Surviving Sepsis
January 16, 2019 would normally have been a day of celebrating my daughter’s birthday, except I was trying to survive a critical situation.
I tried to joke while laying in the hospital emergency room, crying from the pain in my left side, that I had pain like this 23 year ago, but at least at the end of it, I also had a tiny being in my arms. What would possibly come of this? What was wrong with me?
Doped up on morphine for the pain and my boyfriend by my side, I went through ultra sounds and a CT scan. I had a kidney stone. A really big kidney stone. It wasn’t going to pass, and I would need surgery. I also had a kidney infection. Did I know about the infection? Nope. I had zero symptoms. No clue I had been sick until that morning when I could barely walk and was vomiting.
Over 8 hours I spent that morning at the hospital on pain medication until they finally thought I was well enough to go home. If I started feeling worse that I would need to come back, “there is a chance you could become septic”. “Remember to contact the urologist in the morning to schedule surgery”. I would never get that chance.
No less than two hours later, I laid on our bed, and watched as my boyfriend called 911. He pleaded with them to hurry. I was, indeed, dying. I knew it too. It was the most helpless feeling, laying there knowing that there was absolutely nothing that I could do to change the course of events that was happening. Would I last until the ambulance got there? Would I die in our bed? I didn’t want to die in our bed. I didn’t want him to watch me die either. I didn’t want any of my loved ones to witness my death.
The ambulance showed up, and I was on my way to the hospital. All medication they tried to give me in the ambulance, I vomited up. The tech was having difficulties getting an IV in my arm. I didn’t care. I just didn’t want to die in the ambulance.
Things after that were hectic. The same ER doctor told me that they were going to be transferring me to another hospital. I had sepsis and I had went into sepsis shock. They needed to put an IV in my neck to be able to give me medicine to keep me alive. I asked her if I was going by ambulance. She said “If you go by ambulance you won’t make it. You are being airlifted. The helicopter is on its way.”
I joked when with the air crew as they loaded me into the helicopter. Told them that I always wanted to ride in a helicopter, but this wasn’t the way I wanted it. It was dark, I couldn’t see because I was strapped to a board, and I just wanted to sleep. They wouldn’t let me sleep though. They kept asking me if I was still with them.
I don’t remember much after arriving at Aspirus Wausau. It was a whirlwind. I was going to get a port put in a vein in my neck for the medicine to stabilize me. The anesthesiologist was on his way to get me ready for emergency surgery. The kidney stone had moved and was blocking the flow. I said “Maurice is trying to kill me”. I felt I had no other choice but to name the very item that was trying to take me from this earth. They must have all thought I had lost my mind. I must be hallucinating. I don’t remember much after the trauma doctor put the port in my neck. I was asleep before I made it half way to surgery to have a stint put in to move the kidney stone out of the way.
I only spent a few hours in ICU which is almost crazy for someone who was in the condition I was in when I arrived. According to sources, the mortality rate of SIRS ranges from 6% to 7% and in septic shock amounts to over 50%. In particular, abdominal sepsis exhibits the highest mortality rate with 72%. The long-term prognosis is equally poor; only approximately 30% survived the first year after hospital admission.
My daughter came down and spend the morning in ICU with me. My sister would take days off from work to stay in that hospital by my side taking care of me better than any one nurse ever could. I was in constant contact with my boyfriend who needed to be home to take care of our pets. I was in good hands.
I spent less than a week in the hospital and was sent home. I ended up readmitted to the local hospital a few days later because the sepsis came back. At least I was close to home this time, and my boyfriend and mom could come see me. I was off from work for close to two months. I would have surgery to break up the kidney stone after the infection was gone, and then later have the stint removed.
Tomorrow morning will be my day of celebrating my one year of surviving. The trauma that came from that night, it hasn’t left. It is still very fresh in my mind, and PTSD is real from health trauma.
I am lucky. One of the few lucky ones to have survived septic shock. I am also one of the few ones to have gone through such a health crisis and have good health insurance. I cannot imagine what I would have done if I didn’t. It took me almost a year to finally add up all the bills to see how much my life was “worth”.
You’re reading that correctly - $134,523.21, of which $42,000.00 of it was for the medevac helicopter. We are still working with the insurance company to cover that bill. When I say I support Healthcare for All - this is why. No one - absolutely no one - should have to worry about how much their life is worth. They should have zero worries when it comes to how are they going to pay a $200.00 doctor bill or over $100k in medical bills. Every life is worth something, and I have no problem paying more in taxes if it means no one will have to think about how much their life is worth and if they can afford to survive.

Wausau, Wisconsin-based Aspirus Health Care is offering signing bonuses of up to $15,000 for nurses with a year of experience.
Hospitals also are turning to nurses who travel from state to state. But that’s expensive, because hospitals around the country are competing for them, driving salaries as high as $6,200 per week, according to postings for travel nursing jobs.
April Hansen, executive vice president at San Diego-based Aya Healthcare, said there are now 31,000 openings for travel nurses, more than twice the number being sought when the pandemic surged in the spring.

Week 5: History of Design

On page 182, we are shown the “Shroder House,” and discuss how it “it appears less like a series of solid volumes than as a conglomeration of individual planes that pass through one another” (Eskilson, 2019). When I read this, I immediately think of the walls of Tartarus, the mythical prison for the Gods. The walls ever so often to prevent intrusion or escaping. Either way, this structural design strikes me as something containing hints of De Stijl architecture from the way that it’s levels seem to fall into each other. This is a hotel in my town of Wausau, WI. I drive past it every single day during work, even delivering to it at certain times. To my eyes, they do not seem as one giant circle, they appear as many different wheels that rotate around themselves. Our book also talks about the off the wall colors that help set features apart. At night we can see that there are lights designed to this this sort of thing. During the day, the beams are all an offset red that makes them identifiable even from the road. It draws the eye!

On page 193, we are introduced to Constructivism and some various examples that stem from that. What caught my attention was the “Monument to the Third International” design, specifically how it was “designed to reach a height of over 1,300 feet, with a spiral skeletal structure encasing a series of government offices” (Eskilson, 2019, p. 193). It was said to have incorporated many different identifiable shapes within the structure as well. My immediate thought ran to a skyscraper in Dubai that I constantly look at in my free time. Although it is not necessarily a physical part of my daily life, I wish it was! This structural design incorporates many different and unique ideas, very similar to constructivism from what I can tell. Judging from what I read, I can gather that these ideas were ahead of their time and almost futuristic in nature. That’s what I perceive when I look at this building. I see them being similar because this building is so unique, what with it’s entire structure almost bending in the shape of a fin or the point that it meets up at the top. We can also see by comparing these two designs just how much time has evolved. Granted not all skyscrapers are this tall, but there are certainly buildings in Dubai that surpass the 1300-foot mark by well over 700 feet. Bottom line, both of these designs were ahead of their time in theory!

An excellent example that caught my eye in the reading was on page 277 when we were shown the poster of “Anonymous, Mightier Yet” (Eskilson, 2019). This is a Second World War poster designed by Britain to entice people to join the military with ideas of being a hero and discovering a new journey. I just wanted to include a poster design above for the National Guard that I am shown constantly while drilling at Fort McCoy. Times have definitely changed, and new methods have been adapted. Now instead of promising exciting adventures, the recruiters simply post the amazing benefits, such a sign-on bonus or free education. This can also go to show how different the situation can be depending on the state of the world. Right now, we are not in the middle of a world war situation, so that could have something to do with the varying theme. Either way, not much else has changed!

One of the larger concepts discussed in our book was Bauhaus. We are shown an example of the Bauhaus buildings in Dessau, Berlin on page 225. The description turns to discussing how “asymmetry was also a key aesthetic component”, how the buildings “were intended to be experienced in three dimensions”, and how “Gropius, in contrast, left these ‘industrial materials’ exposed” (Eskilson, 2019, p. 225). We can see in the image that there are many different buildings, each looking a little different from the last, but still very simple in nature. This reminded me an awful lot of how the Aspirus hospital appears in Wausau. Above is a picture of just one of the various wings of the hospital. Similar to what Gropius said, this is a building that you would need to walk around to fully grasp what it looks like, as each part looks different from the rest. It looks simple in design, with many of the industrial elements still exposed that were used to construct it. To my eyes, this largely relates to a Bauhaus design in today’s standards.

Rating Methodology
Health Inspections Every year, the government assigns inspectors to conduct a formal review of nursing homes for regulatory purposes to meet the mandates outlined for Medicare and Medicaid, this aims to measure and improve the safety of residents across providers. Facilities may also be inspected when complaints are submitted or based on a reported incident. When noncompliance is identified, the facility is served a citation that indicates which regulation that was identified, along with the severity of the incident. Nursing homes are subsequently required to execute a program of resolution in order to meet compliance. Some scenarios require enforcement actions to be applied, such as a civil monetary penalty or withholding of payment(s), to incentivize resolution in a timely manner. Penalties Facilities are applied 2 types of penalties due to non-compliance / accumulation of incidents. Civil penalties are monetary fines that may be applied to a facility based on citations / infractions identified during a review. The severity of a penalty is defined primarily by the size and frequency of the infraction. Quality of Residence Care There are 3 types of resident care ratings, but for this exercise, we primarily focused on the overall quality measure rating. The quality measures (QMs) include 17 data points that are derived from clinical information reported by the respective nursing home and also from Medicare claims data submitted for payment. Ratings are calculated for the QM domain using the 4 most recent quarters for which data are available. A nursing home receives points contingent on performance on each measure (weighting distribution is not equal). Staffing Staffing research is submitted regularly by the facility and is adjusted for the requirement of the facilities residents. For each of registered nurse staff and total staffing, a 1 - 5 rating is applied according to definitions established for each category. These ratings are subsequently combined to assign an overall staffing rating. As an example, to get an overall staffing rating of 5 stars, nursing homes must earn a rating of 5 stars for both registered nurses and total staffing. Nursing homes could also be assigned a 1 star rating should they not have a registered nurse on-site daily, and do not submit staffing data, or which the data cannot be verified.
