"Every year, over 350,000 women die from cervical cancer and another 660,000 are diagnosed. [Note: Plus trans men and other trans people with a cervix.] As a consequence, children are orphaned, families impoverished and communities diminished by the loss of mothers, wives, daughters and sisters.
And yet, unlike most other cancers, almost all these cases and deaths can be averted. We have powerful vaccines that can prevent infection with the human papillomavirus (HPV) that causes cervical cancer; we have diagnostics to detect it early; and we have treatments for those it strikes. With these tools, cervical cancer can not only be stopped; it could become the first cancer to be eliminated. Some high-income countries are already close to elimination, meaning fewer than four cases per 100,000 women.
But in many low- and middle-income countries, these tools are still not available, which is why 94% of cervical cancer deaths occur in those countries.
In 2018, WHO launched a global call to action to eliminate cervical cancer, which was followed in 2020 by the adoption by all 194 WHO Member States of a Global Strategy to Accelerate the Elimination of Cervical Cancer as a Public Health Problem. The strategy calls for countries to achieve three targets by 2030: 90% of girls fully immunised against HPV; 70% of women receiving timely screening; and 90% of those found with precancer or cancer accessing treatment.
These targets are not just aspirational, they are achievable, even in low- and middle-income countries. Bhutan has already reached the targets, the first to do so in the South-East Asia region.
Since introducing the HPV vaccine in 2011, Rwanda has reached vaccine coverage of 90%, and today announced its national goal to reach the 90-70-90 targets three years ahead of schedule, by 2027. Already, in two districts – Gicumbi and Karongi – Rwanda is meeting those goals. Nigeria, which introduced the HPV vaccine in October last year [2023], has already vaccinated 12.3 million girls.
We have the tools and the opportunity to eliminate cervical cancer.
Since WHO issued the global call to action in 2018, more than 60 countries have introduced the HPV vaccine into their immunisation programmes, bringing the total to 144 countries that are routinely protecting girls from cervical cancer in later life. With scientific advances, we can now prevent cervical cancer with just a single dose, which 60 countries are now doing.
The largest provider of HPV vaccines to low- and middle-income countries is Gavi, the Vaccine Alliance, which plans to vaccinate 120 million children between now and 2030. But this plan requires that investments in health are sustained. We are also counting on manufacturers to confirm and honour their commitments to provide HPV vaccines to low- and middle-income countries in the coming years, to avoid the supply constraints that held back progress in the past.
But we cannot rely on vaccines alone. The impact of the rapid scale-up in vaccinating girls now will not be seen for decades, when they reach the adult years when cervical cancer typically appears. To save lives now, we must match the increase in vaccination with increases in screening and treatment.
Decades ago, as more women gained access to pap smears in developed countries, the mortality associated with cervical cancer dropped rapidly. Today, even better tests are available. Over 60 countries now include high-performance HPV tests as part of their screening programs. Women can even collect their own samples for HPV testing, removing more barriers to life-saving services. In Australia – which is on track to become one of the first countries in the world to achieve elimination – more than a quarter of all screening tests are now done this way...
Several countries are also investigating the use of artificial intelligence to enhance the accuracy of screening in resource-limited settings. When women are found with precancerous lesions, many are now treated with portable battery-powered devices, which can be operated in remote locations."

The Anambra State AIDS Control Agency (ANSACA) has disclosed that 3,138 individuals tested positive for HIV out of 257,953 screened between January and October 2024. This reflects a 1.2% positivity rate over the 10-month period. Dr. Afam Obidike, the Commissioner for Health in Anambra, shared these statistics during a media briefing in Awka on Thursday, in observance of World AIDS Day, celebrated globally every December 1st since 1988. This year’s World AIDS Day theme is: ‘Take the Rights Path, Sustain HIV Response, Stop HIV Among Children to End AIDS in Nigeria by 2030.’ Expansion of comprehensive HIV services Obidike highlighted the state’s progress in combating HIV/AIDS, noting the expansion of comprehensive services to 175 healthcare facilities across Anambra. He stated, “Between January and October 2024, 257,953 individuals were tested for HIV, with 3,138 (1.2%) testing positive. “Notably, 99% of those diagnosed were successfully linked to antiretroviral therapy (ART). Currently, 49,067 individuals living with HIV are receiving antiretroviral treatment.” As part of its prevention efforts, the state, with support from partners, distributed 1,406,744 condoms and 26,640 lubricants to promote safe practices among key populations and the general public. The Commissioner emphasized the critical role of the Prevention of Mother-to-Child Transmission (PMTCT) program in reducing pediatric HIV cases in Anambra State. Obidike revealed that spectrum projections for 2024 estimated 5,167 pregnant women in the state required PMTCT services. “Thanks to the efforts of mentor mothers and community partners, 56,199 antenatal attendees were tested for HIV, resulting in the identification of 560 HIV-positive pregnant women (including known and new cases). All were successfully linked to care,” he stated. He also highlighted the adoption of advanced diagnostics, such as m-PIMA machines for Early Infant Diagnosis, which have enhanced the testing and care of HIV-exposed infants, ensuring healthier outcomes for mothers and their babies. Additionally, Obidike noted that the state government has domesticated the HIV Anti-Discrimination Act to safeguard the rights of People Living with HIV, combat stigma, and address gender-based violence. “The state remains committed to achieving health equity and an AIDS-free future for all residents,” he affirmed. There are multiple options for infertile couples, however, treatment depends on risk factors, available diagnostic tools, the skills of the attending physician, and the financial status of the couple. Assisted reproduction centers are also limited in accessibility due to privatization in certain sections of Nigeria. Participants also mentioned low availability of government-owned hospitals with non-availability of doctors, high cost of treatment, and inadequate staff.
Participants reported having heard of adoption but had different understandings of adoption in the context of cultural or religious meanings. For example, some defined adoption as “a situation where [a] barren person tried to comfort him/herself by taking control of a child given birth to by another person.” Many reported adoption as not common in their community and having negative opinions of adoption for various reasons. For example, some viewed adopting a child as suspicious and potentially having ulterior motives to use the child for something, some found it hard to imagine taking care of a child that is not genetically theirs, or some viewed adopted children as bastard children who would destroy their homes.
Overall, many considered adoption as a very last resort. Some even suggested not marrying an infertile person in the first place. The authors found a need for strong advocacy and enlightenment of awareness and utilization of adoption services.

Executive Order on medicaments: FG yet to implement 2 months after
LAGOS — As Nigeria’s pharmaceutical industry eagerly awaits the implementation of the Presidential Executive Order aimed at reducing the cost of essential medicines, and generally revamping the health sector, delays in the implementation process have pushed back the timeline for its takeoff. In June, President Bola Tinubu signed an Executive Order to strengthen Nigeria’s health system by exempting pharmaceutical machinery, equipment, goods, and accessories from tariffs and excise duties, reducing production costs and making healthcare products more affordable. But two months after the pronouncement, checks by Vanguard revealed that the cost of items in the key categories of health equipment covered by the Executive Order remained prohibitively high. Among these are pharmaceutical medications, medical devices such as diagnostic imaging machines, laboratory equipment, surgical instruments, ECG machines, ventilators, syringes, needles, gloves, medical dressings and diagnostic kits for malaria, HIV, etc. News HQ gathered that while the government set a 30-day deadline for development of a harmonisation implementation framework with the ministries of Finance and Industry, Trade, and Investment, the exact timeline for take-off of the Executive Order remains uncertain. On when Nigerians can expect to start seeing the benefits of the Executive Order, Minister of State for Health and Social Welfare, Dr Tunji Alausa, said the government is working diligently to expedite action on the framework that will guide relevant agencies in operationalising and implementing the Executive Order. What govt is doing, by AlausaAlausa said: “We are developing a harmonisation implementation framework with the Ministers of Finance and Industry, Trade and Investment. “Government agencies involved include the National Agency for Food and Drug Administration and Control, NAFDAC, the Federal Inland Revenue Service, FIRS, the Standards Organisation of Nigeria, SON, and the Nigeria Customs Service, NCS. ”He said the government is moving quickly to develop the harmonisation implementation framework within 30 days, adding that once completed, the Executive Order will go into effect quickly. “We have met with the Attorney-General of the Federation about the need to move quickly on this. We are working night and day to get this effected so that Customs will start implementing the process as well as the FIRS. We are setting up a technical working group on this implementation framework so that the Executive Order will take effect immediately. Worried by the apparent lack of progress as the 30-day window has elapsed, players in the health industry have expressed concerns about the potential impact of the delays on the cost of medications and the livelihood of Nigerians even as there is uncertainty regarding the immediate effects of the executive order on drug prices and are urging the government to step up action on the policy. Delay not worth it, people are dying — Oladigbolu On his part, the Immediate past National Chairman of the Association of Community Pharmacists of Nigeria, ACPN, Pharm Wale Oladigbolu, said the delay in implementation of the Executive Order is not worth it because people are dying, even as prices of drugs have continued to spiral up. “We haven’t seen the impact of the Executive Order, that’s at the down end where I practice. Prices of drugs at the retail stores are still very high and we see a lot of people not being able to afford the treatment that they need. Health insurance which should have helped the people is not working in the Nigerian context.“ A whole lot of people who are in the informal sector are not covered by health insurance, so the prices of medicine are high. Affordability is not there, and people’s incomes are strained. And when I say strained, I mean strained. They need to choose between food with high costs, fuel with high costs, and drugs with high costs. So they need to juggle those factors. So people only come to the pharmacy for purely essential things. So things like high blood pressure that does not have a warning, a lot of hypertension patients are dropping their medicines, especially those who have poor health education, they have dropped. They see hypertension as not troubling, so you see a lot of non-adherence to medication because of the high cost of medication. High cost of food. And indeed, high cost of work.“ The delay is not worth it because people are dying. We have not seen an impact or drop in the cost of medicines in Nigeria, so the Executive Order has not had any impact.
