Chancellor, Vice Chancellor, Graduates, Ladies and gentlemen,
It is a great honour and a pleasure for me to be here today at the University of Stirling, especially among you, dear graduates.
You have been studying hard to reach your goal – the academic degree you are receiving today. It takes time and focus to pass all your exams. You have all completed an admirable journey, and your efforts have led to this very special day. My warmest congratulations to you and your families!
I know how much it means to obtain cutting-edge knowledge within a field. My own dedication to the dementia cause is rooted in personal experience. When my mother lived her last years with dementia, I realised how important it is to have the support of skilled and experienced carers.
I saw a need to create a higher education programme that specialises in cognitive illness and dementia care for nurses and assistant nurses. That was the reason why I founded Silviahemmet more than 25 years ago, with the aim of providing improved care and quality of life for people living with dementia and their loved ones. Today, Silviahemmet – together with Swedish universities – also offers highly specialised training in dementia care for several other professions such as physicians, physiotherapists, occupational therapists and case workers. Silviahemmet’s philosophy of care, which aims to ensure the highest possible quality of life for both the person with dementia and their family, has now spread to other countries in Europe, Asia and South America.
Knowledge is exactly what Silviahemmet – and this ceremony – is all about. And sharing knowledge seems to be the only way forward to truly achieve lasting change.
The cooperation between Silviahemmet and universities has led to important networking between practitioners and researchers, and to the continuous development of more knowledge in order to improve care.
Every year we organise the Silviahemmet Master Academy Day, where former students can share their experiences and be inspired. This day is very much appreciated, and has led to new personal connections and projects. Therefore, my advice to all of you is to keep in touch with your university and your fellow graduates! I am sure this will be rewarding in so many ways.
Ladies and gentlemen,
Education, research and inter-professional work is what I believe forms the basis for development. The University of Stirling is a golden example of this. You have not only a wide range of different education and research fields, but also several international collaborations. I am especially grateful for the cooperation you have with us in Sweden when it comes to caring for people with dementia, in order to improve their chances of living not only for longer but also with a higher quality of life.
Education with a purpose was the educational philosophy formulated by your first Chancellor Lord Robbins here at the University of Stirling. This philosophy remains just as relevant today.

Translating the numbers. COVID. What do extra beds mean.
A pet peeve I have with media and government bodies is whenever they go, "we have X number of extra beds" or "we can flex up to this many beds." And you see journalists have this expression of relief come over their faces. As if to say, "you can now have COVID and a machine to keep you alive if you need it."
If it were only that simple.
This does not mean the beds come out of thin air. Or that COVID staff have magically appeared as "extra staffing" in addition to regular hospital beds.

It's just not how it works. Even if you have extra physical rooms and beds, training staff takes years.
It means regular hospital beds are being converted and staff are being transferred from whatever departments they were previously in to staff the COVID wards.
They could be surgeons, anaesthetists, physicians etc. trained to do other jobs, now being re-purposed or re-trained to temporarily staff ICU or COVID wards. It means, less timely care and beds for other diseases that while not life threatening, are no less significant.
No one casually goes to hospital for a cold for instance. Even those really sick with COVID needing hospital admission, are in terrible shape, it's not like a bad case of the "man flu."

I've seen entire public hospitals re-purposed to cater solely to COVID and it means other patients get re-diverted elsewhere where staff may not know long term patients with complex conditions or staff are not used to caring for some of them.
This can lead to higher rates of medical errors or worse long term outcomes. For example, we have expert units in Cystic Fibrosis, organ transplants etc., not all hospitals or generalists are going to be as highly skilled in looking after them. Entire stroke wards have been dissolved or amalgamated with internal medicine units, so some staff can be re-purposed for COVID. There is no one size fits all in medicine, not all the time. We have these systems because we recognize we are human after all, and anything that mitigates errors systemically has been developed for a reason.
You'll also often hear healthcare agencies come out to say they've "halted elective surgeries." This is needed during a crisis, but what are elective surgeries? Why is this term being thrown around? These aren't superfluous necessarily, like a tummy tuck or hernia repair.
Elective can refer to surgery to determine if someone has the early stages of cancer. So, we are going to see an influx of advanced cancers which means, higher mortality, more destructive therapies to control it versus in the very early stages.
And this isn't solely limited to cancer, it's also relevant to a host of other chronic diseases. We really take for granted the care we were providing previously. As an example in multiple countries, cancer services have been delayed or cancelled. There's been delays in diagnoses.
There are so many ramifications that we will be facing for years to come in healthcare and as healthcare practitioners. We're going backwards in the care we provide possibly by decades. I can't imagine what this means for developing countries already under resourced and behind.
At the same time, we're seeing delays in emergency cases being seen, you get used to seeing reports of patients dying of things they wouldn't die of normally (i.e. a ruptured appendix) because hospitals are overwhelmed by COVID. Or patients are delaying presentation to avoid overwhelming hospitals or exposure to COVID. I'm seeing this, I'm seeing much sicker patients with complications I did not ever expect to see. And it's being widely reported.
My other pet peeve is when politicians in particular and journalists, come out and go, we've known for 18 months or 2 years blah blah why were we not better prepared for the 3rd, 4th, 5th peak?
It can take 10 years + to train a doctor. Med school can be 4-6 years depending on where you are in the world. Then you add residency etc. Nurses also need years to adequately train. We can't just pull doctors out of a hat or from other countries - it's a global pandemic.
Our healthcare systems were already operating at capacity before the pandemic. Why on earth would anyone think it would magically meet the challenge of the pandemic without repercussions? Where do you think the extra beds come from?

I wish the media would stop reporting extra beds as something we just pulled out of thin air when we didn't. It's providing a false sense of security and relief. Right now, we need to continue to encourage vaccinations and appropriate pandemic restriction measures, whatever they may be.

COVID VACCINE HESITANCY?
The FDA is likely going to approve the Pfizer Covid vaccine when they meet on December 10th. The Moderna vaccine will probably be approved shortly thereafter. This is an incredible feat of modern medicine, and our best chance to hopefully get our lives back to near normal relatively soon. However, it is new and it was done quickly, so understandably, people may be hesitant to get it; even people who vaccinate against all other diseases.
Will I be getting the vaccine? ABSOLUTELY. As a physician, I will probably have the opportunity to be immunized prior to New Year's and yes, I will be taking it as soon as possible.
But, it is a new vaccine technology and done in record time, so aren't I worried about its safety? Nope, not any more so than any other vaccine or other medical intervention.
Why? Let's start with how this vaccine works. This is an mRNA vaccine. Past vaccines typically use a live but weakened "attenuated" virus, or dead viral material "inactivated" virus, or a piece of the virus's protein or even a toxin produced by the virus. The Covid vaccine is very different. It contains mRNA (messenger genetic material) that encodes for the Covid spike protein. This causes your cells to then produce the Covid spike protein. In contrast, when you are sick with Covid, the virus hijacks your cell to produce many copies of the entire virus. Then it destroys the cell, busting it open to release its newly formed viral particles. When your cells release just the spike protein, it will stimulate your immune system to form antibodies to the Covid spike protein without you getting sick. There is no possibility of getting Covid from the vaccine. When your body is subsequently exposed to Covid, it will quickly recognize the spike protein and destroy it before it can make you sick. This was 95% effective in preventing Covid, which is an even better percentage than most other vaccines. However, you must take both doses (about 3-4 weeks apart).
Am I concerned about it being new? And previously untested? No, I'm not. This type of technology is not entirely new. It has been studied and used in cancer research. They have been making mRNA vaccines and studying them to specifically target proteins on tumor cells and train your immune system to then destroy the tumor. In this case, it is not a vaccine in the preventive sense, as it is targeted to a tumor that you already have. It is not currently widespread because it has to be custom made for each tumor. But, it has been "around the block" for a while now. The technology was also being studied for other Coronaviruses. It never came to fruition, because the diseases never reached pandemic proportions, and then the funding dried up. The mRNA does not enter the nucleus of the cell, and it does not affect your DNA, and therefore has no lasting impact on your cell.
Am I concerned about the speed with which it was developed? Weren't significant corners cut in order to get this out so quickly? No and no. What was cut out of the equation was mostly red tape, and what was added was technology and funding. We were given the genetic code by scientists in China to start vaccine production in January; before Covid was even documented to have reached our shores. From there, the vaccine was developed from the technology we had from the prior Coronavirus and cancer research, and was completed in March. Normally, there would be months of waiting for the FDA to even look at the work done prior to approving Phase 1 trials. Because of the urgent nature of this, it was essentially put on the top of the wait pile, which cut out months of waiting, but did not cut any corners. Between the Moderna and Pfizer vaccines (both mRNA, with a slightly different delivery system), they were tested in 37,000 people in Phase 3 (and an additional 37,000 received a placebo). That is on par with, if not better than the vaccines currently available.
Aren't I concerned that the FDA is about to approve it, and there may be side effects that haven't been seen yet? Nope on this one too. We know from decades of vaccine research, since you typically just get 1, 2 or 3 doses and then you are done with it, that nearly all side effects from vaccines occur in the first 6 weeks. Like other vaccines, minor side effects may occur(soreness at injection site, muscle aches, fever). Severe adverse reactions are extremely rare, and again, occur quickly if they are going to occur at all. As a healthy 40-something year old woman, my risk of dying from Covid is about 1 in 250 to 1 in a 1,000. That is not a rare event! And even if I don't die from it, I could have long term lung damage and other issues that affect my quality of life. Because vaccines are given to healthy people (unlike medications for treating a disease that is already present) they are held to a much higher standard for approval. My risk of having a significant adverse reaction from the Covid vaccine is minuscule in comparison to my risk with Covid. In fact, there have been no severe reactions to the Pfizer or Moderna vaccine to date.
For the rest of the population (outside of healthcare workers or those in nursing homes), who will likely be able to get the vaccine in February or March, there will be even more time passed and more people who have received the vaccine to be the "guinea pigs" here.
Anyone who is pregnant - it has not yet been tested in your group (although I do know that many pregnant front line health care workers are planning to take it). For those under 18 years old - it is also not yet tested. Moderna will be starting a trial shortly. Hopefully it will be approved by summer. But for everyone else, I highly recommend getting it as soon as you can; for you, for your loved ones, for those who cannot (perhaps because they are immunocompromised), for the many businesses that are on the verge of permanent shut down, and for kids to safely return to schools. I will be rolling up my sleeve for it as soon as it is offered.

We have now surpassed 15 million documented Covid cases in our country (5 million of which were in the last 30 days) and over 2,000 people are dying per day. On a daily basis people are transported in air ambulances for both emergency and nonemergency reasons. Often a medical flight is simply ways to transport a patient from one medical facility to another, perhaps in a different state or even country. Let's focus first of all on the medical personnel who are on board. A superb air medical service should come with a skilled team of qualified medical personnel, including doctors, paramedics and nursing staff. Realizing that emergencies may take place during the flight, these professionals are in charge of the patient's well being every measure along the way. They should have all of the required certifications for safe medical air transport. Many cardiac patients, as well as other patients who are critically ill, may require more than one IV line so the medical staff can administer both fluids and life-saving medications. Infusion pumps are vitally important, therefore , on just about every medical flight. Patients who were once considered too ill to travel by air can now be safely transported. It's largely due to the advancements of the infusion pumps which are now used on medical flights. Any one of us may have to take a professional medical flight for any number of reasons.
