Unraveling the Enigma: Exploring the Causes of Infertility in Men and Women
Infertility, a condition that affects millions of couples worldwide, can be a source of immense emotional distress and frustration. While there are numerous factors that can contribute to infertility, understanding its underlying causes is essential for effective diagnosis and treatment. In this article, we delve into the multifaceted causes of infertility in both men and women, shedding light on the complex interplay of biological, environmental, and lifestyle factors.
Causes of Infertility in Women
Ovulation Disorders: Irregular or absent ovulation can hinder conception. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, and premature ovarian insufficiency (POI) can disrupt the ovulation process.
Fallopian Tube Damage: Blockages or damage to the fallopian tubes can prevent the egg from reaching the uterus for fertilization. Previous pelvic infections, endometriosis, or surgery may contribute to fallopian tube issues.
Uterine Abnormalities: Structural abnormalities in the uterus, such as fibroids or polyps, can interfere with embryo implantation and development, leading to infertility.
Age-related Factors: As women age, the quantity and quality of their eggs decline, making conception more challenging. Advanced maternal age is a significant risk factor for infertility.
Causes of Infertility in Men
Low Sperm Count or Quality: Issues with sperm production, motility, or morphology can impair fertility. Factors such as hormonal imbalances, genetic conditions, and lifestyle choices (e.g., smoking, excessive alcohol consumption) can affect sperm health.
Varicocele: A varicocele, an enlargement of the veins within the scrotum, can lead to decreased sperm production and quality. It is a common reversible cause of male infertility.
Testicular Factors: Conditions such as undescended testicles, testicular trauma, or infections can impact sperm production and function, contributing to infertility.
Ejaculatory Disorders: Disorders affecting the ejaculation process, such as retrograde ejaculation or erectile dysfunction, can hinder the delivery of sperm during intercourse.
Seeking Help from a Male Fertility Doctor
For couples struggling with infertility, consulting a male fertility doctor, also known as a reproductive urologist or andrologist, can provide valuable insights and guidance. These specialists are trained to evaluate and treat male infertility issues, offering diagnostic tests, fertility evaluations, and personalized treatment plans to address underlying causes and improve reproductive outcomes.
Conclusion
Infertility can stem from a myriad of factors affecting both men and women. By understanding the potential causes of infertility and seeking specialized care from a male fertility doctor specialist, couples can embark on a journey towards achieving their dream of parenthood. With advancements in reproductive medicine and personalized treatment approaches, there is hope for overcoming infertility challenges and building a family.

I had the delight of interviewing a cancer researcher on stream as a part of my fundraising for Fred Hutch Cancer Center, and now it's up as a VOD!
Holly Harris is an epidemiologist at Fred Hutch who studies the impact of lifestyle, nutrition and genetic factors on women’s health. She focuses specifically on ovarian and breast cancer, as well as the hormonally related conditions that share reproductive risk factors with these diseases such as endometriosis and uterine fibroids. We spoke a lot about ovarian cancer in particular, as that's the cancer my mom had and also one that so few people know about and an area we need a lot more research in.
Until Dec. 14th, I'm raising funds for ovarian cancer research at Fred Hutch! Holly and I chatted while playing Animal Crossing, but otherwise I stream a lot of Splatoon and will be doing so for the next week.

PAP is screening only, not indicated in work up of post-menopausal bleeding
General EMStripe <4 is <1% for endometrial cancer
>4mm then EMBiopsy is indicated
Polps with cancer can be missed on EMB
Therefore remove since you can't guarantee it's not cancer
Other special cases
Tamoxifen (1% increased cancer risk). Uterus looks odd on US, need to do a sonohystogram and Hysteroscopy
Recurrent episodes -> Go to biopsy
Risk factors (Consider biopsy)
Risk Factors for early menopause
Smoking #1 risk factor for early menopause
Null parity
Family history
Chemotherapy
Causes of PMB - > generally do the full work up, assess for cancer, don't stop at vaginal atrophy
Uterine Atrophy
Very thin lining, can be unstable (lack of estrogen)
Exacerbated by blood thinners
Most common etiology with "negative" work up
Uterine Atrophy
Loss of rugation, frail skin, dysparenuia, speckled (bursting of blood vessels)
Consider
Uterine track atrophy
Urethral Caruncles, diverticulae
Tx vaginal estrogen (esterase cream preferred), capsules, rings (lasts 90 days, not great if sexually active, but good if already using a Pessary and/or have recurrent UTIs)
4mg imvexxy (Lowest estrogen dose)
Nightly for 2 weeks, then night twice a week
Assess at 2-3 months
Endometrial polyps
Proliferative
Carcinoma/Hyperplasia
Risk- Increasing age, nulliparity, obesity, increased estrogen exposure, tamoxifen
EIN -> Send to GYN/Onc for biopsy on follow up
Simple Hyperplasia
Stop estrogen, weight loss (obesity),
Pre-cancerous
Treat with local progesterone therapy (Merina IUD) /or monitor /or biopsy
"you don't have to do anything, but it sucks to watch someone get cancer"
Fibroid
3-5% chance of malignancy in the post-menopausal women
If US negative, get saline infusion sonohysterogram (In office), saves the women from Hysteroscopy (In OR)(Go right to this if concern for cancer)
Other
HRT
Pessary
Injury
Prolapse/ulceration
Silver nitrate, estrogen, pessary (or surgery)
Non-GYN Causes of PMB

Laparoscopy for Infertility: A Minimally Invasive Approach to Improving Reproductive Health
![Tumblr media](https://64.media.tumblr.com/67936c40bdc15d5e1f661a2d929aa47a/59aaa7935a0cd50b-5e/s540x810/65e471f514c9a0086ff444019841faa84d044618.jpg)
Infertility is defined as the inability to conceive after one year of regular, unprotected intercourse. Both men and women can face infertility due to various factors, including tubal infections, ovulation disorders, endometriosis, and male-factor infertility. The World Health Organization (WHO) identifies the leading causes of infertility as malnutrition, pelvic tuberculosis, and recurrent infections that cause blockages in the fallopian tubes.
Laparoscopy involves using a laparoscope—a small camera inserted through a small incision—to examine the reproductive organs, allowing doctors to identify abnormalities and determine the best course of action for treatment. One of the prominent reasons for its growing popularity is its minimally invasive nature, which offers a quicker recovery time compared to traditional surgery. Many patients with conditions like endometriosis, tubal blockages, and fibroids have seen significant improvements after undergoing laparoscopic procedures. This method has revolutionized the way Miart specialists approach infertility treatment, combining precision with minimal disruption to the body.
What Causes Infertility?
Infertility can result from a variety of factors:
Ovulation Disorders: Conditions like polycystic ovary syndrome (PCOS), thyroid disorders, and premature ovarian failure can prevent proper egg release.
Scarring From Surgery: Pelvic or cervical surgeries may lead to scarring in the reproductive organs.
Fibroids: Non-cancerous growths in the uterus can block fallopian tubes or hinder the attachment of a fertilized egg.
Endometriosis: When tissue similar to the uterine lining grows outside the uterus, it can cause pain and fertility issues.
Pelvic Inflammatory Disease (PID): A bacterial infection that can damage the fallopian tubes and other parts of the reproductive system.
For men, infertility may stem from poor sperm quality, testicular issues, or ejaculation disorders.
How Laparoscopy Helps Diagnose and Treat Infertility
Laparoscopy is a minimally invasive procedure that allows doctors to view the internal reproductive organs in detail. The procedure is particularly useful in identifying and treating conditions such as:
Blocked fallopian tubes: The laparoscope helps identify and correct blockages or scarring in the fallopian tubes, which can improve fertility.
Fibroids and endometriosis: Laparoscopy can identify these conditions more accurately than other diagnostic methods, allowing for their removal during the procedure.
Pelvic adhesions: Laparoscopy helps detect adhesions that may be hindering fertility, and the surgeon can remove them to enhance conception chances.
Laparoscopy is typically used when other methods, such as imaging or ovulation testing, do not provide a clear diagnosis or if unexplained infertility is suspected.
The Laparoscopy Procedure
Before the procedure, patients must fast for 8 hours. Under general anesthesia, the surgeon makes a small incision near the navel and inserts a laparoscope to view the reproductive organs. Carbon dioxide is pumped into the abdomen to elevate the abdominal wall for a clearer view. The laparoscope, which has a camera, transmits magnified images of the uterus, ovaries, and fallopian tubes to a monitor, helping the doctor identify abnormalities.
The surgeon may inject a dye to check for blockages in the fallopian tubes, and if necessary, perform corrective surgery during the procedure. Afterward, the surgeon removes the instruments, releases the carbon dioxide, and closes the incisions.
Benefits and Risks of Laparoscopy
Benefits:
Minimally invasive: Laparoscopy involves small incisions, leading to less pain, a quicker recovery, and minimal blood loss.
Improved fertility: Conditions like fibroids, endometriosis, and blocked tubes can be treated during the procedure, potentially improving chances of conception.
Quick recovery: Patients typically recover within a day, and most can return to normal activities within a week.
Risks: Though laparoscopies are generally safe, there are risks associated with any surgery, including bleeding, infection, and damage to nearby organs. In rare cases, laparoscopy can lead to complications like blood clots, abdominal hernias, or reactions to anesthesia.
Diagnosis and Future Fertility
Laparoscopy helps doctors accurately diagnose the cause of infertility, whether it's due to blocked fallopian tubes, fibroids, endometriosis, or other conditions. Once the issue is identified, treatments can be performed during the procedure, and the patient can try to conceive naturally afterward.
If conditions like PCOS or infections cause fertility issues , additional treatments like hormonal therapy or antibiotics may be needed. In cases of severely damaged fallopian tubes, in vitro fertilization (IVF) might be recommended.
Conclusion
Laparoscopy is a highly effective procedure for diagnosing and treating infertility. By providing a clear view of the reproductive organs, it allows doctors to detect and treat conditions that may be preventing conception. While the procedure carries some risks, the benefits, including improved fertility and faster recovery, make it a valuable option for couples struggling with infertility.
If you're dealing with unexplained infertility or conditions like fibroids or endometriosis, consult your doctor to see if laparoscopy could be the solution. For expert care, Infertility treatment in Calicut such as Miart specialists offers advanced laparoscopic techniques to improve your chances of parenthood.

Why Does IVF Fail? Reasons and Solutions
In vitro fertilization (IVF) has helped millions of couples worldwide achieve parenthood. However, despite advancements in reproductive technology, IVF success is not guaranteed. Understanding the reasons behind IVF failure and potential solutions can help patients make informed decisions and improve their chances of success.
Common Reasons for IVF Failure
1. Poor Embryo Quality
One of the primary reasons for IVF failure is poor embryo quality. Embryos may fail to implant due to genetic abnormalities, which can result in failed implantation or early miscarriage.
Solution: Preimplantation Genetic Testing (PGT) can help identify chromosomally normal embryos, increasing the chances of a successful pregnancy.
2. Advanced Maternal Age
As women age, their ovarian reserve and egg quality decline, reducing the chances of a successful IVF cycle. Women above the age of 35 have a higher risk of IVF failure due to poor egg quality.
Solution: Consulting an experienced IVF specialist can help determine the best fertility treatment options. Some patients may benefit from donor eggs to improve success rates.
3. Uterine Issues
A healthy uterine lining is crucial for embryo implantation. Conditions such as fibroids, endometrial polyps, or thin endometrial lining can prevent implantation, leading to IVF failure.
Solution: A thorough uterine evaluation, including hysteroscopy and ultrasound, can help detect and treat uterine abnormalities before IVF.
4. Lifestyle Factors
Smoking, excessive alcohol consumption, obesity, and stress can negatively impact fertility and IVF outcomes.
Solution: Adopting a healthy lifestyle, maintaining an optimal weight, and managing stress through meditation or counseling can improve IVF success.
5. Male Factor Infertility
Poor sperm quality, low sperm count, or DNA fragmentation in sperm can contribute to IVF failure.
Solution: Advanced techniques such as Intracytoplasmic Sperm Injection (ICSI) or surgical sperm retrieval can help overcome male infertility issues.
6. Implantation Failure
Even when embryos are of good quality, implantation failure may occur due to immune system issues or an unreceptive endometrium.
Solution: Endometrial receptivity tests and immune system evaluations can help identify and treat underlying problems before embryo transfer.
How to Improve IVF Success Rates
Choose a Reputable IVF Centre: Seeking treatment at a well-established IVF centre in Delhi, such as SCI IVF Hospital, can provide access to advanced fertility treatments and experienced specialists.
Optimize Ovarian Stimulation: Customizing medication protocols based on individual hormonal responses can improve egg quality and quantity.
Consider Assisted Hatching: This technique helps embryos hatch and implant more effectively in cases of previous failed IVF cycles.
Use Frozen Embryo Transfer (FET): FET cycles allow better endometrial preparation, improving implantation rates.
Seek Expert Consultation: Consulting an experienced IVF specialist ensures personalized treatment plans tailored to individual fertility concerns.
Conclusion
IVF failure can be emotionally and physically challenging, but understanding the reasons behind it can help couples make informed choices. Seeking treatment at a leading IVF centre in Delhi, such as SCI IVF Hospital, and following expert advice can significantly enhance the chances of achieving a successful pregnancy. By addressing potential issues and making necessary lifestyle changes, couples can improve their IVF success rates and fulfill their dream of parenthood.

Fibroids in Women: Symptoms, Risks, and Treatment Options
What Are Fibroids?
Fibroids are non-cancerous growths that develop in or around the uterus. They are also called uterine fibroids and are common in women of reproductive age. These growths vary; some are as small as a seed, while others can become large enough to cause discomfort. Although they are not life-threatening, they can lead to serious health problems if left untreated.
Symptoms of Fibroids
Many women with fibroids do not experience symptoms. However, when symptoms appear, they may include:
Heavy or prolonged periods
Pelvic pain or pressure
Frequent urination (fibroids can press against the bladder)
Pain during intercourse
Lower back pain
Bloating or swelling in the lower abdomen
Difficulty getting pregnant
If you experience any of these symptoms, consulting a gynaecologist is vital for proper diagnosis and treatment.
What Causes Fibroids?
The exact cause of fibroids in women is unknown, but several factors may contribute:
Hormones: Estrogen and progesterone help fibroids grow. These hormones are higher during reproductive years, so fibroids are common in women aged 25-50.
Genetics: If your mother or sister had fibroids, you are more likely to develop them.
Pregnancy: Increased hormone levels during pregnancy can cause fibroids to grow faster.
Lifestyle Factors: Obesity, poor diet, and lack of exercise can increase the risk.
Are Fibroids Dangerous?
Most fibroids are harmless and do not require treatment. However, in some cases, they can lead to complications such as:
Severe anemia: Due to heavy periods
Infertility: Large fibroids can block the fallopian tubes
Pregnancy complications: Fibroids can lead to miscarriage or early labour
This is why it is essential to monitor fibroids with regular check-ups.
How Are Fibroids Diagnosed?
A gynaecologist can diagnose fibroids through:
Pelvic Examination: To check for an enlarged uterus
Ultrasound: To detect the size and location of fibroids
MRI or CT Scan: For detailed imaging
Treatment Options for Fibroids
Treatment depends on the size, number, and symptoms of fibroids. Some common treatments include:
1. Medications
Hormonal birth control to regulate periods
Iron supplements for anemia
Pain relievers for cramps
2. Non-Surgical Procedures
Uterine Fibroid Embolization (UFE): Shrinks fibroids by cutting off their blood supply
MRI-Guided Focused Ultrasound: Uses heat to destroy fibroids
3. Surgical Options
Myomectomy: Removes fibroids while keeping the uterus intact
Hysterectomy: Complete removal of the uterus (for severe cases)
When Should You See a Gynaecologist?
If you have
Heavy or painful periods
Difficulty getting pregnant
Constant pelvic pain or pressure
A family history of fibroids
Seeking expert care early can prevent complications and improve your health.

Common Treatments for Uterine Fibroids
![Tumblr media](https://64.media.tumblr.com/257fcc3bf82c676e33d4d5946f151356/60a68e5f16ff3e9b-2d/s540x810/7f2c2a14c6303e467b8eaf8d7ae8fffa794f0cb5.jpg)
Uterine fibroids (also known as uterine leiomyomas) are benign muscular tumors that grow in and throughout the uterus. Extremely common, they affect roughly 40 percent to 80 percent of women by age 50. They may occur in isolation or more commonly in multiples, and can grow from the size of a pea to the size of a watermelon and as such may be seen to bulge from the outside or inside of the reproductive organ.
Among the common symptoms provoked are bloating, pelvic or back pain or pressure, heavy menstrual bleeding, increased urinary frequency or urgency, constipation, infertility or complications with pregnancy. Though fibroids are routinely not cancerous, physicians often find that they can look similar to rare cancerous tumors of the uterine muscle, known as a leiomyosarcoma.
While the causes of fibroids are not known, they appear related to progesterone and estrogen, which are female-specific hormones. Among the potential risk factors are family history which relates to the underlying genetic predisposition, number of pregnancies, lifestyle, and age of first menstruation.
The most common way of ascertaining their presence is through ultrasound. The physician employs sound waves in creating an image of the uterus, which provides information on the size, number, and positioning of fibroids, as well as their vasculature. MRI is applied in monitoring sarcomatous changes and fibroid degeneration.
When fibroids are small or slow-growing, or when there are no symptoms, physicians recommend an expectant management approach, which involves monitoring them through ultrasound annually. Nonsteroidal anti-inflammatory drugs such as ibuprofen (Advil, Motrin) and naproxen (Aleve, Naprosyn) have marginal effectiveness in managing the often painful periods associated with fibroids. Many women fibroids can develop iron deficiency anemia due to heavy menstrual periods, which is also known as menorrhagia. As such, these patients may take iron supplements in combination with multivitamins, which also helps them to process the iron.
Hormonal birth control is another noninvasive supportive treatment pathway, with options ranging from contraceptive pills or implants. Such aids work to reduce the symptoms of menorrhagia, or abnormal uterine bleeding. This approach requires up to three months to take effect.
MRI-guided focused ultrasound is a relatively new procedure. Precisely targeting tissues, the physician focuses ultrasonic energy at fibroid focal points. This denatures the cell proteins, resulting in cellular death. Follow-up studies at six months may indicate a decrease in fibroid volume of up to one-third.
Uterine artery embolization (UAE) involves inserting a tiny catheter into a blood vessel within the inner thigh. The hollow catheter is guided to the uterine blood vessels, into which minute particles are injected into the blood supply supporting each of the fibroids. This radiological approach serves to halt the blood flow to the entire fibroid network, resulting in their death and shrinkage. A minimally invasive, next-generation tool specifically performed by an interventional radiologist, UAE presents the benefit of relatively short procedural length, rapid patient recovery time and minimal to no scarring. The procedure addresses persistent issues such as pelvic pressure and pain, menstrual bleeding, urinary symptoms, and dysmenorrhea (menstrual pain).
The two main surgical approaches are hysterectomy and myomectomy. With myomectomy, the physician aims to surgically remove fibroids lodged in the uterine wall, while preserving the structural integrity the patient’s womb. One goal of the procedure is to help boost the chances of reproductive success in situations where the fibroids may be distorting the uterine cavity, compressing the fallopian tube, or causing recurrent miscarriages. Myomectomy may be performed using the aid of laparoscopy, laparotomy, and hysteroscopy.
By contrast, hysterectomy involves surgical removal of the entire uterus, with the cervix and ovaries typically left in place. This presents the best outcome when it comes to reducing symptoms such as heavy menstrual bleeding and preventing fibroid recurrence but at a significant cost to the patient. On one hand, a hysterectomy is by definition notably invasive and prevents future pregnancies, essentially sterilizing the patient. The three main hysterectomy pathways are abdominal (laparotomy), laparoscopic (often using a surgical robot), and transvaginal surgery. While physicians often prefer the latter route, this may be impossible when large fibroids are present.
Identifying the right treatment for uterine fibroids requires extensive diagnostics and close consultation with a highly qualified physician or medical provider such as an interventional radiologist and gynecologic surgeon.

The Role of Laparoscopy in IVF: How Endoscopic Surgery Can Improve Fertility Treatment
![Tumblr media](https://64.media.tumblr.com/e3a1498eea4047c96f4493af74b0e9d2/f55d21c0365ffa0f-39/s540x810/636f4cc39ec49164a5da0b87e4ed74d5cf08ba86.jpg)
Infertility is defined as a condition where a couple fails to conceive a baby by natural means. This is caused by several factors in both males and females, including age, hormonal imbalances, infections, etc. If you are suffering from this condition you need to visit the clinic of IVF & Endoscopy specialist in Siliguri. The doctor uses endoscopy to diagnose infertility issues and to improve the chances of a successful IVF cycle.
If you have unexplained infertility, the doctor may recommend a laparoscopy to check the problems in the reproductive organs. Laparoscopy plays a major role in IVF treatment by allowing the doctor to examine the pelvic organs. This procedure improves fertility through surgery in the uterus, ovaries, pelvic, and fallopian tubes.
The role of endoscopic surgery in fertility treatment:
Accurate diagnoses of reproductive issues:
The doctor uses various methods to diagnose infertility depending on the situation including;
Ovulation testing: During this procedure, the doctor will perform a blood test to check the amount of progesterone hormone produced during ovulation.
Ovarian reserve testing: This procedure and blood tests are done during this process. This test helps to determine the quality and quantity of eggs available for ovulation.
Hysterosalpingography (HSG): This procedure is done by injecting a contrast fluid into your uterus to check the abnormalities.
Laparoscopy: This is a non-invasive procedure in which the doctor uses a laparoscope to examine your internal reproductive organs. The doctor inserts a device through a small incision to view the uterus, ovaries, and fallopian tubes. This procedure helps to diagnose scarring, endometriosis, blockage of tubes, and ovary abnormalities.
Treatment for common infertility issues:
If infertility is caused due to gynaecological issues you can make an appointment with the best infertility doctor in Siliguri. The doctor treats common conditions for common infertility issues such as;
Endometriosis: Endoscopic surgery uses laparoscopy to treat endometriosis. This is a low-risk and minimally invasive procedure which is performed under general anaesthesia by the doctor. This procedure allows the doctor to identify and remove endometrial tissue growth in the pelvic area.
Tubal blockage: Endoscopic surgery for tubal blockage uses a laparoscopy to unblock the fallopian tubes. During this procedure, the doctor inserts an instrument through the uterus and cervix into the tube to remove the blockage.
Fibroids: Laparoscopic myomectomy is a minimally invasive procedure that uses laparoscopy to remove uterine fibroids. During this procedure, the doctor makes a small incision in the abdomen and inserts this instrument to see and remove fibroids.
Pelvic adhesions: These conditions are primarily treated with laparoscopic surgery. The primary method of removing pelvic adhesions is by cutting scar tissue. However, in some cases, blunt dissection is done during the surgery to pull these adhesions.
Improved embryo implantation:
Implantation is one of the main procedures used during IVF to achieve pregnancy. This procedure is done by attaching an embryo to the wall of the uterus in the endometrial lining. However, the success rate of successful embryo implantation depends on the quality of an embryo and the receptiveness of the uterus. Numerous factors can affect fertility such as uterine fibroids, polyps, and endometrial lesions. However, with endoscopic surgery, these conditions can be addressed, which improves the chances of successful embryo implantation.
Prevents miscarriage:
Miscarriage in IVF refers to the loss of pregnancy which can occur at any stage after an embryo is implanted in the uterus. This condition can be caused due to various factors like embryo quality, chromosomal abnormalities, and implantation difficulties due to underlying health conditions. This includes polycystic ovary syndrome (PCOS), endometriosis, thyroid disorders, and autoimmune disorders which can increase the risk of miscarriage after IVF. However laparoscopic surgery helps the doctor to identify and treat these issues which helps to boost IVF outcomes.
Faster recovery from surgery:
Endoscopic surgery has several benefits over traditional open surgery. This procedure uses laparoscopy, which is a minimal surgical technique that allows the doctor to examine the abnormalities of the abdomen or pelvis. Some of the benefits of this surgery include less pain, lowering the risk of infection and complications, better cosmetic results due to smaller scars, and ensuring a more successful pregnancy. This allows the patient to recover quickly from this surgery before starting the IVF cycle.
Conclusion:
Endoscopy has become an essential part of modern IVF clinics offering both diagnostic and helping people to understand their issues. This is a minimally invasive surgical procedure which helps to treat gynaecological issues that lead to infertility. For this procedure, you can visit the clinic of IVF & Endoscopy Specialist in Siliguri. By evaluating a patient’s health, this procedure allows the doctor to customize treatment plans effectively which enhances the success rate of achieving pregnancy. This procedure also helps to improve embryo implantation and prevents miscarriage improving fertility treatment and offering hope to couples on their journey to parenthood.
