Inflammatory Bowel Disease (IBD), which includes conditions like Crohn’s disease and ulcerative colitis, can affect the digestive system in multiple ways, causing symptoms like chronic diarrhea, abdominal pain, and constipation. While diarrhea is the more typically known issue in IBD, constipation can also become a major concern for many patients. When constipation becomes severe and unresponsive to medication or lifestyle changes, surgery may be considered. Here, Dr. Samrat Jankar, a renowned gastroenterologist & colorectal surgeon in Pune, explains when surgical intervention is considered for constipation in IBD patients.

POEM Procedure for Six-Year Battle Against Achalasia Ends in Triumph
In a breakthrough procedure, PACE Hospitals’ Gastroenterology team successfully performed a POEM (Peroral endoscopic myotomy) surgery on a 46-year-old male patient with Achalasia, significantly improving swallowing function and quality of life.
Chief complaints
A 46-year-old male patient, presented to PACE Hospitals, Hitech City, Hyderabad with a long-standing history of progressive difficulty swallowing both solids and liquids since 2018. Additionally, he had been suffering from a nocturnal cough for the past two weeks, prompting him to seek expert care.
Medical History
Upon closer examination, it was revealed that the patient had been struggling with difficulty swallowing both solids and liquids for several years, and over time, this problem had worsened. He also experienced frequent regurgitation, which affected his daily life and prompted his admission to PACE Hospitals for further treatment.
Diagnosis
After a detailed clinical evaluation and diagnostic workup, including upper gastrointestinal endoscopy and esophageal manometry, the diagnosis of Achalasia Cardia Type II was confirmed.
Achalasia Cardia is a motility disorder of the esophagus, characterized by the failure of the lower esophageal sphincter to relax properly, leading to difficulty in swallowing. In this particular case, the condition was classified as Type II, as per Eckardt’s score, which rated his symptoms as moderate. This condition severely impacted his ability to consume both liquids and solids, significantly reducing his quality of life.

Achalasia cardia is a rare disease in which there is difficulty in swallowing both liquids and solids and the swallowed food is prevented from passing from the esophagus to the stomach.
Cause of this disease: This occurs due to nerve damage in the esophagus, resulting in the esophagus becoming damaged, dilated, its ability to contract and expand is lost, and at the same time the LOS (gateway at the lower part of the esophagus) is narrowed and the food cannot reach the stomach. In this, food accumulates in the esophagus, sometimes coming up into the mouth and feeling bitter in the mouth. Some people mistake it for GERD.
In achalasia, swallowed food comes up from the esophagus, whereas in GERD it comes from the stomach. There is no complete cure for achalasia. Once the esophagus is damaged, the muscle can no longer function properly. However, the symptoms can usually be controlled with the help of medication, endoscopic intervention, and surgery.
Signs and symptoms of achalasia: The signs and symptoms of this disease appear gradually and over time, permanent disability occurs — 1) Inability to swallow food as if food or drink is stuck in the throat; 2) Regurgitation of food or saliva; 3) Heartburn; 4) Belching; 5) Chest pain that comes and goes; 6) Dry cough at night; 7) Pneumonia (food regurgitates into the lungs and causes infection) 8) Frequent vomiting; 9) Weight loss.
Causes of achalasia: The exact cause has not yet been determined. Researchers believe that the main cause is nerve damage in the esophagus. Some have attributed it to viral infections and autoimmunity. Others have also attributed it to hereditary birth defects or some other unknown viral infection.
Diagnosis of achalasia: This disease is often not detected or misdiagnosed in routine examinations because its signs and symptoms are similar to those of other diseases. The following tests are used to diagnose this disease— 1) Esophageal manometry: This test measures whether the esophageal muscle's ability to contract and expand rhythmically to move food from the throat to the stomach is intact; 2) Barium swallow X-ray: After the patient is given a chalky white drink, the contraction and expansion of the esophagus and the opening or narrowing of the LOS or gateway in the lower part of the esophagus are seen. 3) Endoscopy: This test shows the dilation of the esophagus, the food accumulated in the esophagus, and the tightness of the LOS or gateway in the lower part.
Treatment of achalasia: The main goal of this treatment is to widen the narrowed LOS or gateway in the lower part of the esophagus so that food and drinks can easily pass from the esophagus to the stomach without obstruction.
The appropriate treatment depends on the patient's age, physical health condition and the severity of the disease. 1) Treatment through medication: Calcium channel blockers or Nitroglycerin-like medications; 2) Treatment through endoscopy: Botulinum toxin is applied through endoscopy; The narrowed LOS is widened by inflating a balloon through endoscopy; POEM (Peroral Endoscpic Myotomy) is a small surgery through endoscopy to widen the LOS; 3) Surgical treatment: Laparoscopic surgery is performed by Heller Myotomy or Fundoplication.
Above all, through lifestyle changes — Take time, slowly, and develop the habit of chewing and swallowing food well; Drink plenty of water with meals; Make it a habit to go to bed at least 3 hours after eating; Keep the head of the bed elevated or use two pillows instead of one.
To prevent acid reflux from the stomach after balloon dilation or surgery, the following foods should be avoided — Sour fruits, tomato ketchup, chocolate, alcohol, coffee, etc.; If you can take PPI drugs along with it, you can get the desired results.

On this Page, Dr. Arun S Nair is Explaining about the Achalasia Cardia Disease.
What is Achalasia Cardia?
Achalasia cardia is a rare complaint of the esophagus, the pipe that carries food from the mouth to the stomach. It occurs when the lower esophageal sphincter( LES) fails to relax properly, causing difficulty in swallowing, regurgitation of food, and sometimes casket pain. This condition can lead to significant discomfort and nutritive issues if left untreated.
Symptoms of Achalasia Cardia:
Symptoms of achalasia cardia can vary but often include:
Difficulty swallowing (dysphagia) for both solids and liquids
Regurgitation of undigested food
Chest pain, which may increase after eating
Weight loss due to difficulty in eating
Heartburn or acid reflux
Persistent cough, especially at night
The sensation of food being stuck in the chest
If you are expecting any of these symptoms it is important to consult an achalasia-cardia specialist, Dr.Arun Nair also a laparoscopic surgeon in Thrissur to manage this and get effective treatment
Diagnosing Achalasia Cardia by Dr. Arun Nair:
Patient history: Dr. Nair will talk over the patient’s symptoms, medical history, and any former treatments they may have experienced for their symptoms.
Physical examination: Dr. Arun Nair will perform a physical examination to assess the patient’s overall health and look for any signs that may suggest achalasia cardia.
Barium swallow test: This imaging test involves swallowing a barium solution that fleeces the esophagus and allows for X-ray visualization of the esophagus. This can help identify any blockages or abnormalities in the esophagus that may be reflective of achalasia cardia.
Esophageal manometry: This test measures the muscle condensation in the esophagus and how well the lower esophageal sphincter( LES) is performing. Patients with achalasia cardia usually have weak or missing peristalsis( muscle condensation) in their esophagus and a dysfunctional LES.
Endoscopy: Dr. Nair may perform an upper endoscopy to directly visualize the inside of the esophagus and look for any abnormalities or confirmation of achalasia cardia, similar to a narrowing of the lower esophagus or pooling of food.
CT scan or MRI: In some cases, Dr. Arun Nair may order imaging tests like a CT checkup or MRI to further estimate the esophagus and surrounding structures for any signs of achalasia cardia.
Once a diagnosis of achalasia cardia is verified, Dr. Arun S. Nair a famed gastro surgeon in Thrissur, will work with the patient to develop an individualized treatment plan to manage their symptoms and enhance their quality of life.

Rare Disease Day 28 Feb 2025 – Theme and Importance
Rare Disease Day is a worldwide healthcare event, typically observed either on the last day of February or near the last day of February each year, intending to bring together the patients suffering from rare diseases and create a community in which awareness of their rarity profile, their condition, their diagnosis and their treatment are discussed. This year 2025, World Rare Disease Day is observed on 28 February, Friday.
According to the World Health Organization (WHO), the basis of a rare disease can be identified with its epidemiology. The frequency of any rare disease should be less than 6.5-10 per 10,000 people.
An Indian non-profit company, Organization for Rare Diseases India (ORDI), is set up to provide a collective voice for all Indian patients with rare diseases. Given India's enormous population, ORDI considers an illness rare if it affects one in every 5,000 Indians or less. ORDI has listed 263 rare diseases in India. The first 10 of them are:
Acanthocytosis Chorea - A neurological disorder that affects movement in many parts of the body).
Achalasia Cardia - A rare disorder that makes it difficult for food and liquid to pass from the swallowing tube connecting your mouth and stomach (oesophagus).
Acromesomelic Dysplasia – An inherited, skeletal disorder of short stature known as short-limb dwarfism.
Acute Inflammatory Demyelinating Polyneuropathy (AIDP) – A general classification of pathologies affecting the nervous system.
Acute Lymphoblastic Leukaemia – Blood cancer, especially the white blood cells.
Addison's Disease (adrenal insufficiency) - The adrenal glands in the body don't make enough of certain cortisol and aldosterone hormones.
Adrenoleukodystrophy (ALD) - A genetic condition damaging the myelin sheath (membrane) that covers nerve cells in the spinal cord and brain.
Acute inflammatory demyelinating polyneuropathy - An autoimmune disease of the peripheral nervous system, recognised over a century ago.
Alagille syndrome - An inherited condition of bile that builds up in the liver.
Alkaptonuria (black urine disease) - an inherited condition of impaired protein breakdown resulting in the build-up of homogentisic acid.
