👍Or👎@RestoredSmiles Here is the extraction video from the most recent trauma case I previously shared. Here is some of the info from the the previously posted case... I was so happy to be able to help this patient but I sure felt bad for his situation. I did two central incisor crowns for this patients several years ago and hadn’t seen this patient since until this happened. He was so pleased from the previous work he figured I could do it again but this time the treatment plan is not so simple😬. The patient passed out and landed on a metal framework with the mouth taking the bulk of the impact. The patient needed three upper incisors removed as well as had some facial plate and palatal plate bony fractures. The previous crowns didn’t break though😅😬🤦‍♂️. Large augmentation was done in order to place two implants for an implant bridge in approximately 5 months. Releasing incisions (split thickness incisions) are crucial in cases like this in order to get tension free primary closure. Many cc’s of mineralized particulate allograft was used mixed with growth factors to get sticky bone and CGF membranes to help improve outcome. Patient walked out with a tooth retained Essex type retainer. 💉😴😬🤦‍♂️😷😱 Please Don't Pass Out... Anatomic factors (such as enamel pearls) are often associated with advanced localized periodontal destruction (which this patient had). Notice the mobility of the adjacent tooth during the extraction as well. After site was cleaned and grafted, the site and adjacent tooth healed well. 👍Or👎 @RestoredSmiles Rotten Tooth. 😷 When extracting a severely decayed tooth always make sure your forceps are positioned as apically as possible to get the best grip on the tooth. I thought I was positioned well with this tooth and the coronal portion broke right off. After repositioning more apically and with a little finesse the tooth came right out. Check out the curves on this one😜...also I had a throat pack in this patient but they had a severe gag reflex so I removed it because they could not tolerate it. There are a few extraction kits out there and they are definitely not for all cases but when able and indicated can be highly useful and convenient when you want to place an immediate implant in a pristine socket. This lower left canine was nonrestorable due to decay and lack of tooth structure remaining. This video is a bit long but should be worth it and briefly shows all the steps involved in preparing this tooth for extraction with this particular extraction kit that uses only vertical pressure to extract the tooth while stretching and breaking the periodontal ligaments. This patient had extractions years ago and now wants to replace missing teeth with implants. Due to the position of the maxillary sinuses, a sinus lift is needed to allow space for future implants. A double lateral window approach was done as well as repairs to the schneiderian membrane where there were slight perforations.
