Garlean Housing Ideas

I keep getting this amazing image in my mind of what Garlean apartments, flats and houses could be like. So, here is how I like to think of the technologically advanced homes these guys live in.

I like to imagine Garlean houses are very open. There are few doorways with the exception of private rooms such as bedrooms, baths and studies. Electricity is obvious, as is plumbing and tech of convenience that you would not see in other places.

There are panels in the walls that slide out to store your coat, shoes, hat, purse, etc as you come home. There are options for higher ranking individuals to have chutes for both garbage and laundry. For those rank praefectus and above, perhaps they get free laundered garments and trash disposal. The panels slide back in and appear to be just another part of the wall when not in use.

Art and music are an integral part of Garlean culture. Owning art is a necessity if you’re a high ranking officer. IBFAN will do what we can to protect breastfeeding while warning WHO of the risks of getting too close to corporations and the need for a sound Conflicts of Interest (COI) policy.  A policy that will address WHO’s own institutional COI.

The main issues we will be covering will be:  Emergencies (11.2), Health Environment and Climate Change (11.6), Non Communicable Diseases (11.8), Universal Health Coverage (11.5),  Anti Microbial Resistence (11.8) Patient safety, Hygiene in health care facilities (12.5), Health of Refugees and Migrants (12.4), Global Strategy for Women’s, Children’s and Adolescents’ Health (2016–2030) (12.8).

As a global network that protects breastfeeding, IBFAN is pleased to comment on this important topic.

Breastfeeding is a lifeline in emergencies. Babies are born with an undeveloped immune system and are at greatest risk of water-related diseases – with diarrhoeal disease the second biggest killer of under-fives. Breastfeeding provides food, care and immune support, and protects them from the worst of emergency conditions.

Despite this, all too often emergency responses are characterized by unsolicited donations of baby feeding products. This does not only put infant health at risk but also hinders women empowerment. Public appeals for funds often make this worse – with starving babies shown alongside the myths used by the baby food industry. Rarely do they highlight the resilience of breastfeeding or that artificially fed babies face many more risks to survival.

IBFAN’s World Breastfeeding Trends Initiative assessments show that implementation of UN recommendations on this is dismal. WHO can play a key role in reversing this situation by promoting the  Infant and Young Child Feeding in Emergencies – Operational Guidance for Emergency Relief Staff and Programme Managers, which is designed to give all those working in emergencies guidance on appropriate infant and young child feeding and ensure that when breastmilk substitutes are required they are purchased, distributed and used according to the UN’s agreed strict criteria.

We welcome the IOAC recommendation that WHO establishes long-term relationships with key NGOs on the ground and hope you will guard against COI and we look forward to working with WHO to ensure that families in emergencies have the support and protection they need.

Agenda Item 11.5 Universal Health Coverage IBFAN_UHC WORD. IBFAN_UHC PDF.

As one of WHO’s longest-standing public interest partners, whose primary concern is to protect breastfeeding and child health, IBFAN strongly supports the efforts of Member States to ensure publicly funded UHC,  with primary health care and prevention as a foundation.

 It is worrying that many governments are now under attack from those pushing privatised systems, who pay no attention to how easily these can sabotage  public health goals.

Innovative arrangements that can genuinely advance health are to be welcomed –  but they must be carefully screened and monitored by truly independent bodies, without corporate involvement. Public Private Partnerships and Multi-Stakeholder arrangements inevitably lead to private sector involvement in health care planning,  Corproate access to personal data, and most importantly privatisation of services. WHO knows well how easily this can happen, especially in the infant and young child feeding arena.

 While WHO cannot police what happens at country level, it does have an obligation to warn Member States of the risks of inappropriate commercial involvement and how to avoid – not just manage – them.  Indeed WHO now seems to fear being seen as ‘risk averse’ more than being seen as cavalier, even negligent.  We call on WHO and Member States once again  to protect their independence, integrity and trustworthiness and provide Member States with the reliable and independent evidence that shows clearly that public provision of services is the only way to achieve health for all. This issue  is a slippery slope that needs to be navigated very carefully and WHO has a key role to play in getting it right.

Lastly, as a priority, Member States, must monitor and ensure full policy and programme support to the protection, promotion and support breastfeeding, including it in the indicators of monitoring SDGs 2/3 and ensuring full implementation of the International  Code and resolutions, maternity protection and universal Baby Friendly Hospital Initiative.

Agenda Item 11.6  Health, Environment and Climate Change

IBFAN welcomes the opportunity to comment on the draft strategy on health,  environment and climate change.

Much more should be done to protect, promote and support breastfeeding, a practice that has no carbon footprint, contributes to water conservation and helps in the reduction of air pollution.  While we are calling for reduction of avoidable environmental risks, global sales of breastmilk substitutes increase rapidly every year, boosted by sophisticated marketing techniques against which breastfeeding cannot compete. These products are, of course, necessary for babies who are not breastfed, but they need energy to manufacture, materials for packaging, fuel for transport distribution and water, fuel and cleaning agents for daily preparation and use, and they generate Green House Gases.  Between 11-25 kilos of CO2 and more than 4000 liters of water are estimated to be needed along the production pathway to produce just 1 kg of BMS powder. If breastfeeding was considered a public health intervention for climate change prevention and mitigation, more would be done to end misleading marketing of baby food companies and support women who want to breastfeed.

At the very minimum, all governments should fully implement the International Code of Marketing of Breastmilk Substitutes and relevant WHA Resolutions. We regret that goal 12 of the draft strategy does not include the requirement that national and local governments set up proper safeguards against conflicts of interest and commercial influence when facilitating “cross-sectorial cooperation”. Like other green priorities, supporting new parents with breastfeeding now, will benefit future generations.

Agenda Item 11.8.  Anti Microbial Resistence

As an NGO that has attended Codex meetings for more than 20 years, IBFAN is pleased that WHO is providing evidence to support the strengthening of Codex trading standards and texts to minimize and contain antimicrobial resistance throughout the animal and food chain, including through processing.  We hope that all Member States will recognize that this is a threat to global health security that no country can eliminate on its own and that there are inevitable implications for global trade. Codex is a difficult fora that is dominated by powerful corporations and industrialised countries that protect their interests. WHO needs to be there and strong to protect the global health goal in these complex negotiations.

Among the many factors that increase the risk of AMR are poor animal husbandry and crop management, pharmaceutical promotion and over the counter sales that encourage inappropriate use of anti-biotics. We note that only 64 countries have limited the use of critically important antimicrobials (human and animal) for growth promotion in animal food production.

Strong health care systems, cross agency action, surveillance and regulation of all the industries involved is clearly essential.  However,  care must be taken to ensure that any public private collaborations do not muddy the waters, slow everything down and lead to weaker controls and action.   There is evidence that a higher dependence on the private health sector and higher density of private health clinics is associated with increased AMR due to frequent overuse of antibiotics.

Lastly, we support the efforts to promote WASH components within National Action Plans to reduce the incidence of infections. WASH, alongside stricter marketing regulations will protect breastfeeding – a practice that is critically important in reducing the incidence of infections in infants and young children.

Agenda Item 11.8  Prevention and Control of Non-communicable Diseases

The prevention of NCDs requires us all to tackle the power of corporations and their threat to public and planetary health.

WHO’s proposals rely on a mix of regulatory approaches alongside voluntary commitments and self assessments that are to be monitored by a consortium of institutions. This is a major undertaking that risks WHO’s name being exploited for public relations purposes. Indeed, some of the named institutions work closely with the companies on the presentation of their findings.

IBFAN recommends that WHO focuses on its own internal conflicts of interest and sends a clear message that binding regulations, independently monitored using public funds are what’s needed.  Governments cannot be in the driver’s seat and at the same time in a corporate partnership.

WHO’s  baby food marketing and conflicts of interest rules and the British Medical Journal’s recent decision to stop all baby formula adverts – are important models that could be followed by all.  WHO’s work at Codex trading standards is also critically important.. 

Highly processed products, supplements and deceptive biofortification claims will not solve the NCD problem. Instead support small farmers, biodiverse and culturally appropriate foods and prevent soil depletion, deforestation and land-grabbing.

IBFAN statement on Agenda item 6.1, “WHO governance reform processes” and sub-item “Involvement of non-State actors” (Document EB 145/4)

IBFAN has worked alongside WHO for about 40 years and has followed WHO’s reform process, often with alarm. In our experience, when WHA resolutions are translated into legislation, properly monitored, implemented and defended at Codex, they really can and do protect health and have probably saved millions of lives. There is no need to reinvent the wheel. The baby food issue can be used as a model wherever human health is under threat from harmful marketing practices. The call for increased engagement and partnerships with entities that caused these threats will not help. It will lead to misplaced trust, image transfer, confusion and a weakening of resolutions. To stop the public good being subverted, WHO must remind governments that effective legislation, not here-today-gone-tomorrow voluntary commitments, is what’s needed. We disagree with the statement in report 145/4 that WHO’s constitution ‘forsees’ the participation of NSAs in WHO’s governing bodies. WHO’s basic documents have always been clear that NGOs must be “free from concerns that are primarily of a commercial or profit-making nature.” This is one important reason why FENSA must be evaluated. In allowing Businesses and Philanthropies to enjoy Official Status, WHO has taken a huge leap into the dark. It is vital that the risks of such a fundamental change be evaluated, acknowledged and addressed. WHO has a constitutional duty to consult with civil society as rights holders. Corporations do not have these rights. The term “non-State actors” makes it impossible to see what’s really happening and who is really working for the public good. We even all wear the same colour badges. It also places a huge strain on WHO’s due diligence process. WHO is clearly under pressure to be less ‘risk averse’, to use FENSA as an enabling framework for undue public-private entanglements, to not bother correcting its own Conflict of Interest definition.[1] We call on WHO to resist such pressure, stay within its constitutional mandate, safeguard its independence, trustworthiness and integrity and its unique role as the coordinating authority in setting global health norms. Civil society is there to help.

C.A Dated On 23-07-2019 GS-2 PM Paridhan Rozgar Protsahan Yojana Why in news? Pradhan MantriParidhanRozgarProtsahanYojana(PMPRPY)was a labour reform initiative under which Government wasbearing the entire 12% of the employers’ contribution to the Employees Provident Fund Scheme for new employees of garment and made-ups sectors for the first three years of their employment. What has changed?  Earlier, 8.33% of employer’s contribution was being provided by Government under Pradhan MantriRozgarProtsahanYojana(PMRPY) and additional 3.67% of employers’ contribution was made available under PMPRPY to garment and made-ups segments to new workers with wage up to Rs. 15,000/- per month.  With effect from 1.4.2018, Government has started paying full employer’s share i.e. 12% (EPS+EPF) for employees in all sectors (including textiles) for three years to new employees and to existing beneficiaries for their remaining period of three years under PMRPY. The Protection of Human Rights (Amendment) Bill, 2019 Parliament should come together to support Government’s step to make NHRC more inclusive and efficient, said Union Minister of State for Home Affairs, Shri Nityanand Rai, while moving the Protection of Human Rights (Amendment) Bill, 2019 for consideration and passing in Lok Sabha today. Why amendment inthe bill: C.A Dated On 23-07-2019  The Protection of Human Rights Act, 1993 was enacted to provide for the constitution of a National Human Rights Commission (NHRC), the State Human Rights Commission (SHRC) and the Human Rights Courts for protection of human rights.  The NHRC has proposed certain amendments to the Act to address the concerns raised at certain global platforms.  Besides this, certain State Governments have also proposed for amendment of the Act, as they have been facing difficulties in finding suitable candidates to the post of Chairperson of the respective State Commissions owing to the existing eligibility criteria to the said post.  In view of the above, it has become necessary to amend certain provisions of the said Act.  The proposed amendments will enable both the Commission as well as the State Commissions to be more compliant with the Paris Principles concerning its autonomy, independence, pluralism and wide-ranging functions in order to effectively protect and promote human rights. The Protection of Human Rights (Amendment) Bill, 2019, inter alia, provides  that a person who has been a Judge of the Supreme Court is also made eligible to be appointed as Chairperson of the Commission in addition to the person who has been the Chief Justice of India;  to increase the Members of the Commission from two to three of which, one shall be a woman;  to include Chairperson of the National Commission for Backward Classes, Chairperson of the National Commission for Protection of Child Rights and the Chief Commissioner for Persons with Disabilities as deemed Members of the Commission;  to reduce the term of the Chairperson and Members of the Commission and the State Commissions from five to three years and shall be eligible for re-appointment;  to provide that a person who has been a Judge of a High Court is also made eligible to be appointed as Chairperson of the State Commission in addition to the person who has been the Chief Justice of the High Court; and,  to confer upon State Commissions, the functions relating to human rights being discharged by the Union territories, other than the Union territory of Delhi, which will be dealt with by the Commission. C.A Dated On 23-07-2019 One Stop Crisis Centre One Stop Centre (OSC) at the district level maintains the record of complaints received from women affected by violence. Ministry of Women and Child Development reviews the functioning of OSCs from time to time. About the OSC  As per the guidelines, OSCs are required to have the facility for temporary stay up to 5 days with 5 beds.  To address the complaints of aggrieved women, the Scheme of One Stop Centre is to provide for a range of services for violence affected women including police facilitation, legal counselling, psycho-social counselling, medical aid and temporary shelter in an integrated manner under one roof.  Under the scheme, OSCs are required to have a centre administrator, case workers, medical personnel, police facilitation officer, psycho-social counsellor, legal counsellor, security guard, IT staff and multi-purpose workers.  Capacity building of OSC functionaries are undertaken through National, Regional and State trainings and workshops to improve the responsiveness of OSC staff.  OSCs are also integrated with Women Helpline to improve the functioning and responsiveness of OSC staff.  Media and publicity campaigns are undertaken to increase awareness about OSCs at district level. National Medical Commission (NMC) Bill On Monday, Union Health Minister Harsh Vardhan introduced the National Medical Commission (NMC) Bill in Lok Sabha. Background An earlier version of this Bill was introduced in the 16th Lok Sabha, and had passed the scrutiny of the Parliamentary Standing Committee on Health and Family Welfare. However, that Bill lapsed at the end of the term of the last Lok Sabha. Once the NMC Bill is enacted, the Indian Medical Council Act, 1956, will stand repealed. C.A Dated On 23-07-2019 The existing Act provides for the Medical Council of India (MCI), the medical education regulator in India. Why is Medical Council of India being replaced?  The Parliamentary Standing Committee on Health and Family Welfare examined the functioning of the MCI in its 92nd report (in 2016) and was scathing in its criticism:  “The Medical Council of India, when tested on the above touchstone (of producing competent doctors, ensure adherence to quality standards etc) has repeatedly been found short of fulfilling its mandated responsibilities.  Quality of medical education is at its lowest ebb; the current model of medical education is not producing the right type of health professionals that meet the basic health needs of the country because medical education and curricula are not integrated with the needs of our health system;  many of the products coming out of medical colleges are ill-prepared to serve in poor resource settings like Primary Health Centre and even at the district level;  medical graduates lack competence in performing basic health care tasks like conducting normal deliveries;  instances of unethical practice continue to grow due to which respect for the profession has dwindled.”  Such state of affairs are also symptomatic of the rot within and point to a deep systemic malice”. How will the proposed National Medical Commission (NMC) function?  The NMC Bill provides for the constitution of a 25-member NMC selected by a search committee, headed by the Cabinet Secretary, to replace the MCI.  The Bill provides for just one medical entrance test across the country, single exit exam (the final MBBS exam, which will work as a licentiate examination), a screening test for foreign medical graduates, and an entrance test for admission in postgraduate programmes.  The Bill proposes to regulate the fees and other charges of 50 per cent of the total seats in private medical colleges and deemed universities.  A medical advisory council — which will include one member representing each state and Union Territory (vice-chancellors in both cases), chairman of the University Grants Commission, and the director of the National Accreditation and Assessment Council — will advise and make recommendations to the NMC. C.A Dated On 23-07-2019  Four boards — dealing with undergraduate and postgraduate medical education, medical assessment and rating board, and the ethics and medical registration board — will regulate the sector.  The structure is in accordance with the recommendations of the Group of Experts headed by Ranjit Roy Chaudhury, set up by the Union Health Ministry to study the norms for the establishment of medical colleges. Significance The Bill marks a radical change in regulatory philosophy; under the NMC regime, medical colleges will need permission only once — for establishment and recognition. There will be no need for annual renewal, and colleges would be free to increase the number of seats on their own, subject to the present cap of 250. They would also be able start postgraduate courses on their own. Fines for violations, however, are steep — 1.5 times to 10 times the total annual fee charged. What are the changes in the 2019 Bill?  There are two crucial changes, following the recommendations of the Parliamentary Standing Committee on Health and Family Welfare (109th report in 2018).  One, it has dropped a separate exit examination.  Two, it has dropped the provision that allowed practitioners of homoeopathy and Indian systems of medicine to prescribe allopathy medicines after a bridge course. What did the panel say about exit exam? On the National Licentiate Examination, the Committee (in 2018) recommended that the relevant clause be redrafted “so as to make the final year MBBS examination as the licentiate examination”. Ban on colistin C.A Dated On 23-07-2019 Why in news? The Ministry of Health and Family Welfare has issued an order prohibiting the manufacture, sale and distribution of colistin and its formulations for food-producing animals, poultry, aqua farming and animal feed supplements. Significance  Intensivists across the country are rejoicing as the move is a “massive victory” for the movement against anti-microbial resistance.  Colistin is a valuable, last-resort antibiotic that saves lives in critical care units and in recent years, medical professionals have been alarmed by the number of patients who have exhibited resistance to the drug.  Therefore, any move to ensure that arbitrary use of colistin in the food industry, particularly as growth supplements used in animals, poultry, aqua farms, would likely reduce the antimicrobial resistance within the country. GS-3 Impact of Climate Change on Agriculture Context Agriculture sector in India is vulnerable to climate change. Higher temperatures tend to reduce crop yields and favour weed and pest proliferation. Climate change can have negative effects on irrigated crop yields across agro-ecological regions both due to temperature rise and changes in water availability. Rainfed agriculture will be primarily impacted due to rainfall variability and reduction in number of rainy days. Analysis of impact of climate change under National Innovations in Climate Resilient Agriculture (NICRA) project has found that climate change is expected to affect yields, particularly in crops like rice, wheatmaize. Government of India has initiated various actions to mitigate affects of climate change: C.A Dated On 23-07-2019 Varieties and cultivars tolerant to abiotic stresses are developed under strategic research component of NICRA. The technology demonstrations aim at enhancing the adaptive capacity of the farmers and also to cope with climate variability in the vulnerable districts to achieve climate resilient agriculture. Under NICRA, climate resilient technology demonstrations are implemented in 151 climatically vulnerable districts of the country. District Agriculture Contingency Plans have been prepared by ICAR-CRIDA, Hyderabad for 648 districts in the country to address the adverse weather conditions.

Mechanisms of AMR: Mdr Genes and Antibiotics Decoys Retard the New Antibiotic Discovery against Superbugs-Open Access Publishers Many microorganisms cause lethal diseases in human causing loss of lives and property worldwide. Many antibiotics are used to cure deadly infections for the past 75 years with no difficulty. Recent outbreaks of multi-drug resistant bacteria have caused millions of death every year and physicians do not know how to cure KPC2 Klebsiella kneumoniae, NDM1 Escherichia coli or MRSA Staphylococcus aureus and XDR Acinetobacter baumannii infections. Sadly once used ampicillin, streptomycin, azithromycin, tetracycline, and chlormphenicol are useless against those bacteria. Combination therapy using colistin, imipenem, amakacin, ceftizidime and investigation drug ovibactam sometime are giving good clinical efficacy but not sure. In such a situation, heterogeneous phyto-antibiotics and gene medicines have been welcome by medical authorities but AMR calamity remains as mdr genes (amp, bla, tet, cat, aac, aad, aph,sul etc) moved to conjugative plasmids and chromosome of bacteria with target specific alterations in rRNA and porins genes. Keywords: Mdr genes; Anti-microbial resistant; Gene medicine; Phyto-antibiotics Introduction Past 75 years are the golden era of drug development and several types of antibiotics are in centre stage of such discoveries since the discovery of penicillin drug by Alexander Flaming from slime mold Penicillium notatam in 1928 targeting peptidoglycan cell wall biosynthesis of most Gram (+) and Gram (-) bacteria [1]. Since then 1000 derivatives were made alone for penicillins (ampicillin, cefotaxime and imipenem) for better drug usually called penicillinases resistant drug. A professor of biochemistry and microbiology at Rutgers University, Dr. Selman A. Waksman discovered over twenty antibiotics (a word he coined) and introduced procedures of antibiotic production (streptomycin) that led to Nobel Prize in Physiology or Medicine in 1952. However, such dream could not last long as more potent penicillinases called, oxacillinases, cefotaximases, carbapenemases were appeared in bacterial plasmids [2]. New era of biology was begun in 1953 with the discovery of structure of DNA, gene structure, regulation of gene expression and advancement of DNA sequencing, chromosomal structure and RDT work (Figure 1). Profound impact was found by biomolecules separation by ultra centrifugation and HPLC with chemical structure analysis by Mass, NMR and FTIR. Invitebly we got many life saving drugs with known target site although basic DNA, RNA and protein composition in virus to bacteria to human were same [3]. [Click here to view Large Figure 1] Semi-synthetic drugs and anti-microbial resistance Naturally, semi-synthetic drugs were made without choice to overcome the action of multi-drug resistant class located in bacterial plasmids that inactivate the antibiotics by different mode of actions. As for example, ampR cell extract was discovered as early as 1940 and amp gene which produces an enzyme, Beta- lactamase was sequenced in 1965. Now one in three bacteria in river and sea water contained amp gene in large conjugative plasmids that also carry 5-10 other mdr genes and 10-15 Tra and Tnp genes [4]. So journey from 1940-1960, described the isolation of tetracycline, streptomycin, sulfa-drug, ampicillin, amoxicillin, cefoxitin, cefotaxime, erythromycin, nalidixic acid, ciprofloxacin, neomycin, polymyxin, enoxacin, norfloxacin (Figure 2). However, at the almost same time, resistant bacteria to all these antibiotics were developed creating pressure to drug industry for more and more new drug development. However, it is not very easy to develop a drug for human use because it needed at least one billion dollar to develop a drug. What happen to investor if a developed drug is good for few years and then drug resistant microbes appeared when no one want to prescribe that antibiotic because uncertainty of cure of such infections and also delay in treatment and also taken of repeated different antibiotics surely toxic to health and time and monetary loss [5]. [Click here to view Large Figure 2] Drug screening from bacteria against bacteria-a wrong message In fact, now R & D Industry screening new drugs everyday and also computer-guided graphics design and stimulate artificial drug-target interactions have accelerating the new drug development. Screening of new drug from fungi was favourable in sense that in soil and water there is a battle between bacteria and fungi and so fungi will produce anti-bacterial to kill bacteria. That type of selection is good having different genus but what we did that we introduced the battle between actinomycetes and bacteria like neomycin (1946) and actinomycin (1940). And then we introduced the battle between bacteria against bacteria as for example streptomycin is produced from soil bacteria, Streptomyces griseus and also chloramphenicol that eradicate typhoid disease in early decades. What has happened in life of bacteria that all want to destroy it and as a result bacteria are forced to re-arrange its genes to save its life, Hypothesis is not so easy as its own counterpart is enemy and bacteria created many new entity like transposons, integrons, R-plasmids and many DNA rearrangement enzymes like transposes, resolves and integrases and also many topoisomerases and restriction end nucleases [6]. In 1960-1980, we produced 1000 tons of antibiotics in industry and 7000 millions of global peoples now taken antibiotics almost every day or every month to remove the bacteria from intestine and blood to keep healthy. Doctors have forgotten that bacteria needed for human development and intestine should stay (10)12 bacteria for normal synthesis of vitamins which human could not synthesize itself . When such discrepancy was noticed, then probiotic bacteria were used as supplement after each antibiotic therapy. In other word, we used many unnecessary doses of antibiotics as for example, for viral infection, for pain and in food animal growth as well as in agricultural land [7]. Conjugation plasmid-a safe guard of bacteria to transmit genes without failure However that is too late, as bacteria developed another armour against antibiotics by using its very urgent plasmids used in conjugation (marriage) that means bacteria could form a sex pilus using Tra proteins coded by 62kb plasmid called F'- plasmid which usually did not carry MDR genes. What bacteria did that combined R-plasmid with F'-plasmid and such plasmid is known today as conjugative MDR plasmid which could be large as 100-500kb and such plasmids are hard to purify by plasmid purification method for molecular biological study being contaminated with bacteria chromosome (2000-5000kb) [8]. Never the less CsCl density gradient centrifugation and Pulse Field Gel Electrophoresis have help to isolate such plasmids with purity and also fully sequenced.
