Pfizer's and Moderna's formulas have been the go-to on pharmacy shelves, but Novavax remains an important option.
We're on the cusp of the fall season, which means respiratory viruses like flu, RSV and COVID are expected to keep spreading as weather cools and more people gather indoors.
Luckily, we've got vaccines in stock to help prevent respiratory viruses from turning into severe illnesses. In addition to flu vaccines for the general public and RSV vaccines for older adults and pregnant people, new COVID vaccines from Pfizer-BioNTech, Moderna and Novavax are available this season to reduce the risk of hospitalization.
This means adults have a choice in which COVID vaccine they receive: an mRNA vaccine by Moderna or Pfizer, or Novavax, a protein-based vaccine that targets the virus in a more "traditional" way. All three have been authorized by the US Food and Drug Administration. While Moderna and Pfizer have been widely used over the last few years, the Novavax vaccine is building up a bit of a following.
Novavax, a protein-based vaccine, is an option for those who don't want or can't take an mRNA vaccine. Novavax may also be appealing to those wanting to experiment with the "mix-and-match" approach to COVID boosters as a way to potentially strengthen the immune response.
"Even though mRNA vaccines dominate the market for COVID vaccines, it remains important to have multiple different types of technologies against various pathogens because each may have specific use cases," Dr. Amesh Adalja, an infectious disease expert and senior scholar with Johns Hopkins Center for Health Security, said in an email.
Here's everything we know about Novavax this year.
How is Novavax different from Pfizer and Moderna? Novavax is a protein-based vaccine, which people have associated with a "traditional" approach to vaccination. This is compared with mRNA technology, which does not use dead or weakened virus as an ingredient in the vaccine but instead uses genetic code to instruct the recipient's immune system to respond.
However, Adalja said that calling Novavax traditional may be a "misnomer" because it brings its own innovation to the table. Novavax uses an insect virus that has been genetically engineered to express spike proteins, Adalja explained, which are then incorporated into the vaccine.
"The vaccine itself is coupled with an immune system booster, called an adjuvant, which increases its immunity," he said, referencing a component existing vaccines have also incorporated.
This year, there are also slight differences between Novavax and Pfizer and Moderna's updated vaccines. Both mRNA vaccines have been tweaked to target the KP.2 strain of COVID-19, which is a slightly more recent version of the virus than what Novavax targets, which is KP.2's "parent" JN.1. While the FDA ultimately decided KP.2 was preferred in vaccines, all of them are expected to help protect against severe disease and death.
Who should get Novavax? Does Novavax have different side effects? Novavax was authorized by the FDA for use in adults and children 12 and older, so younger kids can't get this vaccine. But for most adults, which COVID vaccine you should choose depends on your preference and what your neighborhood pharmacy has in stock.
People may opt for Novavax for different reasons, though. For people who do not want to take an mRNA vaccine, having a protein-based vaccine like Novavax available means they can still be vaccinated for the fall and winter season.
Other people may be interested in Novavax for its use in the "mix-and-match" approach to boosting, which in the past has been associated with a strong immune response.
There is some early research that suggests Novavax may have fewer short-term side effects, such as muscle fatigue and nausea, but "we can't say this for sure," Joshua Murdock, a pharmacist and pharmacy editor of GoodRx, said in an email.
"This isn't proven, and side effects do vary by person," Murdock said. He added the CDC doesn't recommend one vaccine over the other, even in people who are immunocompromised.
In general, mRNA vaccines have been found to be fairly "reactogenic" compared to other vaccines, Adalja said, noting that it also depends on the individual. But if someone had a bad experience with the mRNA vaccine, Adalja said, they "may fare better with the Novavax vaccine."
Some flu-like side effects can be expected post-vaccine, no matter which one you choose. This includes symptoms like headache, tiredness, a sore arm and even chills. Not experiencing symptoms doesn't mean your immune system isn't kicking in, but experiencing some side effects may signal that your immune system is responding to the jolt, so to speak.
In rare cases, myocarditis or heart inflammation problems have been associated with COVID vaccination, particularly in younger men and adolescents within the two weeks following vaccination. Research so far shows that Novavax, like mRNA vaccines, may also carry this rare side effect though.
Following high levels of COVID this summer in the US, more information will be needed to see how all vaccines and their freshly targeted formulas fare against the virus that's expected to continue to spread this fall and winter.
"There's no strong evidence that one vaccine is preferable to another in specific individuals, but that will be an important avenue to study for more precision-guided vaccine recommendations," Adalja said.

The U.S. is experiencing more than four times as many whooping cough cases compared with last year — a spike that some experts attribute to post-pandemic vaccine fatigue. “With the increase in vaccine hesitancy that has been going on since the Covid-19 pandemic, we’re seeing outbreaks occurring in kids who are not vaccinated,” said Dr. Tina Tan, president-elect of the Infectious Diseases Society of America. Babies are given the DTaP vaccine, which helps protect against three diseases: pertussis, diphtheria and tetanus. The vaccine works well against diphtheria and tetanus, but is less effective over time for pertussis. Advisors to the Food and Drug Administration met Friday to discuss the need for more robust and longer-lasting versions of the whooping cough vaccine. Until next generation vaccines are developed, boosters are recommended about every 10 years, starting in the tween years, as kids start middle school. It’s the tweens and teens whose immunity against whooping cough has waned that are driving outbreaks in many states, experts say.

"Anyone familiar with biology will recognize that biological systems and phenomena are often very complex. The more one studies biology in detail, the more one becomes aware of its intrinsic complexity. Molecular biologists will certainly agree with this. The level of sophistication increases even more as biological systems become more vulnerable, especially when interactions with other biological systems create dynamics that are vital for their maintenance. This is why the level of biological complexity within entire ecosystems culminates at a point that is difficult for humans to fully comprehend.
But what is the biological significance of this complexity? The complexity of biological phenomena often resembles a real spiderweb, where, much like a busy traffic intersection, many paths cross and change direction. By alternating steps in one direction with steps in another, often even opposite direction, it becomes possible to continuously adjust, modulate, and refine biological processes and reactions. This not only leads to a high degree of sophistication, and therefore vulnerability, in vital biological phenomena but also to a high degree of versatility and diversity. It is precisely this variation that is crucial for ensuring that complex, and hence more fragile, biological systems can thrive, even in the face of environmental threats. This is especially true when these threats come from biological agents that have evolved to sustain themselves with a much lower degree of biological complexity and vulnerability. It is precisely the spectrum of variations generated by the complexity of biological processes themselves and the myriads of mutual interactions with other systems that creates a wide range of possibilities for these vulnerable systems to adapt purposefully to threatening environmental factors. This adaptation occurs through the natural selection of the random’ variant(s)’ whose phenotypic complexity confers a competitive fitness advantage in hostile circumstances.
If the responsiveness of autonomously living biological organisms consisted only of a simple black-and-white reaction, then the confrontation with an unfavorable environmental factor would either result in a large-scale extinction of the species or in its widespread survival, ultimately leading to other factors that could also threaten the species' survival (e.g., depletion of food resources or other detrimental consequences of overpopulation). However, when the adaptability of an entire ecosystem—such as an animal population interacting with a pathogenic germ—is undermined by an unnatural, ill-adapted large-scale intervention, the population's capacity to effectively control the pathogen is at risk of gradually declining. It is not difficult to understand that this is particularly true when the 'hostile' environmental threat comes from a viable biological agent that, due to its primitive reproduction strategy, is much less vulnerable because it only needs to survive and reproduce within the safe environment of a host cell, such as a virus. In this way, the roles are reversed, and the more evolved and vulnerable organism gradually loses its ability to adapt, giving the more primitive adversary a strategic advantage.
For those who have followed my insights and predictions regarding the endpoint of the ongoing immune escape pandemic, this precisely explains how mass vaccination against SARS-CoV-2 during the pandemic has actually benefited the virus's adaptation, rather than that of the Covid 19 (C19)-vaccinated population. The systematic breakthrough infections in C19-vaccinated individuals have led to a collective reduction in the immune system’s adaptability (i.e., adaptive immunity) of highly C-19 vaccinated populations, thereby improving the survival chances of the less vulnerable virus. The adaptive capacity of highly C-19 vaccinated populations has eventually been limited to protection against virulence through polyreactive non-neutralizing antibodies (PNNAbs), a protection that we know does not evolve further and cannot prevent the virus from spreading (and thus adapting). Because the evolutionary dynamics of the virus, fueled by nonpharmaceutical infection-prevention measures and mass vaccination, have caused the antigenic stimulus that maintained the concentration of these antibodies to disappear, it seems more than plausible that this human-induced viral dynamics will ultimately pull the emergency brake to prevent the extinction of highly C-19 vaccinated populations. Indeed, if the collective immunological adaptability is restricted to the extent that it prevents the development of herd immunity, then only a higher degree of adaptability of the virus can offer a solution. The evidence of the virus's increasing adaptability has become clearly visible for quite some time, especially since the emergence of Omicron. At the current stage of this immune escape pandemic, the virus's adaptability is only further stimulated by the increasingly reduced capacity of highly C-19 vaccinated populations to control viral spread and replication.
There is no doubt that under the significant but 'misplaced' immune pressure exerted by highly C-19 vaccinated populations, the virus’s unchecked adaptability can only escalate. This escalation could ultimately give the unvaccinated part of these populations the opportunity to establish herd immunity through epigenetic reprogramming (‘adaptation’) of their innate immune cells, thereby breaking the cycle of ongoing immune escape by the virus.
It is also logical that when the robust adaptability of vulnerable biological entities is disrupted, it may take some time before their stability is collectively undermined and the system collapses under hostile pressure. Nevertheless, it’s clear that when small cracks in a dam’s wall begin to expand into larger fissures, the system’s stability deteriorates very rapidly, and thus the metastable system can suddenly collapse, much like a thunderbolt striking out of a clear sky. Such entities will therefore likely be caught off guard…
Together with those who criticize the timeline I predicted for the termination of the C-19 immune escape pandemic, I am amazed by the remarkable resilience of the complex biological system involved. This can only mean that the ability of our mammalian immune system to collectively adapt to far less biologically complex pathogens is truly spectacular and shows a resilience of unprecedented magnitude. Such remarkable resilience can only be destroyed by large-scale, unnatural and thoughtless immune interventions, but not in a way that nature has not provided a contingency plan to ensure the survival of our species..."

The FDA Just Approved an Updated Vaccines, and Officials Say Paxlovid is Still Effective in Preventing Severe Cases.

The summer of 2024—the fifth since the COVID-19 pandemic began—is projected to be the biggest summer wave of COVID infections to date.
Since early May, COVID infections have steadily increased in the United States, Europe, Singapore, New Zealand, and Australia. The U.S. Centers for Disease Control and Prevention estimates that COVID-19 infections are currently increasing in 25 states based on data from emergency department visits. However, hospitalizations and deaths from COVID remain at their lowest levels.
Now, the U.S. Food and Drug Administration has approved updated vaccines to protect against current variants of the virus.
This recent surge has been driven mainly by a new group of closely related Covid subvariants, known collectively as "FLiRT."
As the summer winds down, students across the U.S. will return to school. Traditionally, this also coincides with the season of respiratory viruses, such as flu, RSV, and increasingly COVID.
"Not sure what will happen this fall and winter," says Kei Sato, a virologist at the University of Tokyo. While the FLiRT variants are likely to keep evolving after summer, entirely new subvariants cannot be ruled out. "An Omicron-like event” seems to have occurred every year in the fall since 2021, says Sato.
Here's what you need to know about the new variants and the new vaccines.
What Are FLiRT Variants?
The "FLiRT" variant family includes the majority of currently circulating variants, identified with the letters KP, JN, and the variant LB.1.
The unofficial name "FLiRT" is an acronym for a set of mutations on the spike protein of SARS-CoV-2, the virus that causes COVID-19. The virus uses spike protein to bind with ACE2 receptors in our nose and lung cells to cause infection.
All proteins are made up of amino acids that string together like beads. Mutations can change one amino acid to another, thereby altering the behavior of the protein and making the virus more or less infectious, or able to dodge immunity.
The FLiRT subvariant family members are descended from the JN.1 variant that was dominant in the U.S. in early 2024. JN.1 itself was highly unusual because it acquired 41 mutations that differentiated it from Omicron XBB.1.5, which is the variant upon which the current bivalent COVID booster is based.
Should You Get The New Vaccines?
The two updated mRNA vaccines, manufactured by Pfizer-BioNTech and Moderna, target a FLiRT variant called KP.2. Anyone over the age of 12 can get the new shots, as long as they haven't received a booster in the last two months.
“Vaccination continues to be the cornerstone of COVID-19 prevention,” Peter Marks, director of the FDA’s Center for Biologics Evaluation and Research said in a statement.
Another vaccine targeting the variant JN.1 and manufactured by Novovax is also under review and could be approved soon.
Previous research also showed that older vaccines based on XBB.1.5, an earlier subvariant of Omicron, were still effective in preventing severe COVID-19. While this vaccine produces antibodies that still target the FLiRT variants, the efficiency is notably reduced. A recent infection from the JN.1 variant also seems to provide strong protection against all the FLiRT variants.
That said, the CDC recommends that everyone six months and older get a COVID vaccine. Those at high risk for serious COVID-19 should get vaccinated with the most recent versions available.
How Alarming Are FLiRT Variants?
Coronaviruses, such as SARS-CoV-2, frequently mutate to avoid recognition by antibodies. The two FLiRT mutations remove the sites on the virus where antibodies bind the SARS-CoV-2 virus.
Additional mutations on the FLiRT variants can either help the virus bind more efficiently to ACE2 receptors making it more infectious, help it evade previous immunity, or both, says Adrian Esterman, an epidemiologist at the University of South Australia, Allied Health & Human Performance in Adelaide, Australia
Early studies show that all existing FLiRT subvariants are very good at dodging previous immunity acquired through multiple COVID vaccinations—including the most recent COVID bivalent booster—or a breakthrough infection from a previous strain of Omicron.
But the good news is that by escaping the antibodies, the FLiRT variants have also seem to lost some ability to infect their target because the virus needed the original antibody-binding sites to bind the ACE2 receptor and enter cells.
"These variants are not yet particularly concerning, even with the new mutations that affect certain aspects of the virus's biology," says Shan-Lu Liu, a virologist at the Ohio State University.
It is common for viruses to acquire mutations that help them dodge immunity, which can affect their ability to infect cells, says Liu. "The viruses can quickly evolve new mutations to restore their infectivity."
But in the meantime, Sato thinks that waning immunity from previous vaccinations and infections, coupled with the FLiRT variant's ability to dodge remaining immunity, are probably the main reason for the recent surge in infections.
Liu also agrees that the currently rising numbers of COVID infections are mostly due to low booster uptake and increased summer travel.
Are COVID Medicines Still Effective?
Emergency department visits, hospitalizations, and deaths have all spiked during this summer but are still much lower compared to earlier waves of the pandemic.
There is no indication that these new FLiRT variants are more dangerous than other Omicron strains.
A study shows that Paxlovid is still effective against FLiRT variants. Other antiviral drugs such as molnupiravir and remdesivir are also expected to work since their mechanism of action is not affected by mutations in the spike protein.

New COVID vaccines have arrived just as a vaccine equity program is ending - Published Aug 28, 2024
This article hits the nail on the the head: “We know the easier we make these vaccines, the more accessible these vaccines are, the more people will get them,” Jetelina said. “And they are expensive, especially for uninsured people; the whole access issue is a major challenge.”
Last week, the Food and Drug Administration approved two COVID-19 vaccines earlier than expected due to record-breaking surges of the virus. Instead of releasing them in the fall, following the pattern of previous coronavirus vaccine roll outs, they are available as early as this week. However, for some people, time is already running out to get the updated shots.
For the nearly 27 million adults who do not have health insurance, the new coronavirus vaccines will only be available for free until August 31. That’s because the Center for Disease Control and Prevention’s Bridge Access Program, which provided free coronavirus vaccines to uninsured adults, will end. Over the last year, the program provided free vaccines for nearly 1.5 million people since it launched in September 2023. Without insurance, the vaccine can cost at least $115.
“Uninsured adults are going to have more difficulty accessing the vaccines this fall,” Dr. Kelly Moore, president and CEO of Immunize.org, a nonprofit organization that works to increase vaccination rates and is funded by the CDC, told Salon. “Budget cuts on the federal government side have eliminated that program, and the amount of money available to purchase the COVID vaccine and offer it to people who are uninsured is quite limited.”
While the program was meant to be temporary, the original termination date for the program was slotted for December 2024. However, the fiscal 2024 government funding bill rescinded $4.3 billion in COVID-19 funding, some of which was being used for the program, as reported by The Hill in May. After August 31 of this year, local health departments might have a small amount of free vaccine available, but supply is expected to be limited for those without health insurance. While the Biden administration advocated for a more permanent solution in 2025, Congress hasn’t agreed to appropriate the funds.
“We hoped that the Bridge Access Program would be a bridge to a more permanent solution, a safety net program that would ensure affordable access to recommended vaccines for uninsured adults,” Moore said. “But unfortunately, it's turned out to be a bridge to nowhere.”
The ending of the program comes at a time when public health officials will be tasked with the assignment of getting vaccines into more peoples’ arms. The optics of last year’s vaccine roll-out was chaos, and not as many people received the vaccines as health officials hoped. Only about 28% of Americans received updated shots last year, a decline from 69% when the first round of vaccines were released.
In a statement to Salon, a spokesperson for the CDC said the Department of Health and Human Services (HHS) has started a campaign called "Risk Less. Do More." to increase uptake of vaccines, including for COVID. The public health agency said it is also "providing additional resources to support access to COVID vaccines this fall and winter season to serve needs of the most vulnerable populations, including uninsured adults, farmworkers, tribal populations and others."
"Much of that support will be provided to the 64 state and local public health departments through their immunization programs," the agency said.
This year, the two new vaccines are updated versions of the mRNA vaccines produced by Moderna and Pfizer-BioNTech, They are specifically designed to vaccinate people against the KP.2 strain, which has been driving a large proportion of infections this summer. For the last several weeks, related but different strains KP.3 and KP.3.1.1 have been the ones most responsible for infections lately. The vaccines still offer cross-protection against these variants, but it's another indicator that this virus will always be changing, evolving new ways of evading our immunity.

“They have an updated vaccine formula, and that's a good thing,” Katelyn Jetelina, an epidemiologist and author of the newsletter Your Local Epidemiologist, told Salon. “This virus mutates incredibly quickly, and so we need to patch up our immunity walls to make sure we can recognize this vaccine or recognize the virus as it changes, and that is one driving factor why we have updated vaccines every fall.”
The vaccines are recommended for everyone six months and older, with a few exceptions. If you’ve had a recent COVID-19 infection, officials recommend waiting two to three months to get the vaccine. For everyone five years old and older, even if they’ve never been vaccinated against the coronavirus before, these two new vaccines only require one shot. Anyone younger than five may have multiple doses depending on whether they’ve been vaccinated before or not and what vaccine they received.

“I'm curious if that will move the needle, but I will say I'm not holding my breath,” she said. “I don't think a lot of other things have changed since last year, people are still tired of hearing about COVID, there's still a lot of confusing information out there, and I don't know how many minds we can really, truly change from season to season.”
Jetelina said she is also concerned about the Bridge Access Program ending.
“We know the easier we make these vaccines, the more accessible these vaccines are, the more people will get them,” Jetelina said. “And they are expensive, especially for uninsured people; the whole access issue is a major challenge.”
