Askaris for mental health! Monday 30th September
Travelling south again, it was cool and a bit rainy most of the day. I always feel cheated when Africa isn’t warm and sunny.
Our first call of the day was to Ruharo Mission Hospital, notable for its eye clinics in particular; it also has a department for children with disabilities, amongst other specialist services.
Madame Stella is the intriguing executive director, previously the Diocesan HR director. She has great plans (“I have a dream” were the precise words) amongst which were her ideas for developing a centre of excellence for psychiatry. The church has donated a plot of land and she imagined that JF might build her a specialist ward. Happily it didn’t take much to convince her that that would be very expensive to build and run, and if she could enable the development of community mental health services she might well manage with very few beds, maybe on a medical ward.
Yowari, the Askari!
We met Patrick, the excellent young clinical officer (“handsome and hardworking” according to Madame S) who might well lead the development of a community service. And, impressively, an askari – a security guard – who was included in the mhGAP rollout, and who has trained his fellow askaris. He – Mr Yowasi Twinomunjuni -found it all very interesting indeed and so did his students. And apparently they just loved their certificates.
By lunchtime it was getting quite cold, and we were glad to have hot soup in the Satellite Motel on the Kabale Road.
Rushoroza Hospital
On then to Rushoroza, where we found Dr Martin, probably the youngest of the Executive Directors we’ve met on this trip. He kept assuring us that he and his colleague were general practitioners, and practised psychiatry as GPs, but he would love to do even better for their patients. He’s certainly doing well including MH in the health education talks they give on Radio Maria. They also use radio to advertise when and where the hospital is doing community clinics.

you can see the pressure on the land. The cultivation goes quite high on the hills. They have drained the swamps on the right to cultivate the ground, but the swamp held the water so the rivers flow better in the dry season.
And so, finally, we came to Kisiizi, where it all began. A warm welcome as always, and supper with Hanna and Ian Spillman (medical superintendent), before settling into Round House 2.
Nine years ago Linda, Avril (Jamie’s Mum) and I came to Kisiizi to explore whether we might form a link between their mental health service and Cheshire and Wirral Partnership, for mutual learning.
Much has happened since then. I don’t think any of us could have guessed at the extraordinary developments that are now becoming possible throughout Uganda through the faith hospitals and their inspiring young leaders in mental health.

It was 11 o’clock at night, and the Turner family was just getting ready to go to bed. A pre-Christmas dinner turned into quite the long night, so all of the aunts and uncles and cousins had left a little over an hour ago. Alex and his dad were having a beer together and watching a rerun of a soccer game on tv, relaxing after quite a draining day. His mom had run herself rugged, and she was upstairs in their tiny family home while the boys unwound. Out of the blue, they heard a knock from the door. Thinking it was just a relative who’d forgotten one of their children’s jackets or a toy, Alex offered to get the door. When he opened it, he was completely shocked to see who was standing there. It was Miles, drenched in rain and shivering, with an apologetic look on his face "Miles....!” He was absolutely delighted by the surprise until he saw the other lad’s trembling figure and his voice tailed off into concern. “Oh love, what are you doing here?" Alex gushed, hands flitting everywhere like he didn’t know where to start, from Miles' dripping wet lapels to his equally damp hair. He stood in he doorway and brushed Alex's hands away from their fussing and sniffled quietly. He was clearly freezing, he wasn’t exactly dressed for the weather either. He said nothing by way of greeting but he wrapped his arms around himself, looking small and definitely shaking just a little. “Who is it, Al?” Penny called from inside somewhere, voice already getting closer and Alex steels himself for the barrage of questions he couldn’t actually answer because he didn’t yet know how to. Alex pulled him inside quickly. “It... It’s Miles,” he called but already she’d come through to the hallway, pausing from her bustling about upstairs. “Miles? Oh honey, you look soaked through, come on into the warmth love, you must be freezing! Let me get the kettle on!” “Mum, I’m taking Miles up to get a change of clothes,” she nodded her approval, and then turned back to head into the living room, clearly trusting that Alex could handle everything efficiently. “Al," he grumbled and Alex's frown deepened at the whine in his voice. As was often the case with Miles, he didn't bother to explain anything properly, just stepped closer and shivered hard as he shrugged off his jacket apologising over and over for getting the hallway wet, voice just a little hoarse and holding a melancholy edge that Alex didn't like in the slightest. “Miles!" He interrupted the fifth apology quickly and hated the way Miles looked up with eyes wide as saucers, seemingly startled at the raised voice biting his lip gently and sniffling quietly. “Sorry,” came yet another whisper. "What's wrong?" Alex mumbled, trying to ooze gentleness into his tone and letting his gaze wash over his boyfriend. Miles was not only shivering from the cold, apparently he was also shaking just lightly and forcing his rough breathing. His eyes were red rimmed and slightly bloodshot, like he’d been crying. His cheeks were flushed from the harsh weather and his hair was unusually tousled. He really did look terrible. Alex came to wondering what it was that Miles was actually doing there. He wanted to ask but something told him it could wait, that for now he should make sure he changed into some warm, dry clothes and received a great deal of tenderness. Alex assumed there was something else going on, he was sure of it but didn't want to push anything by asking. All he knew was that Miles was there, two days before Christmas, showing up on his doorstep out of the blue when he should’ve been staying in the Wirral over the holidays, with his family. Alex sighed gently. "Alright love, doesn't matter right now, you just c'mere hmm," he suggested, waiting for Miles to come to him and then wrapping his arms around the man protectively. Miles seemed to melt into the hug, slowly closing his arms around Alex's waist and resting his head against his shoulder. “You're ok Miles, let's get you out of these clothes though eh? 'Fore you catch summat hmm.” He spoke gently, certain he was more or less talking to himself. He was concerned, worried, hands all the while soothing, one running up and down Miles' spine, coaxing him to stop shivering, the other running through his hair at the back of his head, feeling when Miles nodded slowly. He pulled away and blinked hard at him. Even his eyes looked sad and before he could try to apologise yet again Alex cut him off, steering him instead to the staircase and ushering him up to the bathroom. Being quiet lest he should startle him. "You want me to run you a bath, or d'you wanna take a shower? Can you stand?" He asked quietly, hesitant. Miles suddenly looked so exhausted under the bright light of the bathroom, causing an alarm to go off in Alex’ head. “God.... love, are you okay? H-have you been sleeping? Eating?" Miles didn't respond immediately but made some sort of noise of discomfort that had Alex hushing him gently and offering him the lightest of kisses on his temple. “Not much of either if I'm honest," Miles choked into his collar bone. "Don't be mad at me, please," he pleaded, hoarse voice so so quiet and cracking often like it hadn’t been used for a while. He was a wreck over something, his eyes foggy and misted and going glassy. “I'm not mad, not even a little bit, oh love, of course I'm not mad, I'm just worried, honey... hey.... oh look at me?" He asked gently as possible, fingertip pressing into the underside of his chin slightly. Miles' eyes reluctantly met his own and Alex felt his heart clench at the way they glazed over, from tears he assumed but maybe from sickness he didn't know. “I didn’t know where to go, Al,” he breathed out. His voice is barely audible. “Well then, you did the right thing, you know you can always, always come to me.” The boy bit his lip, eyes watering. “Why 'aven't you been taking care o' yerself love?" He murmured into the quiet, carefully working on Miles’s shirt buttons and trying to ignore how the shirt itself was crumpled and not exactly fresh on. Miles frowned, cleared his throat and then half whispered his response. "Haven't uhm ... Monday? I dunno.... I didn't... Didn't much fancy cooking, or food to be honest... an’ I 'aven't been sleepin' much in general, s'probly why I feel so shit to start wiv. I might be comin’ down wiv sumthin’." Alex blinked at the logic of his statements. "Monday?" He echoed. "Miles, love it's Thursday! You... you didn't think about coming to me sooner? You never even phoned me once! Fuck it's nearly Friday even, I didn't text you for days cuz you were at yer mum's! Fuck why didn't you stay at hers? What’s going on, baby?" He frowned as soon as he saw Miles' expression change, bottom lip clasped between his teeth again. He looked broken. “Oh! Oh love, no, no I'm not mad at you, darling no, don't cry Miles, you're ok, c'mere, cmon, and I didn't mean it to sound like I’m pushing you away, I'll take care of you babe, shhhh," he soothed gently until Miles just took a deep breath and blinked his eyes closed, letting the tears fall down his cheeks silently. He collapsed into Alex's open arms and let the older man card his fingers through his wet hair, let him keep whispering soft words and let him loop his arms round his waist while pressing gentle kisses to his head. Eventually Miles' tears dampening his shoulder seemed to cease and he just sniffled quietly, arms still rigidly by his side instead of around Alex's waist. Alex carefully pried him away, holding onto his shoulders and frowning at just how cold he must be as he was shivering hard. “Ok?" He asked gently, getting a slightly unconvincing nod in response. "Right, so do you want a bath? Or shower? I'll stay with you... Unless you want me to go?" He questioned quietly. "No I- you can go, I-I'll have a shower, th-thanks Al," he managed, little but a whisper and forced more than Alex was comfortable with. "Umm, ok, well there's towels and such, you errr, you just take your time yeh? Get warm, and um the steam will do wonders. Are you cold?” Miles nodded slowly, "yeh jus' a bit cold," he uttered. Alex nodded in response and couldn't help pulling Miles into another hug, just for the way his forehead crinkled when he mumbled something about a headache. "I'll leave some clothes out in my room Okay? Come downstairs when you’re ready aye? Or d’you want me to come up?” Miles hesitated for just long enough that Alex could understand. “Alright, I’ll be here when you're done, love," he promised, gently pressing his lips to Miles' temple. He worried all the more as he closed the door on the bathroom and immediately left some comfortable, warm clothes out for the lad. The shower was still going so he hurried downstairs to speak to his parents and explain what little he knew about the situation and to fetch Miles a cup of steaming tea. “Al? Is Miles there love?” His mum asked, no doubt hearing him come down the stairs. “He’s having a quick shower, Mum he was freezing.” His concern must’ve been obvious because his mum suddenly fixed him with a serious look, mirrored by his dad. “Alright love, I’m making some tea, but perhaps an Irish coffee might be best?” Alex sighed quietly. “Yeh maybe, I dunno I think he hasn’t been sleeping too much so maybe something caffeine free?” She narrowed her eyes at him. “Al, darling, is he okay? You know we don’t mind Miles staying in the slightest, in fact I’m very very happy to have him here... but there’s one or two blanks to fill in, love... is he staying with us for Christmas?” “Mum, I don’t know anything, he just told me ‘I didn’t know where to go’ and I told him he did the right thing coming here because he’s always welcome with me, no matter what. But mum he hasn’t really been eating or sleeping much since Monday, he’s... I don’t know what’s wrong Mum, something serious. I’m really worried.” She nodded immediately. “Of course love, well you did the right thing telling him he’s welcome here, and don’t you worry we’ll take good care of him okay? Why don’t you run him up a cup of tea and make sure he’s physically okay hmm? We don’t want him feeling worse because he caught the flu or something hmm? Alright, and give us a shout if you need us for anything!” Alex nodded his thanks, mumbled some goodnights and then hurried upstairs with a cup of tea for Miles and himself. When he opened the door, he heard the shower turning off. “Here love, I brought you some tea and got some clothes laid out for you. Feeling any better?” “A little. Thank you, Al.” Miles gave him a small smile, but he was exhausted and it showed on his face. “It’s no problem, you know that. I’ll step out while you get dressed.” “What, like you haven’t seen me like this before?” At least he still had his humor. “For me mum’s sake, la.” Alex smiled, before giving him a quick kiss on the forehead and stepping into the hallway. After a few moments, Miles opened the door for Alex to come back into the bedroom. He was dressed in Alex’ pajamas, which were three sizes too big since that’s how Alex liked. He couldn’t help but smile when he saw Miles in his clothing, he’d seen it hundreds of times but each time brought a smile to his face. “Oh darlin...” Alex immediately went to embrace Miles, wrapping the boy in a tight hug. Miles only let out a sigh, and Alex felt some of the tension leave Miles’ body. “Do you wanna talk...about this? Like... what happened?” He began, not sure if Miles was even remotely ready, considering the state he had been in less than thirty minutes ago. “Yeah... Maybe... I’d like to lay down first. I’m exhausted.” “Of course, love.” They moved to Alex’ bed, under the comfortable and warm sheets but still sitting up enough to drink their tea. Alex gave Miles all the time he needed, deciding it would be better to just comfort him than to prod with questions. They sat with Miles resting his head on Alex’ shoulder, while he gently ran his fingers through Miles’ hair. After a long moment of comfortable silence, he finally spoke. “So... what happened was...” He let out a sigh, trying his best not to let his emotions best him. Alex just held him closer and placed a soft kiss on the top of his head. “Just take your time, love.” Alex tried his best to reassure him, and Miles simply nodded, taking in a deep breath. “I was with me mum, we were supposed to have dinner with me aunt later on... And as we were chatting and all I let slip about us, in more or less words. She proper freaked out, asked me if you were a bird, I dunno maybe she was hoping I was meaning a different Alex and all, and I couldn’t lie to her, ya know?” Miles’ eyes started to fill and threatened to spill over, so Alex only held him closer as he continued. “So I told her no, it’s you, and the look on her face you’d think I’d committed a crime! Then she goes on about breaking her heart and all that, tears, everything. It was bloody awful. Then she tells me to get out, she won’t even look at me. I’m begging her, right? Just begging her to let me talk to her but she’s having none, won’t even let me grab me things and shoving me out the door, slams it behind me. I only had me wallet on me, no phone. I stayed at a friends for a night, or two, I can’t remember. But I knew I had to see ya, I couldn’t handle it, and I had enough cash for a train ticket so I came here. I walked from the train station.” They say together for a few silent moments, Alex didn’t dare say anything lest he interrupt Miles. He was filled with anger, sadness, grief, even, and was desperately hoping his eyes would not betray him and unleash their tears. He only spoke when Miles looked up at him expectantly. “I....don’t know what to say... That’s terrible! Awful! Where is her heart? On Christmas no less! I can’t believe she’s done this.” Alex found himself sputtering, motioning with his hands as if he was trying to herd words together. “You know, I better not tell me mum about this, she’ll ring your mum up the minute I tell her and give her an earful. You don’t mess with Penny, plus I think she likes you more than me.” “No, I believe you, she does like me more.” Miles replied, giving Alex an actually genuine smile. Alex shook his head and chuckled before turning serious once again. “I’m glad you came here. It was the right thing to do. I’m so sorry that happened. You’re the kindest person I know, you don’t deserve this. And you can stay here until term starts back and we’ll go back to school together.” “You’re parents don’t mind? This was pretty unexpected...” “Absolutely not. My mum scolded me for not bringing you home for Christmas with me. They’ll be happy to have you. I am, too.” Miles finally leaned up and placed a small kiss to Alex’ lips, hesitant and quick as if too much movement would shatter the moment around them. Alex cupped Miles’ cheek and gave him a small smile before giving him a kiss to his forehead. “Merry Christmas, Al.” He said in a whisper. “Merry Christmas, love.”

Did you know our bubbles work a treat in the rain?! Designed and made in North Wales, of course our bubbles work a treat in the wind AND rain. There are several wonderful things that happen when it’s raining, and unless the raindrops are particularly heavy and fat – our bubbles LOVE the rain. Firstly, when it’s raining the humidity levels rise – and because our bubbles are made up of 97% water, they are literally mostly made of water themselves. Rain, and local humidity can actually replace the water contend of a bubble that would otherwise evaporate. So wet humid conditions allow the bubbles to be BIGGER and last for LONGER. Pretty cool eh? Raindrops will also go through the bubble film without popping it. For the very same reason, that if you get your hand very wet, you can actually put your fingers THROUGH the bubble film! Give it a go! This same clever bit of physics also means, that grass, leaves, ground, anything else wet, won’t pop the bubbles!! So, they will simply sit on surfaces for longer, allowing you to watch and observe and oohh and ahhh. Have a good look at them as they sit in the rain. You will see the colour thicknesses dancing around as the water drains down through the bubble film as it reacts to gravity. And finally, the lack of evaporation with the high humidity will mean that the bubble film will retain its thickness for longer, which means the colours, the purples, the greens, the golds will be richer for longer.
