidneys, it'll probably actually help protect them from damage.
So now the only animal fats I am going to eat for a while are from cream. The fats might not be great for my kidneys but I need the calcium and iodine and I don't want my liver going fully to shit... But I -do- need to buy more honey so I am not tempted to switch to refined sugar T~T which I don't normally eat much of at all. Going to try to get iodine from foods and not by having more salt. Going to -in fact- keep forgetting to salt my food like I have been lately.
Gee it would be so cool if I could just submit some blood tests for approval and my reasons for it and have a practitioner of some kind who's job it is to be like "sounds legit" and have a lab do them and send me the results. Free health care shouldn't mean needing someone's permission to know what's going on inside my body. Tell me what my serum phosphorus levels are, and also I want to be checked for chronic hepatitis still, and also while we're at it can we check my thyroid levels and my ana right now too? There has to be a way to get set up to test some of these things at home, right?
