Abstract
New knowledge about diseases on the molecular level demonstrates that inflammation is a common determinant for seemingly very different disorders. Long standing high levels of pro-inflammatory cytokines is an important factor also for cancer. In fact, the pro-inflammatory cytokines may stimulate normal cells, such as immune cells and fibroblasts, to produce more growth factors and cytokines that the cancer cells take advantage of and a vicious circle occurs. More focus is now on this interaction in cancer research and how to break it. Pancreas cancer is a typical example with a lot of stroma in relation to the cancer cells. Cardiac glycosides, especially in the form of digitoxin, have very interesting properties when it comes to target these mechanisms on the molecular level.

Introduction
The innate immune system plays a pivotal role for eradicating cancer cells in humans. Apoptosis (programmed cell death) in cancer cells triggered by immune cells, such as human NK cells, is a mechanism for the human body to get rid of malignant cells [1]. During studies how to strengthen the innate immune system against cancer we found that cardiac glycosides, digoxin and digitoxin, are potent inducers of apoptosis in different types of cancer cells in doses that are not toxic for humans. Actually, we were the first to show apoptosis in cancer cells by digitalis [2,3]. The last years several new molecular modes of action of the cardiac glycosides of impact for cancer and inflammatory diseases have been shown. Clinical studies of digitalis for cancer are ongoing and in progress, however, almost all use digoxin [4]. Our data indicate that digitoxin has more potent anti-cancer effects, in concentrations that are non-toxic for humans, than digoxin [2-6]. Digitoxin is of interest to use as single agent and in combination with chemotherapy in clinical cancer trials and may also be valuable to suppress harmful inflammatory reactions. Digitoxin may also be combined with radiotherapy and/or new antibodies, such as PD-L1 antibodies, to enhance the anti-cancer effects. This mini-review summarizes some of the most interesting properties of the cardiac glycosides from a clinical point of view.
Background
William Withering made digitalis a common drug to treat cardiac congestion by his book. ”An account of the Foxglove and some of its Medical uses with practical remarks on the dropsy, and other diseases”, published in 1785. Digitalis extract had been used in folk medicine much earlier, but Withering’s observations led to a more rational use. Plant extracts containing cardiac glycosides, for example from Nerium Oleander, have been documented in remedies against cancer as early as in the Middle Ages from different parts of the world [3]. Today digitalis, in the form of the cardiac glycosides digoxin and digitoxin, is used to treat cardiac congestion and some arrhythmias, however, the use is decreasing and digitoxin has been taken off the markets in many countries as it has been replaced by other cardiac drugs. Generally, digoxin is regarded easier to dosage than digitoxin and digoxin has been more common to use than digitoxin. Digoxin has a shorter half-life than digitoxin. The chemical difference is that digoxin has an extra hydroxyl group (-OH) compared to digitoxin; i.e. digoxin is more hydrophilic and is excreted by the kidneys. Digitoxin is mainly metabolized in the liver and excreted by the faeces. Digitoxin is lipophilic and pass the blood-brain barrier and reach the CNS and higher than plasma concentrations in the liver, kidneys, brain and heart [3]. The renal function has a” normal decline” by increasing age, in contrast to liver function, and most patients with cardiac congestion are older, thus, digitoxin may be safer in this patient population [7]. Recently, a debate among cardiologists has started; maybe digitalis has been replaced too early by newer drugs. Studies comparing the newer cardiac drugs to digitalis are lacking [8]. Digitalis has the unique capability to increase the inotropic effect of the cardiac muscle without increasing the oxygen consumption. Interestingly, healthy volunteers taking digitoxin in therapeutic dose for treating cardiac congestion, had reduced diastolic blood pressure and heart rate during overnight sleep, probably because of increased parasympathetic activity or decreased sympathetic activity [9]. Time will show if digitoxin will have a renaissance in cardiology.
Properties of Cardiac Glycosides Pivotal for Cancer and Inflammation
Jens C. Skou made the first discovery of an ion-transporting enzyme, the ubiquitous plasma membrane Na+/K+ ATPase in 1957. The main pharmacological effect of clinical benefit of the cardiac glycosides seemed to be inhibition of the Na+/K+ ATPase on cardiac muscle cells and the subsequent ion changes with increased intracellular calcium ion concentration leading to a positive inotropic effect on the heart. Since we reported apoptosis in cancer cells by cardiac glycosides, it has become obvious that the Na+/ K+ ATPase have two distinct roles; the well known as an ion pump and in addition as a protein-protein signal transducer. Increased intracellular calcium ion concentration is a part of the apoptotic cascade and in addition the Na+/ K+ ATPase signals through several other signaling pathways that regulate apoptosis and proliferation, such as through the pathways of EGFR and other tyrosine kinase receptors [10]. Notably, digitoxin is a strong inhibitor of the transcription factor NF-κB and can abolish the production of proinflammatory cytokines [11]. The tumor micro-environment and stroma directly influences the progression of solid tumors through secretion of growth factors and extracellular matrix depositions. CAFs support angiogenesis and cancer cell invasion and metastasis. TGF-β from cancer cells start differentiation of fibroblasts to CAFs. This mechanism seems to be present for different types of solid tumors, such as glioblastoma, pancreas cancer, prostate cancer and also for several hematological malignancies. Pro-inflammatory cytokines such as IL-6 and TNF-α are often involved in this interplay between the cancer cells and stroma cells as well [12-15]. Cardiac glycosides have the ability to inhibit this process [11,12,16]. The capability of digitoxin to inhibit NF-κB and by that abolish the pro-inflammatory cytokines, may be useful for an array of chronic inflammatory disorders such as ulcerative colitis, Crohn´s disease, systemic sclerosis among other diseases. However, real high levels of the pro-inflammatory cytokines may be acute life threatening. The bird flu virus, H5N1, killed many young healthy adults with intact immune systems. The main cause of death of these individuals were respiratory distress caused by a “cytokine storm”, hyper-production of pro-inflammatory cytokines; the immune system overreacted towards the virus [17,18]. It was great concern during the H5N1 epidemic how to handle the situation as the accessible antiviral drugs were not very effective. It would be worthwhile to evaluate digitoxin in this context if such a situation occurs again and we do not have any other effective drugs.
Digitoxin makes the cancer cells immunogenic, especially in combination with some type of chemotherapy [19,20]. The immunogenic effect of digitoxin as well as the inhibition of IL-6 and similar pro-inflammatory factors may make digitoxin an effective combination with newly developed PD-L1 antibodies [21]. In addition, we and others have shown a radio-sensitizing effect of cardiac glycosides on cancer cells [3]. Naturally, one wonder if all these interesting molecular effects of the cardiac glycosides are clinically relevant. Epidemiological studies may give some indications. We examined a population of more than 9000 patients on digitoxin for cardiac disease and correlated to data in the Norwegian Cancer Registry. This population has serious cardiac disease and eventual anti-cancer effects may be masked behind the high morbidity and mortality due to the cardiac conditions. Still, significant anti-cancer effects were detected for leukemia/lymphoma and urothelial and kidney cancer [22]. Several epidemiological studies on digitalis and cancer have been done, most concerning digoxin, and the results differ considerably. Some studies find anti-cancer effects, others not, some studies indicate an increased risk for cancer in a population on digitalis [23-25].
It has been hypothesized that the anti-cancer effect of digitalis is due to an estrogen effect and that this estrogen effect also could increase the risk for some cancers [26]. However, in our experiments we found estrogen expressing breast cancer cells to be less sensitive for digitoxin compared to receptor negative breast cancer cells [3]. Recent data confirm that mechanisms other than interaction with hormone receptors are the main effect of digitalis on breast cancer cells [27].
Digitoxin has complex dose dependent mechanisms of action; lower concentrations may induce transcription of survival genes whereas higher concentrations can induce cell death by caspase activation and apoptosis and even higher concentrations induce cell death through necrosis due to heavily disrupted ion homeostasis. Digitoxin seems to target several signaling mechanisms of crucial roles for cancer cells simultaneously and that might make it more difficult for the cancer cells to develop resistance [3,10].
Conclusion
Despite the vast number of in vitro studies, epidemiological studies and clinical cancer studies done the last 10+ years, still we do not know if any cardiac glycoside will have a role in oncology. Most of the clinical studies so far are on digoxin and a few other cardiac glycosides, however, digitoxin seems most promising as an eventual anti-cancer drug. Considering the molecular modes of actions and pharmacokinetics of digitoxin; glioblastoma, pancreatic cancer, leukemia/lymphoma, kidney cancer, urothelial carcinoma and cholangiocarcinoma are some of the most interesting types of cancer for digitoxin treatment. Digitoxin can be combined with chemotherapy, immune therapy and radiotherapy in these clinical trials.

The U.S. Food and Drug Administration (FDA) didn’t remove partially hydrogenated oils from the generally recognized as safe (GRAS) list until 2015,1 based on evidence suggesting their removal could prevent thousands of heart attacks and deaths each year.
In reality, research by Fred Kummerow, dating back to 1957, showed that trans fat interferes with the basic functioning of cellular membranes. Even small amounts of manufactured trans fat have been shown to have adverse effects on your heart, insulin sensitivity and neurological system.
Processed Vegetable Oils Do More Harm Than Good
In response to research and public opinion, many restaurants have since turned from partially hydrogenated oils to 100 percent vegetable oil. However, while these oils do not have trans fats, they’re just as bad, if not worse. There are three significant reasons for this:
When heated, vegetable oils degrade to extremely toxic oxidation products, including cyclic aldehydes,2 which have been linked to neurodegenerative diseases and certain types of cancer. In her book, Teicholz cites research showing that aldehydes cause toxic shock in animals through gastric damage
Vegetable oils are a concentrated source of omega-6 linoleic acid, which has led to a severe imbalance between the omega-6 to omega-3 ratio in most people’s diets
Many of the vegetable oils produced today — especially corn and soy — are products of genetic engineering (GE) and a significant source of glyphosate exposure, and glyphosate has also been linked to gut damage and other health problems
In summary, processed vegetable oils (polyunsaturated fat) harm health by:
Creating high amounts of oxidation products when used in cooking (as they are very susceptible to heat), including aldehydes, which are what cause oxidized low-density lipoprotein (LDL) associated with heart disease. Aldehydes also crosslink tau protein and create neurofibrillary tangles, thereby contributing to the development of neurodegenerative diseases.
Damaging the endothelium (the cells lining your blood vessels) and causing an increase in penetration of LDL and very low-density lipoprotein (VLDL) particles into the subendothelium. In other words, these oils get integrated in your cell and mitochondrial membranes, and once these membranes are impaired, it sets the stage for all sorts of health problems.
Damaging your mitochondria and DNA by making your cell membranes more permeable, allowing things to enter that shouldn’t.
Making the cell membrane less fluid, which impacts hormone transporters in the cell membrane and slows your metabolic rate.
Inhibiting cardiolipin, an important component of the inner membrane of your mitochondria that needs to be saturated in DHA in order for it to function properly.
Cardiolipin can be likened to a cellular alarm system that triggers apoptosis (cell death) by signaling caspase-3 when something goes wrong with the cell. If the cardiolipin is not saturated with DHA, it cannot signal caspase-3, and hence apoptosis does not occur. As a result, dysfunctional cells are allowed to continue to grow, which can turn into a cancerous cell.
Inhibiting the removal of senescent cells, i.e., aged, damaged or crippled cells that have lost the ability to reproduce and produce inflammatory cytokines that rapidly accelerate disease and aging.
Stripping your liver of glutathione (which produces antioxidant enzymes), thereby lowering your antioxidant defenses.3
Inhibiting delta-6 desaturase (delta-6), an enzyme involved in the conversion of short-chained omega-3s to longer chained omega-3s in your liver.4
Exposing you to toxic 4-hydroxynonenal (4HNE), which forms during the processing of most vegetable oils, even if the oil is obtained from organic crops. 4HNE is highly toxic, especially to your gut bacteria, and consumption of 4HNE has been correlated with having an obesogenic balance of gut flora. It also causes DNA damage and instigates free radical cascades that damage your mitochondrial membranes.5
Exposing you to glyphosate residues, as most vegetable oils are made with genetically engineered crops. Glyphosate has been shown to disrupt the tight junctions in your gut and increase penetration of foreign invaders, especially heated proteins, which can cause allergies.
Address Your Omega-6 to Omega-3 Ratio to Protect Your Health
Marine-based omega-3 is one of the most important fats in the human diet, as docosahexaenoic acid (DHA) and eicosapentaenoic acid (EPA) are actually key structural elements of cells, including your brain cells, and not just simple fuel. If you don’t have enough DHA and EPA, your body’s ability to repair and maintain healthy cell structures is seriously impaired.
Unfortunately, in the past 100 years, our omega-6 intake has nearly tripled while our intake of omega-3 has decreased 10fold, and this imbalance has also likely played a significant role in our skyrocketing disease rates. Eating too much damaged omega-6 fat — found in abundance in processed vegetable oils — and too little animal-based omega-3 sets the stage for diabetes, cardiovascular disease, rheumatoid arthritis, cancer, depression and Alzheimer’s, just to name a few.
Now, omega-6 fat in and of itself is not the problem. The problem is that most people get far too much of it, and insufficient amounts of omega-3, and that most of the omega-6 people eat has been damaged and oxidized through processing. Evidence implicating excessive consumption of omega-6-rich vegetable oils as a direct cause of heart disease include but is not limited to:6
The amount of linoleic acid in adipose tissue and platelets is positively associated with coronary artery disease, and studies7 measuring changes in linoleic acid concentrations in adipose tissue in Americans show concentrations increased from 9.1 percent in 1959 to 21.5 percent in 2008. This increase also paralleled increases in the prevalence of obesity, diabetes and asthma.
Conversely, the long-chained omega-3s docosahexaenoic acid (DHA) and eicosapentaenoic acid (EPA) have been shown to protect against coronary artery disease, which is why maintaining a healthy balance between omega-3 and omega-6 is so important.
Patients with atherosclerosis have higher amounts of linoleic acid oxidation products in their plasma, low-density lipoprotein (LDL) and atherosclerotic plaques.
Oxidation of linoleic acid begins before any clinical signs of atherosclerosis become apparent.
When the endothelium (the interior lining of your blood vessels) is exposed to linoleic acid, LDL transfer across the endothelium is increased and this is an essential step in the atherosclerotic process.
Low linoleic acid diets reduce LDL oxidation.
A meta-analysis of randomized controlled trials in humans showed that when saturated fat and trans fat are replaced with omega-6 PUFAs, all-cause mortality, ischemic heart disease mortality and cardiovascular mortality increase.
Oxidation products of linoleic acid are found in infarcted tissue.
The linoleic acid metabolite 9-HODE is a strong promoter of inflammation, and may be both a marker for and inducer of atherosclerosis.
How to Balance Your Omega Fat Ratios
Ideally, you want to maintain a ratio somewhere in the range of (4-2)-to-1 of omega-6 and omega-3 fats. This, however, is nearly impossible if you’re regularly eating processed foods or restaurant fare, as these are loaded with omega-6 from industrial vegetable oils like corn oil and canola oil.
While you do need omega-6, it should be in its unprocessed form, not industrial vegetable oils. Good sources are whole, raw plant seeds and tree nuts. In summary, to correct your omega-6 to omega-3 ratio, you typically need to do two things:
1. Significantly decrease intake of damaged omega-6 by avoiding processed foods and foods cooked in vegetable oil at high temperatures. A number of studies8,9 have found that people who regularly eat deep-fried foods have a significantly increased risk of stroke and death.
Common sources of harmful omega-6 to avoid include corn oil, canola oil, soy oil, hydrogenated or partially hydrogenated fats, margarine and shortening.
2. Increase your intake of marine-based omega-3 fats. Ideal sources include small fatty fish such as sardines, anchovies and herring, along with wild-caught Alaskan salmon, or a supplement such as krill oil.
Healthiest Fats for Cooking
Getting back to cooking oils, if vegetable oils are “out,” what should you use to cook with? Healthy alternatives include:
• Coconut oil — This is what I believe is the best cooking oil. It has a number of valuable health benefits, including a positive effect on your heart and antimicrobial properties. It’s also a great source of energy, thanks to its medium-chain fatty acids (MCFAs). When consumed, the MCFAs are digested and converted by your liver into energy that you can immediately use. Coconut oil also helps stimulate your metabolism to encourage a healthy weight profile.
• Grass fed butter — Raw, organic butter made from healthy grass fed cows’ milk contains many valuable nutrients, including vitamins A, D, E and K2. Furthermore, it contains various minerals and antioxidants that support good health.
• Organic ghee, which has been used for cooking for thousands of years, is another good choice.
• Olive oil — This oil contains healthy fatty acids that can help lower your risk of heart disease. While the standard recommendation has been to avoid using olive oil for cooking and to only use it cold, recent research10 in which 10 popular cooking oils were compared, contradicts this advice, showing extra-virgin olive oil actually scored best for both oxidative stability and lack of harmful compounds produced when heated.
A word of caution is warranted, however. Fake olive oil abounds, so it’s important to take the time to investigate your sources. Tests reveal anywhere from 60 to 90 percent of the olive oils sold in American grocery stores and restaurants are adulterated with cheap vegetable oils or nonhuman-grade olive oils, which are harmful to health in a number of ways.11

Peanut oil and sesame oil are two other healthy options. While both are high in omega-6, peanut oil is high in antioxidants, and sesame oil has been shown to benefit diabetics. The caveat with these two oils is that you need to consume them unheated and in moderation, so as not to throw off your omega-6 to omega-3 ratio.

Black Seed Oil — The Forgotten Gem
Black seed (Nigella Sativa) oil is another exceptional fat with a long history of use in traditional systems of medicine, including Ayurveda and Siddha. The most abundant active plant chemical in black seed is thymoquinone; other bioactive compounds include α-hederin, alkaloids, flavonoids, antioxidants and fatty acids.

How to Use Black Seed Oil
Black seed oil is a highly undervalued and oft-forgotten kitchen staple. When used in cooking, it imparts a warm, slightly bitter flavor that tastes something like a blend of thyme, oregano and nutmeg.
A mixture of black seed oil, honey and garlic also makes for a powerful tonic that can help soothe coughs and boost immunity, especially during cold and flu season or if you feel like you’re coming down with an infection.28
Like all seeds, black seed oil is high in polyunsaturated fats. So, when taken in excess, it could make your mitochondrial membranes more susceptible to oxidation.
For this reason, I suggest limiting your daily intake to 1 to 2 tablespoons or less. A simple way to get a small amount of black seed oil into your diet on a regular basis is to use it in your homemade dressing. Here are a few suggestions:
Mix apple cider vinegar, black seed oil, fresh lemon juice, cilantro and tahini. Experiment with the ratios to enhance the flavor you enjoy the most
A simple and yummy dressing that goes particularly well with broccoli, asparagus or salad greens includes: 1 tablespoon apple cider vinegar, 1 tablespoon lemon juice, one-half teaspoon minced garlic, a dash of ground black pepper and a few fresh basil leaves, chopped
Alternatively, you can use apple cider vinegar and/or black seed oil as substitutes for other oils and vinegars in whatever dressing recipe you’re already using. Keep in mind that the black seed oil does have a spicy kick to it, so substituting the full amount may make it too spicy. Start by adding just a small amount, and experiment to find the ratio of vinegar, olive oil and black seed oil you enjoy

What Is Propolis? What Is Its Origin?
In Greek, “pro” means “at the entrance to” and “polis” means “community” or “city.” Propolis is a natural product used in hive defense by the honeybees. It is also known as bee glue – for obvious reasons.
Propolis is a natural resin-like mixture synthesized by the bees. They collect the lipophilic materials on leaves and leaf buds, mucilages, gums, resins, lattices, pollen, waxes, and large amounts of plant-based flavonoids from different plants at different temperate climatic zones. These are mixed with beeswax and bee salivary enzymes (β-glucosidase) (1), (2).
As this natural resin has a waxy texture, the bees use propolis in the construction and repair of their hives. It is used for sealing the cracks and smoothing out the internal walls. Propolis also offers protection from invading predators, microbes, snakes, lizards, and extreme climate (3).
The smart Egyptians, Greeks, Incas, Romans, and contemporary civilizations used propolis as a preservative, antiseptic, antipyretic, disinfectant, and vulnerary agent (1).

What Is Propolis Made Of?
The composition of propolis depends on the various plants the bees have collected it from. It is generally composed of 50% resin, 30% wax, 10% essential oils, 5% pollen, and 5% miscellaneous substances (3).
The miscellaneous 5% includes minerals and organic compounds. There are phenolic acids, their esters, flavonoids, terpenes, aromatic aldehydes and alcohols, fatty acids, β-steroids, and stilbenes. Flavonoids such as genistein, quercetin, kaempferol, luteolin, chrysin, galagin, and apigenin are the most active components (3), (4).
What’s noteworthy is the varying composition of propolis with geography and climate. Meaning, if you examine propolis from Europe, you will find phytochemicals like pinocembrin, pinobanksin, chrysin, galangin, caffeic acid, ferulic acid, and cinnamic acid (3).
On the other hand, Australian propolis would have pinostrobin, xanthorrheol, pterostilbene, sakuranetin, stilbenes, prenylated tetrahydroxy stilbenes, and prenylated cinnamic acids (3).

1. Heals Wounds, Burns, And Acne

Wound healing is a finely tuned complex cascade of phases like hemostasis, inflammation, cell proliferation, and tissue remodeling.
The flavonoid content of propolis brought about accelerated wound healing in in-vitro studies. It regulates the extracellular matrix (ECM) components according to the phase of wound repair (4).

This product effectively reduced acne and erythematous scars better than the synthetic counterpart (6).

Due to its antibacterial and anti-inflammatory properties, propolis can help in preventing dental caries, cavities, gingivitis, and periodontal disease (7).
Some oral bacteria (e.g.: Streptococcus mutans) colonize on the tooth surface and form dental plaques. They do so by synthesizing polysaccharides from sucrose, water-insoluble glucan, etc. The polyphenols in propolis block the bacterial enzymes involved in forming the dental plaques (7).
A 50% propolis extract exhibited antiseptic effects against pulp gangrene in rats. It interacts with synthetic compounds in mouthwashes, like chlorhexidine, to kill various dental microbes and prevent their adhesion and accumulation (7).

Several hair growth promoters, oral and topical, have been approved by the FDA to solve this crisis. However, natural remedies seem to be safer and rewarding in this case (8).

Of all the ingredients, chrysin and caffeic acid phenethyl ester (CAPE) are identified as key (9).
CAPE targets oral tumor cells along with the protein cascades operating in/around them. Chrysin demonstrates anti-inflammatory and caspase enzyme-inducing effects on blood and other cancer cells (9).
5. Eliminates Bacteria, Fungi, And Viruses
Bee glue is known to fight viral diseases like herpes and HIV-1. It is particularly effective against upper respiratory tract-infecting bacteria, especially those bacterial infections that overlap viral ones (11), (12).
This property can mainly be attributed to the flavonoids pinocembrin, galangin, and pinobanksin. These active compounds can arrest microbial cell division, collapse the cell wall and membrane, inhibit protein synthesis, and ultimately kill the pathogen (13).
