BEST OF THE BI-MONTH
The Wager by Donna Jo Napoli (2010): Don Giovanni de la Fortuna, a nineteen-year-old nobleman in medieval Sicily, loses his entire fortune to a tidal wave and soon finds himself on the brink of starvation. That’s when the Devil comes knocking with an offer: endless money for the rest of his life if he doesn’t bathe, cut his hair, shave, or change his clothes for three years, three months, and three days. This is a retelling of a lesser-known Sicilian fairy tale and, next to the sublime Breath, it’s Napoli’s best work. Instead of taking the easy route of making Don Giovanni a stupid brat who learns to be nicer and more frugal, she complicates things by making him sweet and resourceful from the beginning, as well as callow and somewhat thoughtless. (His first action after seeing the damage wrought by the tidal wave is to go out and help bury the dead for three straight days.) This makes the message of the book more powerful; if someone deep-down good and intelligent can stand to think more about others and help the less fortunate, then clearly that lesson applies to everyone, not just the worst sort of rich people. Don Giovanni’s unprocessed grief over his long-dead parents and longing for human connection are also very affecting.
WORST OF THE BI-MONTH
Spinners by Donna Jo Napoli and Richard Tchen (1999): In medieval-ish Scotland, a poor tailor longs to marry his sweetheart, a spinner, but her father will only consent if the tailor can show he’ll be a good provider. The tailor tries to make a dress that appears to be made of gold and succeeds; however, he still loses his sweetheart to a rich miller and his health to a magic spinning wheel (as one does). Years later, the sweetheart’s daughter, now a skilled spinner in her own right, finds herself in trouble when a king gets the wrong impression about her being able to spin straw into gold. File this one under “cool idea, half-assed execution.” After a certain point, Napoli seems to run out of her own ideas and just follows “Rumpelstiltskin” to its original conclusion. This wouldn’t be great for any fairy-tale retelling, but the ludicrous “Rumpelstiltskin” needs more reworking than most. Also, the tailor’s sweetheart is such an ableist tool! I’d get it if she chose the rich miller out of concern for financial security, but she just dumps the tailor because the magic spinning wheel basically gave him a supernatural stroke and she thinks it made him evil? You can do better, baby!
REST OF THE BI-MONTH
The Cartoonist by Betsy Byars (1978): Alfie Mason, a quiet eleven-year-old, takes refuge from his unhappy family in the tiny attic of his ramshackle house, drawing faintly absurd cartoons. Then his ne’er-do-well older brother Bubba loses his job, prompting a way-too-excited Mrs. Mason to decide to renovate the attic into a bedroom...so Alfie barricades himself in the attic and throws the family into chaos without saying a word. I first read this book when I was eleven, and even then I found it deeply upsetting. Mrs. Mason seems incapable of seeing anyone but Bubba as a full human being, and she never regrets hurting Alfie or her daughter Alma in order to benefit her eldest. The best Alfie and Alma can do is call her out on it--Alfie through his silent protest, Alma by finally standing up for herself and her little brother--and try to move on. It’s certainly an unvarnished message for a middle-grade novel, but it’s not a bad one, given that some parents are just like that.
Shrill by Lindy West (2016): In this memoir, Lindy West reflects on her personal experiences with fatphobia, the general strangeness of having a human body, abortion, the ethics of comedy, and Internet trolls, among other subjects. This book was genuinely inspiring and amusing to me at a time when I greatly needed a lot of confidence and some laughs, and for that I am eternally grateful. The humor can feel very social-media-circa-2015, but there are worse things than a book capturing a specific moment.
Cracker Jackson by Betsy Byars (1985): Eleven-year-old “Cracker” Jackson Hunter realizes that Alma, his beloved former babysitter, is being physically abused by her husband. Even though his divorced parents forbid it and Alma herself warns him against angering her husband, he tries his best to help her, with mixed results. By all rights, this middle-grade novel should be a tonal mess--Jackson and his best friend Goat get involved in some legit Wacky Schemes--but instead it’s a moving portrait of a kid who has to deal with gut-wrenching adult realities while also navigating sixth-grade drama. I also loved Jackson’s three parental figures. They’re all flawed--Jackson’s mom is a worrywart about stuff that doesn’t matter, his dad can’t hold a conversation with him without lapsing into Dracula impressions, and Alma sometimes treats him more like a peer than a kid--but they all clearly care about him and try to make things okay.
Not the Duke’s Darling by Elizabeth Hoyt (2018): Years ago, a horrific murder and a dubious attempt at revenge tore apart the lives of Christopher Renshawe and Lady Freya de Moray. Now he’s a widowed duke with severe claustrophobia and a blackmailer on his case, while she’s an undercover spy for a secret society of Scottish witches who help women. (Awesome.) (Also some of them are lesbians.) When they end up at the same house party, she vows to keep hating him for wronging her family, but does that last long? No, because they’re reasonably good at communicating and can appreciate each other’s goals! This spooky Georgian romance didn’t knock my socks off, but it’s a good start to Hoyt’s new Greycourt series and it has a light touch with the serious issues it handles.
Mrs. Martin’s Incomparable Adventure by Courtney Milan (2019): Violetta Beauchamps, a sixty-nine-year-old* bookkeeper, is cheated out of her pension by her landlord boss. In desperation, she hatches her own retirement plan: swindling Bertrice Martin, a wealthy seventy-three-year-old widow, by pretending to be her insolvent nephew’s landlady. Bertrice has refused to pay her nephew’s debts on principle, but she’s willing to make an exception if Violetta will help pester him into vacating his lodgings. Shenanigans and old-lady romance ensue. This mid-Victorian-set romance novella is like an ambiguous image (for example: that picture that’s either a vase or two faces in profile). Look at it as the tale of two L.M.-Montgomery-style elderly women falling in love, and it’s delightful; look at it for deep social commentary, and it’s pretty simplistic and sometimes even callous. I enjoyed it, but it only works on certain levels.
Summer of the Swans by Betsy Byars (1970): Lately, fourteen-year-old Sara Godfrey has been feeling awkward and out of charity with everyone: her absentee father, her plainspoken aunt, her beautiful older sister, the other kids at school, and even her little brother Charlie, who has been mostly nonverbal and easily disoriented since sustaining serious brain damage during a childhood illness. When Charlie goes missing in the night, though, her only thought is to find him. Despite loving Byars, I avoided this Newberry winner as a kid because it looked kind of boring. It is a little sedate in a classic-American-coming-of-age-story way--part “The Scarlet Ibis,” part Judy Blume--but I still loved Sara, who is always ready to throw down, and I found the depiction of Charlie to be surprisingly sensitive for the time. (The language is outdated, but the passages from Charlie’s POV aren’t condescending, plus he isn’t killed off, as I initially feared.) The descriptions of the coal-ravaged West Virginia countryside are also very evocative.
The TV Kid by Betsy Byars (1974): Lenny, a preteen living with his single mom at the kitschy Kentucky motel she owns, struggles in school and has no friends. (His family moves around a lot and he probably has a learning disability.) He has two sources of solace: watching TV and sneaking into the abandoned lake houses in his neighborhood. One day, though, his favorite hobbies get him into trouble. This was one of my favorite Byars books as a kid, even though I was not familiar with the TV landscape of 1974. I liked it a little less this time, but not because it was dated; instead, I was disconcerted by how pro-getting-bitten-by-a-rattlesnake it is. Also, a significant portion of the story is devoted to a child suffering horrible pain from a snakebite, which is harder to take as an adult reader. Still, it’s got some of that classic Byars melancholy.
The Cybil War by Betsy Byars (1981): Eleven-year-old Simon has had a crush on his classmate Cybil for years, because she does awesome stuff like advocate for more active roles for girls in the yearly school pageants. He’s not inspired to act on his feelings, though, until his awful best friend Tony decides he likes Cybil and starts talking shit to her about Simon. There’s a lot to like about this book. Cybil, with her nonchalant confidence and kindness, is a wonderful character, and Simon’s thorough admiration for her is adorable. I also like how Byars ties Simon’s complicated feelings about his deadbeat dad to his efforts to navigate small-scale fifth-grade drama; both weigh heavily on him, and Byars is never condescending about this. Yet the book’s not Byars’s best, mostly because of the lack of generosity towards Cybil’s fat friend Harriet and, to a lesser extent, Tony.
Sex and Violence by Carrie Mesrobian (2013): Seventeen-year-old Evan doesn’t do serious relationships, instead preferring to hook up with girls and ghost them when he starts having feels. (His family moves around a lot and he’s got some trauma.) Then one girl’s jealous ex orchestrates a horrific assault on them both, leading Evan’s distant widowed dad to take his traumatized son back to their Minnesota hometown. It turns out okay. I liked this novel a lot more once I accepted it as an intentionally messy coming-of-age novel, rather than an issue novel...but it was still a little too messy for its own good. I felt like I was supposed to condemn Evan for having casual sex, something that’s both morally neutral and natural enough for a teen who moves every year, yet the narrative all but endorses his contempt for lower-class girls. I was also uncomfortable with the revelation that Evan was a survivor of statutory rape. It seemed like he was being punished by the narrative only for hyper-sexuality that clearly stemmed from trauma--with a physical assault with some strong sexual implications, no less--but let off the hook for his thoughtless middle-class-boy prejudices. I did feel for him, though, and that carried me through most of the book.
October Wind by Susan Wiggs (1991): In late-fifteenth-century Spain, Cristóbal Colón (aka Christopher Columbus) tries to convince Queen Isabella to fund a westward expedition. Meanwhile, nobleman Joseph Sarmiento learns an enormous secret about his background and must decide whether to alter the course of his life. During this time, Rafael Viscaino, a young scribe, strives to rise in the world while his friends, aspiring doctor Catalina and cheerful but troubled half-Roma Santiago, have their own struggles. This historical novel (which just barely qualifies as a romance) has a lot of potential, but it wastes too much time on Columbus and Isabella, plus it gives them more credit than they deserve. Wiggs should’ve focused on Joseph, the sexiest and most likable character, and made more of his eventual relationship with Anacaona, a Guanahani woman. Or else she should’ve just made it a poly romance with Rafael/Catalina/Santiago, which she comes this close to doing.
Love’s Willing Servant by Avis Worthington (1980): Left penniless by her father and betrayed by her childhood sweetheart, Lettice Clifford decides to take herself to her sister’s home in colonial Virginia and get a rich husband. She’s surprised to find herself sharing a ship with Geoffrey Finch, a neighbor who has been betrayed by his evil twin and sold into indentured servitude. When his indenture ends up getting bought by her brother-in-law, they grow closer, but multiple creepy people and Bacon’s Rebellion threaten their love. I didn’t care much for Lettice, though, because she’s usually either boring or kind of a dick.

Trump’s announcement Friday, on Twitter, that he and first lady Melania Trump tested positive for the coronavirus, and the deep uncertainty that accompanied it, permeated the global news cycle, upending countless plans and sparking comment everywhere from presidential offices to the thousands looking to weigh in on social media.
The positive test reading for the leader of the world’s largest economy adds more uncertainty to investors’ worries, including, most prominently, how the infection might affect the Nov. 3 election between Trump, a Republican, and Democrat Joe Biden. U.S. stock futures and Asian shares fell in the wake of the news. The future contracts for both the S&P 500 and the Dow industrials lost 1.9%. Oil prices also slipped. Stock prices in Japan and Australia tumbled.
“To say this potentially could be a big deal is an understatement,” Rabobank said in a commentary. “Anyway, everything now takes a backseat to the latest incredible twist in this U.S. election campaign.”
World leaders and officials were quick to weigh in, and there was both sympathy and something approaching schadenfreude.
“Wishing my friend @POTUS @realDonaldTrump and @FLOTUS a quick recovery and good health,” Indian Prime Minister Narendra Modi tweeted.
U.S.-India ties have prospered under Trump, and India is seen as a partner to balance China’s growing weight in Asia.
Italian right-wing opposition leader Matteo Salvini tweeted: “In Italy and in the world, whoever celebrates the illness of a man or of a woman, and who comes to wish the death of a neighbor, confirms what he is: An idiot without soul. A hug to Melania and Donald.″
British Prime Minister Boris Johnson, who was hospitalized for a week in April after he contracted COVID-19, wished Trump a “speedy recovery.”
“Our best wishes go to the president and the first lady, but it demonstrates that no one is immune from COVID-19 and catching it. So it shows that no matter the precautions, we are all susceptible to this,” Australian Agriculture Minister David Littleproud, deputy leader of the conservative Nationals party, said on Australian Broadcasting Corp. TV. “A trying time, and it just goes to show that a global pandemic can in fact touch anybody, even the president of the United States.”
Tokyo Gov. Yuriko Koike, speaking at a weekly news conference, did not mention Trump’s reluctance to wear masks when asked about his infection, but she said the news “reminded me of how widely masks are worn in Japan.”
Major media across the globe also played up the announcement, with bulletins crawling across TV screens in Seoul, Tokyo, Taipei, Dubai and Beijing.
China’s official Xinhua News Agency flashed the news, and an anchor on state broadcaster CCTV announced it; there was no immediate comment from the government Friday, the second day of an eight-day national holiday.
The positive test result for Trump and his wife was the most searched topic in China on the widely used social media app Weibo a few hours after the announcement, with most comments mocking or critical.
One user darkly joked that Trump had finally tweeted something positive.
The Chinese government has bristled at Trump’s attempts to blame China, where the disease emerged, for the pandemic and called for global cooperation in fighting it, a message that has resonated with the public.
Hu Xijin, the outspoken editor of the state-owned Global Times newspaper, tweeted in English that “President Trump and the first lady have paid the price for his gamble to play down the COVID-19.”
Multiple Arab news media outlets on Friday continuously broadcast footage of Trump and his wife after the virus announcement.
Al-Arabiya, a Saudi-owned satellite channel based in Dubai, cut to a long live shot of the White House. Qatari state-owned Al Jazeera channel brought in four commentators to discuss the “prevailing state of uncertainty” in the United States, questioning whether Trump could effectively steer a reelection campaign and run the country from quarantine.
Iranian state television announced Trump had the virus, an anchor breaking the news with an unflattering image of the U.S. president surrounded by what appeared to be giant coronaviruses. U.S.-Iran ties have suffered since Trump unilaterally withdrew the United States from Tehran’s nuclear deal with world powers and reimposed crushing sanctions.
Social media platforms were ablaze with quick reaction.
Would Trump blame the Chinese? Would he thumb his nose at his critics and enemies by breezing through the quarantine without serious symptoms, tweeting away from the White House? Would he become gravely ill, or worse, and, if he did, what would that mean for the U.S. election, one of the most contentious in recent history?
While the uncertainty seemed palpable on a scroll through various nations’ social media, many of the comments seemed to revel in the announcement.
“Here comes a chance for him to actually try out his idea of injecting disinfectant into himself and fighting back (against allegations that) it was fake news!” tweeted Hiroyuki Nishimura, a Japanese internet entrepreneur, referring to an idea Trump floated earlier this year for treatment.
Keio University economics professor Masaru Kaneko tweeted that populist leaders like British Prime Minister Boris Johnson and Brazilian President Jair Bolsonaro “got infected because they tended not to take the coronavirus seriously. The two other leaders seriously tackled (the virus) after they got infected themselves. Will the United States follow their examples?”
And the satirical Australian news site, the Betoota Advocate, posted a story with this headline: “Trump Family Records More Cases Of Community Transmission Than Entire State Of Queensland.”

Breonna Taylor’s Family Agrees To A $12 million Settlement With The City Of Louisville

After months of Black Lives Matter protests across the country and even the world,

Taylor’s boyfriend thought that they were intruders and fired on one of them. This caused the officers to come down on them with several bullets, leaving Breonna Taylor dead. She was an emergency room technician who got tangled into an ex-boyfriend’s drug crimes even after she was moving on from it. The police found no illegal substances in her home [1].
There had been protests everywhere in the country for around 6 months, demanding justice for Breonna Taylor and other black lives who had been wrongfully killed like her at the hands of police violence. Mayor Greg Fischer of Louisville expressed his outrage at the event as well and publicly stated how his “administration is not waiting to move ahead with needed reforms to prevent a tragedy like this from ever happening again,” [1].
The mayor did stand by this statement as the city did enact numerous changes to how their police conduct itself. The policing changes “require more oversight by top commanders, and make mandatory safeguards that were common practice in the department but were not followed the night of the March 13 raid. They would also put in place an early warning system to flag officers who have been accused of excessive force.” [1]. The city also introduced Breonna’s Law which banned no knock search warrants, the kind of warrant the officers had on the night that Breonna Taylor was killed. These changes combined with the $12 million reached in the settlement makes this a landmark event that you don’t see very often.
The protesters were demanding that the officers who shot Breonna Taylor be arrested, the fact that they weren’t is what contributed greatly to the outrage of concerned protesters around the country. However, there are complications regarding the behavior of the police officers. People want the officers to face criminal charges but they are protected under Kentucky’s statute with their ability to use lethal force if it is in self defense. The officers were shot at before they started shooting so legally it can be argued that they were within their right to shoot, even if they may have been shooting an excessive and unnecessary amount of times [1]. If there were charges brought against the officers they likely wouldn’t result in conviction for two out of the three officers, the third officer was not standing directly in front of Breonna Taylor’s boyfriend when he fired, was seen to be shooting recklessly, and so has been fired from his position [1]. This does not sit well with many protesters because they see this incident as the murder of an innocent person without any real consequences. This event is seen by citizens as even more horrific because it is believed that no legal action is being taken against the officers due to Breonna Taylor being a black woman, and there have been patterns of police brutality targeted at people of color in the United States for centuries.
Critics of this enormous settlement have been saying that the the city of Louisville settled far too soon in this case and that they did not stand up for the police officers, that they stood “idly by as a protest leader accused officers of murder during a press conference Tuesday” [2].
Officers at the conference were saying that using the term murder is very premature seeing as a proper investigation of the case has not yet been completed. They are also critical of Louisville Mayor Greg Fischer because he allowed a freedom activist to speak at the conference and call the police department “corrupt”. They were especially annoyed because the mayor was getting praise when he technically is in charge of the police department [2]. They feel that they are not being supported by the city in this matter.
The movement has been very charged and emotional because citizens feel that they are not being supported when certain police officers use excessive force against people of color and rarely face legal consequences. Breonna Taylor’s name has become a symbol for the Black Lives Matter movement and attached to her name is outcry for all of the black individuals that were seen as wrongfully killed at the hands of the police. This settlement is a step in the right direction for the movement but advocates still demand more justice.

As millions across the globe adjust to a new way of living and working, separated from family, friends and co-workers, our reliance on satellite services for connectivity and data has become even more critical. The coronavirus pandemic has made clear the consequences of being ill-prepared and failing to take preventive measures, despite warnings from experts about the inevitability of a global pandemic. Unfortunately, a similar scenario is playing out in space. As orbits become more densely populated, and conjunctions and near misses more likely, we need to act now to protect space assets and their orbital neighborhoods.
Orbital neighborhoods – from LEO to MEO to GEO – are changing. Once the purview of government agencies and multinational telecom corporations, orbital regimes are going through a period of uncertainty and transition. Large constellations, falling launch and satellite development costs, and innovative concepts in technology and service are changing how these “neighborhoods” are being used and the way in which they bring benefits to the global population. In order to protect this environment from an epidemic, we must advance the technical standards and policy initiatives that support this growth.
KEEP OUR ORBITAL NEIGHBORHOOD CLEAN
As with any new development, numerous adjacent services are sprouting up to provide support to the satellite residents in these orbital communities. One path leaves us operating in an environment of constant concern and uncertainty and the other builds a thriving and prosperous orbital community.
We have been here before — terrestrially — and are learning the lessons of sustainability on Earth. Corporations are seeing the economic and societal value of sustainable communities — not just for environmental responsibility and the benefit of future generations, but for the near-term bottom line. Companies do not focus on environmental sustainability as a nice-to-have cost center for corporate social responsibility; it is a necessity for steady business growth and economic viability.
AWARENESS OF DEBRIS RISKS ARE GROWING
We still have a long way to go before we reach an equivalent level of environmental preparedness in orbit.
The bad news: Though we have not even really begun to launch the bulk of assets that will be put into orbit over the next decade, the number of conjunctions and near misses continues to grow. As the capability and interest in space situational awareness increases, our eyes are being opened to the incredible risk of a crowded and unpredictable orbital environment. Most concerning is the number of debris-on-debris near misses that are more prevalent. Whether those near misses were always happening and we were just blissfully ignorant before the advent of better SSA and transparency, or this is truly an uptick in frequency, is irrelevant. It is only a matter of time before one of these near misses becomes more than just a close call that we casually discuss at conferences, and instead becomes a collision that has irrevocably altered how we live and work on Earth and in our orbital neighborhood.
The good news: Operators and policymakers recognize that a collision would be disastrous for our nascent community and seriously hinder future economic growth. We are starting to see a growing level of awareness around solving the technical and policy challenges that will need to be addressed in order to create a sustainable market. Space situational awareness products and services are being refined to serve the new orbital neighborhood. Technologies like RFID tags and drag sails are being tested to support identification and hasten deorbit time frames. And, there are now several concepts and demonstrations of remediation technology. Later this year, Astroscale will conduct the first fully commercial debris removal demonstration with our ELSA-d mission.

Industry is also starting to coalesce around needed practices that go above regulatory and international minimum guidelines. The Consortium for Execution and Rendezvous of Servicing Operations (CONFERS), an industry-led group consisting of company representatives from around the world that are practicing rendezvous and proximity operations, has been driving the conversation on best practices and standards for future orbital activities since the summer of 2018. The Space Safety Coalition announced in 2019 a set of best practices for the sustainability of space operations that included, for example, recommendations for exchanging information and increasing successful disposal after the end of mission. The coalition has also set a goal for operators with propulsion years of end of mission, as compared to the international guideline of 25 years.
Governments and international organizations, too, are starting to take action to build a modern, sustainable, and global orbital neighborhood. Last year, the UN COPUOS’ long-term sustainability guidelines were officially adopted to help ensure the long-term sustainable use of outer space and debris mitigation standards were updated in the International Standards Organization. Individual states are also taking steps to implement stronger regulations. The United States developed Space Policy Directive-3 (SPD-3) in 2019 which has led to updated Orbital Debris Mitigation Standard Practices. Japan is advocating updates to space debris mitigation policies and the new European Operations Framework/Peraspera is developing recommendations on regulations, licensing and standards for in-orbit servicing. From a technical and policy perspective the world is taking notice of orbital sustainability — and taking action.
MOVING FORWARD, SUSTAINABLY
All of these efforts are contributing to the maintenance of healthy and prosperous orbits. As we consider our future efforts to be good orbital neighbors, we should implement three key actions: Prepare, Repair and Remove.
1. Prepare – Integrating simple design changes prior to launch, like adding a grapple fixture or docking plate, make it easier (and thus less risky and expensive) to conduct servicing missions. OneWeb set the standard for responsible utilization of our orbital environment by including docking mechanisms on all of the satellites it launched, with plans to include a different mechanism on all future launches. The news about OneWeb’s uncertain future should not diminish the important example it was setting for orbital sustainability.
2. Repair — The space community now has more options at hand to service and keep healthy orbital investments. In February 2020, Northrop Grumman’s Mission Extension Vehicle (MEV-1) docked with Intelsat-901, the first docking of two satellites in GEO and the first time anywhere that this happened with a satellite that was not previously designed for a docking. 3. Remove — We must think seriously about remediating the space environment of the most dangerous and massive pieces of space junk. In the last few months, the Japan Aerospace Exploration Agency and European Space Agency both announced missions focused on the eventual removal of a piece of existing debris from orbit. In each case companies have been selected, Astroscale in the former and ClearSpace in the later, to take the first steps toward groundbreaking missions that would actively deorbit a large piece of debris in the next several years.
It is an honor to be a part of an industry that is continuing to connect the world, supporting society and essential operations during this critical time. The importance of prevention has never been more apparent. The benefits of satellites in Earth’s orbit are clear, but the risks of debris are real. Let’s expedite the development of the technology and policies that will encourage us to prepare our satellites for servicing before launch, repair them when possible and remove them when no longer useful. Our orbital neighborhoods are too precious of a resource to ignore and these steps are necessary to safeguard this industry’s future.

You’re probably familiar with the term “Medicare for All.” Maybe you’ve heard Bernie Sanders or Elizabeth Warren talk about it. But do you really know what it means? What it could mean for you and your family? Medicare for All has been shoved front and center as public support for universal health care surges amid the COVID-19 pandemic. In the last two weeks, millions of Americans have lost their jobs—and with it their health insurance.
“The weaknesses in our system have been exposed, the fact that our system depends on being insured and being able to afford it,” says Marcia Angell, MD, a physician, former editor in chief of the New England Journal of Medicine, and corresponding member of the faculty of global health and social medicine at Harvard Medical School.
As of Thursday, April 2, worldwide detections of the virus had approached 1 million with dwindling supplies available for treatment. And as doctors and nurses put their lives on the line to treat not only patients sick for coronavirus but patients with other health problems, the idea of our current healthcare system being the best healthcare system is eroding far and wide.

Donald Moore, MD, a primary care physician and longtime Medicare for All advocate, says patients would benefit most from a Medicare for All system. He believes this because for the past five years he hasn’t accepted commercial insurance.
“So many people think that they want to hold on to their insurance, that what they have is as good as it gets, that they can’t do any better,” says Dr. Moore, who has an independent practice in Brooklyn. “I can tell you as a doctor, I know you could do better because I’m the one who used to take the bad insurance that you have.”
A common critique of Medicare for All is that such an ambitious and costly program is too much for the federal government to handle. But experts like Dr. Moore and Dr. Angell say Medicare provides precedent enough to show it’s well within our capabilities as a nation. Medicare for All is an adaptation of the United States’ current Medicare system. Medicare is a single-payer (that payer is the government) health care system that everyone, regardless of income, becomes eligible for when they turn 65. Younger people with disabilities, and people with end-stage renal disease are also eligible. Though the current form of Medicare would need some tweaking before it became Medicare for All (buh-bye, co-pays and deductibles), Dr. Moore says it’s already a great model for what health care looks like when it reaches huge swaths of the population. According to November 2019 data, 61.2 million people use Medicare each month.

The reason Medicare for All is the most popular part of the healthcare system is that the benefits cannot be cut and the package is the same for everyone, Dr. Angell says, no matter how much you need and how much you use. This, in particular, is relevant given the fact that jobless claims soared to a record 6.6 million in the last week in March, meaning many whose employers provided their health insurance are no longer insured or have to pay out of pocket for insurance plans.
There are other options, though. COBRA, the Consolidated Omnibus Budget Reconciliation Act, is a way to keep the insurance plan you were on with your employer, albeit paying for it entirely by yourself, which is likely $600 or more per month. Recent laws have confirmed uninsured people won’t have to pay out of pocket for COVID-19 testing, and the Affordable Care Act or medicaid (a state and federal program, somewhat like medicare) can help insure unemployed people if they are of a certain income level, though every state handles medicaid differently. While COVID-19 testing and treatment is of obvious concern, other healthcare treatment can’t be forgotten.
Outside of COVID-19, medical costs can be a huge burden, with research from a 2019 study in the American Journal of Public Health showing that more than 60 percent of people who file for bankruptcy do so because of medical debt.
“As healthcare costs increase, all businesses, including all big businesses, are saying, ‘We can’t afford health insurance, you need a better system,’” says Dr. Moore. “Big businesses can’t afford it, the city can’t afford it, and definitely the small business and the individual can’t afford it. As a society, we can afford it, as a country we can afford it—and most countries do afford it.”
More than 60 percent of people who file for bankruptcy do so because of medical debt.
Dr. Angell says the United States spends about 2.5 times the average per capita as compared to other member countries of the Organisation for Economic Co-operation and Development (OECD)—36 of the wealthiest countries in the world. At the same time as we spend more money, we get much less for it.
“Without [Medicare for All], not everyone is covered, you have to be in the private sector, you pay growing deductibles copayments and employers are trying to get out of it by not covering the inflation,” says Dr. Angell. “Medicare for All is much cheaper and you get more. It can’t be tailored according to how often you get sick or if you have a chronic illness.” It’s also infinitely more efficient, she says.
Under the current system in the United States, with such high costs for health care our of pocket, it’s easy to wonder: where does all the money go? “We have fewer hospital beds, fewer doctors and nurses than other countries, and we provide more outpatient tests and procedures like MRIs because these are the money makers, so when you add in a pandemic, we are just not prepared for it,” says Dr. Angell.
A 2018 survey from the American Hospital Association showed that hospitals in the U.S. have about 2.8 beds per 1,000 people, compared with other countries: Italy has 3.2 beds per 1,000 people, China has 4.3 beds per 1,000 people, France has 6.5 per 1,000, and South Korea has 12 beds for every 1,000 people.
Healthcare in the U.S. is treated like a market commodity instead of like a public service, says Dr. Angell, where all the pieces are working more or less independently of one another and often in competition.
“There’s a chain that starts with employers that hire insurance companies and they decide what people are going to pay and who will be covered, and they have marketing departments and billing agencies and all kinds of satellite businesses feeding at the same trough as the healthcare dollar makes its way from the employers through the insurance through multiple businesses trying to make part of that dollar for their overhead and other costs,” says Dr. Angell. “Each level as you go along, money is skimmed off the top until it’s hard to say what amount actually goes to the provider for health care.”
So what’s the upside here? You might be wondering if there is one. At the very least, the current pandemic has put the fragility of the healthcare system on display, one where those who can afford to get treatment get it. Case in point: rich celebrities touted on social media that they had somehow secured access to COVID-19 tests while the rest of the country wondered why there still aren’t enough tests for the rest of us.
While progressives in politics continue to fight for Medicare for All, the pandemic could very well help the cause and turn people’s heads in the direction of at least thinking it would be better for them and for everyone. When life is more important than money, we all win.
