What are the causes of kidney failure/kidney disease in children?
Kidney failure is the condition when your kidneys stop working well enough to survive without dialysis or a kidney transplant.
Kidney failure or renal failure has two different types - acute renal failure and chronic renal failure. Acute kidney failure occurs when your kidneys suddenly stop functioning properly whereas chronic kidney failure develops over time. However, acute kidney failure may be reversible while chronic kidney failure can lead to permanent kidney failure.
The causes, symptoms and treatments of acute and chronic renal failure are different.
What can cause kidney failure in a child?
The causes and symptoms of kidney disease in children are often different than in adults. A child has more risk of developing acute kidney disease if he/she has suffered from blood loss, surgery, or a blockage in the urinary tract in the past.
Some of the common causes of kidney disease in children include:
1. Birth defects
Most children born with a kidney defect have problems in their urinary tract. Some children are born with a narrow or blocked urethra. Other conditions include:
Having born with only one kidney
Having born with two kidneys that don’t function
Being born with a kidney in the wrong location
2. Genetic diseases
Family history and genetics play a major role in one’s health through passing genes of a parent to their child. There are many kidney diseases that get developed by genetic disorders such as Polycystic kidney disease (PKD), Alport syndrome etc.
3. Urinary tract infections (UTIs)
Recurrent urinary tract infections and some certain bladder infections can harm the kidneys. Urinary tract infections in infants and young children are common and show symptoms like frequent, painful urination that appears cloudy or bloody.
4. Nephrotic syndrome
Nephrotic syndrome usually affects children at the age of 2 to 6. The most common causes of nephrotic syndrome in children are inflammation, infection and diabetes.
5. Systemic diseases
Systemic diseases like diabetes and lupus can cause kidney disease. Though diabetes is not a common cause of kidney disease in children, a child can develop both type 1 and type 2 diabetes that involves the risk of kidney problems.
Most children with renal failure are treated by a pediatric nephrologist. The treatment of kidney disorders in children depends on the degree of kidney function that remains.

Infection of the Kidney | Acute Pyelonephritis Treatment in Bangalore | Nelivigi Multispeciality Hospital

Acute pyelonephritis (APN) is a technical term for infection of the kidney. It is a part of the spectrum of urinary tract infection or UTI. UTI can occur anywhere from kidney, ureter, urinary bladder, prostate and urethra.

The individual complains of pain abdomen, fever and chills. However there are many who do not suffer from all these symptoms and may only have some of them. Fatigue, generalized weakness and vomiting also are common. They may not have symptoms like burning urination or painful urination. APN usually is seen in middle aged and elderly people and less in young people. It is rarely seen in children.

APN is almost always caused by infections by common urinary infection causing bacteria like E. coli, Proteus, Enterobacter and Pseudomonas. Most of these infections are hematogenous which means the infection spreads through blood stream. Infection ascending from urinary bladder is less common. Patients with underlying diabetes are especially prone to be affected and those with uncontrolled diabetes even more so. It is also seen in individuals with kidney stones or stones blocking the ureters. Another common but less recognized cause is prostatic enlargement with urinary infection in the elderly males.

APN is diagnosed based on history and clinical examination. The patient will have fever, chills and pain abdomen. There will be pain on pressing the upper back on either side. The urologist will order a set of tests to confirm the diagnosis. These are Complete Blood Count, urine culture and sensitivity, serum creatinine, blood sugar level blood culture and ultrasound of abdomen. In case ultrasound is not conclusive, a CT scan of abdomen is done.

The patient is advised hospitalization for ease of evaluation and treatment. The mainstay of treatment is injectable antibiotics, drugs to relieve fever and pain. Rest helps in faster recovery. In patients with diabetes sometimes ultrasound and CT scan show blockage to the kidneys due to pus flakes or sloughed off debris from the kidneys. There could be blockage due to stones too. In such cases endoscopy is done, debris or stone is removed and DJ stent is inserted. This is a synthetic tube to relieve the blockage and control infection. This tube is kept for a few weeks and removed once the infection is controlled. In all cases, the underlying diabetes has to be controlled without which infection control is very difficult. Even after the patient is off antibiotics, strict control of diabetes is mandatory to avoid recurrences. In case the urologist feels that prostate enlargement is the cause of APN, then prostate surgery is warranted.
Outcomes
With modern imaging, antibiotics and endoscopy, acute pyelonephritis doesn’t carry the morbidity it used to have a few decades ago.  However uncontrolled, prolonged and repeated infections can result in permanent kidney damage .Despite modern advances, there are rare situations where it can still result in death of the patient due to severe uncontrolled infection. Therefore it is worth reiterating the importance of controlling diabetes.

Urinary tract infections are one of the most common infections afflicting both males and females.
The urinary tract consists of kidneys, ureters, urinary bladder, prostate, and the urethra and any of these organs can be involved. Infection of the kidney is called pyelonephritis, infection of the bladder is called cystitis, infection of the prostate is called prostatitis and infection of the urethra is called urethritis.
They can occur in any age but certain age groups are more predisposed. In males, the most common age group is in the elderly which is after the age of 50. This is the age in which prostate enlargement is seen and is responsible for UTIs.
In the females, there is a bimodal distribution with a surge in infections with the first surge occurring after the onset of sexual intercourse and after marriage. This goes by the name of ‘honeymoon cystitis’. The second surge in the females is in their forties and fifties with the onset of menopause where hormonal changes at menopause increase the incidence of UTIs.
UTIs are seen in children too where they could be associated with congenital abnormalities of the urinary tract like PUJ obstruction, vesicoureteric refux, megaureter, posterior urethral valves etc.
The most troublesome infections are seen in females where they could be recurrent. This leads to a situation where some suffer from urine infections regularly despite antibiotics.
Symptoms of UTI
The most common symptom is dysuria which is the medical term for burning urination. The next most common symptom is increased urine frequency where the person has to visit the toilet very often in both day and night. This could be associated with urgency in urination and sometimes urine leak. The other common symptom is a pain in the lower abdomen (suprapubic region) and low backache. The patient may feel feverish and fatigued but usually, in infections of the urinary bladder, the fever is of low grade whereas, in kidney infections, the fever is high grade and associated with chills.
Investigations
The most important test is a urine examination. It is a simple and affordable test which can give a diagnosis in a few minutes. The doctor can start the treatment immediately. Urine can be sent for culture and sensitivity but the results will be ready only after 3 days. Complete Blood Count can establish if the urine infection has spread to the bloodstream. In complicated infections, an ultrasound abdomen will help in finding out if any other abnormality is present. Depending upon the complexity of the infection, more tests like CT scan may be needed which will be decided by the treating doctor.
Treatment:
The mainstay of the treatment is antibiotics. Most of the times, oral antibiotics are enough. However, in kidney infections, injection antibiotics are needed. In case it is severe, the patient may need hospitalization for observation and treatment. The antibiotics need to be taken for a minimum of three days and in severe infections may need to be continued for 15 days. Rest and hydration are important too so that the person recovers fast. There are many supportive treatments like cranberry extracts and alkaline solutions. However, they are mostly supportive but not the mainstay in treatment. Alkaline solutions are very popular and help in reducing the acidity of the urine so that the burning sensation may reduce. Cranberry capsules may help in long term prevention of infections but the evidence is not strong.
