【洢蓮絲】EllANSÉ洢蓮絲中國上市前臨床實驗 邀請林上立醫師指導與經驗分享

  過去的美容醫學，是屬於玻尿酸的黃金年代，舉凡各種輪廓雕塑到抗老治療，都可以以玻尿酸解決。不過隨著科技的進步，各種材質、儀器推陳出新，每次的進步都以「效果更好、更精緻」「副作用更少」的方向邁進。

以抗老領域來說，過去的拉皮手術已經逐漸式微，當然不是說這個手術不好、沒有效，而是對於現代繁忙的都市人，要忍受術後冗長的修復期，必須長時間請假、不能上班、拒絕各種社交生活，是比較不合時宜的；也因此微整形手術的修復期快速、效果立即等特點大受歡迎。

註解：林上立醫師強調，拉皮手術仍適合老化較嚴重、年齡層較高的族群，甚至跟一些膠原蛋白增生材質相互搭配，讓術後效果更精緻。

  尤其Ellanse洢蓮絲（又稱依戀詩、聚己內酯、PCL，市場上俗稱少女針，香港稱易麗適），因兼具玻尿酸的立即雙重效果，及3D聚左旋乳酸後續持續增生膠原蛋白之特性，市場反應熱烈。對於抗老拉提領域來說，更讓醫師們有一項更精良的武器。

儘管Ellanse洢蓮絲2010於歐洲全面推廣，且於2011年更得到「歐洲皮膚科創新科技大奬」 殊榮。2015年（前年）衛生福利部核准於台灣上市。而自從上市以來，林上立醫師累積的大量的案例，而且是「令人驚艷」的案例，因此獲邀至歐洲各國、亞洲其他地區等指導、分享，連國外的醫師都好奇「到底是如何將鬆弛、下垂、圓潤的Ｕ臉型，變成緊緻的Ｖ」，因此獲得邀請，至北京參加洢蓮絲在中國上市前的臨床計畫指導。

  ▌核准上市前的臨床實驗

一個國家，如果要核准一樣藥品、儀器或是注射材質，首先廠商必須提出這項產品的成分、核准治療區域、適應症、用途，以及過去國外相關的研究資料，如文獻、臨床實驗報告、相關不良反應記載。

 以台灣為例，審核過程當中，食藥署會針對這個產品，經過無數次、反覆、仔細地開會討論，針對資訊不夠明確、齊全的疑點，會要求廠商提出更多的相關文獻、報告或是證據，甚至邀請國外的學者專家，當然是對於這個產品熟悉的專家學者與會說明，仔細且謹慎的審核，待各個細節都確認、沒有疑慮後，產品才會獲得上市許可。因為關係到國人的福祉利益，過程必定得謹慎萬分，通常從申請產品到核准上市，所要耗費的時間通常是以「年」計算。

那麼中國呢？在核准上市前的繁文縟節可是比台灣繁瑣。 除了如同台灣上市上述的資訊及步驟外，中國必須【進行自己的臨床試驗】。

 而執行臨床試驗的團隊，這個臨床試驗正是全中國全三大的頂級醫學中心，分別為北京醫院、北京空軍總醫院、北京協和醫院(就好像是我們這邊的台大醫院、長庚醫院)，同時由頂尖的主任級醫師率領主持。

  而林上立醫師正因此產品的使用經驗、熟悉程度及令人驚豔的案例，及對於皮膚本身、老化相關學理構造、生理學之專業，在中國官方執行臨床試驗前夕，林醫師特地赴北京，與這三家指標性的醫院主任及其團隊，深入地對於洢蓮絲的特性、注射時需要注意的事項，其後續相關的反應探討及分析。

第一家為北京醫院。其實林上立醫師與北京醫院主任是第二次見面了。第一次主任是特地飛到林上立醫師的診所看林上立醫師打針。這位主任手下還主持好幾個臨床試驗，其中也包含了台灣即將上市的SculpSure非侵入式體雕療程。

  無意中發現了這台PicoSure 755蜂巢皮秒雷射，原來因為當時在林上立醫師的診所看到這台雷射，與跟林醫師討教一番後，也買了了一台PicoSure皮秒雷射。

  第二家為北京醫學美容第一大醫院，空軍總醫院。一天皮膚科門診量有3000人次。

  其實中國目前並沒有任何已核准上市的【膠原蛋白增生劑】，(儘管在抗老治療的領域上，全球的趨勢已經從單純的填充轉換成膠原的增生)。所以在合法使用的前提下，絕大部分的醫師，過去因為僅僅是接觸到單純填充材質，如「玻尿酸」，因此對於膠原蛋白增生劑這類的產品會感到陌生。畢竟「填充」與「膠原增生劑」的概念是完全不同的。

  簡單來說，填充概念其實就是凹多少、補多少，如臉頰凹陷與下垂，醫師就會估大約2CC的玻尿酸，將該凹陷處給填平；不過膠原增生劑，會刺激皮膚長膠原蛋白，注射在不同的部位、深度也都會有不同的效果，因此這必須靠經驗值的累積，若依循著填補的邏輯注射，術後絕對是「超乎想像」(的難看)！

  膠原蛋白新生已經是抗老治療的新趨勢，甚至是潮流，無法抵擋。加上林上立醫師資質對於進步、新穎的各種材質、儀器，永遠是保持高度熱忱，同時在每次治療中不斷累積經驗，不停地修正、進化，甚至是結合不同材質的優勢，讓治療效果更接近期待。

  當然，在教學、演講的交錯中，有那麼一點點的小幸確，那就是美景與美食。在赴北京之前，林上立醫師先是去了趟青島參加了個會議，希望藉由林上立醫師在抗老治療領域的經驗，給予一些指導與建議。

由於青島這個城市景致及氛圍實在是迷人，忍不住要分享一下。

  中國人對於「食」確實重視。不只是味覺、嗅覺，視覺上更是講究。其中有一餐正是在這船上吃的。
