I've been listening lately, only a short way in but yeah, he's an unreliable narrator in like three distinct ways.
He starts out by straight up telling us he's withholding important information about his past for now, so there's that, which he continues to vague about. And then there's the whole thing where he has regular, massive hallucinations, so uh, yeah...but beyond that?
Even at his most lucid we're fighting an uphill battle against the combined factors of Howie being generally oblivious, thick as a brick sandwich, and with some sort of ASD vibes too. I get the impression that even if he did have the ability to intuit basic social cues (he doesn't), there'd still only be about a 50/50 chance he'd ever actually notice them. And about a 0% chance he'd draw the right conclusions from them.
The Milkman of St. Gaff's is good but like. So stressful. Like unreliable narrators are great i love an unreliable narrator but Howie is more like. An oblivious narrator. A dumbass narrator. A narrator so dense he will form a black hole. A narrator I want to strangle.

Not all of these are new but they're all good and several are fantasy: The Goblet Wire - Surreal microfiction with weird fantasy vibes following people playing a strange telephone RPG Two Flat Earthers Kidnap A Freemason - Comedy adventure about idiot conspiracy theorists, has a very clear beginning, middle and end.
Wizard Seeking Wizard - Comic fantasy wizard 'dating' program (includes wizards looking for romance, friendship/collaboration and rivalry.)
The Mistholme Museum of Mystery Morbidity and Mortality - Episodic with overarching plot. Audio tour guide takes you on a trip through a museum of strange and fantastic objects.
The Orbiting Human Circus - Chill, whimsical and sometimes very emotional podcast about a janitor who works at the Eiffel Tower, where a profoundly radio variety program is recorded.
The Dungeon Economic Model - Short comic fantasy 'informative' programming explaining how your town could benefit financially from a monster-filled dungeon.
Midst - Fantasy/scifi/weird fiction following a tough monster hunter, a stressed squire and a shady businessman. Unique worldbuilding, even more unique and fun, high-energy narration.
Inn Between - Follows a DnD-like adventuring party, but ONLY during their stops at a tavern between adventures. The Great Chameleon War - Weird, surreal fiction with soft narration. I couldn't possibly tell you what this is about, even I don't really know, but it's great to listen to. If you like the Goblet Wire try this one. The Deca Tapes - Short murder mystery in a scifi setting. Gastronaut - Scifi. A food critic travels through space, reviewing mars cuisine and being drawn into an escalating series of conflicts and skirmishes.
Within the Wires - Skip season one if you don't want any horror content. Alternate history found footage tapes, from a society built around surveillance. Explores relationships, human drama and uncomfortable decisions.
The Milkman of St Gaff's - Weird Kafkaesque comedy set during mid-century wartime, following Howie the milkman - a malevolent moron who makes life worse for everyone.
Wooden Overcoats - In my opinion, the best comedy podcast out there, period. The misadventures of two warring funeral homes on a small British island. The comedic writing and voice acting are truly unmatched.
