Short-term treatment of active duodenal ulcer: 20 mg daily for 4-8 weeks.
Short-term treatment of erosive esophagitis or poorly responsive gastroesophageal reflux disease: 20 mg daily for 4-8 weeks.
Pathological hypersecretory conditions including Zollinger-Ellison syndrome, multiple endocrine adenomas, and systemic mastocytosis: Initial dose is 60 mg once a day. Doses up to 120 mg given three times a day have been administered. Some patients have been treated continuously for more than 5 years.
Maintenance therapy of healing erosive esophagitis: 20 mg given once daily.
To maintain the healing of erosive esophagitis and gastric ulcers: 20 mg given once daily.
For the treatment of GERD or other acid-related disorders in pediatric patients over the age of 2 years: For patients <20 kg give 10 mg daily. For patients >20kg give 20 mg daily.
As an adjunct in the treatment of H. pylori infection:
Dual Therapy: Omeprazole 40 mg once daily with clarithromycin 500 mg given 3 times a day for 14 days. If an ulcer was present at initiation of treatment, follow with omeprazole 20 mg once a day for 14 days.
Triple Therapy: Omeprazole 20 mg with clarithromycin 500 mg plus amoxicillin 1000 mg given twice daily for 10 days. If an ulcer was present at initiation of therapy, follow with omeprazole 20 mg once a day for 18 days.
Monitor: BMD, Mg
Pharmacology/Pharmacokinetics: Omeprazole is a proton-pump inhibitor (PPI) that suppresses the H+/K+ ATPase enzyme system at the secretory surface of the gastric parietal cell. The enzyme system is involved in the movement and production of gastric acid. Omeprazole is acid-labile and is formulated as enteric-coated granules. Onset after oral administration occurs within 1 hour and reached a maximum at 2 hours. The binding to the (H+/K+)-ATPase results in antisecretory activity that lasts for more than 72 hours.
Drug Interactions: Increases plasma levels of diazepam, phenytoin, and warfarin by inhibiting oxidative metabolism. May also interact with other drugs metabolized via the cytochrome P450 system (cyclosporine, disulfiram, benzodiazepine). Since the pharmacological effect of omeprazole increases gastric pH, absorption of drugs in which bioavailability is determined by gastric pH may be changed. This would include ketoconazole, digoxin, cyanocobalamin, ampicillin esters, and iron salts. Antacids may be used concomitantly. Voriconazole may increase plasma levels. Omeprazole inhibits the metabolism of cilostazol. Omeprazole prevents the conversion of clopidogrel to its active metabolite.
Contraindications/Precautions: Animal studies have shown a dose-related increased risk for gastric carcinoid tumors with long-term use. The levels of drug needed to produce tumors far exceeded the normal daily dose (up to 352 times the human dose). Pregnancy Category C.
Adverse Effects: Side effects are rare but may include back pain, cough, constipation, rash, and dizziness.
Patient Consultation:
Take 30 minutes prior to eating.
Antacids may be taken at the same time.
Store in a cool, dry place away from sunlight and children.
If a dose is missed, take it as soon as possible. If it is closer to the time of your next dose than the dose you missed, skip the missed dose and return to your dosing schedule. Do not double doses.
Contact a physician if the above side effects are severe or persistent.
For patients who have difficulty swallowing capsules, the contents of a capsule can be added to one tablespoon of applesauce, swallowed immediately and followed by a glass of cool water. The contents of the capsule should not be crushed or chewed.

No matter the end game on that insurance companies and doctors, rightfully so, do not generally support prescriptions of more than 30-days duration. So now people are looking at alternatives and one that is getting a lot of attention is antibiotics for fish as a possible substitute since they can be close to those used in humans and they are NOT regulated, they can be bought on Amazon or at pet stores over the counter. 
Some doctors will prescribe antibiotics for survival storage, but not very many as their abuse can lead to their ineffectiveness, your immune system can work against you. So is has been rumored that many preppers explore fish antibiotics as an alternative. They’re commonly sold in human doses and available without a prescription.

Price fluctuations and availability in the market or in stores are some of the challenges that one could encounter when going for antibiotics. These challenges are caused by demand for these drugs; you can imagine when humans start requesting fish antibiotics, there would definitely be an increased demand which has a direct effect on the price. Another case maybe that the pharmaceutical company may consider it pertinent to withdraw a particular brand of antibiotic from the market completely and replace it with a better and more effective one. Often times antibiotic removal is related to profits of the pharmaceutical manufacturers no matter the viability of the medication. For example, if an antibiotic is not selling well in the market there is a high probability that its production will be reduced or stopped entirely.
Where are my reliable and available sources?  
Aside from the fluctuation in the availability of these drugs, again probably caused by their increasing demand and to a lesser extent, manufacturers profit issues as earlier discussed above, I don’t have a recommended source.  Although Amazon would could guarantee an easy purchase of fish medications.
In conclusion, fish antibiotics in recent times have filled in seamlessly for its human equivalent. Consequently, it appears to be an essential for your survival kit, although like I mentioned in the guide, requires confirmation from some qualified medical personnel which of course is important to avoid drug abuse related issues. So, I will advise that you get one before buying a can of fish antibiotics.

Ingredient: Lansoprazole.
Dosage Form*:
Delayed-Release Capsule: 15 mg.
*Also available in 30 mg capsules and 15 mg and 30 mg orally dissolvable tablets by prescription only.
FDA Indication/Dosages:
For the treatment of frequent heartburn which occurs two or more days a week in adults over the age of 18 years: One capsule taken 30 minutes before breakfast every day for 14 consecutive days. A 14-day course may be repeated once every 4 months. It may take 1-4 days for full effect. Do not chew or crush the tablets.
Pharmacology: Lansoprazole inhibits the H+/K+ ATPase enzyme system at the secretory surface of the gastric parietal cell. This enzyme system is involved in the movement and production of gastric acid. Because this enzyme system is regarded as the acid (proton) pump within the parietal cell, lansoprazole has been characterized as a gastric acid-pump inhibitor, in that it blocks the final step of acid production. This effect is dose-related and leads inhibition of both basal and stimulated gastric acid secretion irrespective of the stimulus. The absorption of lansoprazole is rapid with peak levels occurring within 1.7 hours and having a half-life of approximately 1.5 hours. Absorption rate and amount is reduced by as much as 50% if the drug is given 30 minutes after food as opposed to the fasting condition. Lansoprazole is thought to be transformed into two active species which inhibit acid secretion within the parietal cell canaliculus but are not present in the systemic circulation. Therefore, although the plasma half-life is less than 2 hours the acid inhibitory effect lasts more than 24 hours.
Evaluation: Acid indigestion or heartburn is usually described as a burning or sourness in an area posterior to the sternum. It is caused by a dysfunction of the lower esophageal sphincter which allows gastric contents to flow upward into the esophagus. It is usually preceded by overeating, eating acidic foods, alcohol ingestion, or stress. The patient should be advised to avoid the causes of the indigestion. Frequent or chronic complaints may be a sign of a more serious upper GI disease and the patient should be referred to a physician.
Contraindications/Precautions: Prevacid is contraindicated in patients with known hypersensitivity to any component of the formulation. Symptomatic response to therapy with lansoprazole does not preclude the presence of gastric malignancy. The long-lasting inhibition of gastric acid secretion may interfere with the absorption of drugs where gastric pH is an important determinant of bioavailability (e.g., ketoconazole, ampicillin esters, iron salts, digoxin, atazanavir). Patients may require titration of their theophylline dosage when lansoprazole is started or stopped to ensure clinically effective blood levels. May inhibit the metabolism of tacrolimus.
Adverse Effects: May cause diarrhea, abdominal pain, nausea, and headache.
Patient Consultation:
Take 30 minutes prior to eating.
Antacids may be taken at the same time.
Consult a physician if you have had heartburn for over 3 months.
Consult a physician if you have heartburn with lightheadedness, sweating, dizziness, chest or shoulder pain with shortness of breath, pain in the arms or neck, or frequent wheezing.
Do not use for more than 14 days or more frequently than once every4 months unless directed to do so by a physician.
Do not use if you have trouble swallowing food, are vomiting blood, or have black or bloody stools.
