As for how Irene and Esmund leave, I think Esmund finally died in O'Khasis. I think he died in Irene's arms, peaceful, finally able to realize his mistakes. Finally able to apologize to her for everything that happened between them. Finally able to tell her that he loves her unequivocally and that due to this love he can't wait to let her go.

And then he died. He died with a smile on his face. And he was buried in the O'Khasis Burial Grounds. As per his request, he wasn't given a grand statue befitting of his status as a savior of Ru'aun. He was given a headstone, like everyone else. And his achievements were carved into it, like everyone else.
