parental death tw, cancer tw, hostages tw vedant was the second of two sons, born on a rainy day in delhi. his life was marked by loss within hours of his birth when his mother passed away due to complications in the delivery. his father, while unwilling, was compelled to remarry by his family. they moved to the united states shortly after, and his step-mother showered both him and his older brother, santosh with love and care. while they saw the relationship between his father and mother was nonexistent, the children never felt the strain of her marital relationship impact her maternal love. when his brother was diagnosed with a rare and aggressive form of leukemia, vedant was the presumed donor for a bone marrow transplant. the fact that he was not a match and therefore could not help his brother haunts him to this day. within a few years of her birth they said goodbye to santosh as well. grappling with their grief, ved threw himself into school. after graduation, he attended georgetown and then received an mba from stanford. he began to work with the fbi in hostage negotiations. within a few years he had excelled at work and was handling international negotiations. one such mission took him to syria where he disobeyed instructions in saving mahira zahara garcia. upon returning to the us, he was put on probation and after a few more episodes of poor performance since returning, he was asked to take a break. he returned to wilmington to be with family and work through the demons that caused him to buckle under pressure for the first time.
IN THE WEEDS . . .
Born during the monsoon season in Delhi, Vedant’s very first moments in the world brought tragedy with them. He was only minutes old when his mother passed during child birth, leaving with him little more than her name as his middle name. And thus began Vedant Gagan Arora's series of loss and trauma. His father tried to raise his two sons on his own but family pressure and lack of much needed support led him to marry once more. Where Gagan had been the woman he loved, his new wife was the one who could love his children. And he did not need more than the memories of his late wife and the maternal instinct of his new life partner. Soon after the marriage, they moved to the United States to their first home in Iowa. Vedant grew up knowing only his step-mother as his own. And she absolutely doted on both him and his older brother, Santosh. But it was no secret that she showered her oldest son with the most love given his vivacious personality.
Yet his childhood too was marked by struggle. With a rare and aggressive form of cancer that required a bone marrow transplant, the obvious choice was Vedant. After all, they shared the same blood, who could be a more perfect match? Unfortunately, it was discovered that Vedant was not a match and once again he was faced with the possibility of losing a family member. And it would once again be attributable to him. The thought alone haunted him as he grew even more withdrawn despite the family never voicing such sentiments to him. A part of him felt as though he were a bad omen, bringing destruction and loss wherever he went. They had a few years of absolute happiness when they found a donor. Then once again, Santosh relapsed and this time, there was no treatment that could succeed in prolonging his life. The Dasaris said goodbye to one more family member and plunged into misery immediately. The light and happiness of their house vanishing with their oldest.
It took years to heal, and even then the emptiness could be felt throughout the family. Vedant, having always been introspective and closed off excelled in his academics. It was the only thing he could do to keep himself sane. He ended up attending Georgetown University soon after leaving Wilmington, studying psychology. Thinking it were better for his family if he were away from them, things improved slightly in his absence. Or at least, he was not privy to the ways in which his sister carried the burden of ensuring the family stayed intact and afloat. He continued his education, pursuing an MBA at Stanford University a while later. Vedant was hardly made for business, and it was perhaps for that reason that his career path took a far more dangerous turn. Feeling as though it was hardly fulfilling to work as a consultant or banker like many of his classmates, Vedant instead ended up working at the FBI as a hostage negotiator. Driven by a desire to actually be the source of happiness in the lives of strangers, he quickly rose the ranks as his negotiation skills were unparalleled.
His career soon took him from D.C. to international conflicts. With a few years of training under his belt, he whisked off to negotiate higher stakes situations. His entire career was marked by success. Unlike his childhood, in which he lost two family members, Vedant this time around managed to reunite a number of families with their loved ones. He was all primed to work his way up the ladder. But it all changed when he disobeyed orders in Syria. They were two journalists. A woman and a cameraman, caught in the middle of a hostage situation. Vedant was stationed nearby with orders to wait for further instruction. The tensions were high, as were the demands. Ones that the United States government would have to comply with if they were to rescue their own. The crushing guilt of his childhood came crashing back when after nearly 18 days of hostile negotiations, they were no closer to success. It was then that the thought was floated. Perhaps they had never intended to release the hostage at all. With negotiations at a standstill, it seemed that the government was growing less and less inclined to give into the demands and bring the two journalists back to their family. The thought was unconceivable to Vedant and, for the first time, he went against the orders he was given and sought to save the two himself. He could not watch yet another person lose their life because of his failings.
It was only when he entered the compound that he realized that they had already lost one. He had already failed one family. Throwing his life on the line, he made sure to bring Mahira Zahara Garcia out with him. And while the rescue was a success, it also cost him his promotion. It cost him the work that he was doing and that had gained him such accolade. When he returned, he began to work on smaller cases. They were all local, nothing nearly as high stakes. Added to that was the trauma that returned. The years of wickedness and desperation that he had seen in his life. The worst of human kind and the worst emotions one could feel. They came crashing down and the agency appointed therapist suggested that he take some time off. Reorient himself. He returned to his hometown of Wilmington, wondering if surrounding himself with his family, the people with whom he had first experienced loss, would allow him to heal from within.

Tobacco – The Most Common Preventable Cause Of Cancer
During the early days of my training as a medical oncologist at AIIMS, New Delhi, I was alarmed to see patients as young as 20 years being diagnosed with advanced stages of oral cavity cancers and dying within one year of diagnosis despite treatment. Tobacco chewing is the single most important cause of these cancers and in certain parts of the country, it is so prevalent that it has become the most common cause of cancer.
Research points out that Tobacco abuse is the most common cause of cancer. It is responsible for 10 different types of cancers and collectively they are referred to as ‘Tobacco-related cancers’. They are head and neck cancers (mostly related to tobacco chewing), Lung cancer (primarily due to tobacco smoking), esophageal (food pipe) cancer, stomach cancer, pancreatic cancer, kidney cancer, bladder cancer, liver cancer, cervix cancer and acute myeloid leukemia (a type of blood cancer). Together, these account for almost 40% of the cancer burden worldwide, making tobacco the single most common cause of cancer.
Tobacco smoke has at least 70 chemicals that cause cancer, called as carcinogens. Tobacco chewing causes irritation of mucosa in the oral cavity which on long term exposure turns into cancer. An indirect exposure to tobacco smoke from people who smoke around you can place you at a higher risk of getting cancer. This is called ‘passive smoking’ and is especially important to pregnant women as it is shown to be one of the causes of childhood cancers.
If the current trend in smoking and population growth continue, the number of current smokers is expected to reach 2 billion worldwide by 2030. The WHO (World Health Organization) estimates that one out of two young people who start smoking and continue smoking throughout their lives will develop ‘Tobacco-Related Cancer’ (TRC). The numbers are alarming. According to the latest GLOBOCAN data (2018) for India, there were 1.1 million new cases of cancers occurring in a year and 2.2 million people living with cancer. Of these, at least 40% are tobacco-related. Even these reports are likely to be an under-representation due to lack of a good cancer registry covering an adequate population.
What these numbers would suggest is that cancer incidence is growing at an alarming rate and unfortunately a very significant proportion of it is due to a preventable cause, tobacco abuse. The need of the hour is a strong effort from Government and regulatory authorities to curb tobacco. Effective policies, public education through popular media and partnership of all stakeholders across multiple sectors are some of the ways. Every year on 31st May, the WHO and its global partners celebrate World No Tobacco Day (WNTD). The annual campaign is an opportunity to raise awareness on the harmful and deadly effects of tobacco use and second-hand smoke exposure, and to discourage the use of tobacco in any form.
The focus of World No Tobacco Day 2019 is on “tobacco and lung health.” The campaign aims to increase awareness on the negative impact of tobacco on people’s lung health, from cancer to chronic respiratory disease.
The focus is not just on Government and such campaigns but on the public as well to understand the risks. Peers to peers, parents to children should talk about the negative impact of tobacco. Its time to ‘kick the butt’!

Cutaneous T-cell lymphomas : These lymphomas start in the skin. Skin lymphomas account for about 5% of all lymphomas.
Angioimmunoblastic T-cell lymphoma: This lymphoma accounts for about 4% of all lymphomas. It is more common in old age. It involve  lymph nodes and bone marrow as well as the spleen or liver, which can get enlarged. People with this lymphoma usually have fever, weight loss, skin rashes and often develop infections.
Extranodal natural killer/T-cell lymphoma, nasal type:  This rare type lymphoma  involves the upper airway passages, mostly nose and upper throat, but it can also invade the skin, digestive tract, and other organs. Cells of this lymphoma are similar in some ways to natural killer (NK) cells, another type of lymphocyte.
Enteropathy-associated intestinal T-cell lymphoma (EATL):EATL is a lymphoma that grows in the lining of the intestine. This lymphoma is  common in the small intestine, but can also appear in the colon.
This lymphoma occurs in  people with celiac disease (also called gluten-sensitive enteropathy). Celiac disease is an autoimmune disease in which eating gluten, causes the immune system to attack the lining of the intestine and other parts of the body. It is rare among people who have had celiac disease since childhood, and is more common in people diagnosed as older adults. This lymphoma is more common in men than women.
Anaplastic large cell lymphoma (ALCL): About 2% of lymphomas are of this type. It is more common in young people (including children), but it can also affect older adults. This type of lymphoma tends to be fast-growing, but many people with this lymphoma can be cured.
There are different forms of Anaplastic large cell lymphoma

Systemic ALCLcan affect the lymph nodes and other organs, including the skin.
Systemic ALCL is divided into 2 types based on whether the lymphoma cells have a change in the ALK gene.

SYMPTOMS
Symptoms vary according to the specific type of T-cell lymphoma.

Chemotherapy combination called CHOP, which includes cyclophosphamide, vincristine, and prednisone Newer chemo drugs, such as pralatrexate (Folotyn) Targeted drugs, such as bortezomib (Velcade), belinostat (Beleodaq), or romidepsin (Istodax) Immunotherapy drugs, such as alemtuzumab (Campath) and denileukin diftitox (Ontak) In advanced cases, one may need maintenance chemotherapy for up to two years.
Light therapy UVA and UVB light can kill cancer cells on the skin. Light therapy is usually given several times in a week using special lamps. UVA light treatment is generally combined with drugs called psoralens. UVA light activates the psoralens to kill cancer cells.
Radiation Radiation therapy uses radioactive particles to destroy cancer cells. The beams can be directed to the affected skin so that internal organs aren’t affected.
Extracorporeal photopheresis This is used to treat mycosis fungoides or Sézary syndrome.  Two-day procedure, in which blood will be removed and treated with UV light and drugs that activate when exposed to the light, killing cancer cells.
Stem cell transplant

Riddhima Kapoor has been revisiting memories of her time with late father Rishi Kapoor. She has now shared a childhood picture with Rishi and one from mother Neetu Kapoor’s birthday celebrations on her Instagram stories.
Rishi breathed his last on April 30. He had been battling leukemia for the past two years. He had returned from New York after a year-long treatment and was even declared cancer free last year.
The childhood picture shows Riddhima as a young girl, twinning with Rishi in white. The other picture shows Neetu sitting in front of a cake on her birthday. Riddhima is seen sitting close to her while Rishi, Riddhima’s husband Bharat Sahni and daughter Samara are standing behind them.

Last week, Riddhima shared a selfie with her brother, actor Ranbir Kapoor, and their mom. Sharing the picture on her Instagram stories, Riddhima wrote: “Got your back ma. Your pillars.” She had also shared a couple of monochrome pictures recently. One of the images has Rishi holding his mother, the late Krishna Raj Kapoor’s hand while dancing. Captioning the image, Riddhima wrote: “Reunited with his most favourite person.” The other image was from Rishi and Neetu Singh’s wedding celebrations.

She arrived in Mumbai from New Delhi on May 2 by road to be with her mother and Ranbir Kapoor. Riddhima did not get permission to fly down due to the nationwide COVID-19 lockdown, and could not be in Mumbai on time to attend her father’s last rites.

Owing to the lockdown, Riddhima, who lives in Delhi, had to travel over 1400 km by road all the way to Mumbai, in order to be with her mother, in the time of bereavement. She couldn’t attend Rishi’s last rites. She later accompanied Neetu, Ranbir and his girlfriend Alia BHatt for the immersion of Rishi’s ashes at Banganga.
