A Firm Foundation
Poor balance and weaknesses in feet and ankles can severely limit mobility. Lin points to a literature review indicating that over 80 percent of older adults who worked with podiatrists felt that the intervention was beneficial.
“It’s not just the physical improvements,” he says. “Confidence also has a role to play. Fear is a big factor. When we don’t feel confident about our physical performance, it’s like a self-fulfilling prophecy.”

Falls and Foot Treatments
Although most people undergoing podiatry treatments feel confident that it’s working, researchers are more cautious in drawing conclusions: “The burden of proof for researchers is a heavy one. For example, if a study deals with foot exercises and falls, there must be no doubt that foot exercises rather than individual physical differences between subjects are responsible for any differences.” Lin explains.
“But even those who can’t produce evidence that podiatry works to reduce falls can’t produce evidence that it doesn’t. They’re calling for further research.”
However, the British Medical Journal has published a study indicating a “significant” reduction in the incidence of falls in older adults following intervention by podiatrists. The lead author, an NHS researcher, concludes that podiatry is a cost-effective intervention that can reduce the number of NHS patients requiring treatment for falls.
The study involved a multi-faceted approach which included advice on footwear choices, orthotics, foot and ankle exercises, and fall prevention education.
“People may find this confusing,” says Lin, “One researcher says that further study is needed while another says there’s a significant difference – but study design has a lot to do with the conclusions drawn.”

Foot Anomalies and Foot Pain
Foot problems affect one in three people over the age of 65, and once again, Lin has the third-party evidence to prove it. “Foot pain has long been considered a contributing factor when it comes to falls in older people. When your feet hurt, you will favour them, and that means you develop a less steady gait. That, in turn, affects balance – and the logical consequence is that you’re more likely to fall.”
Although Lin is best known for his work as a sports podiatrist, he works with people of all ages and from all walks of life. Foot pain, it seems, is not limited to any specific segment of the population. “I work with kids too,” he explains. “They can have postural and gait problems that need to be corrected before they cause further problems. No matter who you are, if your feet hurt, you have a problem. Sore feet aren’t normal.”

Lin is concerned about the high incidence of young people, particularly young women, dropping out of sports and avoiding physical activity. There are several barriers to sports participation that women face, including confidence issues, but Lin worries that the tendency towards injury could have a role to play too.
“If you’re inclined to get hurt when you perform a certain activity, you’re going to start avoiding it,” he says. “You might not be put off the first time it happens, but if it keeps happening, you’re going to quit.”

However, as he points out, the primary focus of sports medicine is the prevention of injuries. “Sports test your body, so if you have any existing weaknesses, they will come to the fore,” he explains. “If we work together to correct the cause of an injury, you’re stronger than ever, and your performance improves accordingly. There are lifelong benefits.”
“Of course, we do help our patients to recover from existing injuries but treating the symptoms without addressing the cause means it’s only a matter of time before the athlete suffers from the same injury again, or even suffer from other pain and injures down the track.”
It’s a well-known fact that most professional teams have a network of sports health professionals working with the team and its coach. But Lin says that public awareness is growing, and he’s seeing more casual athletes visiting his Chatswood and Sydney CBD practices than before.
“Doctors have always known that sports podiatry can be helpful, but now we’re seeing more members of the public who aren’t in the big-name teams and who aren’t being referred,” he says.

Lin says that quite often, symptoms only appear towards the later stage of a problem. In most cases, some degree of irreversible damage has likely occurred by the time pain is experienced. “Pain is a sign your body has already compensated enough for a problem and is trying to tell you to do something about it, so don’t ignore the symptoms from your feet and legs.”
Common symptoms such as morning foot stiffness, general foot aches, calf tightness or tightness under the arch often get overlooked as they do not normally prevent people from doing anything. However, Lin warns that more extensive damage often starts from these symptoms.
Lin says, “Growing pain” in children should be considered in the same way. Children may grow out of their foot pain as they age but there may be lifelong or future problems as their growing bones and muscles adapt to an inefficient way of functioning.
Pain is a gift which signals there is something wrong with the body. “It’s like the warning lights in your car, they flash for a while before it breaks down.” So, stop ignoring pain, listen to your body and get your feet assessed even if you’re only experiencing minor symptoms. “There is always something that can be done to optimise your foot function and prevent future problems,” Lin concludes.

“There are a lot of things that can cause low back pain,” Lin says, “But a lot of people don’t realise it could come from their feet.”
Why Foot Problems Cause More than Sore Feet
The logic is straightforward, says Mark. Because we, unlike any other animal on the planet, walk around on our hind legs. As a result, our legs and feet take a lot of strain. Although we’ve evolved to be upright, there are still a few variations in the gene pool that, while they don’t get us down in the literal sense, certainly do so in the figurative one.
It’s an obvious chain reaction. Muscles, joints, and ligaments strain when the feet don’t carry their load correctly, and since our backs also take up the lot of the physical strain inherent in our upright posture, the chain reaction is easy to follow.
Foot problems can cause pain in the ankles, shins, knees, hips and lower back – and some people experience the full chain of pain from the feet upward.

Why Orthotics Aren’t First Choice
Orthotics could be a way to correct lower back problems caused by foot issues, but Wei says they’re not a solution they prefer for their patients. They support the foot and strive to correct the way that the feet carry their load. But not all of them are created equal, and they only help you when you’re wearing them- there’s no long-term correction.
A look through research on the topic of orthotics doesn’t show anything very conclusive, but it’s interesting to note that some widely-cited trials looked at “off-the-peg” arch supports rather than custom orthotics. Other studies show more positive results, but note that patients were told about the theory behind their custom orthotics and speculate that this may have heightened the placebo effect.
“We do occasionally use orthotics, and they help some people” says Mark, but we prefer to use them as adjunct to therapies that correct the real problem that causes pain. If we can achieve a solution without making our patients depend on orthotics, we will.”
Low Back Pain Advice
Getting to the root of the matter is the best solution for those with low back pain, says Lin. His mission isn’t just treating pain – it’s also about preventing its return.
“There are a lot of possible causes to examine and eliminate,” say Lin. “It could be your office chair – or it could be the way you walk.”
He suggests that we begin with the physical activities we perform most. “If you spend most of your day seated, your posture or your desk and chair are likely to blame,” says Mark. “If you spend most of your day standing or walking, your feet, your shoes, and your posture are among the most likely culprits.
If you’re still not sure, Wei suggests starting from the feet and working your way up. “Our assessment will help you to make progress towards finding the cause of your lower back pain,” he says. “And if it lies in your feet, or if the way you use your lower limbs are contributing to it, then we have the solutions to the problem too.”

Kinesiology Sports Taping: The Science
It turns out that there’s quite a lot of science behind kinesiology tape – but that its proponents and detractors hotly contest each other’s claims. And when bias is removed, things get even more puzzling.
In an impartial laboratory environment, one study may seem to show results in favour of kinesiology sports taping while another seems to draw a blank. Study design and scope could be to blame, but that hasn’t stopped athletes from getting serious about the placement of kinesiology tape.
What we do know is that kinesiology sports taping was the brainchild of a Japanese chiropractor, Kenzo Kase. He started his work in the 1970s, commercialised the tape 14 years on, and by the 2008 Olympics, kinesiology tape was everywhere to be seen – if only because Kenzo’s company donated a lot of tape to Olympic athletes. .
It was around about then that people started asking about the science behind kinesiology sports taping. And at the time, there wasn’t much – only theories that seemed to make sense and a history of use in Japanese sports. By this time, there’s a bit more to work with, but still nothing that looks conclusive or compelling.
Why Athletes and Amateurs Wear Kinesiology Tape
Despite controversy in the scientific community, athletes seem to love kinesiology tape. They talk about cutting out painkillers, preventing injuries, relieving inflammation, recovering faster after getting hurt, and getting extra support.
They also speak of greater confidence when wearing the tape – a feeling that they can push themselves further and achieve more. And, of course, some might just be wearing it for fun.

A Sports Podiatrist’s Opinion
Mark Lin believes in using techniques that work for his clients, and kinesiology sports taping is one of the things that his clients say are getting results.
“Maybe it’s just because it gives them more confidence,” he says. “But my reasoning when I first began to use this form of treatment was that wearing something on a part of the body you need to concentrate on is going to help you with awareness as to how it’s positioned and how it’s working.”
“Plus, there’s a light support benefit. Kinesiology tape isn’t unyielding like other strapping tapes. It stretches like skin. So, you’re getting extra support, but you aren’t immobilising anything – you’re not limiting movement.”
Not a Cure-All
Lin doesn’t see kinesiology sports taping as a cure-all. Instead, he thinks it’s a helpful addition to other therapies – and certainly not the right intervention for all the clients he sees.
In podiatry, he says, professionals are dealing with something that could be as unique as your fingerprint – the complex machinery of the foot and its interaction with the lower limbs. As a result, there is no sports podiatry how-to guide that works for absolutely everyone, and by the same token, taping won’t solve everybody’s sports injury issues.
“The kinesiology taping debate is easily settled on an individual level,” says Lin. “If, after assessment, we believe that it is among the interventions that might help you, we can work together to see whether it seems to improve outcomes.”

When Your Feet Make Your Back Hurt
Foot problems affect the way you stand and the way you walk. An incorrect gait, for example, can place a lot of strain on the back or even the shoulders, and in time, this will result in injuries related to the daily stress inflicted on them. With the injury, comes pain – and you’re left wondering why your back always seems to be sore. 
People with flat feet are easily-identifiable victims. They’re almost sure to compensate with a less-than-ideal gait, but flat feet, and other physical variations that affect how we walk, are not the only reason why your feet could be placing stress on your back. Tight-fitting, high-fashion shoes and towering heels are obvious culprits.
“Don’t get me wrong,” says Lin, “I like the way high-heeled shoes look as much as the next person, but if your footwear changes the way you walk, and you wear the same kind of footwear day after day, you can expect problems.”
“Whatever the reason for your feet triggering back pain, we need to see the feet as being linked to the rest of the body. What affects the feet affects the ankles which in turn affects the knees. When the knees start taking stain, that’s transferred to the hips, and from there, the back isn’t a big jump away.”
What to Do if You Experience Persistent Back Pain
If you experience persistent back pain, your doctor is the first port of call, but as one source told us, that isn’t always a solution.  “My doctor gives me anti-inflammatories – and when they’re finished, I’m in pain again,” a colleague told me. “I’ve been for back X-rays, but I was just told that nothing seems to be wrong.”
Finding the source of back pain is a process of elimination that not all GPs will embark on. In fairness, podiatry is a specialised field, and few GPs will be qualified to assess the way you walk as a possible cause for your discomfort. While Mark Lin assures us that he receives many referrals from family doctors, those who are weary of symptomatic treatment may decide to take the matter into their own hands.
“I don’t need to have a referral to assess a problem that may originate in the lower limbs,” says Mark. “At The Footwork Clinic, we often see people who came to us with long-standing pain issues and want our opinion. Sometimes, it’s back pain. At others, it’s discomfort in the limbs and feet.”

Podiatry is Not a Cure-All
While back pain can originate with foot problems, Mark Lin is quick to disclaim the possibility of a cure-all through podiatry. “Our discipline is scientific. We can help some people. We may have to turn others away, referring them to the relevant practitioners to address their problem,” he says. 
“At the same time, our patients can be assured of a thorough assessment and an interview. That may point towards the right therapy to achieve relief from pain, and ideally, permanent correction of the issue that’s causing it. If the biomechanics of the feet are involved, we can either provide or contribute to a solution.”

“We’ll also prescribe non-invasive therapies including exercise – and some of them are things you can do yourself at home. We also provide functional rehabilitation with manual therapies to restore movement in the ankles and stabilise the feet, building a strong foundation of the body, and once we achieve better function and movement of the feet, many of our clients experience improvement in back pain.”

Why Foot Orthotics may not be the Best Solution to Bunions
A bunion is a hard, bony bump that develops at the base of the big toe. As bunion sufferers will know, they can cause considerable discomfort. Less well-known is the fact that they can lead to secondary foot problems like arthritis or hammertoes.

The Problem With Orthotics
Orthotics certainly can help people with bunions, but they aren’t a cure. It’s a common misconception, says Lin. “People feel better when wearing orthotics, and as a result, they may think that their bunions will improve over time, but the orthotics aren’t addressing the problem itself. To do so, we need to work on joint alignment, improving foot strength, and improving movement.”
And there’s the most obvious problem of all: you can’t wear orthotics all the time. While you’re wearing your orthotics, they help to take the strain off your feet and legs. Movement becomes easier. But the minute you take off your shoes, you don’t have orthotics to help you, and you’re back where you started. Orthotics don’t actually correct bunions. They only provide relief.
While this may seem acceptable on the surface, our feet, along with the rest of our bodies, deteriorate as years go by. In time, we may become unable to enjoy simple pleasures like walking barefoot on the beach. In effect, we become dependent on our orthotics losing, as Mark Lin terms it, “the freedom of our feet.”
Functional Foot Solutions for Bunions
Since bunions are the result of poor foot function, functional rehabilitation is the key to effective treatment that goes beyond symptomatic relief. Podiatry techniques can help, says Lin. “It’s possible to reduce bunions and address the toe misalignment that caused them. And, because we’re dealing with the causes of bunions, pain management becomes easier. Most importantly, we can prevent bunions from becoming worse over time.”
But, Lin explains, the benefits go beyond bunions. “Because we’re addressing functional problems in the feet and legs, we also prevent the secondary problems, such as arthritis, that are associated with bunions. A bunion is a mis-alignment of the joint of the big toe, where the first metatarsal (the inner bone) shifts inward, and the adjoining proximal phalange (the outer bone) moves in the opposite direction.
The visually prominent bulge or ‘bump’ is often mistaken to be an excess growth of bone, but the protrusion is actually due to the mis-alignment and gradual movement of these two bones in the big toe.

Bunions can often carry along with them their own stereotypes.
Some assume that bunions only affect the elderly, remembering a menacing protrusion submerging from grandpa’s soft and unsuspecting slipper.
Others have assumed that they are a peril faced mainly by women, due to the choices of some to elect beauty over practicality, therein putting their foot-health in peril.
Perhaps there are some that relate the bunion to those in careers such as teaching or nursing, where the job demands long hours on their feet.

Even the demands on one’s feet, whether it be bustling between hospital rooms or sweating through interval training, is not a direct cause of a bunion condition.”
Most frequently, a bunion is a hereditary condition.
Lin explains that “the structure and shape of the foot are passed down genetically, and these factors play an important role in the probability of developing or not developing a bunion within one’s lifetime. Some foot types that are at increased risk of bunion development are feet with low arches, loose joints and tendons, or those with flat feet.”
While they are unlikely to cause a bunion independently, there are additional factors that can trigger or exacerbate any underlying bunion conditions.
Access to practical and functional shoe options can affect the experience of bunions, as “shoes that fit tightly to the foot, particularly around the front of the foot, heightens the likelihood of bunion development” advises Lin.
“We also do see many athletes, along with those in careers which require large amounts of time on their feet, experiencing bunions. The extra stress put on the foot from these activities makes the foot more susceptible to bunions, along with a variety of other foot injuries and conditions.”

How to stay free of pain and injuries as a runner
Running continues to gain popularity as a form of exercise, and for good reason – it’s an effective antidote to the modern sedentary lifestyle, it doesn’t require any special equipment and it delivers a dopamine boost. Yes, fitness is important, but the joints of the feet and legs absorb a lot of impact during running and it’s vital to give them a rest.
Conditions like Achilles tendinitis are very common, this occurs when the tendons connecting the heel to the lower leg muscles become inflamed. This can be caused by improper footwear, tight calf muscles or having naturally flat feet and can be avoided to some extent by a proper stretching and strengthening programme of the calf muscles.

Runners knee, or Patellofemoral pain syndrome, refers to pain in the front of the knee and around the kneecap, and is generally caused either by maltracking of the kneecap and/or overuse from vigorous exercise.”
Other conditions that runners can suffer from are Morton’s neuroma, Metatarsalgia, Metatarsal stress fractures, Shin splints, plantar fasciitis, calf muscle strain or tear and ankle sprain.
Morton’s neuroma – a painful condition that occurs as a result of the thickening of the tissue surrounding one of the nerves leading to the toes.
Shin splints – amongst the most common of running injuries, this occurs when the muscles, tendons, or the fascia covering the shinbone become inflamed and sore.
Metatarsalgia – characterised by painful inflammation in the forefoot (ball of the foot) as a result of increased pressure in this area.
Metatarsal stress fractures –  tiny cracks in the bone, these are a sure sign of too much repetitive force on the leg bones.
An occurrence of any of these conditions is an indicator that there are some biomechanical issues going on that have put the feet and legs under too much strain, and seeing a sports podiatrist is recommended.
