I actually did die...clinically that is. November 8, 2010 my heart was stopped & I was placed on a heart-lung bypass machine for 5 1/2 hours while surgeons and nurses worked to give me a new lease on life. Atrial Fibrillation Common After TAVR and Aortic Valve Replacement
MedicalResearch.com Interview with: Rajat Kalra, MBCh Cardiovascular Division University of Minnesota, Minneapolis MedicalResearch.com: What is the background for this study? Response: New-onset atrial fibrillation after aortic valve procedures is thought to occur frequently after aortic valve procedures, such as transcatheter aortic valve implantation (TAVI) and surgical aortic valve replacement (AVR). However, the incidence estimates and implications of this new-onset atrial fibrillation in the contemporary era are unclear. We sought to examine the incidence of atrial fibrillation after aortic valve procedures, compare the incidence between TAVI and AVR, and evaluate the associated morbidity and mortality implications using a ‘big data’ approach. This big data approach employed the National Inpatient Sample and was validated in the New York State Inpatient Database. Both are publicly available datasets that are developed as part of the Healthcare Cost and Utilization Project, a federal-state-industry partnership that is sponsored by the Agency for Healthcare Research and Quality. MedicalResearch.com: What are the main findings? Response: We found that the incidence of atrial fibrillation was ~50% in TAVI and AVR hospitalizations in the National Inpatient Sample cohort. The hospitalizations with new-onset atrial fibrillation had increased length of stay, odds of in-hospital stroke, and odds of in-hospital mortality. In our validation cohort, we found that new-onset atrial fibrillation occurred in ~15% of TAVI procedures and ~30% of AVR procedures. While the incidence estimates of new-onset atrial fibrillation were lower, the odds of in-hospital mortality were very similar in the validation cohort. MedicalResearch.com: What should readers take away from your report? Response: Atrial fibrillation is commonly associated with TAVI and AVR and has important morbidity and mortality implications. Clinicians should consider discussing the possibility of atrial fibrillation as a post-procedural complication as part of the shared decision-making process prior to aortic valve procedures. MedicalResearch.com: What recommendations do you have for future research as a result of this work? Response: New-onset atrial fibrillation is likely due to a combination of patient-based and procedural factors. Now that our group and others have highlighted the importance of peri-procedural atrial fibrillation, the next step for our field is to identify preventive strategies to limit the incidence of new-onset atrial fibrillation after aortic valve procedures and the associated morbidity and mortality burden. At this stage, may be difficult since many of these factors are not modifiable. However, the evolution of valve design and peri-procedural management strategies may offer important advances in atrial fibrillation prevention. Response: Our findings also reiterate the urgent need to find a balanced approach for peri-procedural anticoagulation for aortic valve procedures. This is an area that is already rife with debate in modern-day cardiovascular medicine, given the prognostic associations of atrial fibrillation that we (and others) have demonstrated and the emerging data regarding transcatheter aortic valve thrombosis. Waiting for my Aortic valve replacement
I am in hospital at the moment waiting to have a heart operation to replace my aortic valve. I am writing this blog for something to do so if you get bored don't worry, I won't be offended.
And life couldn't be more different. This blog is for people who are on the way to an operation or will be in the future. My dear darling daughter is extremely worried that I have too much time on my hands doing nothing so together we came up with the idea of writing my experiences - as she says I am so funny the way I say and describe things that it may be nice for other people to know that it's’ not so bad. With a good sense of humour and the patience anybody can go through it and come out in the other side.
We need a beginning so let's talk about when it all happened...
Everything in my life has a dash of comedy and of course it was my 60th birthday party. I had people from London coming to stay in my house and others from around where I live.
The day before I had a very busy day at work, went to a funeral, supermarket, pick up someone from the train station, prepare food for the next day, go to a friends house etc etc etc.
It was a very long day but one or two glasses of wine in the evening (maybe 3 to be honest) was a must. I was very tired and after a day like mine anybody would have been (At least that’s what I thought).
Next day early start. Lots to do and prepare after all it was my big 60.
The plan was to go for a meal in the evening at a local restaurant with about another 8 people. Throughout the day I thought ‘I’m not going to drink a lot today, I have to be at my best, I have a dinner to cook for about 25 people on Sunday’ so in my head I was saying ‘Eli behave it has got to be perfect nothing can go wrong’.
Saturday all going well according to my plans. In my head so far so good...
Everybody got dolled up and as always I was looking pretty with my new white dress lovely high heals, the blings to be honest the lot..
My husband had the idea of a pub crawl before the dinner so we went for pints and wine. It was nice to be the birthday girl, didn't even need to ask for drinks they just appeared from nowhere, just like as if the glass before gave birth to another one. Magic.
Again I didn’t want to drink too much as I had a big dinner to cook the next day.. a Feijoada which is a Brazilian dish (I am brasilian by the way).
We managed to get to the restaurant and everybody was starving. I don't remember what I ordered for my starter but I ate all of my brother's in law’s mussels - they were delicious! He wasn't impressed by my contribution to help him but hey it was my big 60 and I could do anything...
It was very hot in the restaurant and I remember saying to my husband that I was going to the toilet and I remember him say to my daughter to go with me............ The rest is history.
I am going to let my daughter describe what happened that night because I don't have any memory of it.
