Contents: The notice of construction lien must include:
The name of the reputed owner or, if unknown, the record owner.
The name of the person who employed the claimant.
The dates when the claimant first and last provided labor or materials.
A description of the property sufficient for identification.
The claimant’s name, address, and phone number.
The amount claimed.
The claimant’s signature.
An acknowledgment or certificate as required by Title 57, Chapter 3.
For liens on owner-occupied residences, a statement on the steps the owner can take to remove the lien (Utah Code § 38-1a-502).
Recording: This notice must be recorded in the county where the property is located, and the county recorder will index it accordingly. The notice is considered effective from the time of recording, giving public notice of the lien.

Much of Barbara's work in films was routine. Interestingly, the one role she will always be identified with is also one of the smallest parts given her during her brief tenure as leading lady. In the very last segment of All About Eve (1950, 20th Century-Fox), Barbara turns up in the role of "Phoebe", a devious school girl and wannabe actress who shows startling promise as a future schemer along the lines of her equally ruthless idol, "Eve Harrington", superbly played by Anne Baxter. Barbara's image is enshrined in the picture's very last scene--posing in front of a three-way mirror while clutching Baxter's just-received "Sarah Siddons" acting award. It is this brief, breathtaking moment for which she will best be remembered. The Hollywood Reporter said of her performance, "Barbara Bates comes on the screen in the last few moments to more or less sum up the whole action and point of the story. It's odd that a bit should count for so much, and in the hands of Miss Bates all the required points are fulfilled." Despite a seemingly successful career, Barbara's life, both on and off screen, started unraveling. She became a victim of extreme mood swings, insecurity, ill health, and chronic depression. Bates tried to salvage her career and traveled to England to find work. She was signed on as a contract player with the J. Arthur Rank Organization, only to drop out of two leading roles in one month. Bates continued to be too emotionally unstable to work and in 1957, her contract with the Rank Organization was canceled. Her last onscreen appearance would come in an episode of The Saint that aired in November 1962. In 1960, Barbara and her husband moved back to the United States and got an apartment in Beverly Hills. Later that year, Coan was diagnosed with cancer. Bates remained devoted to her husband and rarely left his bedside, but the strain was too much for her. She attempted suicide by slashing her wrists and was rushed to Cedars-Sinai Hospital where she soon recovered. In January 1967, Bates' husband, Cecil Coan, died of cancer. She was devastated by his death and fell apart. Later that year, she retreated back to Denver and fell out of the public eye. For a time, Bates worked as a secretary, a dental assistant, and as a hospital aide. In December 1968, she married for the second time to a childhood friend, sportscaster William Reed. Despite her new marriage and location, she remained increasingly despondent and depressed. On March 18, 1969, just four months after her marriage to Reed, Barbara Bates committed suicide in her mother's garage by carbon monoxide poisoning. She was 43 years old. She is buried at Crown Hill Cemetery in Jefferson County, Colorado. Barbara's suicide was a sad and tragic ending to a promising Hollywood beauty who seemed destined to having it all.
