Cornea Transplant Surgery: Beyond Darkness
Vision is a remarkable gift that allows us to experience the world around us in vivid detail, capturing the beauty of nature, the faces of loved ones, and the intricate patterns of life. However, for many individuals, the world remains shrouded in darkness due to corneal disorders that impair their ability to see clearly. Cornea transplant surgery, also known as corneal transplantation or keratoplasty, has emerged as a beacon of hope for those seeking to escape this darkness and regain the gift of sight.
The Cornea: Window to the World
The cornea is the clear, dome-shaped surface at the front of the eye that acts as a protective barrier and plays a critical role in focusing light onto the retina. It is responsible for a significant portion of the eye's refractive power, aiding in the formation of clear images. However, various factors such as injury, infection, genetic conditions, and degenerative diseases can damage the cornea, leading to visual impairment and even blindness.
Unveiling the Surgical Solution
Cornea transplant surgery has revolutionized the treatment of corneal disorders, offering a ray of hope to those whose vision has been compromised. This surgical procedure involves the replacement of the damaged or diseased cornea with a healthy cornea from a deceased donor. The surgery can be a partial-thickness transplant (where only a portion of the cornea is replaced) or a full-thickness transplant (where the entire cornea is replaced).
Types of Cornea Transplant Surgeries:
Penetrating Keratoplasty (PKP): This is the traditional full-thickness cornea transplant, where a circular section of the central cornea is removed and replaced with a donor cornea. PKP is suitable for cases of severe corneal scarring or disease affecting multiple layers of the cornea.
Endothelial Keratoplasty (EK): EK includes techniques like Descemet's Stripping Endothelial Keratoplasty (DSEK) and Descemet's Membrane Endothelial Keratoplasty (DMEK). These procedures target the innermost layer of the cornea, called the endothelium. EK is preferred when only the endothelial layer is affected, as it allows for faster visual recovery and reduced risk of complications.
Deep Anterior Lamellar Keratoplasty (DALK): DALK involves removing the front layers of the cornea while leaving the endothelial layer intact. This technique is suitable for cases where the disease primarily affects the outer layers of the cornea.
Beyond Darkness: Transforming Lives
The impact of cornea transplant surgery on patients' lives cannot be overstated. For many individuals, the surgery means moving from a life of limited sight or blindness to one of renewed clarity and independence. The procedure not only enhances visual acuity but also improves the quality of life by enabling individuals to participate more fully in daily activities, pursue careers, and engage in hobbies they may have thought were lost forever.
However, the journey from darkness to light is not without its challenges. Finding suitable donor corneas, post-operative care, and the potential for rejection are all factors that must be carefully managed to ensure the long-term success of the surgery.
A Glimpse into the Future
Advancements in medical technology continue to propel cornea transplant surgery forward. Researchers are exploring innovative techniques to enhance graft survival rates, reduce the need for long-term medications, and improve visual outcomes. Additionally, regenerative medicine and tissue engineering are on the horizon, holding the promise of creating lab-grown corneal tissue to eliminate the need for donor corneas altogether.
Conclusion
Cornea transplant surgery stands as a testament to human ingenuity and compassion, offering a lifeline to those trapped in a world of darkness. Through the expertise of skilled surgeons, the generosity of donors, and the resilience of patients, this surgical procedure has transformed countless lives, reaffirming the power of medical science to bring light to the shadows. As technology continues to evolve, the journey from darkness to vision promises to become even more remarkable, guiding individuals towards a future filled with vibrant colors, breathtaking vistas, and the boundless beauty of the world around them.

Amniotic Membrane: Structure and Function
The amniotic membrane comprises several layers and can be divided into two different layers, the chorion and amnion, further separated by a jelly-like layer. It entails three material types, regenerative molecules, biologically active cells, and collagen and extracellular matrix. Collagen adds structural strength to the membrane, whereas extracellular matrix provides structure and has specialized proteins like laminins, fibronectin, proteoglycans, etc.
Further, biologically active cells comprise stem cells that revitalize new cellular materials within the membrane lining. Regenerative molecules are central to growth and healing, encompassing metalloproteinases, platelet-derived growth factors, and fibroblast growth factors.
Amniotic Membrane Market by Application
Amniotic membrane (AM) has varied applications in regenerative medicine. Over the years, its clinical applications have advanced from corneal and skin repair to solubilized powder injections, amniotic cytokine extract, and micronized dehydrated membrane to regenerate tendons, cartilage, and muscles. Also, AM is a source of several stem cells types and is a highly biocompatible natural scaffold. Thus, with the increasing number of reconstructive and cosmetic surgeries globally, the surgical wounds segment is expected to continue its revenue growth trend throughout the forecast period.
Some of the prominent and resurging AM applications are:
1. Ophthalmology
Ophthalmology is the fastest-growing segment in the amniotic membrane application market. It has become one of the key adjuvant treatments in ophthalmic surgery, with advancements in tissue engineering, ease of use, enhanced obtaining process, and tectonic properties. Additionally, tissue engineering has enabled its use in classic forms and drops.
2. Skin Graft
Amniotic membrane was first used as a skin substitute in 1910. As an alternative for the dressing of skin graft donor sites, it offers substantial benefits like decreased number of dressing changes and enhanced wound healing process.
Additionally, it has similar features as human skin since it originates from ectoderm. As per studies, it is further evaluated to prevent associated infections, traumas, and dehydration conventionally observed in disrupted skin wounds. Moreover, AM serves as a cost-efficient material for wound dressing.
3. Sports Medicine
AM has promising applications in sports medicine, since it is a source of highly organized collagen, matrix proteins, immunomodulators, anti-inflammatory and anti-fibrotic cytokines, and pluripotent cells. When applied as tissue engineering scaffolds, such aspects enhance tissue organization in treating and healing arthritic joints.
Also, around 14 companies offer commercial AM products despite sports medicine leaning towards animal and in vitro studies. Studies state that such products’ formulation changes their mechanical and biological properties, and an in-depth understanding of differences will enable the use of AM-derived products in sports medicine research.
Product Innovations: Amniotic Membrane Market Trend 2022
The development of various preservation methods has helped overcome AM’s limitations like high disease transmission risk and limited shelf life. Alongside, commercially available amniotic products implement dehydration preservation methods or cryopreservation methods to sustain the regenerative properties of amniotic membranes.
Moreover, there are over 80 commercial placenta membrane products available in the market. Some prominent firms operating in the market include Katena Products Inc, Bio-Tissue Inc, MIMEDX, Amino Technology LLC, etc.
The two leading AM products are cryopreserved AM and dehydrated AM, with the former harboring the largest revenue share in the product segment.
Dehydrated Amniotic Membranes
These are traditionally used in surgeries associated with ocular surface reconstruction. They are obtained from the human placenta surface after a healthy birth, wherein the donor tissue is evaluated for communicable diseases. However, matching donors is not a requisite since donor tissue is absorbed through the healing process.
In addition, dehydration enables AM tissue storage in a shelf-stable state and its active biological components preservation, required for medical uses. Dehydrated AM is used for several applications, the most common being treating neurotrophic corneal ulcer, a non-healing ocular surface defect. Besides, it can be employed to minimize scar tissue during ocular surface healing.
Cryopreserved Amniotic Membranes
These retain the specific biologic matrix that facilitates AM’s regenerative healing and anti-inflammatory properties, leading to fewer clinic visits and long-term relief. This is because cryopreserved amniotic membranes last longer on the eye. Also, these are FDA-approved for their anti-scarring and anti-angiogenesis properties. Moreover, cryopreserved AMs are advantageous compared to other therapies in terms of corneal defects.
The Progress Here on:
Amniotic membrane has had clinical applications for 100 years. Despite a slow start in clinical use, it has made inroads into several fields of medicine and surgery in the last 50 years. Besides, its beneficial properties and growing availability due to innovative storage methods offer lucrative growth opportunities for the global amniotic membrane market.

Abstract: Great success of the modern keratorefractive surgery and its wide spread require high attention to the biometric parameters of the eye in pre- and post-operation period. The most important parameters are those characterizing the cornea, such as central corneal thickness and its changes, regenerative response of corneal tissue and its regulation, as well as the intraocular pressure (IOP) and its correlation with corneal parameters. Morphological structure of the cornea is altered, during various, and first of all laser surgeries with refractive, reconstructive, optical and other purposes. For the patients after corneal surgery an accurate IOP measurement without corneal contact would be desirable. The transpalpebral transscleral tonometer Diaton is small and mobile. It is a non-corneal device, which allows for IOP-measurement in sitting and supine position. recording of stationary and expansive equipment like slit lamp and air puff machines, in seated or in lying position of the patient, and it is independent from mains supply. A resent clinical study revealed that the results of IOP measurement using transpalpebral transscleral tonometry in patients with refraction anomaly before and after keratophotorefractive surgery do not depend on central corneal thickness and are similar to CCT-corrected IOP-values measured by Goldmann Applanation Tonometry. A reproducibility study with Diaton showed recently that when performed by experienced or well-trained individuals, IOPmeasurements with Diaton possess high reproducibility and low inter-grader variability. Moreover the measuring method has been shown to be easy to learn over a short time. Clinical application of transpalpebral transscleral tonometry with Diaton makes IOP-measurement efficient and economical. No additional instrumental examination, like measurement of CCT is required.  This allows for the application of Diaton as a screening tonometry device both in clinical and non-clinical environment, as  well as in rural areas. A particular advantage of Diaton is its ability to measure IOP in situations of contraindication to the corneal tonometry, e.g., ulcer or IOP-monitoring after keratorefractive surg https://www.instagram.com/p/B0t0RRKBSnU/?igshid=10tgtsydliqvp
