Setting a New Course of Action in Sobriety
In order to succeed in alcoholism treatment you are going to have to set a new course of action in your life. This means that you need to eliminate certain habits and routines from your existing life and replace those with new, healthier habits.
Often times the outside world (meaning non addicts and non alcoholics) believe that recovery or sobriety simply means not drinking any more, walking away from any drug or alcohol use, and generally being “done” with that life forever–as if it is as simple as flipping on and off a light switch.
The reality is that after the alcoholic stops drinking booze every day, they are left with a disease that still tries to manifest itself in other ways, tripping them up and causing them to relapse. The truth is that any alcoholic could potentially “white knuckle it” and stick it out while dry, but that person will be completely miserable unless they are working some kind of recovery program. They may or may not remain sober, but if all they did was to put down the drugs and the booze, then they are going to be very unhappy until they learn a better way to live their life.
So the process of starting out in recovery really begins with the question: “How exactly should I be living my life?” That is what the recovering alcoholic is really struggling with when they attempt to get clean and sober. Sure, they have to actually stop putting alcohol into their body, and they may do things such as going to inpatient treatment or AA meetings, but they are still going to be facing this question at some point in their journey, which essentially amounts to: “Now that I no longer define my life in terms of getting drunk or high all the time (“partying” if you will), then how exactly do I define myself?”
So in order to set a new course of action in recovery you need to clear out a space for this new action to occur. What this means is that you must let go of the old life before you can start sculpting your new life. I have found that the most reliable way for this to happen for most people is through a trip to inpatient rehab, typically of the 28 day variety. Going to inpatient treatment gives you a chance to “reset” your life so that you are then able to start out on a new course. Those who purposely avoid treatment are going to struggle to be able to turn their life around with any real degree of success. While inpatient rehab is not a sure fire cure, it is definitely the best possible path for most people who are struggling with substance abuse. Inpatient gives you the most advantages compared with most other treatment solutions. This is likely because inpatient treatment encompasses most other treatment solutions such as counseling, therapy, peer support, aftercare services, group therapy, 12 step support, and so on.
So after you have (hopefully) taken the smart advice to attend an inpatient treatment center, your next step is to figure out how to transition out to the “real world” and somehow make your life work now that you are sober. One of the things that you are going to notice very quickly is that your life will seem to revolve around solving problems. During this time of early recovery, you must remind yourself to stay grateful because the set of problems that you are now solving in life have been drastically upgraded.
You used to worry about having enough drugs or booze, avoiding the police while breaking various laws, and generally trying to keep yourself alive in spite of the massive amount of abuse that you are putting your body through due to your addiction. Those were some serious problems and they had real consequences for a lot of struggling addicts and alcoholics. If you are one of the lucky few individuals who somehow made the leap into addiction recovery, then you know that you still have problems in your life today, but those problems are seriously different now. Today you have to worry about a job perhaps, getting to an AA meeting in the evening, and helping out a newcomer with their fourth and fifth step next weekend. You still have problems but they are not nearly as bad as your problems were during active addiction, and that is a huge blessing. You would do well to remember that these upgraded problems are actually a blessing in your life and not a curse.
So in order to set a course of action in recovery you need to first set a direction. That direction should come from a decision that you make, possibly with the help of a therapist or a sponsor in recovery, and that decision should be based on getting the biggest bang for your “personal growth buck” so to speak.
Which is another way of saying that all of us have various problems, hang ups, and defects that we could be working on fixing right now. Whichever thing that you have would provide the most freedom and relief in your life is the thing that you should work on next.
In the very beginning that “thing” was to get clean and sober, to go through detox, and to start living sober. Let’s assume that you have done that, or that you are set to do that. What next?
What happens next is that you start to assess your life. You start to work with recovery coaches, peer support, AA and NA members, sponsors, and therapists in order to figure out what your next change is that you need to make. Look for your “pain points,” try to figure out what is causing the bulk of your anxiety or unhappiness lately, and then zero in on fixing whatever that is.
You may need some advice or guidance in order to determine what your next big priority in recovery should be. Take the time to analyze and determine this with the help of others.
Once you have your next objective, your plan should be to gather as much information as possible in terms of conquering that particular problem, and then testing out advice and strategies that you receive in order to make the necessary changes. You may need to seek advice from your sponsor, your therapist, or from people in AA and NA meetings. You may need to test more than one solution before you stumble on the thing that works best for you. But you persist in this goal and you keep hammering away at it until you fix the issue that is causing you anxiety or grief in your life.
Then you simply reset and it over again–analyze your life and find the biggest pain point, all over again. Then make another plan, gather more advice, find others who have conquered this same problem for themselves, and get to work at it. You should not tolerate having problems, anxiety, or loss of freedom in your life. If you want to set a new course of action then you simply need to make the decision, start asking for help, and set the goal that you want to achieve. You can get through sobriety this way and improve your life a great deal to boot. Good luck!

Good luck!

No matter where you are in the world, there is never enough healthcare to meet demand. There are too many human bodies out there to be maintained, tended to and healed. But we’re living in an age of automation and AI. Couldn’t there be an answer in technology?
Havas LYNX’s Interactive Director Adam Emmott thinks so. “With healthcare systems being pushed to their limits, there is a need to understand and implement how intelligent, AI-driven chatbots could help ease some of the burden and perhaps improve data infrastructures,” he says. “As we move in to new ways of delivering better ‘beyond-the-pill services’ in healthcare, the chatbot is emerging as an interesting opportunity to support.”
With healthcare professionals stretched thin, live face-to-face time with a healthcare professional can be variable. “These live conversations can be poor quality, average quality, or amazing, and the reality is that not enough are amazing,” says Dr John Reeves, Chief Medical Officer at conversationHEALTH, a Toronto-based startup that builds chatbots for healthcare and adjacent sectors. “These one-to-one conversations cannot scale to meet real time customer demand,” he adds.  He believes branded conversational AI can step in to plug this gap. “Chatbots allow brands to capture the very best conversations (and those that are on-brand message) and scale to millions of end customers. And they do so in an on-demand, personalised way.”
You may have already encountered services like Babylon, Sensely and your.md that demonstrate the power of tracking user healthcare data in meaningful ways, but also using that data to make meaningful diagnoses. “It is still early days,” admits Adam, “but with the use of machine learning and huge data lakes of usable medical information being collected, it should be inevitable that we could see much needed support from chatbots in healthcare provision.”
The end game of that thinking is hard for us to countenance in 2018: a world where you can chat to an AI about your symptoms and receive a diagnosis that you can trust.  Adam imagines the power of good conversational AI in the mental health space. “Supported, meaningful, conversation obviously lends itself to provide support in talking therapies. When we are able to create truly sensitive, personal conversations available whenever a user needs it, we could be onto something incredibly powerful. Connect that with deep learning, we begin start something groundbreaking in the understanding of mental health, from provision to diagnosis.”
That’s getting ahead of ourselves though. Chatbots aren’t likely to replace your doctor anytime soon. “There isn’t really a strategy around AI and bots that I’ve seen any health system or pharma company put together,” says Ritesh Patel, Chief Digital Officer for Health and Wellness at Ogilvy. “You’ll see point solutions coming through, not necessarily a strategic view of using this technology to really change the delivery of care model. Entrepreneurs will go after the market faster than health systems will. They’ll see a need, see how much the area is worth and go for it.”
Pharmaceutical brands are taking the chatbot arms race seriously, as Dr John is witnessing. “Compared to the three previous big digital opportunities (web, apps, social), conversational solutions (messaging, chatbots, voice) are gaining traction at a faster rate,” he says. “Pharma sees massive potential in the use of AI across the enterprise, including marketing communications to reimagine the way they engage their customers in a more ‘modern’ way. I mean, texting and talking is how life is lived now. No one wants to be searching or navigating through web sites, articles, brochures, videos to resolve a specific need.” Ritesh observes that there are two arms races among the pharma clients in this space. One of them is the race to be first to market. In this case, being first is valuable, he believes: “You’ll be the first chatbot people use and they’ll remember you’re the one. And when someone else comes along, unless they have something amazingly different or unique to offer you, it’s much less likely you’re going to use it.”
The second arms race is the technology and the ability for image recognition, AI and natural language processing. “I think it’ll drive a lot of these bots to become voice-activated bots,” says Ritesh. “Right now while it’s chat and you’re typing. I think you’re going to see an explosion of voice. Where now you see people saying ‘OK Google’ and ‘Hey Siri’, you may get a doctor saying ‘Hey Pfizer’ or ‘Hey Merck’.”
Chatbots from pharma brands will appear in doctors’ offices before they appear on patients’ phones more generally, Ritesh predicts, and he can imagine how they might make physicians’ lives significantly smoother. “The doctor doesn’t wait until the pharma rep arrives at 2pm on a Thursday to hold all their questions. The brand’s website is highly complex and tough to navigate to get the content you need. So if I’m doing my rounds and I need a bit more information about the drug I’m going to prescribe to you, I’ve got two ways I could do that - look it up on the website of the pharma company and try and find the dosing information or I could just whip out my phone and say ‘Hey Merck M.D., what’s the dosing information for this medication? I’ve got a 55-year-old female who’s taking this medication and this medication. Can I also give them this? And how many milligrams?’ So that instant access to bits of data that I need during my day is the value proposition for a chatbot for healthcare professionals.”
Naturally, this is great marketing for the pharma brand with the best bot. Whatever makes doctors’ lives easiest will benefit from doctors considering that brand’s products more often.
There’s also a compelling argument for chatbots’ use around clinical trials. Chat apps on a patient’s phone could make the collection of data more natural as well as reminding patients to take their drugs when they need to and report any side effects. Many jobs that once took up a nurse’s time can be automated. “It used to be that I gave you a book and you wrote down everything that occurred on a daily basis,” says Ritesh. “Now the bot does that. Anyone who has a mobile phone can install it and you can log your day with this medicine you’re taking during the trial. It also checks in with you - a virtual nurse: ‘How are you feeling today? Did you have any adverse effects?’ Sometimes people don’t report them because they don’t think it’s a side effect.”
Alongside the pharma brands, we are likely to start seeing chatbots emerging from advocacy groups for specific conditions to raise awareness around these diseases. In the UK the Asthma Foundation recently launched a chatbot for living with asthma that was sponsored by Boehringer Ingelheim called Tabatha. It’s a Facebook Messenger bot created to educate and help people. It ran as a pilot earlier this year and Ritesh found it interesting how well it was received. “It’s a generational thing. The millennials are adopting these things in droves because it’s part of their whole workflow. They’re good with things like chat because of text and Snapchat and all these apps.”
Tumblr media
The concept of a chatbot has been proliferating for years now and we see them all over the web in customer service roles and it is now easier than ever for health and pharma brands and their agencies to build them. Adam notes that services such as Dialogflow and Bluemix do “most of the heavy lifting” for developers these days. “The challenge, as always with digital, is making them good! We are in that awkward toddling phase like we have seen with all new tech. We may have small pockets of awesome in our agencies, however, making those function across all disciplines is the challenge. As the development tools get better, functionality will undoubtedly improve. Hopefully, within the next 18 months, we’ll start to see chatbot personalities coming from outside of the big players and offering real value in healthcare research.”
The decisions the industry makes now will be vital, setting the bar by which conversational health solutions are measured for years to come, until we reach that point when we have trustworthy AI doctors. “Ultimately, we will begin to see patients ‘converse’ with their digital twins, a bot that will understand who we are, what we like and what healthcare issues we have faced in the past,” says Adam. “When considering the build of these kinds of interactions, I often think of the base interactions as ‘starter yeast’! Whatever core interactions we create and define now will form the ‘personalities’ of our chatbots of the future. Now more than ever, we have to get these foundations correct.”
Ritesh realises the challenge here comes from brands’ motivations. “Most marketers want to tell you stuff. They don’t want to help you. So how do you fix it so that you’re having a conversation and that leads to you giving me something of value so that I keep coming back to your brand? That builds a brand.”
Bot agencies tend to compete on the tech side and it’s easy to get lost in functionality, but as Dr John recognises, “all tech does is allow anyone to create a so-so bot. The magic lies in the conversation strategy and execution within this highly regulated industry (and how technology can enable this).”
One of the main reasons healthcare has always lagged behind in the innovation space is that it’s important. It’s literally a matter of life or death. So naturally, innovation by health and pharma brands faces a lot of scrutiny.
“As soon as we open up this area of conversation though, we inevitably end up in rabbit hole of ‘how much can and should we know?’, ‘who owns my data?’, and ‘how the hell will we approve this?!’” laughs Adam. “The major challenge for pharma clients is the age-old issue of compliance. How do we create meaningful, compliant experiences for our customers that go beyond the linear multiple-choice pathways provided by most pharma chatbots?” Giving medical advice via an AI? That’s scary for corporate lawyers. “If you give bad advice and somebody dies, then what? You have to be overly cautious. Anything that slaps of providing medical advice is going to be difficult,” says Ritesh.
But difficult doesn’t mean impossible. Dr John denounces the perception that pharma can’t play in the messaging space. The fear is that AI might generate conversations that might include incorrect or off-message information and put the client at legal risk. “The reality is that pharma solutions can leverage AI in a different way,” he says, “meaning that all conversations are pre-structured and pre-approved and AI is used only to understand the user questions and then serve up the right conversation (from an extensive library which is pre-approved).”
Another huge paranoia is the great bogeyman of 2018 - use of personal data. Health issues revolve around the most personal kind of data - age, location, lifestyle choices - and chatbots often end up on Facebook Messenger as a natural environment for them to operate in. “For healthcare, the question is in the access to usable data sets,” posits Adam. “How do we let private companies have access to personal information? Even with the current level of chatbot capability, they could help provide the much-needed improvements in infrastructure. Enter blockchain. Once we have patient data available on a robust, secure and universal platform then maybe we can start to think of a future with android doctors.”
The final challenge for health chatbots is a more subtle, human one. Do you trust a robot doctor? “On the patient side I think trust is going to be enormous,” says Ritesh. “I think that’s why some of these bots in healthcare will come from patient advocacy groups, which is a trusted resource in patients’ lives if they’re living with a disease. They’re more likely to trust it than something coming from a pharma brand.”
None of these challenges are insurmountable though. Pharma brands are undeterred. Ritesh has been watching this space and predicts that we’re on the brink of something. “In 2019 you’re going to see an explosion of healthcare professional bots from pharma companies. The advocacy groups will be a bit slower. It will reach healthcare professionals before consumers.”
Dr John and conversationHEALTH are actively working with eight global pharma companies. “All are aggressively moving into the conversation world, he says, “with two quickly moving to voice solutions for healthcare professionals. Our belief is that all brands will have conversational solution (just as all have websites). Why would a user want to search for answers when they can simply ask and get one immediately?”

Say Goodbye to Addiction Relapse Forever
What does it take in order to overcome a drug or alcohol addiction and avoid relapse forever?
For starters, I would encourage you to forget about the concept of “staying sober forever” and instead adopt the mindset that they encourage in traditional recovery programs, which is the “day at a time” philosophy. If you do it right and you work a good recovery program then you may very well stay clean and sober until the day that you pass away. However, we don’t want to create an unnecessary burden on ourselves by placing this demand on our sobriety right out of the gate. Better to take it slow and try to be realistic about your journey in recovery. This is not to say that you are doomed to relapse, but rather that you want to keep things in proper perspective and focus on getting through each day sober. Tomorrow can take care of itself and you do not need the added anxiety of having to project into the future too much.
In order to do well in addiction recovery you need a strong foundation. I would recommend that you start your journey by going to an inpatient treatment center. If you do not know how to go about doing this then simply pick up the phone and start making some calls. Call up a treatment center and inquire as to how you can get into rehab, what the terms are, what insurance is needed, if you qualify for a different rehab center, and so on. They will help to direct you to the help that you need to recover.
Now after you get an appointment scheduled to attend inpatient treatment you are well on your way to recovery, but there is plenty more ahead of you. Do not be discouraged if you are at this point though as you have your entire sobriety ahead of you and lots of help available. The nice thing about addiction treatment is that you do not have to do it alone. You will have a group of peers–both in rehab as well as at AA or NA meetings or other support groups–who are going to help you along the path to healing. You don’t have to do it alone.
My biggest recommendation to you following a stay at an inpatient treatment center is that you follow through with the aftercare recommendations that they give you. They are going to want you to go to IOP groups, counseling, therapy, AA meetings, and things of that sort. The specific recommendations may be different from these listed here but the basic premise is always the same: You need social support and fellowship with others in recovery if you are going to have a chance at staying clean and sober. Trying to do it all by yourself after leaving rehab is not a good option. You need other people in order to recover.
Now after you have gone through treatment and you have been following up with things like group therapy, one on one counseling sessions, AA and NA sponsorship, and various meetings, you still need to do a bit more in order to really build out a full life for yourself in recovery.
In other words, even if you go to rehab and you follow up with AA you can still become complacent in your recovery and end up relapsing.
Therefore you need to create a strategy for long term recovery in which you do more than just sit in meetings every day. You need a plan for recovery that allows you to push yourself to learn more, to improve yourself, and to keep growing as a person in recovery.
So how exactly do we go about doing this? How do we go from being relatively stable in early recovery programs to living a successful life in long term sobriety?
For me, the solution involved in taking a lot of suggestions and advice from other people, implementing their advice in my own life, and testing out their ideas.
This is a critical concept in my opinion, and you should start practicing this as soon as you are stable in early recovery.
So how do you do this? Go to your sponsor, your therapist, or your counselor and ask them for advice and direction. Ask them what your main priority should be right now. If they say something vague like “Just try to stay clean and sober” then pin them down and say “What is one thing you would recommend that I do every single day in order to make that happen?”
This is an important question because it drives at one of the critical concepts for successful recovery, and that is building positive habits.
Think about it: If you go to a single AA meeting today, and then you never go again, what have you accomplished? What good is it to go to a single AA meeting and then never return there?
Instead, what is powerful is doing 90 AA meetings in 90 days–which is why that gets suggested all the time in recovery programs. You see, it is the daily habit that is powerful. It is the ongoing habit of plugging into that recovery source every day that gives you strength in your program. It is in that habit that you give yourself a chance to embrace the fellowship, to make new friends, to learn some lessons on a deeper level.
The power is not so much in the individual action itself, but in the habit or the lifestyle change that can result from the habit.
You can expand this idea beyond just going to daily AA meetings. For example, at one point I was asking people in recovery what they did in order to stay happy and healthy in their life outside of going to meetings. So I started hearing things like “I go jogging” or “I go to the gym” or “I do yoga” or “I do seated meditation” or “I walk around in nature.”
So I started to take suggestions like this and emulate what these successful people were doing in their own lives to see if it enhanced my own life.
What I found is that some of their suggestions were a great fit for me and really helped my recovery, while other suggestions were not so great.
So I eventually dropped the advice that did not work for me and I continued to focus on the habits that seem to empower me. So I started working out and exercising on a regular basis, and this has had a huge impact on the quality of my sobriety. I also started to experiment with seated meditation, and this has given me some amazing benefits as well. In particular, I seem to get more intellectual breakthroughs when I meditate, and I tend to get emotionally balanced when I work out physically. Both practices seem to reduce anxiety and also help me to sleep better.
I never would have started either of those habits on my own unless someone had suggested them to me first. Luckily I was open minded enough when I got those suggestions to be able to give those habits a chance to work in my life. So what I would recommend to you is that you be open to suggestions from people in recovery who are already successful, who already have the sort of life that you want to live. Take advice from those people and then put their suggestions into action. Test it out and see if it works for you. If not, move on and find another suggestion. Good luck to you on your journey!

Good luck!
