Why Kawaei Rina and Iriyama Anna were attacked?
A mentally ill man had lost his job and wanted to blame someone for his misfortune. He saw AKB on TV and got angry and jealous that these girls were making more money than him and decided to cause harm to them.
The man basically just got the handshake ticket that anyone can get through the limited CDs in-store and went to the venue. He chose the shortest line, which happened to be the line with Annin, Kawaei, and a few others. Unfortunately Annin and Kawaei were at the front and got attacked, though a male staff member tried to protect and fight the man off.
So despite some beliefs, this man was not a fan of AKB, but rather despised them so much that he would go as far to attack them. Though each lane had security and staff, the organizers failed to do any sort of security checks prior to entering the venue, causing the incident to occur.
As a result, handshake events since then have beefed up security significantly. Of course, they have mandatory bag checks and metal detectors. Each lane has double the security and staff. When fans approach the end of the lane, they leave their bags in a basket monitored by staff. Staff check both palms to make sure they are not holding anything, and then they may proceed. They added an extra fence between fan and member, and fans are no longer allowed to lean over the fence when greeting the member.
You may see old AKB photo sessions where member and fan would stand right next to each other... that definitely would never happen now. Since the incident, there will always be some sort of barricade/fence between member and fan. Not tall enough for a handshake to be impossible, but wide enough so that any potential harm could be broken apart by a staff and a member could escape.

Rinrin: Because it's sitting right in front of us, and it's broken.

Ayaka: If it matters, probably not, but Ayanan was the last one to use it.
Ayanan: Liar, I don't even drink that crap!
Ayaka: Oh, really? Then what were you doing by the coffee counter earlier?
Ayanan: I use the wooden stirrers to push back my cuticles, everyone knows that Ayaka!
Moekyun: Okay, let's not fight. I broke it, let me pay for it Myao-san.
Myao: No, who broke it?
Annin:
Annin: Yuihan's been awfully quiet--
Yuihan: Really?
Annin: Yeah, really!
*arguments errupt*
Myao, to the camera: I broke it. It burnt my hand so I punched it.
Myao: I predict ten minutes from now they'll be at each other's throats with war paint on their faces and a pig head on a stick.
Myao: Good. It was getting a little chummy around here.
