Pterocarpus marsupium and Gymnema Sylvestre Powder in Alleviation of Biochemical Abnormalities Associated with Human Type 2 Diabetes Mellitus- Juniper Publishers

Abstract
Objectives: This observational study investigated the antihyperglycemic, antihyperlipidemic and antioxidant potential of Pterocarpus marsupium (Vijaysaar) and Gymnema sylvestre (Gudmar) (“VG”) Therapy in patients with type 2 diabetes mellitus.
Methods: Subjects with type 2 diabetes mellitus were administered a phytochemical formulation consisting 50 ml of aqueous extract derived from 3 gm “VG” powder was administered twice a day for six months. The blood glucose levels were monitored at monthly intervals; glycosylated hemoglobin, lipid profile and biomarkers of oxidative stress, liver and kidney functions were monitored at three monthly intervals.
Results: Daily administration of the “VG” Therapy regularly for 6 months resulted in significant reduction of blood glucose and glycosylated hemoglobin levels. There was also a significant increase in high-density lipoprotein cholesterol levels with concomitant decrease in total cholesterol, triglyceride, low-density lipoprotein cholesterol, and very-low-density lipoprotein levels. Significant improvement in the biochemical markers of oxidative stress was recorded.
Conclusion: TThe results suggest that herbal formulation “VG” is beneficial in improving glucose and lipid homeostasis in type 2 diabetics and may serve as an adjunct therapy.

Introduction
Type 2 diabetes mellitus is reaching epidemic proportions worldwide and is a complex metabolic disorder characterized by hyperglycemia and dysregulated lipid metabolism [1,2]. Abdominal obesity and dyslipidemia (increased triglycerides, high LDL-cholesterol and low HDL-cholesterol), and hypertension, as well as altered platelet function [3] are the major risk factors contributing to type 2 diabetes. The metabolic defects that underlie the development of type 2 diabetes may include islet β-cells dysfunction, hyperglucagonemia, elevated endogenous glucose production, development of peripheral insulin resistance, inflammation of adipose tissue, and dysregulation of adipokines production [4]. Chronic elevation of blood glucose which is a central factor in the production of reactive oxygen species (ROS) [5] which in turn promote cellular damage and contribute to the development and progression of diabetic complications, such as cardiovascular disease, nephropathy, retinal blindness, neuropathy, and peripheral gangrene [6]. The conventional allopathic drugs, sulfonylureas, meglitinides, biguanides, metformin, thiazolidinediones, miglitol, acarbose, etc. although effective [7], are associated with minor or major side effects on long term use [8]. Statins used in the treatment of dyslipidemia have their own limitations [9].
Plant based Indian traditional medicines such as Ayurveda have been used since ancient times in the treatment of diabetes [10]. Ethnobotanical studies of traditional herbal medicine used for diabetes have identified more than 1,200 species of plants with hypoglycemic activity [10]. Medicinal plants are frequently considered to be less toxic and free from side effects than synthetic ones. The antidiabetic effects of several plant extracts and herbal formulations and their bioactive compounds have been identified and characterized for the treatment of diabetes mellitus [11,12].
Polyherbal formulations [13], have been shown to exhibit antidiabetic, antihyperlipidemic and antioxidant potential in animal models as well as in diabetic patients [14]. The phytochemical based formulations consisting of multiple herbs are liable to produce a large number of metabolites that may act on multiple targets in the body and hence, polyherbal formulations (Which are used in traditional practice) are preferred over mono therapeutic ones. Although, phytochemical based formulations have extensively been used, studies on their proof of efficacy are limited [15].
Gymnema sylvestre (Gudmar) is a climber grown in tropical forests of South-East and belongs to the family Asclepiadaceae G. sylvestre is considered to have a great anti-diabetic potential and used in several anti-diabetic formulations. There are several reports in animal models as well as in diabetic patients which shows that extract of G. sylvestre leaves have anti-diabetic potential [16,17], and these effects are attributes due to increase in insulin secretion [18]. Pterocarpus marsupium (Vijaysaar) is the most versatile plant with a wide spectrum of biological activities and belongs to the family Fabaceae. The P. marsupium is reported to be anti-diabetic and associated with anti-obesity, anti-hyperlipidemic, antiinflammatory, anti-oxidative, antitumorigenic functions [19-21]. The aim of this study is to explore the therapeutic potential of combined effects of the regular consumption of Pterocarpus marsupium and Gymnema sylvestre in human subjects with type 2 diabetes mellitus.
Materials and Methods
Study sample
A total of 45 type 2 diabetic patients attending the weekend diabetes clinic run by the Centre for Translational Research, School of Studies in Biochemistry, Jiwaji University, India expressed their willingness to participate in the study. Of these, 35subjects met the inclusion criteria who were then explained the necessity of maintaining defined lifestyle pattern during the course of the study. Out of 35 subjects, 8 subjects were eliminated during the course of the study due to non-compliance. The remaining 27 subjects took the drug regimen regularly as per the experimental design. Figure 1 shows the number of subjects enrolled and drop outs during the course of study. The patients’ baseline characteristics are summarized in (Table 1).

Study design
The study design included following steps:
a) Selection of patients meeting inclusion criteria and those consented to participate in the study.
b) Measurement of anthropometric and biochemical parameters once before and at selected intervals during the course of the study.
c) Administration of “VG” Therapy in defined dose daily for a period of 6 months.
d) Measurement of parameters at the end of the study.
e) Analysis of the data.
Inclusion criteria
a) Non-insulin dependent diabetes diagnosed, as per the criteria of the World Health Organization;
b) Both genders between the ages of 30-65 years;
c) Body Mass Index in the range of 18.5 to 40;
d) Participants who understood the benefits of the study and signed a written informed consent;
Exclusion criteria
a) Presently using other blood glucose regulating agents;
b) Daily intake of alcoholic beverages;
c) Smokers consuming more than 1 pack/day;
d) Patients diagnosed as type I and insulin dependent type II diabetics;
e) Patients with hepatic or renal disease, pancreatitis, cardiac problems, uncontrolled hypertension, malnutrition and severe immune deficiency.
f) Non-compliance during the course of study.
Before starting the study all participating subjects were given a verbal explanation about the objectives of the study, nature of drug formulation (Kwath) to be consumed daily, (“VG” Therapy) rationale and duration of therapy in local language. They were asked to avoid a carbohydrate rich diet and advised regular walking for about 4-5 km during the course of study. The patients who consented to participate in the study were registered, anthropometric measurements; weight, height and waist were recorded at the beginning and at selected intervals during the study period. The patients were kept exclusively on “VG” therapy.
Composition of drug (VG) formulation
The “VG” formulation consisted of a mixture of Pterocarpus marsupium (Vijaysaar), and Gymnema sylvestre (Gurmar) (Table 2) and was provided by the M/S Dindayal Industries Ltd. India. The individual plants were identified by Prof. A.K. Jain, School of Studies Botany, Institute of Ethnobiology Jiwaji University Gwalior. Voucher specimens were prepared and deposited at the Centre (IOE-501 and IOE-505).
Composition of VG formulation: 1:1.
Preparation of VG kwath
3 gm of VG powder was suspended in 100 ml drinking water, kept overnight, boiled till 50% water is evaporated, cooled to room temperature and consumed on an empty stomach twice a day for six months under the supervision of an Ayurvedic Physician.
Biochemical parameters
The fasting and postprandial plasma glucose levels were monitored at monthly intervals, while the glycosylated hemoglobin (HbA1c), biomarkers of oxidative stress viz., Superoxide dismutase (SOD), catalase, reduced glutathione (GSH), Thiobarbituric Acid Reactive Substances (TBARS) and lipid profile, biochemical markers of kidney and liver functions were monitored at baseline, at the middle (3 month) and at the end (6 month) of the therapy.
Fasting and postprandial plasma glucose was estimated by the Glucose oxidase/Peroxidase method [22]. Glycosylated hemoglobin (HbA1c) was estimated by the ion exchange resin method [23]. Estimation of plasma total cholesterol by the Cholesterol oxidase Phenolaminophenazone CHOD-PAP method [24], triglyceride by GPO-PAP method [25], High density lipoprotein cholesterol (HDL) by Polyethylene glycol/CHOD-PAP method [26], Low-density lipoprotein cholesterol (LDL) and Very-low density lipoprotein cholesterol (VLDL) were calculated by the Friedewald formula, urea by the modified Berthelot method [27], uric acid by uricase/PAP method [28], creatinine by modified Jaffe’s kinetic method [29], Alanine transaminase (SGPT or ALT) and serum Aspertate transaminase (SGOT or AST) by the modified International Federation of Clinical Chemistry method [30], and bilirubin by method of L. Jendrassik [31] was assayed using standard kits from Crest Biosystems, Goa (India). Markers of oxidative stress such as super oxide dismutase (SOD) Winterbourn et al. [32], catalase by Sinha, [33], glutathione (GSH) Ellman et al. [34], and Thiobarbituric Acid Reactive Substances (TBARS) Ohkawa et al. [35] were checked.
Ethical clearance
The study protocol was duly approved by the Institutional Human Ethics Committee (JU/IHEC/2013-A/13).
Statistical analysis
Statistical analysis was carried out using a paired t test (Sigma stat 3.5). A p-value *p<0.05, **p<0.001 was considered statistically significant.
Results
Effect of VG therapy on hyper-glycemia
Table 3 shows the fasting and postprandial blood glucose levels at monthly intervals in subjects on “VG” therapy. A significant decrease was recorded for fasting (13.5% P<0.05), postprandial (15.0%, <0.001) glucose levels and glycosylated hemoglobin (9.8%) at the end of six months therapy.

Effect of VG therapy on lipidemia
Table 4 shows the results of the lipid profile of patients on “VG” therapy. Total cholesterol, triglycerides, LDL and VLDL were significantly decreased after six months therapy by 9.6%, 14.4%, 15.7%, and 14.4% respectively (P<0.05). HDL cholesterol was elevated from32.50±1.11 to 35.69 ± 1.02 (P<0.001).

Effect of VG therapy on biomarkers of oxidative stress
Significant (P<0.05, P<0.001), improvements in GSH level (from 1.98±0.17 to 2.59±0.21 mg/dl), SOD activity (from 0.63±0.05 to 0.94±0.08 μM/min/mg protein), catalase activity (from 7.64±0.24 to 9.44±0.23 μM/min/mg protein) were recorded. A significant decrease (P<0.001) in TBARS (from 457.19±8.09 to 415.15±7.47 (moles of Malondialdehyde/ml of blood) were recorded at the end of six months therapy.
Effect of VG therapy on markers of toxicity
The effect of “VG” therapy on kidney function was monitored by estimating urea, uric acid and creatinine levels in plasma at specified intervals during the course of therapy. The data presented in Table 5 showed significant (P<0.05), reductions in Urea (from 29.97±1.20 to 27.10±0.88mg/dl) and uric acid (from 5.08±0.20 to 4.67±0.20 mg/dl) and notable reduction in creatinine (from 0.78±0.06 to 0.73±0.05) at the end of six months therapy.

Significant (P< 0.05), variations in biochemical markers of liver functions, namely bilirubin (from 0.83±0.06 to 0.70±0.04 mg/ml) and SGOT (from 22.47±1.61 to 18.00±1.04 IU/L) as well as SGPT (from 23.60±2.32 to 19.26±1.38 IU/L), at the end of six months was observed (Table 6).
Effect of VG therapy on hypertension and body mass index
Table 7 shows variations in systolic blood pressure (from 133.70±2.59 to 129.22±2.39 mmHg), diastolic blood pressure (from 81.70±0.97 to 79.56±0.93 mmHg) and body mass index (from 25.21±0.64 to 24.22±0.60 kg/m2) at the end of the six months therapy.
Discussion
Currently, plant based therapies (main or supplementary) are being used globally for the management of type 2 diabetes. In India, the indigenous systems of medicine viz., Ayurveda, Siddha & Unani employ plant based extracts for treatment of diabetes (Premeha in Ayurveda). Most of the Ayurveda based formulations depend on plants/herbs based decoctions, and powders. In the present study, administration of the aqueous extract of VG that consisted of equal amounts of the ingredients for over a period of 6 months to type 2 diabetic subjects resulted in significant improvements of altered biochemical markers. Significant alleviation in glucose homeostasis, as marked by noticeable changes in glycosylated hemoglobin, blood glucose levels, and lipid profile were observed. The G. sylvestre leaf extracts are reported to reduce hyperglycaemia in experimental animals [16], and human type II diabetics [17]. The G. sylvestre is reported to promote insulin secretion, probably by regeneration of pancreatic beta cells [17,18]. The gymnemagenin and gymnemic acids in G. sylvestre, showed to exhibit potent antioxidant, and hepatoprotective activities [36]. Ethanolic extract of G. sylvestre was reported to possess anti‑apoptotic potential in STZ‑induced diabetic cardiomyopathy, and affected the levels of leptin, insulin, dyslipidemia, apolipoproteins, lipids, LDH (serum lactate dehydrogenase) in experimental models [37]. Administration of deacylgymnemic acid DAGA (200mg/kg), a derivative of gymnemic acid reduced Systolic blood pressure and significantly improved the fasting plasma glucose and HOMA-IR (homeostatis model assessment insulin resistance) with marginal improvement in lipid profile [38]. The G. sylvestre is also reported to inhibit absorption of glucose from intestine. The atomic arrangement of gymnemic acid molecules is similar to that of glucose molecules and they reduce the blood glucose level by preventing the absorption of sugar molecule in the intestine. It is attributed by filling the receptor location in the absorptive external layer of the intestine by gymnemic acid molecules [39].
In a controlled study, an aqueous extract of the leaves of G. sylvestre, (GS4) when administered at the dose of 400mg/day, to subjects with insulin-dependent diabetes mellitus (IDDM), the insulin requirement came down together with fasting blood glucose, HbA1c and serum lipid levels soon after therapy. However, the GS4 administration failed to check glycosylated haemoglobin and glycosylated plasma protein levels on long term follow up. The GS4 therapy is reported to enhance insulin secretion rate, either by regeneration or revitalisation of the beta cells [40].
The effectiveness of GS4, further investigated as a supplementary drug to conventional oral anti-hyperglycemic drugs. Administration of GS4 (400mg/day) to 22 Type 2 diabetic patients receiving conventional therapy resulted in significant reductions in blood glucose and, glycosylated hemoglobin level. In addition, five of the 22 diabetic patients showed improved glycemic control with GS4 alone and were discontinue their conventional drug [17].
In another study, supplementation of the diet with G. sylvestre at the dose of 500 mg per day for a period of 3 months reduced polyphagia, fatigue, blood glucose levels with concurrent reductions in HbA1c, lipid profile suggesting beneficial effect of G. sylvestre supplementation in the management of diabetes mellitus [41].
Pterocarpus marsupium is traditionally used in Indian ayurvedic medicine for the treatment of diabetes and has been shown to control diabetes in experimental animals [42]. The phenolic C-glycosides present in P. marsupium regulated glucose homeostasis and significantly decreased the TNF-α level in experimental diabetes [43]. The phenolics, marsupsin, pterosupin, pterostilbene and (-)-epicatechin present in P. marsupium have been identified as the blood sugar lowering components [20,44].
The alcohol extract of the bark of P. marsupium Roxb shown to exhibit potent antidiabetic activity and corrected the metabolic alterations in diabetic rats [42]. Aqueous extracts of P. marsupium Linn significantly attenuated hyper insulinaemia as well as hypertriglyceridaemia and also prevented the alteration in metabolic patterns [45]. The Indian Council of Medical Research undertook an anti-diabetic phase II open trial at four centers across Indian using Vijaysar (Pterocarpus marsupium). Vijaysar was tested in newly-diagnosed or untreated non-insulin dependent diabetes mellitus (NIDDM) patients between 35 and 60 age for 12 weeks. Among the 93 patients who completed 12 wk of treatment, both the fasting and postprandial blood glucose levels fell significantly (P < 0.001), by 32 and 45mg/dl at 12 wk from the initial means of 151 and 216mg/dl respectively. Mean HbA1c Other laboratory parameters remained stable during the designated treatment period of 12 wk. Also, no side-effects were reported [46].
Our previous study done with the polyherbal formulation, “GSPF kwath” which contains Vijaysar and Gurmar as two of the ingredients has shown antidiabetic and antioxidative effects [13]. “GSPF kwath”, showed 23.5%, 26.7% and 11.7% reductions in fasting, postprandial blood glucose and HbA1c respectively. In the present study “VG” containing only two of the components of GSPF proved highly beneficial in restoring not only hyperglycemia but also lipidemia and antioxidative potential. The changes observed on glycemic, lipidemic and oxidative stress. Oral administration of “VG” therapy for 6 months had no adverse effects, either on kidney or liver functions and in fact showed a significant improvement in the functioning of these vital organs.
Conclusion
In conclusion, the “VG” administration to type 2 diabetic patients with varying degrees of glycemia, lipidemia and oxidative stress provide an efficient alternative to conventional antidiabetic drugs when coupled with active lifestyle and dietary changes. Significant decrease in biochemical markers viz., blood glucose, HbA1c, total cholesterol, triglycerides, LDL-cholesterol, and VLDL-cholesterol and higher values of HDL, following VG therapy are testimony to the anti-diabetic efficacy of VG. Further, the formulation did not show any adverse effects on liver and kidney functions and may be a potential natural and safe therapy for treatment and prevention of diabetic complications but randomized controlled trials are required to confirm these findings.

Good for What Ails You

Due to the powerful phenolic compounds found in ginger, this spice has been studied for its benefits for a wide range of ailments and health disorders. While often referred to as a root, ginger is in fact a spice that contains potent antioxidants such as shogaols, zingerone, and gingerols.1 These strong compounds have been linked to providing relief for dozens of issues, including cough, bronchitis, upper respiratory tract infections, migraine headaches, motion and morning sickness, rheumatoid arthritis, post-surgical pain, flatulence, loss of appetite, diarrhea, upset stomach, stomachache, colic, dyspepsia, and general and chemotherapy-induced nausea. It has even been used topically as an analgesic.1
Ginger has been used as a home remedy for thousands of years.
Ginger’s History
The health benefits of ginger have a history almost as old as civilization itself. Historians have found references to the use of ginger, known botanically as Zingiber officinale, in the writings of nearly every ancient society, including Chinese, Indian, and Roman. In fact, the ancient Roman Empire first started importing ginger nearly two thousand years ago.2 Due to its alluring smell, great taste, and medicinal properties, the spice became extraordinarily popular in Europe. This rise in demand, combined with the expensive cost of shipping the product from Asia, forced Europeans to find ways to make ginger less expensive and more readily available. They did this by introducing ginger to the West Indies, Mexico, and South America, and then exporting it back to Europe. Since ginger thrives in a tropical climate, it is produced in the United States in Hawaii, Florida, and California.1 Internationally, it is exported mainly from Jamaica, Fiji, India, China, and Australia.
Ginger’s Benefits
Motion Sickness
Many prescription or over-the-counter products on the market that are developed to combat motion sickness or nausea have side effects like drowsiness and dry mouth. Ginger has shown in several studies that it may provide significant relief to the above symptoms without the detrimental side effects.3
In one study comparing ginger to a placebo in an attempt to measure its ability to reduce symptoms associated with motion sickness, ginger was measured to be potentially more effective than placebo.3 This trial studied 80 beginner-level sailors who were prone to motion sickness. Half of the group took powdered ginger and the other half took placebo. The half who took ginger experienced a reduction in vomiting and cold sweats compared to placebo.
Morning Sickness
Ginger has been used as a home remedy for thousands of years, with one of the longest known uses as a treatment for morning sickness accompanying pregnancy. Researchers presented a study that finally put this folk remedy to the test. In a double-blind, randomized, placebo-controlled trial, a group of pregnant women ingested 125 mg of ginger extract four times a day for four days.4 All the women were less than 20 weeks pregnant, and the women who ingested the ginger showed significantly reduced symptoms of morning sickness, compared with women who consumed placebo.
Inflammation
Ginger contains ultra-potent anti-inflammatory compounds called gingerols, which are the substances that many scientists believe are responsible for the reduction in inflammation people experience when they start taking ginger supplements regularly. Case in point, a recent study involving patients who tried conventional drugs to alleviate painful symptoms found that 100% of patients with muscular discomfort experienced relief of pain or swelling when they consumed ginger on a daily basis.2
In a compelling study published in Osteoarthritis and Cartilage, 29 patients with debilitating arthritis in the knee participated in a placebo-controlled, double-blind, crossover study.5 The study had each patient start with placebo or ginger, and then switch after 3 months, writing down the results. At the end of the first half-year of the study, the patients who were taking ginger experienced significantly less pain than those on the placebo. Additionally, the patients using ginger reported improvements on a standardized scale used to assess mobility.
A separate study measuring the anti-inflammatory properties of red ginger in the Journal of Medicinal Food found positive results as well.6 In this study, Japanese researchers prepared a 40% ethanolic extract from dried red ginger and evaluated its anti-inflammatory activity. After confirming that antioxidant shogaols and gingerols suppressed production of inflammatory mediators, the researchers discovered that red ginger extract “has a potent suppressive effect on acute and chronic inflammation, and inhibition of macrophage activation seems to be involved in this anti-inflammatory effect.”6
Ginger Tips
Choose fresh ginger over dried ginger to preserve the high levels of gingerol content.
Fresh ginger root should be firm, smooth, and free of bruises and mold.
Fresh ginger can be stored in your refrigerator for up to three weeks if left unpeeled.
Dried ginger should be kept in a tightly sealed glass container in a dry, cool, and dark place.
The center of the ginger root is the most flavorful.
When shredding ginger, be sure to shred in the direction of the fibers.
Helps Reduce Chemotherapy-Induced Nausea
Chemotherapy-induced nausea is the type of stomach unease or discomfort that begins within 24 hours after the administration of chemotherapy. Many patients treated for cancer experience this awful side effect, which can lead to vomiting, dry heaving, and overall sickness. It is known that meals high in protein may offer some protection against the onset of nausea, and a study recently tested whether high-protein meals with added ginger had any beneficial effect.
A team at Siena College in Loudonville, New York, studied this hypothesis on 28 patients with cancer who were receiving chemotherapy for the first time.7 Control group patients ate their normal diets, while another group drank a high-protein shake with ginger twice daily. The results were striking.
Patients taking the high-protein shake with ginger reported significantly fewer instances of nausea, and electrical measurements of the stomach muscles revealed less gastric dysrhythmia, leading the researchers to conclude that high protein meals with ginger reduced the nausea that follows chemotherapy.7
Colon Cancer Prevention
In a study done by scientists at the University of Minnesota, gingerol exhibited anti-inflammatory and antitumorigenic effects.8 At first, scientists were unsure about why gingerol displayed such effectiveness against colon cancer, but the study revealed that the leukotriene A(4) hydrolase protein (LTA[4]H), whose activity can act as a catalyst for colon cancer cells, is targeted by gingerol.8
The researchers gave a group of specially bred mice an injection of gingerol three times a week before and after injecting human colorectal cancer cells into them. Tumors first appeared 15 days after mice were injected, but only 4 tumors were discovered in the gingerol group, as opposed to 13 in the control mice.
The findings revealed that “gingerol effectively suppressed tumor growth in mice by inhibition of LTA(4)H activity…these findings support the anticancer efficacy of gingerol for the prevention of colorectal cancer.”8
Fights Ovarian Cancer
While the effectiveness of ginger extracts against inflammation and tumors has been shown above, perhaps ginger has the most promising effect in the fight against ovarian cancer.
At the 97th Annual Meeting of the American Association for Cancer Research, Dr. Rebecca Liu and her team from the University of Michigan made a presentation about gingerol’s ability to kill ovarian cancer cells by inducing apoptosis (programmed cell death) and autophagocytosis (self-digestion).9
The presentation focused on Dr. Liu’s team’s experiments that examined the effects of a whole ginger extract containing 5% gingerol on several different ovarian cancer lines. While traditional chemotherapeutic agents suppress inflammation the same way ginger is purported to, the danger is that cancer cells may become resistant to the drugs. Dr. Liu believes that ginger may be unique in this respect because cancer cells show no sign of becoming resistant to its cancer-fighting properties. Her team’s data showing that exposure to ginger extract caused cell death in all ovarian cancer lines studied bolster this claim.9

Unfortunately, resistance to chemotherapeutic agents is the most frequent occurrence during treatment, which indicates a need for the discovery of novel therapeutic anticancer drugs.The tumor-suppression effect of eight different cucurbitacins was evaluated in gastric cancer cell lines, and the Cucurbitacin E (CuE) showing the greatest effect was used in further studies to explore the mechanism and potential synergistic effect of Dox bothand.Compared with other cucurbitacins, CuE showed the greatest antiproliferative activity against the gastric cancer cell lines. Further investigations revealed that CuE suppressed the growth of gastric cancer cell lines through the induction of G2/M arrest and subsequent apoptosis by impairing AKt activation and reducing its expression in gastric cancer cells. Furthermore, our results indicate that CuE can significantly enhance the cytotoxicity of doxorubicin (Dox) bothand.In summary, we present the first evidence of the efficacy of CuE for the inhibition of gastric cancer growth and the synergistic antitumorigenic effect of CuE and Dox, bothand.

CBD dispensaries have popped up in many states all over the country in people’s hunt for wellness. But CBD oil can also be used to help dogs with anxiety and other issues. What is CBD and how does it work? And, will it give your dog the munchies? First, let’s define some of the terms that can be confusing.
“Cannabidiol, also known as CBD, is a cannabinoid �� a naturally occurring compound found in both hemp and cannabis (cannabis is also referred to as marijuana),” says Jodi Ziskin, director of communications with Treatibles in Petaluma, California.
Hemp and marijuana are “Both members of the Cannabis sativa plant and share similarities but have very distinct differences due to each plant’s biological structure,” says Jon Neveloff, partner with King Kanine in Fort Lauderdale, Florida. Consider that a rice cake and rice pudding are from the same plant but quite different.
“CBD is one of hundreds of compounds found in cannabis. It is the most prevalent compound in hemp, a special strain of cannabis that has only trace amounts of THC, the substance in medical and recreational strains of hemp that is associated with psychoactive effects,” says Joey DiFrancesco, CEO and founder of LolaHemp in New York City. This is crucial because THC can be toxic to pets. (For more THC specifics, see “Why the Farm Bill Matters” on page 38.) “CBD is non-psychoactive,” Joey adds. It won’t get your dog high in any way.
Hemp Oil vs CBD Oil
Both CBD oil and hemp oil come from the same plant, but CBD oil comes from the flowers, leaves and stalk of the hemp plant, while hemp oil comes from the seeds of industrialized hemp plant.
Hemp oil is a nutritional supplement full of nutrients that can be used in cooking and for moisturizing — like in soaps or lotions. It has a variety of other manufacturing uses, such as making plastic and as a bio-diesel fuel.
CBD oil is a supplement used for medicinal purposes, like treating various conditions, such as inflammation and anxiety, among others.
Help for anxiety
Many owners report benefits to using CBD oil for dogs, and CBD oil has been recommended to assist with many ailments that plague our furry friends, including anxiety.
“The most common conditions that relate to anxiety include location and separation anxiety, as well as noise phobias,” says Annie Chrysler, CBD specialist and product manager with SpaRoom based in Cleveland, Ohio. “For example, CBD oil can provide noticeable relief to dogs who demonstrate car ride anxiety or who experience anxiety when visiting the veterinarian’s office. CBD oil can also provide noticeable relief for dogs who have noise sensitivities such as fireworks and thunderstorms.”
Amanda Howland, co-founder and CTO of ElleVet Sciences in Portland, Maine, explains further. “Pain and anxiety are so interrelated in dogs that we wanted to have both a profound anti-anxiety effect and an extremely effective pain management effect, to help the pet feel better in every way.”
Angie Krause, DVM, with Boulder Holistic Vet in Boulder, Colorado, says in her experience CBD is consistently effective for treating dogs with mild to moderate anxiety. “For severe generalized anxiety, I have more success with drugs like Prozac,” she adds.
So how does it work? It’s based on science; the same science that explains how CBD works with man and man’s best friend, who have some biology in common. That’s because all mammals have an endocannabinoid system (ECS.)

Jillian Dutson, marketing and advertising manager with Pet Releaf, based in Littleton, Colorado, explains. “When your dog consumes CBD, his ECS is activated. The ECS works as a two-way communication system with various systems in the body such as the immune, nervous and digestive systems just to name a few. This two-way communication allows these parts of the body to speak to one another to help them function at optimal levels of health.”
And there’s more to it: “Although further studies are needed, initial findings show that CBD binds to a number of endocannabinoid receptors,” says Lauren Brychell, marketing coordinator with cbdMD in Charlotte, North Carolina. “These receptors regulate everything from inflammation to pain perception, and CBD has shown the ability to potentially raise dopamine levels and improve mood while reducing symptoms of anxiety.”
Speaking of studies, “Most of the research done on cannabidiol, indeed cannabis writ large, has taken place outside of veterinary research, which is not uncommon, as vet research typically lags behind human medicine,” Joey says.
More benefits and dosage
There are many common uses for CBD oil in dogs, other than anxiety. “I use CBD for dogs with seizures, arthritis, cancer, pain, chronic inflammation and allergies,” Dr. Krause says.
Joey, too, cites success in using CBD for the Big C. It can “Shrink tumors in several types of cancer both because it appears to have antitumorigenic properties of its own and because it appears to enhance the effectiveness of some chemotherapy agents.”
CBD oil has also been used to treat the following ailments:
Sleep issues
Vomiting and nausea
Muscle spasms
Glaucoma
Digestive issues and appetite loss
Skin conditions
As with any supplement or medication, getting the correct dose is crucial. Many of the manufacturers state dosing suggestions should be determined based on each dog’s individual stats and condition. Some suggest generic guidelines as a starting point: 1 mg per 10 pounds of body weight twice a day; Dr. Krause recommends 0.5 mg/kg of CBD twice daily for anxiety.
Of course, always get input from your dog’s vet on any treatment. “First, many symptoms that CBD may help with can be an indication of a deeper and more serious medical problem that needs to be treated,” Joey says. “Second, CBD oil can interfere with the metabolization of other drugs (in a similar way that grapefruit does). Third, the answer to dosing questions is dependent on what condition is being addressed.”
And don’t assume you and Fido are in for a quick fix. “While some dog owners may notice an immediate difference in their pet, we suggest waiting two to three weeks to see the full effects,” Lauren says.
Giving to your dog
Whatever the dosage, you have a couple of options on how to administer CBD oil: topically or ingested. But which is better?
“The answer ultimately depends on the individual dog,” Annie says. “More commonly, topical application can provide a more localized sense of relief, such as pain relief in the hips and legs. Ingesting can provide a broader sense relief, such as separation anxiety relief.”
Angela Ardolino, founder and CEO of CBD Dog Health in Tampa, Florida, lists other conditions that lend themselves to direct applications. “It has incredible benefits for skin issues, including allergies, hot spots, bug bites, skin tumors, warts and cysts when applied topically,” she says.
If you do decide to go the oral route you have more choices here, too. According to Angela, “The best way to administer CBD is to lift the dog’s lip and administer it right onto the gums (it is absorbed through capillaries in the gums).” Putting it under the tongue is also common. CBD oil can be mixed in with your dog’s food or treats made with CBD oil.
It may come down to what your dog — and wallet — prefer. “The oils and treats that I use in the practice have the same hemp extract in both,” Dr. Krause says. “The oil is more cost effective.”
Why the Farm Bill Matters
In December 2018, President Trump signed the much anticipated 2018 Farm Bill into law. Whether you’ve got a city dog or a country dog this is something that can affect dog owners everywhere regarding CBD. In short, the Farm Bill officially legalizes cultivating and producing industrial hemp and removing the crop from the federal list of controlled substances.

That last part may also free up your veterinarian to be able to discuss CBD oil more in depth with you. The Federal Drug Enforcement Administration named cannabis and cannabinoid products as Schedule I controlled substances, with no accepted medical use. That hindered veterinarians’ ability to administer or prescribe them. Some vets opted not to talk about CBD as a treatment because of these restrictions.
“Now that the Farm Bill has passed, I believe this will not be an issue moving forward,” says Angie Krause, DVM, with Boulder Holistic Vet in Boulder, Colorado. “Hemp is legal at a federal level, and each state will decide how they want to regulate hemp. If a state declares hemp illegal, it may limit the veterinarian’s ability to prescribe or discuss hemp/CBD.”
The Farm Bill will:
Define industrial hemp broadly to cover all parts of the Cannabis plant including seeds, derivatives, extracts, cannabinoids, etc. as long as it has a THC level of 0.3 percent or less.
Remove hemp completely from the Controlled Substances Act (CSA).
Make the USDA the sole federal regulatory agency overseeing hemp cultivation.
Authorize and fund hemp research as part of the Supplemental and Alternative Crops program and the Critical Agricultural Materials Act.
Regarding that research, Heidi Hill, Holistic Hound founder and trained homeopath in Berkeley, California, says, “We are thrilled with this victory … This decision will also fund and allow long-overdue research into the many health and wellness benefits of this amazing plant.”
A final look at terminology
Some manufacturers tout CBD isolate while others say full spectrum is better. Here’s the difference and an industry professional’s take on each:
Full spectrum, or whole plant, CBD contains all other cannabinoids found in the marijuana plant, including minute amounts of THC. “[Full spectrum] contains all the beneficial constituents of the whole plant, including other cannabinoids, terpenes, flavonoids and fatty acids. These naturally occurring components all work together for enhanced benefit and what is called ‘the entourage effect,’” says Heidi Hill, Holistic Hound founder and trained homeopath in Berkeley, California.
CBD isolate is purified CBD that has been extracted from the marijuana plant and isolated, hence the name, from the other cannabinoids. “Our knowledge about CBD and its benefits are well-established, but we are just beginning to learn about the possible benefits of other compounds in the hemp plant. To claim the benefit of a full spectrum product over CBD itself, is premature,” says Joshua Sosnow, DVM, chief medical officer of CompanionCBD and owner of Arizona practices North Scottsdale Animal Hospital and Desert View Animal Hospital.

Sharing pros and cons
If you have other pets at home with similar conditions, they may also benefit from the same product your dog uses — with different dosages, of course. “There is a big difference in dosing for dogs and cats,” Amanda says. “Cats are not small dogs! The half-life in cats is extremely short, meaning they metabolize it very quickly. We have very specific dosing for dogs and for cats based on science.”
According to Colette Florido, founder and president of CR Pets Thrive in St. Petersburg, Florida, says, “A high-quality CBD product will work equally well for both cats and dogs. The main difference you’ll find between options for cats and dogs is what other flavors are combined with the CBD, knowing that each pet has their preferences.”
Some CBD products for cats come in catnip or fish flavors, while beef or peanut butter flavors are used for dogs.
The most important part about sharing CBD oil is to never share products made for humans with your dog or any other pets. “It is vital to make sure that the oil you are buying is not just a human product with a paw print on the label,” Angela says. “Human products may contain artificial flavoring, like xylitol, that is toxic to dogs.”
Joey gave a flavor-specific example of toxicity. “Wintergreen oil, which is perfectly fine for people and probably won’t bother a dog, is potentially toxic to cats. It is commonly found in CBD products made for people.”
There is much more research to be done on using CBD oil for your dog but hopefully you now know more about how it works and how it might help. And just to confirm: CBD oil won’t give your dog the munchies. If your dog is craving Scooby snacks, it has nothing to do with a supplement.

Jon Neveloff, partner with King Kanine in Fort Lauderdale, Florida, says, “The most important question is: Are there lab results on this bottle of CBD? If so, what are they testing for?”
“Know where the hemp is grown,” recommends Angie Krause, DVM, with Boulder Holistic Vet in Boulder, Colorado. “U.S. hemp grown organically is preferred. Extraction technique is very important. For cats, I prefer CO2 extraction. This ensures there are no harmful chemical residues. A guaranteed analysis should be available from the manufacturer to quantify the concentration of CBD in each ml.”
Colette Florido, founder and president of CR Pets Thrive in St. Petersburg, Florida, says to “ask to see the company’s certificate of analysis (COA) and lab tests. If the company can’t or won’t [share it], do not buy. It means they don’t test for pesticides, solvents, mold, yeast and E. coli. An ethical CBD company will proudly show you their COA.”
Jillian Dutson, marketing and advertising manager for Pet Releaf, in Littleton, Colorado, suggests you ask these questions when comparing CBD products:
Where is their hemp grown and can they prove it?
If claiming organic, do they have the necessary certifications to verify these claims?
How much CBD is in their products and do they have consistent third-party testings to confirm these amounts?
Jodi Ziskin, director of communications with Treatibles in Petaluma, California, adds these:
Does the milligram count on the label reflect the amount of hemp oil or does it include the carrier oil(s)?
Is the company a National Animal Supplement Council (NASC) member and are they fully compliant with their packaging and marketing?
“We bought and tested about 20 different products we got from pet stores and online, and out of the 20, none had what was advertised on the label and some had no CBD at all,” said Amanda Howland, co-founder and CTO of ElleVet Sciences in Portland, Maine. “Pet owners need to be careful and research the company.”
