Fathoming serosurvey data, with cautionThe data from the ICMR’s Fourth National Seroprevalence Survey have clear implications for the situation in India today

The past two weeks have seen the release of the Fourth National Seroprevalence Survey for COVID-19 (https://bit.ly/3lklMzJ), as well as the reporting of data of serosurveys independently conducted by State governments such as in Tamil Nadu. The data from the Indian Council of Medical Research (ICMR) conducted national serosurvey are truly remarkable, indicating that between December-January, when the third serosurvey was conducted, and June-July 2021, at least 40% of the population of India, over 500 million people, were exposed to the novel coronavirus of which a small proportion have antibodies because they were vaccinated. The data also show for the first time that over half of all children tested have already been infected, which is expected and reassuring.
Not from infection alone
Such a high rate of infection seems incredible, and if true, needs to serve as a warning to India and the world because it shows the consequences of a combination of variants, inappropriate population behaviour and delayed implementation of public health measures. It is, of course, evident, that all of the seropositivity did not come from infection alone, but given that the seropositivity rate was 67% and the serosurvey reports that a quarter of eligible adults were vaccinated, a large proportion of the antibodies resulted from detected and undetected infections in the first or second waves, with the sharp seropositivity rise indicating that the bulk must have occurred during the second wave. With less than 32 million positive cases, of which many may be positive more than once, and the likelihood of waning antibodies leading to some of those previously infected testing negative subsequently, the seropositivity of 67% is an under-estimate. There are other caveats as well.
Although the serosurvey methodology is similar to previous rounds and sampled individuals in 70 districts in 21 States, it is not clear how representative the survey really is — given the reported two-dose vaccine coverage of 13% in adults which is much higher than the national coverage at the time, and the known high heterogeneity between and within States. This is where State-level data can be useful, and the third Tamil Nadu serosurvey, conducted in over 26,000 participants and compared to previous rounds allows comparison of at least district-level information.
Data from the States
The National Serosurvey data indicated that Madhya Pradesh, Bihar, Rajasthan and Gujarat have over 75% seropositivity, while only Maharashtra, Assam and Kerala were below 60%, with Kerala the lowest, at 44.4%. Although heterogeneity across districts is likely, the fact that the national serosurveys were repeated in the same districts allows for some comparison of seropositivity rates over time. For example, in Kerala, when the national rates were approximately 0.7%, 7% and 24%, the corresponding rates were 0.3%, 0.9% and 11.6% during the first three rounds of serosurveys, showing a consistently lower exposure than the national average, but a large increase between the third and fourth serosurveys.
If we examine the data from Tamil Nadu, where the State-led serosurveys were done in October 2020, April and July 2021, some figures jump out. There appeared to be no change from October to April at the State level, with 31% and 29% positivity, but then there was a massive increase to 66%. Comparing the ICMR survey in three districts with the 38 district surveys of the Tamil Nadu Directorate of Public Health, shows that the ICMR serosurveys found a seropositivity of 43%, 24% and 31% in the districts of Chennai, Coimbatore and Thiruvannamalai in the serosurvey in December 2020-January 2021. The corresponding figures from the State serosurvey were 41%, 22% and 36% in October 2020, 49%, 20% and 34% in April 2021, which rose to 82%, 43% and 68% in July 2021. Some districts showed marked variability, with the central and southern districts showing significant declines in antibodies between October 2020 and April 2021 followed by a large increase in July 2021. For example, Madurai went from 40% to 19% to 79% and Thoothukudi from 39% to 21% to 72%.
Explaining variation
What explains these variations and how can these data be used at the national-State levels? First, there was inconsistency in the assays used for antibody estimation, with the ICMR using an ELISA test that measured antibodies to inactivated virus in its first round and then switching to a method that measured antibodies to nucleocapsid and spike proteins. Tamil Nadu used a chemiluminescence method for all its assays, while serosurveys in Delhi, Mumbai, Pune and other local serosurveys have used different methods — which means that some of the variations may be due to the testing methods which have varying sensitivity and specificity for antibodies directed against different viral antigens. Second, the sampling methods across serosurveys have differed with the ICMR using a cross-sectional survey in 70 districts that were decided/based on reported cases with some lack of clarity on how villages/wards were chosen, while others have used population proportional to size, directed and convenience sampling. Third, the age groups have varied across surveys and with time and not all serosurveys have reported age-stratified results. Fourth, antibodies decay with time, as illustrated in the data from Tamil Nadu’s first and second serosurveys.
So, analyses need to account for the underestimation of exposure. There are many other considerations for analysis, such as the proportion of people included who have reported previous SARS-CoV-2 infection or exposure or the proportion vaccinated. But in general, serosurveys more accurately reflect the experience of the population with an infectious agent than reported cases. This is especially true in India, where without targeted effort and sometimes despite detailed search, it is not possible to discern the reasons for testing, the numbers of individuals that have contributed to cumulative testing, the locations where tests were done or the types of tests used — all of which are essential to understanding the strengths and limitations of the data on reported cases, and the test positivity rate.
Reported cases represent the tip of the iceberg for a virus that can cause asymptomatic infection or result in mild symptoms that are not distinguishable from other respiratory viral infections. Serological data has value for public health because knowing where and how many have already been exposed to the virus gives a clearer picture of how and when infection has penetrated the population. The data from the ICMR’s Fourth National Seroprevalence Survey therefore, have clear implications for the situation in India today.
The Kerala example
When Kerala is reporting the bulk of the country’s positive cases and has a reproductive rate of infection of greater than 1, how concerned should we be? Seroepidemiology shows that the State has over 18 months been able to limit the exposure of its population, and vaccination data indicates that the State is outperforming most others. In general, the health-care infrastructure has not been overwhelmed, deaths are low, and shortages of oxygen and hospital beds have not been reported. Relaxation of restrictions over Eid on July 21 are likely to have resulted in cases which should peak in the first week of August, but there is a need to ensure continued population compliance with restrictions, particularly with Onam approaching and a need to ramp up vaccination. In fact, given Kerala’s success in controlling the virus so far, a key consideration should be whether States and districts with the lowest seroprevalence should receive a greater proportion of the vaccines while supplies are limited.
The immunity gap identified in Kerala demonstrates the value of serosurveillance and the need to ensure that we continue to use this very valuable tool not just to record the history of viral circulation in India but also to inform decisions going forward. At the State and district levels, what can be changed based on serosurvey data? Increasing vaccination in areas with the lowest exposure in parallel with ensuring high testing levels and health-care system preparedness in areas with high vulnerability are immediate responses.
However, serosurveys need to be continued and the data integrated with testing, vaccination and clinical data. These are needed to understand ongoing infection rates, age distribution of infections and cases, where variations could be driven by seasonal coronavirus exposure, vaccination and the effect of waning antibodies, all of which will be essential to inform policies on the need and use of booster doses and long-term system preparedness.

In Puri, more than Covid, ban from ‘gateway to heaven’ scares faithful
NEW DELHI: At 8am every day, 60-yearold Durga Prasad Das Mohapatra goes to the Jagannath Temple in Puri to offer prayers. This has been his routine for the last 45 years. Mohapatra is the daitapati nijog or the general secretary of the group of 185 servitors who are engaged in the Rath Yatra in Puri every year. If the 12th century temple is at the centre of Mohapatra’s existence, his life’s desire is to be cremated at Swargadwar — the crematorium in Puri that many Hindus believe to be ‘the gateway to heaven’. Mohapatra is among the countless who harbour this wish. But now, coronavirus has come in the way of faith. The local administration has prohibited cremation of Covid-19 victims at Swargadwar. And this ban has given rise to an extraordinary situation. The stigma attached with the disease and the fear of spending lonely days in a hospital, added to the apprehension of not getting a cremation at Swargadwar is said to be making people — especially the elderly — risk their lives. People are avoiding getting tested and not approaching doctors when symptoms of Covid show up, say residents of this seaside town in Odisha. During the lockdown, the Puri administration had barred outsiders, even those who hadn’t died of Covid, from being cremated at Swargadwar. Earlier, people not only from across Odisha but also Jharkhand, Bihar, West Bengal and Chhattisgarh travelled to Puri to cremate their kin. Over 100 funeral pyres used to be lit here daily before the pandemic, which dipped to 20-25 with the curb on non-locals, according to sub-collector Bhabataran Sahoo, also the secretary of Swargadwar Seva Samiti. Currently, Swargadwar sees 40-50 cremations a day. The restriction on non-Puri people didn’t go down well with the public and the ban was relaxed in October. For those dying of Covid-19 in Puri, Odisha government has created a separate crematorium. “People who cremate the Covid dead are trained and steps are taken as per protocol,” says Saroj Kumar Swain, additional executive officer of Puri municipality. “We are more concerned about the safety of the living than the wishes of those who want to attain heaven after death,” he says. Sub-collector Sahoo says the cremation ground staff quiz those accompanying a hearse and ascertain the cause of death if a medical certificate isn’t provided. “Some people have been turned back after the details they provided raised suspicion of a Covid death,” adds Sahoo. Swain says corpses aren’t tested for Covid but neighbours inform them if they suspect that a death is related to the disease. But there is no answer as to why separate arrangements can’t be made for Covid victims at Swargadwar like in many other crematoriums. Repeated calls and messages sent by TOI drew no response from Puri collector Balwant Singh and municipality executive officer Bijaya Kumar Dash. On the ground, there’s a running battle between faith and reality. Shibasis Tripathy hails from Bhubaneswar and works in Bengaluru, where he has lived 11 years. “But I still desire to get the final send-off from Swargadwar,” he says. The urge is stronger among the servitors at Puri’s Jagannath temple. “Servitors’ mukhagni (lighting of the funeral pyre) is done with Vaishnavagni or fire from the grand kitchen at Jagannath temple,” says Das Mohapatra. Servitors believe that their lifelong service would come to nothing if their final rites aren’t performed at Swargadwar with the sacred fire. “Over 600 servitors and their family members have tested positive for Covid and around 25 have died,” says a senior servitor and temple official who didn’t want to be named. He further says, “The numbers must be higher as many are hiding symptoms. It is especially true for the elderly who fear that they would be packed off to hospitals in Bhubaneswar and Cuttack and denied final rites at Swargadwar.” Dr JP Das, a government doctor in Puri, says people were not only dodging tests but also hiding Covid symptoms. “The fear of being isolated in hospitals and denied cremation at Swargadwar were major deterrents,” he says. How many elderly people have made Puri their home in the wish for a cremation at Swargadwar? “It is difficult to say as no survey has been done,” states sub-collector Sahoo. According to Das Mohapatra, over 1,000 retirees have bought land and built houses in Puri. Then there are hundreds of widows and widowers who stay at various mutts and lead a religious life with the sole goal of attaining heaven through Swargadwar. Dr Das says that fear and stigma attached to Covid have ebbed. And with Covid cases dipping, it seems the worst is behind for this temple town. “Since October, reported cases of infection among the servitors of Jagannath temple too have come down,” says Das Mohapatra, who himself was Covid-positive and emerged victorious “with blessings of Lord Jagannath and modern medical care at a Covid quarantine centre.”

They trudged hundreds of kilometers on foot lugging their meagre belongings, clung to vehicles, crowded bus and railway stations to get a ride home after they lost their jobs in the cities. Now, a few of them are set to return – by a plane. The contrast couldn’t have been starker.
Bengaluru-based developers Prestige Group has said they are going to fly back migrant labourers from West Bengal and Bihar to Hyderabad to expedite completion of projects, halted by the coronavirus-induced lockdown, in Telangana.
The group has three high-profile projects in Hyderabad.
Even after Centre relaxing the lockdown norms and allowing construction activities to begin, major project sites are in a state of limbo as 99 per cent of the estimated three and a half lakh construction labourers in Telangana are migrants. Most of these workers left for their native states after the lockdown kicked in, stripping them off their means of livelihood in the city.
The first batch of 10 carpenters from Bihar is scheduled to fly in on June 6.
R Suresh Kumar, Vice President, Business Operations for Prestige Group says the response is need-based.
“Let us say the cost of each air ticket is about Rs 5,000 for a flight coming in from Patna. The company estimates that a ticket for an AC coach would cost almost as much, so would the cost of hiring a bus. We may fly in more workers depending on the need.”
The migrant exodus crippled many sectors like construction and real estate. Pushed into a corner by a skeletal workforce, several contractors and developers are now keen to help migrant workers to take to the skies as they realise that without them their business will not take off.

Bihar government extends lockdown till June 30
Patna, June 1 The Bihar government has extended the coronavirus-induced lockdown in the state till June 30 after the Center unveiled the guidelines for a phase-wise lifting of restrictions outside containment zones. The state government has allowed relaxations in economic activities. Now shops and business establishments can restart but will have to the social distancing protocols. The state government which, in its earlier order, had allowed shops to open on alternate days have now allowed them to open on all days. This has come as a big relief for the traders. The planning for the opening of shops have also been changed and now these shops can work together work 9 pm, prior shops needed to pull down shutters at 6 pm. The Bihar government has also allowed inter-state bus services to continue operations from today (June 1). The Mithapur Bus Depot which had been deserted for the past two months was abuzz with activity today as people were seen boarding buses to their destinations. At the Mithapur Bus Depot, a special administrative camp has been set up to screen the handling of transports. The transport administrators have been given rules for continuing tasks. Each bus leaving the bus depot will have to be sanitized, social distancing must be followed, travellers should wear veils and no swarming will be permitted at the bus depot. The administration has clear that strict action will be made any bus operator found defying government orders. NEW DELHI: All the speculation on lifting the nation-wide lockdown came to rest as the government of India has extended the lockdown by two weeks up to May 17, 2020. The lockdown 2.0 was to end on May 3, 2020.
There will a complete ban on air travel, trains and inter-state road transport for the general public while educational institutions, theatres, malls, hotels and bars will also remain shut, but govt gave some relaxations for various business activities and people’s movement within green zones.
Various recruitment activities
As the lockdown has been further extended by two more weeks in India, it means no educational activities will take place during this period. The entrance exams which have been either postponed or deferred will have to be reworked considering the current state of affairs in the country. Union Public Service Commission (UPSC), Staff Selection Commission (SSC) and other state PSCs will also be working on their schedule of conducting competition activities.
School Exams and evaluation The Central Board of Secondary Education (CBSE) had reiterated that it would conduct pending class 10 and 12 board exams in 29 subjects but had not announced the schedule yet. The remaining CBSE board exams would now have to be postponed further.
The board was not able to conduct class 10 and 12 exams on eight examination days due to the coronavirus outbreak. Further, due to the law and order situation in northeast Delhi, the board was not able to conduct exams on four examination days, while a very small number of students from and around this district were not able to appear in exams on six days.
Delhi Deputy Chief Minister Manish Sisodia had suggested that students be marked on the basis of internal assessment as it is not feasible to conduct the pending exams now.
The Bihar board has already announced results for three streams of class 12 and is in the process of evaluation of class 10 exams.
The Uttar Pradesh board is yet to begin the evaluation and will take a call in this regard soon. University/Colleges semester exams and beginning of academic session 2020-21
The University Grants Commission (UGC) on Wednesday said that terminal semester students exams will be held in the month of July.
The new academic session for freshers may begin in universities from September and for already enrolled students in August in the wake of the COVID-19 outbreak in the country, the UGC told universities.
Competitive exams like NEET/JEE
According to the HRD ministry officials, as of now there are plans to conduct competitive exams, including JEE and NEET, in June for admission to engineering and medical colleges.
In the present scenario, the entrance exam conducting body like NTA, will have to rejig their schedule further. UGC, CBSE, NTA and state boards are expected to announce their revise schedule in the coming days.
